Intracranial teratomas are tumors that occur mainly in childhood and extremely rare in adulthood. They account up to 50% congenital CNS tumors. In this article a case of mature teratoma in 33 year-old female patient with progressive visual impairment is presented. Differential diagnosis at preoperative stage was difficult. Outcome analysis of neuroimaging study method and clinical picture provided evidence of epidermal cyst. Transnasal endoscopic approach as a treatment method was chosen, but during the surgery the atypical tissue for epidermal cyst was identified with tight adhesion to the right internal carotid artery which limited the extent of surgical tretment. Pathohistological and immune histochemical study detected mature teratoma. Detailed visual impairment dynamic and instrumental methods of diagnosis during postoperative supervision are presented in the article. The choice of management, namely, surgical intervention using extended endoscopic transnasal approach is considered to be controversial and risky among different authors taking into account intraoperative characteristics of this tumor.

Purpose: This report describes a case of bilateral primary angle closure (PAC) progressing to unilateral end-stage primary angle closure glaucoma (PACG) associated with treatment for coronavirus disease-19 (COVID-19) infection. Methods: A 64-year-old man came to our attention because of blurred vision after a 2-month hospital stay for treatment of COVID-19 infection. Examination findings revealed PACG, with severe visual impairment in the right eye and PAC in the left eye due to plateau iris syndrome. The patient’s severe clinical condition and prolonged systemic therapy masked the symptoms and delayed the diagnosis. Medical chart review disclosed the multifactorial causes of the visual impairment. Ultrasound biomicroscopy (UBM) aided in diagnosis and subsequent therapy. Results: The cause behind the primary angle closure and the iridotrabecular contact was eliminated by bilateral cataract extraction, goniosynechialysis, and myotic therapy. Conclusions: COVID-19 treatment may pose an increased risk for PAC. Accurate recording of patient and family ophthalmic history is essential to prevent its onset. Recognition of early signs of PAC is key to averting its progression to PACG.

<b><i>Introduction:</i></b> Nasal chondromesenchymal tumors (NCMT) are rare benign neoplasms that usually present in children &#x3c;1 year of age. They can display rapid growth and significant local bony remodeling that can mimic a malignant process. Of the ∼50 published cases to date, few have documented the need for neurosurgical intervention. We herein report a NCMT in an infant treated with a staged cranial and transnasal approach, as well as summarize the available literature on this pathology. <b><i>Case Report:</i></b> A newborn male with a compromised airway was noted to have a large sinonasal lesion. After stabilization, MRI demonstrated a 4-cm enhancing mass with diffuse sinus involvement and significant extension into the anterior cranial fossa, with displacement of the optic apparatus and hypothalamic pituitary axis. After an initial biopsy, the patient underwent a bifrontal craniofacial approach at 2 months of age, followed by a second-stage transnasal endoscopic approach at 15 months which resulted in a complete resection. There were no neurosurgical complications. Pathology was consistent with a NCMT. <b><i>Discussion:</i></b> Although rare, neurosurgical involvement is critical for the treatment of NCMTs with intracranial extension. Staged cranial and endonasal endoscopic approaches may be needed for complete resection of such lesions.

SummaryCor triatriatum sinister is a rare congenital cardiac anomaly that has been identified in 0.1% of children with congenital heart disease. It is defined as a fibromuscular membrane that divides the left atrium into two chambers: a superior (proximal) that in most cases receives drainage from the pulmonary veins and an inferior (distal) chamber that communicates with the mitral valve and the left atrium. Cor triatriatum sinister can be an isolated lesion (approximately 25% of cases), but in many cases it is associated with other congenital cardiovascular anomalies, the most common one being – atrial septal defect(3). Symptoms in patients with cor triatriatum sinister are related to obstruction of pulmonary venous drainage, pressure loading of the right side of the heart and congestive cardiac failure. Depending on the severity of the obstruction and presence of associated cardiac anomalies it can be diagnosed at any age. Diagnosis is usually achieved by echocardiography in early infancy. Elective treatment method is surgical excision of the membrane. Here we present a pediatric patient (4 months old) presenting in cardiogenic shock with a successful correction of isolated cor triatriatum sinister. To confirm diagnosis and success of surgical repair, transthoracic and transesophageal echocardiography were used.

An intramuscular epidermal cyst without subcutaneous involvement is extremely rare. The patient presented with low back pain. Spinal magnetic resonance imaging indicated spindle-shaped mass in the right erector spinae muscles located at L3 to L5 level without any connection with subcutaneous tissue. The patient underwent operation for removal of intramuscular cystic lesion. The tumor was totally removed and pathologically confirmed with epidermal cyst. Unlike typical epidermal cyst, the patient had a characteristic clinical manifestation of mild low back pain without palpable mass and local tenderness.

Domestic wastewater is one of the most interested environmental issues in Vietnam, especially in big cities and suburban residential areas. Most of the wastewater was not treated in the right way before discharging to environment. In this research, the author used water hyacinth as a main aquatic plant in aquatic pond model to treat wastewater. The experiment was operated continuously in 1 year with hydraulic retention time (HRT) is 11 and 18 days. Hydraulic loading rate (HRL) is 300 and 500 m3/ha/day, respectively. Treatment efficiency of the model for TSS is 90% (remaining 6 -12mg/l); COD, BOD5 is 63 - 81% (remaining 10 - 48mg/l); TP is 48 – 50% (remaining 3.5 - 9.8mg/l); TKN is 63 - 75% (remaining 8 - 17mg/l). Polluted parameters in effluent were lower than A and B levels of Vietnam standard QCVN 14: 2008/BTNMT and QCVN 40: 2011/BTNMT. The doubling time of water hyacinth in summer is 18days, and in autumn and winter is 28.5 days. Experiment results showed that we can use water hyacinth in aquatic pond to treat domestic wastewater with medium scale. We can apply this natural treatment method for residential areas by utilizing existing natural ponds and abandoned agricultural land with capacity up to 500m3/ha day. However, to get better efficiency we should combine with other aquatic plant species to treat wastewater and improve environmental landscape. Nước thải – xử lý nước thải sinh hoạt hiện đang là một trong những vấn đề môi trường được quan tâm tại Việt Nam nhất là tại các thành phố lớn và các khu dân cư. Hầu hết lượng nước thải chưa được xử lý đúng cách trước khi thải ra môi trường. Trong nghiên cứu này, tác giả sử dụng mô hình hồ thủy sinh và sử dụng cây Bèo lục bình để xử lý nước thải sinh hoạt của một khu dân thuộc huyện Từ Liêm, tại khu vực này nước thải bị thải trực tiếp vào sông Nhuệ. Thực nghiệm đã được tiến hành trong khoảng thời gian một năm, trải qua các mùa của khu vực miền Bắc với hai chế độ vận hành HRT là 18 và 11 ngày, tương ứng với tải trọng thủy lực HRL là 300 và 500 m3/ha/ngày. Kết quả cho thấy mô hình thủy sinh sử dụng cây bèo lục bình cho kết quả tốt, hiệu suất xử lý với các chất ô nhiễm đạt được như sau: chất rắn lơ lửng đạt 90%, COD, BOD5 đạt 63 - 81%, Phốt pho tổng giảm tới 48 - 50%, Nitơ tổng giảm tới 63 - 75%. Hàm lượng các chất ô nhiễm trong nước thải đầu ra của mô hình đều thấp hơn ngưỡng A và B của các tiêu chuẩn QCVN 14: 2008/BTNMT và QCVN 40: 2011/BTNMT. Bên cạnh đó tác giả cũng đã xác định định được tốc độ sinh trưởng của cây bèo tại khu vực miền Bắc là 18 ngày vào mùa hè và 28.5 ngày vào mùa thu đông. Kết quả nghiên cứu cho thấy có thể sử dụng bèo lục bình để xử lý nước thải sinh hoạt, với qui mô vừa và nhỏ và nên áp dụng cho các khu vực ven đô, nông thôn nơi có diện tích đất rộng hoặc tại các hồ sinh thái của các khu đô thị. Tuy nhiên, để hiệu quả tốt hơn ta cần kết hợp với nhiều loại thực vật thủy sinh khác để ngoài tác dụng xử lý nước thải mà còn tạo cảnh quan môi trường xung quanh.

We report a case of a lipoma inside the eustachian tube, an extremely rare location for this lesion. To the best of our knowledge, this is only the second such case that has been described in the literature. The patient was a 47-year-old man, a fighter pilot, who was referred to our hospital with a 3-year history of (1) fullness in the right ear secondary to recurrent serous otitis media and (2) right ear pain, which was especially acute during fights. Nasopharyngeal endoscopy, computed tomography, and magnetic resonance imaging detected the presence of a well-encapsulated lesion inside the eustachian tube; macroscopic and radiologic findings identified the mass as a lipoma. The lesion was completely removed via transnasal endoscopy. Histopathologic evaluation confirmed the diagnosis of lipoma. The patient's postoperative course was favorable, and he was able to fly again without any ear complaints. Radiologic examination is useful for the diagnosis and preoperative evaluation of this benign tumor. Lesions located in the lower part of the eustachian tube can be easily removed via a transnasal endoscopic approach.

Posterior reversible encephalopathy syndrome (PRES) is a temporary condition that is diagnosed clinically, neurologically, and radiologically. Its symptoms vary, and nonspecific headaches, confusion, impairment of consciousness, nausea, vomiting, and visual impairment may occur. Acute hypertension often accompanies these symptoms. Patients can also suffer from convulsions, cortical visual impairment, and coma. Diagnosis can be difficult due to focal neurologic signs. Nevertheless, knowing the clinical risk factors can lead to the right diagnosis. It has been reported that this condition may also occur during organ transplantation, immunosuppressive treatment, and autoimmune diseases and chemotherapy, and also patients with eclampsia. In this paper, a 21-year-old, 31-week pregnant woman, who had been diagnosed with PRES and thanks to early diagnosis and treatment had fully recovered and discharged from the intensive care unit, is presented, and the relevant literature is discussed.

Dislocation of the hip usually results from a high-energy injury sustained during a road accident. Inveterate dislocations persisting for many months or years are extremely rare. Selection of an appropriate treatment method is not easy and is always associated with the risk of serious complications. The present authors hope that a description of the course of diagnostic work-up and treatment of a patient in whom a hip dislocation persisted for 42 years will prove interesting and helpful for orthopaedists who may encounter such a case in their practice. The patient sustained a dislocation of the right hip in 1974. He did not agree to undergo reduction immediately after the injury. Initially, he experienced extremely severe pain and difficulty walking, but gradually learned to walk without crutches and even took up a job. The limb was considerably shortened with only minimal movement in the hip joint. The pelvic geometry was altered and spinal scoliosis developed. In the last several years, the patient experienced a significant increase in pain and a decrease in function that prevented him from walking unassisted. Following a thorough physical examination, and based on computed tomography images, the patient was qualified for hip arthroplasty. An analysis of the available literature prompted the present authors to use a cemented implant and not to use bone grafts. Early treatment outcomes are good.

Following surgery in the USA in 1992 to remove a large right cerebello-pontine angle tumour, a 39-year-old woman developed severe brainstem and cerebellar infarction. This left her with severe visual impairment and ataxia. She became able to communicate by means of an adapted finger-spelling alphabet. She had total hearing loss in the right ear and a mild to moderately severe sensorineural hearing loss in the left ear, and severe tinnitus heard throughout the head. Additionally, she experienced hypersensitivity to sound above normal conversational levels, which evoked a synaesthetic feeling of coldness across her upper torso. Previous linear analogue hearing aid fitting had not been beneficial for either hearing or tinnitus. Careful fitting of a digital hearing aid, together with tinnitus counselling, inhibited the patient's tinnitus to 25 per cent of its former intensity after a six month acclimatisation period, and improved communication.
