This review article provides information on the role of different methods for pregnant women to prepare for childbirth according to the data regarding both foreign and domestic practices in obstetrics. The article highlights the issue of partner support starting from when pregnancy is established and gestation, through to childbirth and the postpartum period. According to the research, it is clear that the physiological course of the pregnancy directly depends on the psycho-emotional state of a pregnant woman, which impacts not only the course of pregnancy and childbirth, but also the condition of the fetus and newborn.The results of the research show that the method of psychophysiological preparation and partner support undoubtedly has a positive influence on the consequent childbirth: the duration of childbirth and the frequency of anomalies during maternity activity are reduced; surgery is performed less; the frequency of cervical and perineum ruptures, as well as the amount of blood loss, are decreased; intrauterine hypoxia and birth asphyxia are less common; and the manifestation of obstetric aggression decreases. Furthermore, the technologies of prenatal preparation are aimed at the demedicalization of childbirth and the minimization of the use of pharmacological drugs which have an ambiguous influence on a parturient, a fetus and a newborn.Therefore, the data mentioned above reveal the role of prenatal preparation methods of married couples for childbirth not only in the reduction of obstetric and perinatal complications in relatively healthy, pregnant women, but also in the choice of anesthesia methods, taking safety and efficiency into account.Unfortunately, the decreased interest toward psychophysiological preparation for childbirth by pregnant women and their partners, as well as medical institutions in general, was revealed while studying the issue of preparation for childbirth.Consequently, prenatal preparation of married couples must be obligatory in the system of obstetric care in Ukraine at all medical levels.

The aim: To study the features of the course of gestation and perinatal outcomes of delivery in women with vitamin D lack. Materials and methods: The article presents the results of studies of the characteristics of the course of pregnancy and delivery outcomes in 50 patients with vitamin D lack compared with a group of 50 somatically healthy pregnant women with normal level of 25 (OH) D. In order to establish a lack of vitamin D in pregnant women in the 10-12, 20-22, 30-32 weeks of gestation electrochemiluminiscence method by using a test system EURIMMUN (Germany) in the blood serum level of 25-hydroxycalciferol (25 (OH) D) in pregnant women. Results: When analyzing the structure of complications in women with vitamin D lack during pregnancy and childbirth we found out that risk of premature birth and premature births dominated among all the complications, respectively (58.0% and 36.0%) against (12.0% and 16.0%), p <0.05. Vitamin D lack in pregnant women is often associated with a wide range of obstetric and perinatal complications, namely: preeclampsia, gestational diabetes, bacterial vaginosis , premature rupture of membranes, placental abruption, abnormal labor activity, fetal distress that required delivery by Caesarean section. Conclusions: An analysis of the course of pregnancy and childbirth in women of thematic groups proved the expediency of an individual approach to the therapy of obstetric pathology among women with vitamin D lack. Despite the level of 25 (OH) D in the blood serum of a pregnant woman of 30 ng / ml and below, it is advisable to prescribe vitamin D for prophylaxes and treatment of Vitamin D deficiency in mother and fetus.

The objective: to study features of the somatic and genesial anamnesis at pregnant women with an undifferentiated dysplasia of connecting tissue for possibility of early diagnostics of the subsequent obstetric and perinatal complications. Patients and methods. 100 patients with clinical-laboratory signs an undifferentiated dysplasia of connecting tissue are surveyed. On the basis of the received results of the patient were divided into two groups: the main – 50 women (expression і6 points) regarded as patients with the expressed undifferentiated dysplasia of connecting tissue and group of comparison – 50 women (expression <6 points), regarded as patients without the connecting tissue expressed by an undifferentiated dysplasia. Results. Results of the conducted researches testify to an important role of the established features of the somatic and genesial anamnesis at pregnant women with clinical-laboratory and functional signs of an undifferentiated dysplasia of a connecting tissue. Conclusion. The received results need to be used for carrying out pregravidarny preparation and forecasting of possible obstetric and perinatal complications at pregnancy. This article presents the results of the retrospective study of the childbirth history of 106 pregnant women in whom labor was complicated by premature rupture of the membranes, delivery in the Bukhara regional perinatal center for the period 2017-2019 years. The results show the significant role of premature rupture of the membranes in the development of obstetrics and perinatal complications, especially in women with a history of somatic and gynecological anamnesis

Background: Diabetes mellitus is the most frequently encountered endocrine disorder in pregnancy and is associated with adverse outcomes if remain undiagnosed or untreated. This study was done to compare the maternal and perinatal outcome of pregestational diabetes mellitus (PGDM) with that of gestational diabetes (GDM). Methods: This observational analytical study with group comparison was carried out in the Department of Obstetrics and Gynaecology, BIRDEM General Hospital, Dhaka from July 2015 to June 2016. This study was conducted on two groups of pregnant women: group A (PGDM) and group B (GDM). A total of 100 cases were selected and in each group 50 pregnant women were enrolled. Singleton pregnancy and age 18 to 45 years were included. Pregnancy with hypertension, heart disease, renal disease and other metabolic disorders were excluded. Results: In this study mean gestational age of the study subjects were 35.72 ± 2.61 weeks and 36.58 ± 2.34 weeks in PGDM and GDM groups. History of GDM [34.0% vs 16.0%], intrauterine death (IUD) [14.0% vs 2.0%] and abortion [22.0% vas 6.0%] were significantly higher among PGDM patients than GDM patients. Regarding present pregnancy complications, polyhydramnios [32.0% vs 14.0%], preterm delivery [38.0% vs 20.0%], vulvovaginitis [28.0% vs 12.0%] and premature rupture of membrane (PROM) [24.0% vs 8.0%] were significantly higher in PGDM than GDM patients. Wound infection was significantly high in PGDM groups [35.7% vs 11.1%] among the patients delivered by lower uterine Cesarean section (LUCS). Regarding perinatal complications, hypoglycemia [22.0% vs 8.0%], birth asphyxia [24.0% vs 8.0%], RDS [24.0% vs 8.0%] were significantly higher among PGDM comparing GDM patients. Conclusion: The maternal and perinatal outcomes of pregestational diabetes mellitus were less favorable than those of gestational diabetes mellitus. Birdem Med J 2019; 9(2): 127-132

The endpoint of the negative impact of adverse processes in the mother’s body with influenza is the formation of placental insufficiency, the basis of which is a violation of the uteroplacental blood flow. The objective: to study the features of the course of pregnancy, the state of the fetus and the newborn after the influenza in the first trimester of pregnancy. Materials and methods. 120 women who had the influenza in the first trimester of pregnancy were examined. In 68 (56.7%) pregnant women signs of feto-placental dysfunction were observed, 2 groups were distinguished: the main group - 68 patients with feto-placental dysfunction, the comparison group – 52 pregnant women without signs of feto-placental insufficiency. Results. 3 times more often than women without manifestations of placental insufficiency (42.6% versus 15.4%; p<0.05) a severe course of influenza was observed, it accompanied by a high frequency of clinical manifestations, including in almost all patients (95.6% versus 67.3%; p<0.05), body temperature rose to 38 °C and higher, and in 61.7% of women it stayed for 4–6 days (versus 11.5%; p<0.05). Among the complications of influenza: bronchitis (25.0% versus 9.3%; p<0.05), pneumonia (17.6% versus 5.7%; p<0.05), sinusitis (17.6% versus 7.7%; p<0.05). The threat of abortion was noted in 57.4% of cases, the threat of preterm birth was observed in 39.7% of women. Most often, placental dysfunction was associated with fetal distress (76.5% versus 13.5%; p<0.05) and growth retardation (54.4% versus 3.8%; p<0.05). 32.4% versus 13.5% of women had preeclampsia (p<0.05). The frequency of both polyhydramnios (17.6%) and low water (10.3%) is significantly higher. By cesarean section, 35.3% women were delivered (versus 15.4%, (p<0.05). Delivery was preterm in 17.6% of women versus 7.7% (p<0.05). Premature discharge of amniotic fluid (17.6%) and pathological blood loss during childbirth (16.2%), fetal distress during childbirth (48.5% versus 9.6%; p<0.05) were noted. Maternal placental dysfunction, fetal distress, prematurity (17.6%) and malnutrition (22.1%) led to a high incidence of birth asphyxia (46.5% versus 19.2%, p <0.05). Half (51.5%) of children had disadaptation syndromes, most often neurological disorders (32.4% versus 11.5%; p<0.05) and respiratory disorders (27.9% versus 7.7%; p<0.05). Conclusion. Influenza in early pregnancy with a severe course and a high frequency of complications is associated with a high frequency of feto-placental dysfunction and other obstetric and perinatal complications, which requires a more detailed study to determine risk factors and develop tactics for managing this category of pregnant women. In Ukraine, each year born about 1,000 children ELBW, representing 0.3% of all newborns. The survival of infants with ELBW in our country has a slow upward trend and does not exceed 50%. Infants with ELBW have the highest risk of death and are the group most at risk for the development of chronic diseases of the respiratory and nervous systems and sensory apparatus and associated disability. Considering the growth of the number of children at private ELBW on the background PPROM, high perinatal morbidity and infant mortality, reduced health and reproductive capacity of women becoming urgency tactics of pregnancy and labor with premature rupture of membranes. The objective: perynatalniyh reduce complications in terms of 22-28 weeks of pregnancy complicated by premature rupture of membranes based on developing the tactics of pregnancy and childbirth. Patients and methods. To reduce perinatal complications analyzed the effect of different methods of pregnancy and childbirth on the state of newborns in gestational age 22-28 weeks against the backdrop of premature discharge of amniotic fluid. The control group consisted of 56 women who had less than a day anhydrous term. In the study group applied expectant tactics of anhydrous long term. Depending on the length of the term of anhydrous main group was divided into 2 subgroups. Sub-IA amounted to 86 pregnant women with anhydrous period of 5 days, and IP subgroup totaled 64 pregnant women with anhydrous period of 25 days. Statistical analysis of the results was performed using Microsoft Excel. Probability difference calculated by Student’s t-criterion. Results. Аnalyzing the structure and frequency of obstetric and perinatal complications in women in the period 22-28 weeks of pregnancy with different duration waterless terms, different terms of delivery proved the effectiveness of the tactics of delivery, which is used in perinatal centers m. Kyiv. This correlates with clinical data - statistical analysis of indicators of neonatal mortality and morbidity. Conclusion. Оf the research found that the rates of neonatal mortality and morbidity affecting: duration of anhydrous interval, gestational period, methods of delivery. Long expectant tactics leads to a real reduction of RDS, but an increase in the incidence of chorioamnionitis and neonatal infection. Therefore, wait-recommended tactics for 5 days, followed by delivery of women. At 22-27 weeks of pregnancy delivery path has no impact on rates of neonatal morbidity. The advantage in the delivery of women in the period 27-28 weeks, especially in the immature cervix and pelvic peredlezhenni fetal provided cesarean section. Aims: This study was conducted to assess the fetal and maternal outcomes following cesarean section in Trishuli District Hospital at Nuwakot, NepalMethods: A hospital based study was conducted in Trishuli District Hospital Nuwakot from 14th March 2012 to 13th March 2013 among 327 pregnant women who underwent cesarean section in Trishuli District Hospital Nuwakot and met the inclusion criteria of this study.Results: The most common indication of caesarean section was (n=139, 42.6%) foetal distress (n=98, 30% moderate to thick meconium stained liquor; n=41, 12.6% foetal tachybradycardia). Among maternal complications, wound infections was seen in seven (2.1%) pregnant women, uterine rupture in two (0.6%), post partum haemorrhage in one (0.3%), post partum eclampsia in one (0.3%) and peurperial pyrexia in one (0.3%). Among perinatal complications, birth asphyxia was present in 10 (3%) neonates, neonatal sepsis in seven (2%) and perinatal death in 12 (3.5%) neonates.Conclusions: Better BEmOC reduced the number of CEmOC requirement. Providing CEmOC in district level is the vital measure in safe motherhood for reducing maternal and perinatal morbidities and mortalities. But for improving maternal and perinatal outcomes in district level, there should be adequate skilled manpower in maternity ward and operation theature, adequate infrastructures in preoperative, operative and postoperative wards, blood bank and neonatal high dependency unit in CEmOC.

BACKGROUND: Abortion remains a problem in the obstetric care as it is one of the causes of maternal and fetal death. Bleeding or miscarriage threat in the first trimester comprises 20% to 25% of all pregnant women, and 50% of them will end as spontaneous abortion. The major cause of maternal death in abortion is hemorrhage. Some recent studies have indicated that MMP-9 is important in the process of embryo implantation into the endometrium and spontaneous abortion occurs when there is an overwhelming increase of MMP-9. This data indicate the importance of a further research to elucidate the role of MMP-9 in spontaneous abortion.METHODS: This was a cross sectional study, included 70 pregnant women with gestational age <20 weeks who came for examination and treatment at the Department of Obstetrics and Gynecology, Medical Faculty of Padjadjaran University/Hasan Sadikin Hospital, Bandung. RESULTS: There were differences in the MMP-9 levels in spontaneous abortion compared to normal pregnancy and MMP-9 had a cut-off point >1221.7 with the sensitivity of 48.6% specificity of 80%, and accuracy of 60.9%.SUMMARY: The MMP-9 level in spontaneous abortion was higher than in normal pregnancy.KEYWORDS: MMP-9, spontaneous abortion, normal pregnancy

The objective: determination of the effectiveness of prevention and medicinal correction of placental dysfunction in pregnant women with chronic salpingo-oophoritis.Materials and methods. The study included 100 pregnant women (basic group) with chronic salpingo-oophoritis, of these, I group (50 women) included patients who received proposed treatment and prevention approach, II group (50 women) received a common set of treatment and prevention measures. The proposed treatment-and-prophylactic approach included prophylactic administration to pregnant women with chronic salpingo-oophoritis from the 8th to the 10th and from the 16th to the 18th week inclusive of natural micronized progesterone at a dose of 100 mg intravaginally twice a day for two weeks. Venotonik and angioprotector were prescribed prophylactically at dose 600 mg from 18 weeks to 37-38 weeks of pregnancy. For therapeutic purposes (blood flow disorders in the uterine arteries and vessels of the umbilical cord that are determined by Doppler), the drug was prescribed at dose 600 mg twice a day for 3–4 weeks.Results. A lower frequency of obstetric and perinatal complications was found in the women in I group than in the patients in the II group. Thus, the rate of placental dysfunction was 6.0±3.36% vs. 34.0±6.7%, respectively, oligohydramnios – 6.0±3.36% and 12.0±4.59%, polyhydramnios – 10.0±4.24% and 22.0±5.86%, fetal distress during childbirth – 6.0±3.36% and 14.0±4.91%, cesarean section – 12.0±4.6% and 24.0±4.27%. In the group of patients with treatment-and-prophylactic approach, the mean of the body weight of newborns was 3486.7±312.71 g, and in women who received a common set of treatment-and-prophylactic measures – 3099.8±295.69 g. The mean of the assessment of the condition of newborns by the Apgar scale at the 1st minute – 7.92±1.54 and 6.81±1.16 points, respectively.Conclusions. The use of micronized natural progesterone and the drug diosmin 600 mg in pregnant women with a history of chronic salpingo-oophoritis reduces the rate of obstetric and perinatal complications during pregnancy.

The article highlights the psychological features and functional status of the autonomic nervous system in single and married pregnant women and methods of preventing complications associated with impaired psychoautonomic regulation in pregnant women. The main purpose of the study is to improve the psychological status and functional status of the autonomic nervous system during pregnancy and childbirth in single women by developing and implementing a complex of therapeutic and preventive measures. Systematization of literary sources and approaches to solving the problem of gestational complications in women deprived of partner support showed that the course of pregnancy and childbirth in single women has a high risk of perinatal complications: increase in the frequency of preeclampsia, anemia, placental dysfunction and nonstatus evaluation nervous system in 50 single women on the background of the proposed complex algorithm, in 50 single women and 50 married women in general adopted pregnancy management and childbirth. The psychological status of women during pregnancy and before childbirth was assessed on the Spielberger and Hanin scale. In order to detect the degree of disorders of the autonomic nervous system and the level of regulation of cardiac activity, cardiointervalography was performed. It has been established that a number of psychoautonomic disorders are characteristic of single pregnant women, deprived of partner support. Against the background of a complex algorithm for managing pregnancies and childbirth in single women, indicators of psychological status impairment decreased significantly (p <0.05): memory impairment, 2fold intelligence; physical and mental discomfort 2.4 times; Feeling anxious about health 2.5 times; anxiety about childbirth, the health of the unborn baby, and the difficulties associated with newborn care 2.4 times; reducing mood and feeling depressed 3 times; manifestations of tension 3 times; sleep disturbance 3.5 times; panic, fear and despair 3.5 times. Indicators of cardiointervalography showed a significant normalization of the state of the autonomic nervous system: an increase in the coefficient of variation 1.3 times, against the background of a simultaneous decrease in the amplitude of the fashion 1.3 times; vegetative equilibrium index 1.2 times; activity index of regulation processes 1.2 times and voltage index 1.2 times. The results obtained may be useful for gynecologists.
