Coherence parameters of cerebral bioelectric activity and blood serum levels of phosphorylated neurofilaments in comorbid alcohol dependence and affective disorders

Введение. Наряду со многими психическими расстройствами алкогольная зависимость и аффективные расстройства являются результатом взаимодействия генетических, социальных и экологических факторов, что сопровождается морфофункциональными изменениями в центральной нервной системе. Тем не менее, основные причины и механизмы развития коморбидности алкоголизма и аффективных расстройств остаются не до конца ясны. Цель исследования - определение функциональной связности и уровня фосфорилированных нейрофиламентов у пациентов с алкогольной зависимостью и коморбидностью алкогольной зависимости и аффективного расстройства. Методика. Обследовано 60 пациентов после детоксикации: 30 пациентов с алкогольной зависимостью и 30 пациентов с коморбидным течением алкогольной зависимости и аффективного расстройства. Контрольную группу составили 20 психически и соматически здоровых лиц, сопоставимых по полу и возрасту. Исследование биоэлектрической активности головного мозга проводилось при помощи 16-канального энцефалографа. Анализировались общие усредненные значения внутри - и межполушарной когерентности. В сыворотках крови определяли содержание фосфорилированных нейрофиламентов методом твердофазного иммуноферментного анализа на полистироловых планшетах, предварительно покрытых куриными пoликлональными антителами. Результаты. При межгрупповом анализе были выявлены статистически значимо более низкие значения когерентности в правой гемисфере у пациентов с коморбидностью алкогольной зависимости и аффективного расстройства по сравнению с пациентами, страдающими только алкогольной зависимостью. Были обнаружены также статистически значимо более высокие значения концентрации нейрофиламентов в группе пациентов с коморбидностью алкогольной зависимости и аффективного расстройства по сравнению со здоровой группой контроля. При сравнении групп пациентов между собой были обнаружены боле высокие значения концентрации нейрофиламентов у пациентов с коморбидностью алкогольной зависимости и аффективного расстройства на уровне тенденции. Заключение. Наличие коморбидности алкоголизма и аффективных расстройств приводят к нейрофизиологическим изменениям в виде снижения функциональной связности коры головного мозга, особенно в правой гемисфере, а также увеличению степени нейронального повреждения. Background. Similar to many mental disorders, alcohol dependence and affective disorders result from interaction of genetic, social, and environmental factors associated with morpho-functional alterations in the central nervous system. However, major causes and mechanisms of the development of comorbid alcoholism and affective disorders are not fully clear. The aim of this study was to determine the functional connectivity and levels of phosphorylated neurofilaments in patients with alcohol dependence and comorbid alcohol dependence and affective disorder. Methods. 60 patients were evaluated after detoxification, including 30 patients with alcohol dependence and 30 patients with comorbid alcohol dependence and affective disorder. The control group consisted of 20 sex- and age-matched, mentally and somatically healthy individuals. Brain bioelectric activity was recorded with a 16-channel encephalograph. Overall average values of intra- and inter-hemispheric coherence were analyzed. Blood serum concentration of phosphorylated neurofilaments was measured by solid-phase enzyme immunoassay on polystyrene plates pre-coated with chicken polyclonal antibodies. Results. The intergroup analysis showed that coherence values for the right hemisphere were significantly lower in patients with comorbid alcohol dependence and affective disorder compared to patients with alcohol dependence alone (p=0.018). Also, concentrations of neurofilaments were significantly higher in the patient group with comorbid alcohol dependence and affective disorder compared to the healthy control group (p=0.042). Comparison of patient groups showed that neurofilament concentrations had a tendency toward higher values in patients with comorbid alcohol dependence and affective disorder (p=0.092). Conclusion. The presence of comorbid alcoholism and affective disorders leads to neurophysiological alterations evident as reduced functional connectivity of the cerebral cortex, particularly in the right hemisphere, as well as to the increased degree of neuronal damage.

Objective: The striatum is unevenly impaired bilaterally in Parkinson's disease (PD). Because the striatum plays a key role in cortico-striatal circuits, we assume that lateralization affects cortico-striatal functional connectivity in PD. The present study sought to evaluate the effect of lateralization on various cortico-striatal circuits through resting-state functional magnetic resonance imaging (fMRI).Methods: Thirty left-onset Parkinson's disease (LPD) patients, 27 right-onset Parkinson's disease (RPD) patients, and 32 normal controls with satisfactory data were recruited. Their demographic, clinical, and neuropsychological information was collected. Resting-state fMRI was performed, and functional connectivity changes of seven subdivisions of the striatum were explored in the two PD groups. In addition, the associations between altered functional connectivity and various clinical and neuropsychological characteristics were analyzed by Pearson's or Spearman's correlation.Results: Directly comparing the LPD and RPD patients demonstrated that the LPD patients had lower FC between the left dorsal rostral putamen and the left orbitofrontal cortex than the RPD patients. In addition, the LPD patients showed aberrant functional connectivity involving several striatal subdivisions in the right hemisphere. The right dorsal caudate, ventral rostral putamen, and superior ventral striatum had decreased functional connectivity with the cerebellum and parietal and occipital lobes relative to the normal control group. The comparison between RPD patients and the controls did not obtain significant difference in functional connectivity. The functional connectivity between the left dorsal rostral putamen and the left orbitofrontal cortex was associated with contralateral motor symptom severity in PD patients.Conclusions: Our findings provide new insights into the distinct characteristics of cortico-striatal circuits in LPD and RPD patients. Lateralization of motor symptoms is associated with lateralized striatal functional connectivity.

In this study, we sought to assess the level of cognitive functioning in patients with comorbid alcohol dependence and affective disorder, as well as to compare the detected changes with the indicators of cognitive tests in patients suffering only from alcoholism or affective disorder. It is suggested that patients with comorbidity may have a more severe cognitive deficit than patients with a single diagnosis.Materials and methods. We examined 100 patients aged 30–50 years before treatment: 30 patients with affective disorders, 40 patients with alcohol dependence and 30 patients with comorbid alcohol dependence and affective disorder. As a control group, 30 mentally and somatically healthy individuals were examined. The level of cognitive functioning was assessed using computer tests Go/No-go, Corsi and Stroop.Results. Statistically significantly lower indicators of cognitive functioning were found in patients with comorbid alcohol dependence and affective disorder in comparison with all the studied groups.Conclusion. The data obtained in the study indicate that the presence of comorbidity of alcohol dependence and affective disorder in patients leads to a significant deterioration in cognitive functions: Executive control, working memory, attention and cognitive flexibility compared to healthy individuals, as well as patients suffering only from alcohol dependence or affective disorder.

Fluency tests are frequently used in clinical practice to asses executive functions. The literature data are not unequivocal although in a great number of papers is pointed out the importance of the left hemisphere, specially of the left frontal lobes in the mediation of phonological fluency and the right hemisphere in the mediation of nonverbal fluency. This paper considers the suitability of fluency tests for the detection of left versus right seizure laterality. The sample consisted of thirty-two epilepsy patients divided into two groups: LHF-participants with the seizure focus in the left hemisphere (n=16), and DHF-participants with the seizure focus in the right hemisphere (n=16), and K-the control group of t age-matched healthy children (n=50) aged 7-11 years. The qualitative and quantitative comparison of the phonological and nonverbal fluency performance was carried out in consideration of the seizure laterality as well as compared to the healthy controls. The results of phonological fluency performance revealed that the performance of the LHF group was significantly reduced as compared to both DHF and K group. The analysis of nonverbal fluency performance revealed that the performance of the DHF group was significantly reduced as compared to both LHF and K group The qualitative analysis obtained valuable data, which could additionally contribute to the neuropsychological evaluation of the left versus right seizure laterality.

The visual span of apprehension for random letter and digit sequences, approximations to English sequences and non-symbolic line stimuli was measured in patients with unilateral cerebral lesions. The left hemisphere group was significantly impaired relative to the right hemisphere group and a control group on all three types of visual span task. The deficit was most marked in patients with left posterior lesions. The visual span deficits were not related to other language deficits. The findings are discussed in terms of a modality-specific defect of visual short-term memory.

Metaphors are frequently used in our daily life communications to comprehend and convey abstract concepts. Different neuroimaging studies also show a wide neural connection to metaphor perception. This study used a brain mapping setup (quantitative electroencephalography) to examine large-scale brain networks in participants when they were submitted to non-metaphorical sentences as compared to creative and conventional metaphorical phrases. To this end, 20 healthy right-handed individuals with Persian as their mother-tongue language consented to participate in the above language task. Having evaluated the functional connectivity across key cortical hubs, no significant or predominant role of the right hemisphere was observed upon comprehension of the conventional and creative metaphorical phrases. Rather, a wide interhemispheric functional connectivity is proposed to play a key part in the above task. Further understanding about the neural networks involved in the semantic processing of metaphors may potentially open new avenues to neural modulation in patients who find it challenging to comprehend figurative language or abstract metaphorical meanings (e.g. schizophrenics).

AbstractHemispherotomy is a treatment for drug-resistant epilepsy with the whole hemisphere involved in seizure onset. As recovery mechanisms are still debated, we characterize functional reorganization with multimodal MRI in two children operated on the right hemisphere (RH). We found that interhemispheric functional connectivity was abolished in both patients. The healthy left hemispheres (LH) displayed focal hyperperfusion in motor and limbic areas, and preserved network-level organization. The disconnected RHs were hypoperfused despite sustained network-level organization. Functional connectivity was increased in the left thalamo-cortical loop and between the cerebelli. The classification probability of the RH corresponding to a minimally conscious state was smaller than for the LH. We conclude that after hemispherotomy, neurological rehabilitation is sustained by cortical disinhibition and reinforcement of connectivity driven by subcortical structures in the remaining hemisphere. Our results highlight the effect of vascularization on functional connectivity and raise inquiries about the conscious state of the isolated hemisphere.

Introduction. Olfactory dysfunction is considered to be an early and relatively important marker of Parkinson's disease (PD). Olfactory studies using objective neurophysiological methods may become one of the diagnostic tests to identify individuals with a high risk of developing PD. The aim of the study was to assess the spectral and topographic characteristics of bioelectrical brain activity in patients with PD during directional perception of olfactory stimuli. Materials and methods. This study included 30 patients with PD (mean age was 66.5 6.5 years). The control group consisted of 20 people without PD (mean age was 65.3 8.5 years). Lavender oil, clove oil, camphor oil and -mercaptoethanol solution (an aversive stimulus) were used for olfactory stimulation, while distilled water was used as a control test. The test subject sat with their eyes closed and inhaled the presented smell for 30 seconds, while an EEG recording was made during this time. Study results. Olfactory stimulation in patients with PD showed increased synchronicity of the 3 rhythm in the right hemisphere, as well as the rhythm in the parieto-occipital regions of both hemispheres. These changes indicate significant activation of internal (endogenous) attention, increased overall, non-specific readiness potential, as well as the involvement of the limbic-reticular complex in olfactory perception. Olfactory perception in the control group was accompanied by reduction in the 1 rhythm amplitude in the parieto-occipital regions bilaterally, which may indicate moderate activation of external (exogenous) attention and the posterior attention system responsible for simple perception processes. Conclusion. Increased 3 rhythm amplitude in the right hemisphere and increased rhythm amplitude, observed during directional olfactory perception, may indicate olfactory dysfunction and should be viewed as an additional indicator when establishing a diagnosis of Parkinsons disease.

Background/Objective: Prenatal opioid exposure (POE) is a growing public health issue that can result in premature birth, Neonatal Abstinence Syndrome (NAS), and adverse neurodevelopmental outcomes. However, the neural basis for these findings remains relatively unknown. In this study, we aimed to investigate the neural correlates of POE based on neonatal thalamocortical functional connectivity using resting state functional magnetic resonance imaging (rs-fMRI).     Methods: In this prospective, IRB-approved study, nineteen neonates with POE and twenty opioid naive (ON) controls underwent non-invasive MRI during natural sleep at mean post-menstrual age (PMA) of 44.7 ± 2.6 and 44.6 ± 2.6 weeks respectively. MR imaging included anatomic T2-weighted images and rs-fMRI. General Linear Model (GLM) seed-based whole brain functional connectivity analysis was performed for each subject, with the right and left thalamus as distinct seed regions. Unpaired mixed-effects group analyses between POE and ON groups were conducted for each seed region corrected for PMA and sex.    Results: Thalamic connectivity to cortical and subcortical structures differed in the POE group compared to the ON control group. The POE group exhibited higher functional connectivity to deep gray structures, frontal, medial prefrontal, parietal, occipital, and anterior temporal cortices compared to controls. The POE group exhibited lower connectivity to the nuclei accumbentes, bilateral caudate nuclei, posterior cingulate gyri, superior frontal gyri, insular, and dorsolateral prefrontal cortices.     Conclusion and Potential Impact: Overall, these novel results suggest the presence of opioid exposure-related alterations in thalamic functional connectivity. Given that the thalamus plays a crucial role in early brain development, the described alterations in thalamocortical and thalamic-subcortical connectivity may have implications in stratifying risk and informing treatment for the adverse neurodevelopmental outcomes associated with POE. Future studies should explore the relationship between POE-associated disruptions in thalamic connectivity and developmental outcomes.

Objectives. Suicide is the second leading cause of adolescent deaths and may be linked to difficulties with inhibitory and emotional processing. This study assessed the neural correlates of cognitive inhibition during emotional processing in adolescents hospitalized for a suicidal crisis. Methods. Event-related potentials were recorded during an emotional Go/NoGo task in 12 adolescents who attempted suicide and 12 age- and sex-matched healthy controls. Results. Compared to the control group, the suicidal group showed significantly reduced positivity at the time of the P3d (difference waveform reflecting NoGo minus Go trials) in response to happy and neutral, but not sad stimuli. For happy stimuli, this group difference was restricted to the right hemisphere. Further analyses indicated that the suicidal group had a reversed pattern of P3 amplitude in response to inhibition, with lower amplitudes in the NoGo compared to the Go conditions. Suicidal symptoms severity strongly correlated with lower amplitude of the P3d in response to neutral faces. Conclusions. These findings provide more insight into inhibition difficulties in adolescents with acute suicidal risk. Interactions between emotional and inhibition processing should be considered when treating acutely suicidal youths.
