ORGANIZATION OF WORK OF NUCLEAR MEDICINE DEPARTMENT AFTER COVID-19

The purpose of work was to develop and implement an algorithm for organizing the work of the nuclear medi-cine department in time of covid-19 epidemy. As a result of the spread of a new coronavirus infection, special requirements are imposed on medical institutions to ensure the safety of patients and medical staff. In ac-cordance with the recommendations of the European association of nuclear medicine, work was carried out on the introduction of preventive examination of per-sonnel, optimization of documentation flow and minimi-zation of direct contact between medical personnel and patients. Patient routing has been adapted, taking into epidemiological standards. The work carried out made it possible to prevent infection of medical staff and pa-tients. The time of contact with patients was reduced to the necessary minimum without any decrease the quality of treatment. Routing deficiencies were identified and eliminated, which significantly extended the time spent by patients in a medical facility. In addition, the load on the medical staff was optimized, which made it possible to improve the quality of medical care provided.

In the last decade, the health care system has been suffering from an imbalance of medical personnel and low efficiency in the use of nursing personnel in practical health care, which significantly affects the quality of medical care. The article reveals the main goals and objectives of the activity of mid-level medical staff in the system of providing medical care to patients in a urological hospital. The need for nurses to comply with the formulated standards is a central link in the process of organizing their activities. The effectiveness of treatment and recovery of patients without reducing the further quality of their lives depends on timely measures for care, psychological assistance, tact and compliance with ethical standards by nurses. All these factors directly affect the prospects for the treatment and rehabilitation of patients with urological pathology, as well as the performance indicators of the urological hospital.

Introduction. This study focuses on the formalization of the institution of mentoring in health care. The mentoring system in Russia has existed for a long time. Still, there is a particular interest in this topic since the requirements for the quality of medical care are changing, new challenges arise in the healthcare sector, and reducing staff turnover and retaining highly qualified specialists in the workplace arises. Targets and goals. The aim of the study is to clarify the need for institutional consolidation of the institution of mentoring. The tasks of the work included identifying factors motivating or demotivating medical workers to participate in the mentoring system, as well as the search for problems of formalizing the institution of mentoring. Material and methods. The author conducted a sociological study in Moscow: a mass questionnaire survey among doctors and nurses in 6 medical institutions subordinated to the Department of Health of the city of Moscow and a series of in-depth structured interviews with representatives of medical organizations in charge of mentoring in their organizations (17). Analysis of quantitative data was carried out using the construction of classification trees using the CHAID method. Results. The main issue that experts propose to spell out when formalizing mentoring is the skills and abilities. Employee willingness to become mentors varies. The study found that physicians over the age of 27 are most likely to become mentors convinced of the need to introduce and expand mentoring in the workplace. Young doctors under the age of 27 are less likely to take on the mentoring function due to a lack of confidence in their qualifications, as well as older colleagues who are dissatisfied with the working conditions in the workplace and do not consider it necessary to introduce or expand a mentoring system in their organization. Conclusion. The study showed that experts have a positive attitude to formalizing the institute of mentoring, noting that there should be a single document of a recommendatory nature.

Annotation. The article is devoted to the problems of motivation of modern specialists of humane professions at the stage of their formation. The most humane profession of humanity is the profession of a doctor. Therefore, the purpose of the article was to study the essence of motivation of medical staff of medical institutions of Ukraine, to determine the problem of managing the system of incentive of medical staff, to generalize and systematize the stages of designing stimulation of medical workers of medical institutions. Materials and methods of research were the analysis of domestic and foreign scientific sources, logical-theoretical procedure; method of system analysis and union; method of generalization (to study the quality of work of medical staff and identify problematic aspects of increasing work motivation). An anonymous survey on the motivation of professional activity of medical workers according to the method of K. Zamfir in the modification of A. Rean and a study on the practice of A. Leonov, S. Velichkovska on the differential assessment of the reduced working capacity of medical workers. Thus, the essence of motivation of medical staff of medical institutions of Ukraine was investigating, problems of management of system of inducement of medical staff are defined, stages of designing of stimulation of medical staff of medical institutions are generalized and systematized. To ensure quality management of motivation of medical staff of the medical institution, the manager must identify and eliminate the causes that negatively affect the reduction of stimulation for successful activities of medical staff and make timely and informed management decisions.

Aim.To carry out a comparative analysis of self-rated health, medical activity, and satisfaction with the quality of medical care in public health institutions by residents of rural settlements of the Untsukul district, Republic of Dagestan.Methods.The study was conducted by the method of questioning 2643 respondents, among them 1453 women and 1181 men.Results.According to the results of the survey, the majority of the interviewed residents of Untsukul district (68.2%) are satisfied with their health. Medical activity of the population at the time of the study was 60.6%. As to respondents who applied to the medical institutions of the Untsukul district during this period, 13.5% are not satisfied with the quality of medical care in public health institutions, 23.5% are fully satisfied, 30% are not fully satisfied.Conclusion.A social survey in the form of a questionnaire is one of the most effective methods of obtaining information about the self-rated health of the population. Timely analysis of medical activity of the population, its satisfaction with the quality of medical care will improve the efficiency of the health system.

The study aimed to evaluate the influence of coronavirus on the functioning of the system of healthcare and the associated pharmaceutical industry in Russia. The study of the influence of coronavirus infection on the system of healthcare and pharmaceutical market in Russia was made using a statistical method of comparison of the parameters. The degree of influence on the system of healthcare was estimated by the monthly dynamic of the morbidity rate, which reflected the load on medical institutions of the budgetary system of healthcare of the RF. The changes on the pharmaceutical market were evaluated through quantitative and price factors: quantitative factor was defined by the dynamics of general volume of sales and price – by the changes of the average-weighted prices on the market. As a result, it was established that the dynamics of the morbidity rate with coronavirus in Russia varied wavelike, significantly increasing in the periods of pandemic peaks in May and December 2020. This indicates that the system of healthcare lacks mechanisms of fighting the infection spread in complicated epidemiologic conditions, the bed capacity is low, and there is a deficit in medical personnel. During pandemics, a significant increase in the drug cost was observed on the pharmaceutical market. Especially, it was evident for drugs used to limit the spread of coronavirus, which was confirmed by the shift of drugs sales to the group of antimicrobials. Coronavirus pandemic significantly affected the structure and tendencies in the development of healthcare and Russian pharmaceutical market.

Aim. To study the opinion of patients with low back pain of judgement of assessibility and quality of medical care in public and private medical centers. Methods. Back pain patients to assess the quality of medical care filled in specially designed questionnaires for each type of medical facilities (two types of questionnaires for each patient who underwent treatment in both outpatient public clinics and private medical centers, total of 600 questionnaires). The poll was anonymous and was performed at one of the private medical centers in august 2013. Patients were randomly picked out from all patients who seek for medical aid in private center being unsatisfied with the quality of medical aid offered at public medical centers or who were on the waiting list for a doctor visit, or with disease worsening not within the working hours of public medical centers (600 questionnaires). The poll included patients of active working age: men aged 20 to 60 years (118 patients, 39.3%) and females aged 20 to 55 years (182 patients, 60.7%). Results. The majority of patients were more satisfied with the quality of the treatment, examination and attitude of medical personnel in private medical centers compared to public institutions. The following issues of providing medical aid to back pain patients were discovered both in private and public settings: untimely referral to specialist, or consultation only by general practitioner without subsequent consultation of neurologist, long waiting lists, incomplete neurologic examination, and low qualification of doctors in choosing appropriate examinations, treatment approach and further prophylaxis. Abovementioned issues were more significant in public settings. Conclusion. The research showed that there is a need in developing uniform standards of medical care in case of a back pain in outpatients that do not yet exist.

The purpose of the study was to analyze the state of satisfaction of the population in the provision of medical services and to observe the changes taking place in this area in connection with the start of the national project “Lean Polyclinic”. The studies were conducted in the form of a questionnaire survey of patients of two medical institutions, namely: the Regional Public Health Institutions “Children’s City Hospital No. 2” and “B. I. Alperovich City Clinical Hospital No. 3”.The material of the study was the responses on paper of the interviewed patients. The average indicators of patient satisfaction with the received medical services were determined by means of statistical calculations. The survey was conducted at the start of the project in May‑July of 2017 and again in October‑December of 2017.Results. The initial survey revealed the presence of such problems as insufficient number of automated workplaces; heavy workload of the doctor when working in the Medical Information System of the Tomsk Region; imperfection of the card filing cabinet; insufficient number of racks for storing outpatient cards, front-office and back-office were located in the same room and did not have a partition, which created noise and interfered with the work of call-center operators; the need to contact the registry to get the outpatient card when the visit was previously assigned; difficult orientation of patients in the clinic; long-term preventive examination of children aged 1 year; mixed flows of healthy and sick patients; and lack of personnel. The repeated survey reflected the changes that occurred in connection with the start of the “Lean Polyclinic” project: the number of detected violations tended to decrease, which indicated the improvement in the quality of services and the improvement in the organization of medical personnel work.Conclusion. Thus, the present study established that the use of lean production technologies contributed to the quality of medical services and it had a beneficial effect on the processes of organizing the work of medical institutions.

The problem of infectious safety is extremely urgent in modern medicine. To date, it is not possible to determine the reliable prevalence of nosocomial infections, since this indicator is significantly underestimated according to available statistical reporting. The development and implementation of a sanitary and hygienic monitoring system, organization of the work of the infection control commission is an important element in improving the quality of medical care by a general practitioner.
