scholarly journals Effect of unicornuate uterus on women's reproductive health

Objectives. The effect of the unicornuate uterus on the reproductive health and quality of life of woman has been analyzed. Materials and methods. The study included 26 patients with unicornuate uterus aged 25 to 42 years and 20 women without gynecological pathology. Diagnosis of uterine abnomalies was based on patient complaints, medical history and life history, as well as clinical and laboratory and instrumental examinations. The severity of dysmenorrhea was determined using a visual analogue pain scale, women's quality of life was assessed using a short SF-12 health questionnaire. Results. The average age of the surveyed women was 30.2 ± 3.9 years. All patients with unicornuete uterus by subclasses were distributed as follows: subclass U4a (unicornuate uterus with rudimentary cavity) – 69.2% of patients, subclass U4b (unicornuate uterus without rudimentary cavity in the contralateral horn or with its aplasia) – 30.8%. In all patients was confirmed normal karyotype – 46XX. The combination of a unicornuate uterus with a defect of the urinary system (aplasia, doubling, or kidney dystopia) occurred in 23.1% of women. It has been found that in women with unicornuate uterus, clinical symptoms in 76.9% of cases are accompanied by dysmenorrhea. The unicornuate uterus in 69.2% of cases causes negative reproductive outcomes, among which 26.9% – early spontaneous abortions, 11.5% – ectopic pregnancy. In 46.2% of cases the unicornuate uterus is combined with other gynecological pathology, whereby the incidence of concomitant gynecologic pathology is significantly more frequent in U4a patients than in the U4b subclass (˂0.01). The severity of dysmenorrhea in the main group was 5.8 ± 1.8 points (the difference was significant compared with the control group, p = 0.0032). Conclusions. Due to the low physical subunit, the unicornuate uterus significantly impairs the quality of life of women, which was significantly lower in the group of patients with unicornuate uterus compared to the control group (total SF-12 score: 85.4 ± 11.2 vs. 95.8 ± 12.1; p = 0.002).

OBJECTIVE: To determine the efficacy of the Synaptic 2000 in reducing total knee replacement postoperative pain in a prospective randomized controlled trial.DESIGN: Patients were prospectively randomly assigned to two groups, one receiving seven days of transcutaneous electrical stimulation (TES) therapy in addition to standard postoperative care, and the other receiving standard postoperative care. Both groups received postoperative patient-controlled analgesia and were switched to oral opiates.SETTING: Patients were treated at a university teaching hospital for elective primary unilateral total knee replacement.PATIENTS: All patients consented to enter the study. All had primary osteoarthritis. Fifteen patients were entered into each arm of the study, and all patients completed the study. The mean age of patients was 64 years for treatment and 71 years for controls.OUTCOME MEASURES: Outcome measures were analgesic consumption, return of knee range of motion and score on a visual analogue pain scale.RESULTS: No significant differences were noted in postoperative analgesic use, knee range of motion, length of stay or visual analogue pain scale score. Mean total analgesic use of morphine was 148 mg in the TES group and 108 mg in the control group. The TES group mean knee range of motion on postoperative day 7 was 75.3°; it was 73.6° in the control group. Group mean length of stay was 9.5 days in the TES group and 8.3 days in the control group.

Background Patients with heart failure have low quality of life because of physical impairments and advanced clinical symptoms. One of the main goals of caring for patients with heart failure is to improve their quality of life. Objective The aim of this study was to investigate the effect of the use of a smartphone-based app on the quality of life of patients with heart failure. Methods This randomized controlled clinical trial with a control group was conducted from June to October 2018 in an urban hospital. In this study, 120 patients with heart failure hospitalized in cardiac care units were randomly allocated to control and intervention groups. Besides routine care, patients in the intervention group received a smartphone-based app and used it every day for 3 months. Both the groups completed the Minnesota Living with Heart Failure Questionnaire before entering the study and at 3 months after entering the study. Data were analyzed using the SPSS software V.16. Results The groups showed statistically significant differences in the mean scores of quality of life and its dimensions after the intervention, thereby indicating a better quality of life in the intervention group (P<.001). The effect size of the intervention on the quality of life was 1.85 (95% CI 1.41-2.3). Moreover, the groups showed statistically significant differences in the changes in the quality of life scores and its dimensions (P<.001). Conclusions Use of a smartphone-based app can improve the quality of life in patients with heart failure. The results of our study recommend that digital apps be used for improving the management of patients with heart failure. Objective: To explore the effects of health education based on mobile platform on cognitive function and quality of life of children with pneumonia. Methods: Eighty parents with pneumonia admitted to our hospital from March 2018 to March 2020 were selected as the research subjects. The parents of the control group received routine nursing intervention, and the parents of the observation group received health education intervention based on the mobile platform. After nursing, the nursing effect of the two groups was compared. The cognitive function and quality of life of the two groups were compared after nursing. The time of cough, dyspnea, shortness of breath and lung rales were compared between the two groups. The incidence of rash, abdominal pain, dizziness and headache and parents' satisfaction with nursing were compared between the two groups. Results: Compared with the control group, the nursing effective rate of the observation group was significantly increased (P<0.05). The scores of cognitive function and quality of life in observation group were significantly higher than those in control group (P<0.05). Compared with the control group, the disappearance time of clinical symptoms in the observation group was significantly decreased (P<0.05). The satisfaction of parents in the observation group was significantly higher than that in the control group (P<0.05). Conclusion: Health education based on mobile platform can effectively improve the cognitive function and quality of life of children with pneumonia, contribute to the early recovery of children, and has high clinical application value.

SummaryThe objective of the study was to evaluate the presence of psychopathology in fibromyalgia patients compared to a control group of rheumatic patients with pain. Forty-nine fibromyalgia patients and 33 controls were evaluated using established self-rating scales. Serotonin was determined using a radioenzymatic method. Fibromyalgia patients scored significantly higher than the controls on the majority of the subscales on the SCL-90, the two subscales on the Spielberger State and Trait Anxiety Inventory, the GHQ 12 scale and on a visual analogue pain scale. In a multivariate analysis the somatization dimension followed by the state anxiety accounted for the highest difference. Platelet-poor plasma serotonin, platelet serotonin and platelet uptake index were not significantly different between the groups. In conclusion, fibromyalgia patients presented quantitatively more psychopathology than the control group on a wide range of psychopathologic axes, and this cannot solely be accounted for by their higher level of pain.

76 elderly patients with cardiac heart disease (CHD) (31 male and 45 female) were included in the study. Mean age was 81.6 ± 3.4 (from 76 to 89) years. All investigated patients had stable angina pectoris (I-III class). The duration of disease was 8-15 years. The patients were hypertensive (mean arterial pressure 164.5 ± 5.8 / 100 ± 4.3 mm Hg) and had clinical symptoms of heart failure (II class by NYHA). The mental status was evaluated by variety specific tests. The quality of life was assessed by using questionnaire scale SF-36 Health Stains Survey. All subjects were treated by nitrates. ACE-inhibitors and diuretics. Patienis were divided into 2 groups, clinical data and demographies signs of witch, were comparable. 29 patients (1 group) received Metocard 25-100 mg per day. 27 patients were included in control group. Patients of 1 group demonstrated more pronounced increase of distance in test with 6-minutes walking. The depression level after treatment period was significantly reduced in patients of 1 group compare with control group and it's precede level (Camilton scale). Evaluation of quality of life following treatment period shewed statistically significant improvement of number indexes of scale SF-36 Health Status Survey.

Objective: To explore the clinical effect of applying Buyang Huanwu Decoction in the treatment of sequelae of stroke. Methods: The research work was carried out in our hospital, which was from November 2018 to November 2019. A total of 200 patients were selected for treatment during this period, all of whom were patients with sequelae of stroke, and 100 patients were grouped randomly. One group was given conventional acupuncture treatment as the control group, and the other group was given acupuncture combined with Buyang Huanwu Decoction treatment as the test group. The clinical effects of the two groups of patients were evaluated. Results: After treatment intervention, the treatment rates of the test group and the control group were 94.00% and 78.00%, respectively. The Fahl-Meyer score and Barthel index score of the test group were significantly higher than those of the control group, and the quality of life score was higher. There are significant data differences, which was statistically meaningful (P<0.05), and the test group has a better effect. Conclusion: The clinical effect of applying Buyang Huanwu Decoction in stroke patients is significant, which can improve the clinical symptoms and quality of life of patients, which has positive meaning for clinical development.

Diverticulosis, its associated symptoms and complications are one of the most common pathologies of the gastrointestinal tract in more economically developed countries. Presence of diverticuli and their clinical consequences can be divided into four categories: 1) diverticulosis, i.e. an asymptomatic presence of diverticuli that are usually found by accident 2) symptomatic uncomplicated diverticulosis 3) diverticulitis (acute uncomplicated diverticulitis) 4) complications of diverticulitis (conditions requiring hospital stay). The aim of this study was to retrospectively analyze the efficacy of rifaximin in preventing diverticulitis in patients visiting proctology clinics. The diagnostic criterium for diverticulosis was confirmation by colonoscopy, barium enema or CT colography (virtual colonoscopy) as well as history of at least one documented episode of diverticulosis. History of diverticulosis was evaluated based on medical records, clinical symptoms, elevated level of CRP (>5.0) and/or diagnostic imaging (ultrasound, CT). After setting strict exclusion criteria, 248 patients were qualified for the study out of 686, and they were later divided into two groups: control group (group I – 145 patients) and studied group (group II – 103 patients receiving rifaximin prophylaxis). Diverticulitis rate was comparable in both groups over a period of 6 months before study (p = 0.1306) and 6 months of treatment (p=0.3044). Between the 6th and 12th month of treatment, a significantly lower rate of diverticulitis was noted in the group receiving rifaximin compared to control group (p<0.0001). Patients receiving rifaximin reported higher quality of life (which was assessed using the VAS scale) compared to control group after 12 months. The results confirmed the efficacy of riaximin in prevention of diverticulitis, even in the scheme of repeated courses every 3 months. Not only did application of rifaximin lower the rate of diverticulitis and its complications in patients after an episode of diverticulitis, but also it improved the patients’ quality of life. It seems that diverticulitis prophylaxis based on rifaximin can be economically efficient, however, it requires further research.

To study the clinical effect of Huayu Tongluo Decoction combined with edaravone injection in the treatment of patients with acute cerebral infarction.86 patients with acute cerebral infarction received from our hospital from October 2016 to May 2018 were randomly divided into study group (n=43) and control group (n=43). Routine treatment was performed in both groups. On the basis of this, the control group was treated with edaravone injection, and the research group was treated with Huayu Tongluo Decoction combined with edaravone injection. The effects of treatment, the degree of neurological deficit (NIHSS score), TCM syndrome scores and quality of life (QLI score) before and after treatment were compared. The total effective rate of the study group [95.35% (41/43)] was higher than that of the control group [79.07% (34/43)] (P<0.05). After treatment, the main symptoms, secondary diseases and tongue and pulse of the two groups were treated. The scores were lower than before treatment, and the study group was lower than the control group (P<0.05). After treatment, the NIHSS and QLI scores of the two groups were better than before treatment, and the NIHSS score of the study group was lower than that of the control group, and the QLI score was higher than the control. Group (P<0.05). The clinical effect of Huayu Tongluo Decoction combined with edaravone injection in the treatment of patients with acute cerebral infarction is significant, which can further improve the clinical symptoms, improve neurological function and improve the quality of life.
