Childhood Abuse, Postpartum Depression and Parenting Difficulties: A Literature Review of Associations

Objective: The aim of this paper is to obtain an understanding of the links between maternal postpartum depression, poor parenting and childhood abuse, and the subsequent development of adult psychopathology in children from these families. Method: A literature review was undertaken of studies on postpartum depression looking at parental childhood abuse, parenting and child outcomes, as well as childhood abuse and its association with adult depression and parenting difficulties. Results: Considerable overlap is noted in predisposing factors for major depression, postpartum depression, inadequate parenting and childhood abuse. Links appear to begin in early infancy, suggesting postpartum depression as a possible mechanism for intergenerational transmission of psychopathology. Conclusions: Further research is required to look at the outcome of children of women who are at high risk of chronic or recurrent depression and of difficulties parenting; the postpartum period is the optimum time for identification and intervention.

AbstractMothers who have experienced childhood maltreatment are more likely to have children also exposed to maltreatment, a phenomenon known as intergenerational transmission. Factors in the perinatal period may contribute uniquely to this transmission, but timing effects have not been ascertained. Using structural equation modeling with 1,016 mothers and their 2,032 children in the Environmental Risk Longitudinal Twin Study, we tested the mediating role of postpartum depression between maternal childhood maltreatment and a cascade of negative child outcomes, specifically child exposure to maltreatment, internalizing symptoms, and externalizing symptoms: (a) adjusting for later maternal depression, (b) comparing across sex differences, and (c) examining the relative role of maltreatment subtypes. Mothers who had been maltreated as children, especially those who had experienced emotional or sexual abuse, were at increased risk for postpartum depression. In turn, postpartum depression predicted children’s exposure to maltreatment, followed by emotional and behavioral problems. Indirect effects from maternal childhood maltreatment to child outcomes were robust across child sex and supported significant mediation through postpartum depression; however, this appeared to be carried by mothers’ depression beyond the postpartum period. Identifying and treating postpartum depression, and preventing its recurrence, may help interrupt the intergenerational transmission of maltreatment and its sequelae.

Introduction. The aim of this study is to assess how negative life events and adverse experiences with pregnancy, delivery, the infant(s), and breastfeeding cessation impact on postpartum depression (PPD), specifically in first lifetime and recurrent depression. Method. The study group comprised 104 mothers with a current episode of PPD and a control group of 104 mothers who did not have current PPD. The Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-I) was used for data collection. The course of the depression, adverse experiences, and breastfeeding were assessed by self-reports. Results. In age-adjusted multivariate analyses, mental and physical problems during pregnancy or delivery, postpartum problems with the infant and breastfeeding cessation, and negative life events during the previous 12 months were associated with postpartum depression. Eighteen percent (18%) of the mothers had first depression and 82% recurrent depression. Mental and physical problems during pregnancy or delivery were associated with both first lifetime and recurrent depression. Nevertheless, negative life events and infant/breastfeeding issues associated only with recurrent depression. Conclusion. Factors associated with pregnancy and delivery have an impact on PPD, but in recurrent depression other postnatal and psychosocial factors are also important risk factors.

Despite a large population of registered people with drug addiction, child protection in substance-abusing families is a neglected issue in China. The present study aims to investigate the association between parents’ childhood abuse history and the risk of abusing their own children in substance-abusing Chinese families and also to examine the mediating role of detachment and moderating role of social support during the intergenerational transmission of abuse. A total of 173 men and 116 women were selected using cluster sampling from two compulsory drug rehabilitation centers in Jiangsu Province. Results indicated that one’s childhood abuse history was positively associated with the current perpetration of child abuse for both fathers and mothers. Detachment mediated the linkage between a history of childhood maltreatment and perpetration of child abuse in all types of abuse for both men and women except for women’s sexual abuse. Social support from family members buffered the intergenerational transmission of child abuse for fathers but not for mothers. Child maltreatment in substance-abusing families is an urgent issue that needs measures to prevent the intergenerational transmission of violence in China. Intervention programs could involve helping parents cope with their childhood abuse history through rebuilding secure attachment and facilitating social support for their parenting practices, especially for fathers.

Background. In modern society, there is an intensive increase in the incidence of arterial hypertension among young people, which is mainly due to lifestyle characteristics and related risk factors.Objective: to analyze the results of scientific researches of domestic and foreign authors regarding the study of risk factors in the development of arterial hypertension in children.Material and methods. A qualitative analysis of the current literature data on the risk factors for arterial hypertension and its prevention has been carried out.Results. It has been established that the occurrence of arterial hypertension in childhood is due to not only hereditary, but also a number of environmental risk factors.Conclusions. Further researches are needed to study new and established risk factors that must be considered when conducting primary prevention of the disease in children at high risk.

Pendahulan: Neuritis Optik (ON) didefinisikan sebagai inflamasi pada saraf optik, yang sebagian besar idiopatik. Namun dapat dikaitkan dengan penyebab lain seperti lesi demielinasi, gangguan autoimun, infeksi dan inflamasi. Dari semua ini, multiple sclerosis (MS) adalah penyebab paling umum dari demielinasi ON. Tujuan: Untuk mengetahui diagnosis dan tatalaksana neuritis optik demielinasi. Metode: Artikel ini dibuat dengan metode literature review, melibatkan 29 pustaka baik buku dan jurnal nasional atau internasional. Hasil: ON terjadi karena proses inflamasi yang mengarah pada aktivasi sel-T yang dapat melewati sawar darah otak dan menyebabkan reaksi hipersensitivitas terhadap struktur saraf. Mekanisme pastinya belum diketahui. Diagnosis klinis ON terdiri dari tiga gejala klasik yaitu kehilangan penglihatan, nyeri periokular dan dischromatopsia. Hal ini membutuhkan pemeriksaan oftalmik, neurologis dan sistemik yang cermat untuk membedakan antara ON spesifik dan tidak. Diagnosis banding diperlukan untuk membuat rencana tatalaksana yang tepat. Pembahasan: Menurut Optic Neuritis Treatment Trial (ONTT), pengobatan pertama adalah metilprednisolon intravena dengan pemulihan yang lebih cepat dan lebih sedikit kemungkinan kasus relaps dan konversi ke MS. Namun prednisolon oral saja dikontraindikasikan karena peningkatan risiko relaps. Controlled High-Risk Subjects Avonex® Multiple Sclerosis Prevention Study (CHAMPS) dan Early Treatment of MS study (ETOMS) dan telah melaporkan bahwa pengobatan dengan interferon β-1a, dengan hasil pengurangan risiko karakteristik MS dari MRI. Sensitivitas kontras, penglihatan warna dan bidang visual adalah parameter yang sebagian besar tetap terganggu bahkan setelah pemulihan ketajaman visual yang baik. Simpulan: Tatalaksana pada neuritis optik demielinasi dominan diberikan steroid dan interferon β-1a.   Kata kunci: demielinasi, multipel skeloris, neuritis optik, tatalaksana

Objective: Individuals with a history of childhood maltreatment are particularly vulnerable to a longer course of depression. Immunization theories of resilience suggest that resilience and related factors may buffer against the deleterious effects of early childhood adversity. Trait mindfulness is linked to resilience and may be a pathway to cultivating this dynamic process. In this study, we investigated whether trait mindfulness can buffer against the effects of early childhood maltreatment in predicting lifetime number of months depressed. Method: We recruited 43 previously depressed, currently remitted patients, and retrospectively examined their depression history (using a structured interview, LIFE-SCID), their self-reported experience of maltreatment in the first 16 years of life (MOPS), and their levels of trait mindfulness (MAAS). Results: We found that number of months depressed in a lifetime was positively associated with reported childhood maltreatment, and negatively associated with trait mindfulness. Secondly, we found evidence that trait mindfulness significantly moderated the relationship of early childhood maltreatment and number of months depressed. Specifically, it appears that individuals who report severe histories of maltreatment are especially vulnerable to recurrent depression if they are also reporting low levels of trait mindfulness. Conclusions: Increasing mindfulness may be warranted among individuals reporting a history of childhood abuse with lower baselines of trait mindfulness; however, results of this retrospective study require replication in a larger, prospective trial.
