Challenges for the European SPA Management. Results of the ILIS ProjectIntroduction. The spa industry has been rapidly growing throughout Europe in recent years, which has led to an associated increase in educational demand in spa management. The aim of the study was to identify the most important needs for training and the most significant skills among the spa managers and their personnel throughout Europe. Moreover, it was to research the background of spas were the study had taken place. Material and methods. 30 managers took part in semi structured interviews that used a pre-prepared questionnaire. The answers were analysed with Wilcoxon signed rank test and also the mean values were compared. Results. The needs for training and importance of skills vary from country to country but common areas of interest are: sales, marketing, quality and innovation management, legal regulations, operational and social skills. The complex character of answers to questionnaire was a result of different spa business models and differing domestic spa markets as well as variety of legal environments. Conclusions. Spa tourism creates a significant income among the other tourism disciplines since that further research is needed. Demands for the spa education differed accordingly to a stage of each country spa business development. Spa managers tend to express higher training needs for their employees than for themselves.

Background: CRP, an acute phase reactant, is known to increase up to 1000 folds following inflammation and/or infection, but it’s utility as a significant predictor of difficult LC or need for conversion to OC still remains controversial. Limited studies on levels of hsCRP and its association with difficult cholecystectomy are available in literature, especially in Indian context. It is hypothesized that levels of hsCRP pre-operatively for a given population or an individual patient if high would help a surgeon anticipate better a difficult LC, avoid excessive intra-operative manipulations, go for conversion early, reduce time of surgery and hence also minimise post-operative complications. Methods: The present included 30 clinically diagnosed ‘cholecystitis with cholelithiasis’ patients, and 30 healthy individuals. For accurate comparison of hsCRP levels, a fasting and post-operative, 6 and 12 hours, blood samples were analyzed by ELISA test. Pre-and post-operative hsCRP values were recorded and analyzed using multi-user licensed SPSS- Version 21.0.Results: The mean value of hsCRP in controls was 1.43mg/L. In study group, in pre-operative serum samples, the concentration was 20.46 mg/l. At 6 and 12 hours post-operative periods, the mean values observed were 24.4mg/L and 23.7 mg/L respectively. Compared with controls, the mean values are fairly high and statistically also significant (p-values <0.0001). For the study group comparison of hsCRP value pre-operative and post-operatively 6 hours, using Wilcoxon Signed Rank Test has given the p-value <0.001 indicating that the increased hsCRP concentration in patients’ circulation is highly significant as a predictor of difficult laparoscopic cholecystectomy. Similarly, Comparison of hsCRP value pre-operative and post-operative 12 hours, using Wilcoxon Signed Rank Test revealed the p-value <0.007 showing it as highly significant and a predictor of difficult LC requiring conversion. The preoperative value of 20.46% is significant for an individual planned for LC who may require conversion. Conclusions: The preoperative hsCRP value of 20.64mg/L, as per our study results, can be relied upon as predictors of difficult LC and/or need for conversion not only in the study group in the given population but also in individual cases. Such a value of 20.64mg/L is definitely important for an individual case management. Knowing the preoperative values beforehand and if found high, it places a surgeon on a cautious note to avoid too much of LC manipulations intra-operatively, take an early decision to go for conversion, reduce the time of surgery, minimise post-operative complications and hence also morbidity.

Forceps biopsies of airway lesions have variable yields. The yield increases when combining techniques in order to collect more material. With the use of cryotherapy probes (cryobiopsy) larger specimens can be obtained, resulting in an increase in the diagnostic yield. However, the utility and safety of cryobiopsy with all types of lesions, including flat mucosal lesions, is not established.Aims. Demonstrate the utility/safety of cryobiopsy versus forceps biopsy to sample exophytic and flat airway lesions.Settings and Design. Teaching hospital-based retrospective analysis.Methods. Retrospective analysis of patients undergoing cryobiopsies (singly or combined with forceps biopsies) from August 2008 through August 2010.Statistical Analysis. Wilcoxon signed-rank test.Results. The comparative analysis of 22 patients with cryobiopsy and forceps biopsy of the same lesion showed the mean volumes of material obtained with cryobiopsy were significantly larger (0.696 cm3versus 0.0373 cm3,P=0.0014). Of 31 cryobiopsies performed, one had minor bleeding. Cryopbiopsy allowed sampling of exophytic and flat lesions that were located centrally or distally. Cryobiopsies were shown to be safe, free of artifact, and provided a diagnostic yield of 96.77%.Conclusions. Cryobiopsy allows safe sampling of exophytic and flat airway lesions, with larger specimens, excellent tissue preservation and high diagnostic accuracy.

Abstract Cephalometric norms are important to orthodontists in their diagnosis, treatment, and evaluation of orthodontic treatment outcomes. The purpose of the present study is to establish if orthodontists treat and finish their cases to the cephalometric means or norms. Pre- and post-cephalometric radiographs and dental casts of 35 orthodontically treated cases were analyzed. The Kappa test, Wilcoxon Signed-Rank test, Paired t- test, and Z-test were used for the statistical analysis of the data. The result revealed that orthodontists do not reach the cephalometric mean values post-treatment. However, sagittal maxillomandibular relationship and interlabial gap are the main areas of improvements. This leads to improvement of soft tissue esthetics by camouflaging the skeletal and dental relationship. Citation Al-Balkhi KM. The impregnation of knitted Dacron grafts with biodegradable protein removes the need for preclotting at implantation. The effect of such impregnation on the thrombogenic potential of these prostheses is, however, unknown. We have compared the early thrombogenicity of two impregnated knitted Dacron grafts - Hemashield ( collagen-impregnated Dacron, Meadox) and Gelseal (gelatin-impregnated Dacron, Vascutek) -with conventionally preclotted knitted Dacron in an artificial circulation. Fresh, heparinised human blood, containing 111Indium-labelled platelets, was circulated around identical twin circuits of silicone tubing by a Sarns roller pump at 180 ml/min and 120 mmHg. Into each circuit a different 15 cm × 8 mm type of graft material was mounted. Changes in platelet count, platelet aggregation to collagen, plasma haemoglobin and blood radioactivity were recorded and after 60 minutes of perfusion each graft was sectioned and counted for radioactivity prior to scanning electron microscopy (SEM) of the luminal surface. The mean graft radioactivity divided by the mean blood radioactivity yields a Graft Activity Index (GAI) allowing comparison of platelet deposition and thus thrombogenicity.RESULTS: (mean ± s.e.m.) 8 examples of each graftSEM confirmed these findings with preclotted Dacron having the highest platelet deposition. Both impregnated grafts produced significant early haemolysis within 4 minutes of blood contact. Our findings indicate that the early thrombogenicity of these impregnated grafts is significantly less than conventionally preclotted Dacron, (p< 0.01, Wilcoxon signed rank test), despite the early haemolytic phenomenon and that Hemashield grafts are less thrombogenic than Gelseal grafts. (p<0.05, Wilcoxon signed rank test)

Aim of this trial was to assess (1) the accuracy and precision of electrochemistry-based glucometers CONTOURLINK, CALLA, and LINUS and (2) the diabetes control using Ambulatory Glycaemic Profiles (AGP) as markers of therapeutic effectiveness. Glucometers and COBAS INTEGRA 400 Plus analyzer were used by one laboratory professional to estimate P-glucose (PG) in 112 out-patients. There were 112 sets of 12 PG estimations analyzed. In each set, means of 3 capillary PG estimations on 3 respective glucometers and on INTEGRA analyzer were calculated. The statistical program SPSS, v. 15, was applied. The mean INTEGRA PG values ranged from 2.7 to 25.3 mmol/L. There were strong correlations between mean PG on INTEGRA versus CONTOURLINK, versus CALLA, and versus LINUS; PG deviations from INTEGRA were mostly within the range ±15%. Wilcoxon Signed Rank Test revealed differences between CONTOURLINK-INTEGRA and LINUS-INTEGRA; CALLA-INTEGRA showed no difference. SD INTEGRA = 0.061 mmol/L, SD CONTOURLINK/SD CALLA/SD LINUS were 0.256/0.290/0.286 mmol/L. All patients were trained to perform defined 10-point PG profiles to adapt food, exercise, and insulin doses. The PG differences between all tested glucometers and reference values were in borderline of ISO 15197 but worthy of consideration. AGP are helpful markers of diabetes control.

The aim of this study was to quantify the effect of tonsillectomy on the incidence of sore throats and its co-morbidity in adult patients. One hundred and nineteen adult patients were sent a standard questionnaire regarding their symptoms in the 12 months preceding and following their tonsillectomy. Outcome measures included the incidence of sore throats, total number of days with sore throat, amount of time taken off work or school, and number of visits to the general practitioner (GP). In addition, patients were asked to indicate the duration of their symptoms and whether or not they found the tonsillectomy effective in curing their sore throats.Sixty-six patients (55.5 per cent) returned completed questionnaires. The age of the patients ranged from 16 to 39 years. The mean duration of symptoms was 8.3 years. On average, patients had 8.1 different sore throat episodes, 42 sore throat days, 21.4 days of sore throat related absence from work or school, and 5.9 visits to the GP in the 12 months before their operation. For the 12 months after surgery, these reduced to 0.9 episodes, four days, 2.2 days and 0.6 visits, respectively. This reduction was very significant (p < 0.001, Wilcoxon signed rank test). After their surgery, more than half the patients achieved complete resolution of all the measured parameters mentioned above. Most of the remaining patients achieved at least 50 per cent resolution. Only three patients (4.8 per cent) achieved less than 50 per cent resolution. Ninety-five per cent of the patients found the operation effective in curing their sore throats and were glad they had had surgery. In conclusion, retrospective questionnaire data must be interpreted with some caution, but this study suggests that tonsillectomy is effective in reducing the incidence, duration and co-morbidity of recurrent sore throats in adults; this must be balanced against the post-operative problems in a minority of patients.

Purpose: To describe the biomechanical changes seen after thumb carpometacarpal (CMC) fusion with distal scaphoid excision (DSE), a surgical technique used for pantrapezial disease in high-demand individuals. Methods: A cadaveric study with 13 fresh-frozen cadaveric specimens was conducted. Fluoroscopic measurements in posteroanterior (PA) and lateral planes were performed. Thumb CMC fusion was then simulated. The same images were taken after fusion. The distal scaphoid was excised, and the measurements were obtained again. The angle between the index finger and thumb metacarpals was recorded. Statistical analysis was performed using Wilcoxon signed rank test. Results: The mean arcs of motion in the PA plane pre-fusion, post-fusion, and post-fusion with DSE were 48.5°, 25.1°, and 34.6°, respectively. An increase in arc of 9.5° was obtained after DSE compared with thumb CMC fusion alone. The mean arcs of motion in the lateral plane pre-fusion, post-fusion, and post-fusion with DSE were 53.4°, 22.4°, and 33.6°, respectively. A statistically significant increase in arc of motion of 11.2° in the lateral plane was obtained after DSE compared with thumb CMC fusion alone. This was a 20% and 21% increase in pre-fusion range of motion in the PA and lateral planes, respectively, from fusion alone compared with fusion and DSE. Conclusions: Thumb CMC fusion combined with DSE maintains motion lost in simple CMC arthrodesis.

The disturbance in performing Poyandu routine activities in Puskesmas Bojong Menteng and face-to-face Cadre refresher during COVID-19 pandemic might reduce cadres' motivation and knowledge. A virtual education program through webinars is one of the possible solutions. This community service activity aims to analyze the effects of webinar on nutrition through zoom meeting on cadres' motivation and knowledge regarding 1) the role of cadres during the pandemic, 2) prevention of COVID-19 with balanced nutrition, water, sanitation, hygiene, and physical activity and 3) introduction to Ms. Excel application in smartphone. The webinars were given in 2 days with delivering a pre- and post-test evaluation. The statistical analysis used was Wilcoxon signed-rank test. Participants were 33 Cadres. Cadres have received technical guidance to use the zoom application on their smartphone prior to the webinar schedule. The majority of cadres are in the age of 31-45 years old (45.5%) and have graduated from high school (63.6%). Result shows that the mean of Cadre’s score on knowledge and motivation had increased from 65 to 85 and 90 to 100 (p:<0.05). This concludes that webinars could be a potential way to refresh and educate Posyandu cadres during a pandemic
