Introduction: Breastfeeding promotion is one of the most important strategies against infant mortality and to control child undernourishment. Despite policies and plans to promote and protect breastfeeding in Colombia, its practice is low and its duration is short. Objective: To propose an ecology framework to interpret and incorporate contextual, interpersonal, and individual factors associated with the practice of breastfeeding and duration. Thereby, the plans and policies addressed to promote and pro­tect breastfeeding in Colombia could be reinforced. Conclusions: To implement an ecology framework for Breastfeeding in Colombia, it is necessary to identify the effect of contextual factors in the biggest cultural regions of Colombia, to recognize the limitations of Infant-Friendly Hospital Initiatives to improve exclusive breastfeeding duration, to execute prospective studies in order to identify factors associated with breastfeeding duration, to design and implement plans and policies based on comprehensive planning strategies of healthcare interventions, to develop appropriate and cost-effective extra-institutional strategies aimed at prolonging the duration of breastfeeding, and to implement more reliable breastfeeding surveillance systems.

BACKGROUND: Exclusive breastfeeding is the best form of nutrition for infants during the first 6 months of life. Nevertheless, the practice of breastfeeding is currently declining throughout the world. OBJECTIVE: The objective of this study was to assess the prevalence of exclusive breastfeeding (EBF) practice and its associated factors among mothers living in Marrakesh province, Morocco. METHODS: The data were collected using an interviewer administered questionnaire. Both bivariate and multivariate logistic regression analyses were used to identify factors associated with exclusive breastfeeding practice. RESULTS: Prevalence of exclusive breastfeeding was 50.2%. Mothers with age more than 30 years, residing in rural area, receiving counseling related breastfeeding during antenatal follow up and having knowledge about duration of breastfeeding were more likely to practice exclusive breastfeeding than their counterparts. CONCLUSION: Although the prevalence of breastfeeding in Marrakesh was higher than the last national survey report but it is still insufficient. These findings have to stress authorities and deciders to sensitize mothers and future mothers to increase exclusive breastfeeding practice notably through the involvement of health professionals.

Background Breast milk is the ideal food for infants. According to the 2007 Indonesian National Household Health Survey, only 23% of mothers exclusively breastfeed for six months.Objectives To determine the rate of exclusive breastfeeding in Indonesia, to evaluate factors associated with infant feeding practices, and to compare the nutritional and developmental status between exclusively-breastfed and formula-fed infants.Methods A survey was conducted in hospitals located in 17 provinces in Indonesia. The rate of exclusive breastfeeding was calculated. Many variables were investigated as potential predictors for exclusive breastfeeding using a multivariable logistic regression analysis. Further analysis was performed to compare the nutritional and developmental status between exclusively breastfed and formula-fed infants at the time of survey.Results From 1,804 infant subjects, the overall rate of exclusive breastfeeding was 46.3%, ranging from 10.5% in East Java to 66.9% in Jambi. Predominant breastfeeding, complementary feeding, and formula feeding rates were 14.3%, 8.6%, and 30.7%, respectively. Maternal unemployment was associated with a longer duration of breastfeeding (P=0.000). There were significantly more formula-fed infants who were undernourished compared to exclusively-breastfed infants (14% vs. 8%, P=0.001). There were also significantly more infants in the formula-fed group who had abnormal head circumference compared to those in the exclusively-breastfed group (9% vs. 6%, P=0.031). Child development, as assessed by the Pre-screening Developmental Questionnaire, was similar between the two groups (P=0.996).Conclusion The overall rate of exclusive breastfeeding in Indonesia is 46.3%. Maternal unemployment is associated with longer duration of breastfeeding. Exclusive breastfed infants have significant better growth and head circumference compared to formula fed infants, while the development is similar between the two groups.

Several small studies have shown associations between breastfeeding and genome-wide DNA methylation (DNAm). We performed a comprehensive Epigenome-Wide Association Study (EWAS) to identify associations between breastfeeding and DNAm patterns in childhood. We analysed DNAm data from the Isle of Wight Birth Cohort at birth, 10, 18 and 26 years. The feeding method was categorized as breastfeeding duration >3 months and >6 months, and exclusive breastfeeding duration >3 months. EWASs using robust linear regression were performed to identify differentially methylated positions (DMPs) in breastfed and non-breastfed children at age 10 (false discovery rate of 5%). Differentially methylated regions (DMRs) were identified using comb-p. The persistence of significant associations was evaluated in neonates and individuals at 18 and 26 years. Two DMPs, in genes SNX25 and LINC00840, were significantly associated with breastfeeding duration >6 months at 10 years and was replicated for >3 months of exclusive breastfeeding. Additionally, a significant DMR spanning the gene FDFT1 was identified in 10-year-old children who were exposed to a breastfeeding duration >3 months. None of these signals persisted to 18 or 26 years. This study lends further support for a suggestive role of DNAm in the known benefits of breastfeeding on a child’s future health.

IntroductionKnowing breastfeeding is one of the easiest and cost-effective ways to maintain a good childhealth, majority infants are not exclusively breastfed for the recommended 6 months of age.Thus, the aim of the study was to find out the factors associated with discontinuing exclusivebreastfeeding (EBF) among mothers of infants’ aged 0-6 months. MethodsA analytical study design was used to find out the factors associated with discontinuing EBFamong mothers of infants’ aged 0-6 months in Nepal. A total of 380 mothers of infants whosechild were 0-6 months old were selected using non-probability purposive sampling techniquethrough online survey as people are advised to stay at home during the Covid-19 outbreak. Datawas collected using structured questionnaire with interview technique. ResultsStudy findings revealed that half (50.5%) of the mothers exclusive breastfed their infants withmother’s mean age being 27.39 ± 3.64 years. Discontinuation of EBF varied with mothers age,ethnicity, mother’s educational level, initiation of breastfeeding and sources of informationregarding EBF as newspaper/articles and relatives/friends/neighbors (p=<0.005). The factorsrelated to mother that are associated with discontinuation of EBF were found to be perceivedinsufficient milk, cesarean section delivery, jobholder mother (p= <0.001) respectively. ConclusionsOnly half of respondents exclusively breastfed their infants for the recommended age in whichonly factors related to mother were significant with discontinuation of EBF rather than infants’.Findings of the study emphasizes on counselling the mothers regarding the benefits of EBF toboth child and mother during hospital visits. Background: Breastfeeding is beneficial to mothers, infants and society at large. The nutritional and non-nutritional benefits are gained when practiced in line with recommended durations. Objective: The duration of breastfeeding and related factors among mothers of southeastern Nigeria were explored. Methods: This was a hospital based cross-sectional study using an interviewer-administered questionnaire. Data analysis employed SPSS version 20. Results: A total of 1,833 women were surveyed. Most (93.3%) were aged 20-40 years and had at least secondary education (94,6%). More than half (64.3%) were working class. Many had 1-4 children (91.5%) and a family size of ≤6 (74.2%). Up to 83.3% of the mothers breastfed for ≥12 months. Common reasons advanced for cessation of breastfeeding were pregnancy (29.8%), baby refusing to suckle (10.2%) or old enough to stop breastfeeding (18.0%). No reason for cessation was given for 22.4%. Mothers <20 years practiced non-exclusive breastfeeding or early introduction of complementary feeds, bottle-fed and were significantly least likely to breastfeed for up to one year. Conclusion: A suboptimal number of Nigerian mothers breastfed for ≥12 months. Unscientific reasons were proffered for cessation of breastfeeding. Younger mothers practiced non-exclusive breastfeeding, bottle-fed and gave early complementary feeds, negatively affecting duration of breastfeeding. Sustained individualized health education is advocated to improve duration of breastfeeding and avail young children of its wholesome nutritional benefits. Abstract Background Breastfeeding (BF) is one of the most effective ways to ensure child health and survival. WHO recommend the maintaining the exclusive breastfeeding (EB) up to six months. Unfortunately, its prevalence does not exceed 13% in our country at this age. A better understanding of factors associated with short duration of breastfeeding can allow us to identify the strategies that improve it. This study aimed to determine prevalence of EB and predominant breastfeeding (PB) during the first 2 months and identify their associated factors. Methods We conducted an observational prospective study in two centers of maternal and child health of Monastir (Tunisia) during April and May 2019. Data collection was performed after birth using a self-administered questionnaire and at 4th and 8th postpartum week by telephone interviews. We used the BSES-SF scale to identify self-efficacy in BF and the IIFAS-A scale to measure the mother's attitude towards BF. We used Kaplan Meier survival analysis and Cox Proportional Hazard regression to identify independent factors associated with EB and PB. Results During the study, 437 mothers were enrolled. Prevalence at 8th postpartum week was 6.2% with median duration of eight days and 35.6% with median duration of 21 days respectively for EB and PB. Absence of a breastfeeding experience (HR = 1.31 [1.07-1.61], p = 0.008), the first breastfeeding at beyond 12 hours (HR = 1.47 [1.05-2.07], p = 0.023), the intention of mixed breastfeeding (HR = 1.25 [1- 1.55], p = 0.043) were associated with a shorter duration of EB. Absence of breastfeeding experience (HR = 1.47 [1.05-2.07], p = 0.023), the first breastfeeding at beyond 2 hours (HR1 = 1.72 [1.08-2.73]; p = 0.007) and the non-determination of the duration of breastfeeding (HR = 1.54 [1.43-4.51], p = 0.006) were associated with a shorter duration of PB. Conclusions This study allowed us to identify mothers likely to stop BF early. They should be the target of specific promotion interventions. Key messages Breastfeeding is one of the most effective ways to ensure child health and survival. Information about BF, time of first BF, type and duration of planned BF and self-efficacy deserve a particular attention and constitute key axes of effective promotion interventions of EB and PB.

Background: Extended breastfeeding duration is common in India. Extended breastfeeding protects the infant from infectious disease and promotes child spacing. In the 1990s, the median breastfeeding duration in India was 24 months. Research aim: This study aimed to investigate the median duration of breastfeeding in India and to identify the factors associated with extended breastfeeding to 24 months as recommended by the World Health Organization. Methods: This cross-sectional data analysis used nationally representative data from the 2011–2012 Indian Human Development Survey II. The outcome in this study was extended breastfeeding defined as breastfeeding to 24 months or more. Multivariate logistic regression was used to identify the factors associated with extended breastfeeding. Results: The median duration of breastfeeding was 12 months; approximately 25% of women breastfed 24 months or more. Women were at greater odds of breastfeeding 24 months or more if the infant was a boy compared with a girl, if the women lived in a rural area compared with an urban area, if the women were married at a young age (< 17 vs. 20 years or older at marriage), and if the delivery was assisted by a friend or relative compared with a doctor. Conclusion: The median duration of breastfeeding has decreased by 50% from 1992–1993 to 2011–2012. The women who continue to breastfeed 24 months or more tend to be more traditional (i.e., living in rural areas, marrying young, and having family/friends as birth attendants). Further research to study the health effect of decreased breastfeeding duration is warranted.

Abstract Background Initiation of breastfeeding immediately after birth, exclusive breastfeeding, and continuous breastfeeding for at least 2 years lower the risk of newborn deaths. This study was conducted to examine the trends and factors associated with early initiation of breastfeeding, exclusive breastfeeding and duration of breastfeeding in Ethiopia. Methods Data for this study were extracted from the Ethiopian Demographic and Health Survey 2016. A total of 5122 children were included in the analysis. Multivariate logistic regression analysis, and Cox proportional hazards model were fitted to find the factors associated with breastfeeding practices. Reported p - values < 0.05 or a 95% Confidence Interval of Odds Ratio/Hazard Ratio excluding one was considered as significant association with early initiation of breastfeeding, exclusive breastfeeding, duration of breastfeeding and independent variables. Results About 81.8% of the children initiated breastfeeding within 1 h of birth and during the day before an interview, 47% were exclusively breastfed during the first 6 months. The median duration of breastfeeding was 22 months (22 ± 0.50 months 95% Confidence Interval [CI] 21.01–22.99). Rural residents (Odds Ratio [OR] 0.71, 95% CI 0.51, 0.99), mothers with no antenatal follow up (OR 0.75, 95% CI 0.57, 0.99), caesarean birth (OR 0.80, 95% CI 0.66, 0.96) and home delivery were associated with low initiation of breastfeeding within 1 h of birth. Mothers with no/primary education (OR 0.62, 95% CI 0.40, 0.96), no baby postnatal checkup (OR 0.53, 95% CI 0.39, 0.73), average/larger size of a child at birth (OR 0.80, 95% CI 0.65, 0.99) and deliveries outside of health centers were significantly associated with non-exclusive breastfeeding at the time of the interview. Further, mothers living in Amhara (Hazard Ratio [HR] 1.31, 95% CI 1.05, 1.64), Oromia (HR 1.27, 95% CI 1.04, 1.54), and Benishangul-Gumuz (HR 1.34, 95% CI 1.09, 1.65) regions had a longer duration of breastfeeding while Muslims, employed mothers, multiple births and poor economic level of households were associated with shorter durations of breastfeeding. Conclusions Rural residence, female sex, home delivery, caesarean birth, small birthweight baby and large family size were associated with late initiation of breastfeeding. Living in Affar, Somali, and Harari, primary education level of mothers, giving birth outside of health facilities, no antenatal care follow up, and no postnatal check-up were associated with non-exclusive breastfeeding, while younger age mothers, Muslims, giving birth outside of health facilities, and employed mothers were associated with shorter time to cessation of breastfeeding. Providing health education and counseling for mothers during and after pregnancy should be encouraged.

Background Atopic diseases (AD) are the most common chronicdiseases in childhood, and their incidence has a tendency to increaserecently. Tendency to have atopy could be triggered by many factorsoriginated in early life, including time of breastfeeding cessation.Objective To determine the association between exclusive andduration of breastfeeding and atopy in children with or withoutfamily history of atopic disease.Methods This was an observational clinical epidemiology studyperformed at Babakansari, Padasuka, Garuda Primary Health CareCenter in Bandung from January to March 2006. One hundredfifty of 749 children were randomized from group with and withoutfamily history of AD. They underwent skin prick tests and totalserum IgE level analysis. Atopy was defined as a positive skinprick test to any of the eight allergens tested. History of exclusiveand duration of breastfeeding was obtained from their parents.Significance tests for contingency tables were on the basis of x 2test for association odds ratio with 95% confidence interval.Results Atopy was found in 28.2% of children, of whom 32.4% withand 23.9% without family history of AD. Children exclusivelybreastfed exhibited a reduced risk of atopy (5.8% v 35.3%, OR=0.11,95%CI= 0.03;0.34, P<0.001). The difference of atopy was stronglysignificant between children who had exclusive breastfeeding andthose without exclusive breastfeeding whether or not the subjectshad family history of AD (P<0.001). There was a highly significantrisk reduction for atopy related to prolonged breastfeeding (=6months) (OR=0.37, 95%CI = 0.19 to 0.72, P=0.001). Thedifference of atopy was strongly significant between children whohad prolonged breastfeeding and short breastfeeding duration whetheror not the subjects had family history of AD (P<0.001)Conclusions Exclusive and prolonged breastfeeding decrease atopyin children with as well as without family history of AD.

CONTEXT Sudden infant death syndrome (SIDS) is a leading cause of postneonatal infant mortality. Our previous meta-analyses showed that any breastfeeding is protective against SIDS with exclusive breastfeeding conferring a stronger effect. The duration of breastfeeding required to confer a protective effect is unknown. OBJECTIVE To assess the associations between breastfeeding duration and SIDS. DATA SOURCES Individual-level data from 8 case-control studies. STUDY SELECTION Case-control SIDS studies with breastfeeding data. DATA EXTRACTION Breastfeeding variables, demographic factors, and other potential confounders were identified. Individual-study and pooled analyses were performed. RESULTS A total of 2267 SIDS cases and 6837 control infants were included. In multivariable pooled analysis, breastfeeding for &lt;2 months was not protective (adjusted odds ratio [aOR]: 0.91, 95% confidence interval [CI]: 0.68–1.22). Any breastfeeding ≥2 months was protective, with greater protection seen with increased duration (2–4 months: aOR: 0.60, 95% CI: 0.44– 0.82; 4–6 months: aOR: 0.40, 95% CI: 0.26–0.63; and &gt;6 months: aOR: 0.36, 95% CI: 0.22– 0.61). Although exclusive breastfeeding for &lt;2 months was not protective (aOR: 0.82, 95% CI: 0.59–1.14), longer periods were protective (2–4 months: aOR: 0.61, 95% CI: 0.42–0.87; 4–6 months: aOR: 0.46, 95% CI: 0.29–0.74). LIMITATIONS The variables collected in each study varied slightly, limiting our ability to include all studies in the analysis and control for all confounders. CONCLUSIONS Breastfeeding duration of at least 2 months was associated with half the risk of SIDS. Breastfeeding does not need to be exclusive to confer this protection.
