The objective is to determine the optimal synthetic material for endoprosthetics of the abdominal wall.Material and methods. The experiments were conducted on 50 rabbits of the Chinchilla breed, which were divided into 2 groups of 25 individuals in the each group. In the first group, a lightweight polypropylene endoprosthesis with filament diameter of 90 microns was implanted at the aponeurosis of the anterior abdominal wall, and in the second group – a super-lightweight polypropylene polyvinylidene fluoride endoprothesis with filament diameter of 70 microns. We performed morphometric and histological study of reactive changes in the tissue surrounding the implanted endoprosthesis.Results. In the animals of the second group, the inflammatory response to the implanted prosthesis was on the 3rd day of the experiment lower by 1.3 times, on the 7th day – 1.2 times, on the 10th – 1.1 times; fibroblastic response on the contrary increased 1.2, 1.12, 1.16 times on the same days.Conclusion. When choosing a lightweight synthetic material for hernioplasty, the preference should be given to the super-lightweight polypropylenepolyvinylidene fluoride endoprosthesis.

The healing processes of sutured wounds of soft tissues in most cases depend on the type and quality of the suture material. Any kind of suture material in the tissues of the body is a foreign body that causes various reactive changes from the tissues. The nature of these changes, all other things being equal, is mainly determined by the type of suture material, its thickness and the method of sterilization of the tissues on which the sutures are applied, the trauma of surgery, the infection of the wound and the irritating effect of the threads on the tissues.

ABSTRACT Background: Extramedullary hematopoiesis depends on complex pathophysiological mechanisms linked to hematopoietic stem cells and the proteins considered mediators of the inflammation. The identification of hematopoietic cells outside bone marrow in the adult is an occurrence that can occasionally follows the inflammatory response, was considered a secondary occurrence, but current biomolecular studies have changed that concept. Aim: Describe the presence of clusters of precursor cells of platelets (megakaryocytes), and cells of the inflammatory response in the abdominal wall and spleen of rats with experimentally induced incisional hernias and repaired with different synthetic prostheses. Methods: Twenty-five rats with incisional hernias previously performed, were divided into groups of five animals each: Group 1, repair of the hernia defect without prosthetic implant; Group 2, repair with polypropylene prosthesis; Group 3, repair using polypropylene with low weight; Group 4, the use of polypropylene and polyglecaprone prosthesis; Group 5, of polypropylene and polyglactin prosthesis. All prostheses were cut in rhombus format with area 2,625 cm². The animals were reoperated after 10 days, the abdominal walls were removed with the viscera attached to them and the material was processed for histological study. Results: Megakaryocyte niches in the abdominal wall and spleen, occasionally removed together with the adhesions produced in animals with implantation of prostheses and significant inflammatory reaction. Conclusion: The intense inflammatory reaction due to the prostheses with polypropylene in their composition was disproportionate to the expected response, indicating that further studies should be accomplished including immunophenotyping evaluation and specific panels of monoclonal antibodies to better understand the findings.

Our experimental study shows a morphological response of the anterior abdominal wall to implantation of a combined (bovine pericardium graft and polyester mesh) hernia prosthesis. For this purpose, fifteen chinchillas were operated on. A combined prosthesis was implanted intra-abdominally, where the pericardium graft adjoined the internal organs with the smooth side, while the synthetic material was facing the peritoneum. Morphological and morphometric assessment of the tissue in the area of implantation of the prosthesis was carried out. Morphology of tissues samples was evaluated in 2 weeks, in 1 month and in 2 months after surgery. Our study revealed that the combined prosthesis implanted in the anterior abdominal wall causes an acute inflammatory response, which progresses within a month after the surgery. However, the signs of inflammation reduced at the end of the second month of the experiment. A new connective tissue grew actively into the implant from the side of the polyester mesh. The pericardium graft adjacent to the abdominal cavity with the smooth side did not cause formation of connective tissue. Thus, the combined hernia prosthesis has protected the abdominal cavity from adhesions in the postoperative period.

Deep cervical fascia forms a connective tissue sheath around the thyroid gland. Delicate trabeculae and septa penetrate the gland indistinctly dividing the gland into lobes and lobules which in turn composed of follicles.1,2,3 These follicles are structural units of thyroid gland which varies greatly in size and shape.4 The number of follicles varies in different age groups. The study was carried out to see the percentage of area occupied by follicles in the stained section of thyroid glands in different age groups. The collected samples were grouped as A (3.5 – 20yrs), B (21- 40yrs) & C (41 – 78yrs). Aim. To study the influence of deproteinized dialysate from blood of milk-fed calves on the type of exudative discharge and dynamics of inflammatory reaction after hernia repair with plastics of the anterior abdominal wall with hernioendoprosthesis of polypropylene. Materials and Methods. The study involved 59 patients being on stationary treatment in the surgical department of Kursk Regional Clinical Hospital. Patients were hospitalized for the herniation of small or medium dimension. The patients were divided to two groups: the main (n=30) and control (n=29) groups. After endoprosthetics, the patients of the control group were given complex conservative treatment. The patients of the main group, besides standard treatment, were administered deproteinized dialysate from blood of milk-fed calves intravenously in drips 10 ml + 200 ml of 0.9% sodium chloride solution within 7 days. For cytological examination and determination of the type of cytograms, the traumatic discharge was collected and analyzed using the method of M.F. Kamaev and M.A. Palthsev. Results. Cytomorphometric examination was conducted on the third, fifth and seventh day after endoprosthetics with the aim of studying dynamic changes. Determination of cell composition and also of its changes characteristic of each studied period, was necessary for obtaining further information characterizing inflammatory process in the region of placement of the endoprosthesis. After endoprosthetics in patients who were administered hemodialysate, the inflammatory reaction was less pronounced than in patients who did not receive the preparation. This was associated with a more dynamic change of stages of the inflammatory process. In patients of the main group who received deproteinized dialysate, regenerative type of inflammation first appeared on the fifth day and made 6.9%; by the seventh day the share of patients with the regenerative type rose to 17.5%, while in the patients of the control group no regeneration stage was observed in both periods. Conclusion. Analysis of the efficiency of influence of hemodialysate on the inflammatory reaction in plastics of the anterior abdominal wall with polypropylene endoprosthesis evidences faster course of all stages of inflammation and reduction of its intensity by 10%.
