La estancia en el hospital de niños y niñas es desafortunadamente común y está asociada a regresión en los comportamientos, agresión, falta de cooperación, retraimiento, dificultad para recuperarse, llanto excesivo, y disminución de la comunicación y/o actividad, por lo que el proceso atención enfermero necesita no solo una perspectiva biológica, sino también incluir a las esferas psicológicas y sociales. El rol del personal de enfermería, al ser los principales referentes de cuidado dentro del contexto sanitario, resulta esencial pues son percibidos como la mayor fuente de apoyo e información de los padres, madres y pacientes. En el documento se proporcionan algunas pautas, dirigidas al personal de salud, para aumentar el bienestar integral a través de acciones concretas y de fácil ejecución. Palabras clave: hospitalización, personal de enfermería, humanización de la atención. ABSTRACT The hospital stay of children is unfortunately common and is associated with regression in behaviors, aggression, lack of cooperation, withdrawal, difficulty in recovering, excessive crying, and decreased communication and / or activity, so The nursing care process needs not only a biological perspective, but also includes the psychological and social spheres. The role of the nursing staff, being the main care references within the healthcare context, is essential since they are perceived as the greatest source of support and information from parents and patients. The document provides some guidelines, aimed at health personnel, to increase comprehensive well-being through concrete and easy-to-implement actions. At-homeness, as an aspect of well-being, can be experienced despite living with life-limiting conditions and needs for a palliative approach to care. In nursing homes, older residents with life-limiting conditions face losses and changes which could influence their experience of at-homeness. The aim of this study was to explore how nursing staff enable at-homeness for residents with life-limiting conditions. Interpretive description was employed as the design using data from participant observations and formal and informal interviews related to nursing care situations. The strategies found to be used to enable at-homeness comprising nursing staff presenting themselves as reliable, respecting the resident’s integrity, being responsive to the resident’s needs, collaborating with the resident in decision-making, and through nurturing comforting relationships. The result on how to enable at-homeness could be used as strategies for a person-centered palliative approach in the care for residents in nursing homes.

Background : Emergency conditions are conditions that occur suddenly and are very life-threatening, so help must be given quickly, precisely and correctly. One of the life-threatening conditions is cardiac arrest, which is a condition where the heart cannot function anymore, and requires collaboration between health personnel in providing help and this always ends in death. The incidence of cardiac arrest at the Ulin Regional General Hospital during May to June 2017 was 88 cases and most of the deaths occurred in patients. Cardiac arrest can still be saved by the correct nursing care process so that return of spontaneous circulation can occur.Purpose : Identify and explore the experience of nurses doing nursing care for cardiac arrest.Methods: This type of research is qualitative with a phenomenological approach. The number of participants in this study were 9 nurses in the emergency room and intensive care unit. The process of retrieving data using interview guides, and data analysis using Colaizzy.Results: This study obtained a major theme, "Collaborative Teamwork" when providing nursing care for cardiac arrest. This theme was formed from two sub-themes namely "division of job teams", "collaboration" and KIE to the patient's family.The participant describing the existence of maintained teamwork can help smooth the process of nursing cardiac arrest so that patients can return to the condition of return of spontaneous circulation.

The definition of the role of hospitals and communities in terms of the response to patients’ health care needs is essential in the Lombardy region health-care reform development (LR. 23/2015). The stratification of patients according to clinical severity and care complexity for adequate clinical health care, is achieved by delineating care settings, staff standards, required technical equipment and crucial aspects of clinical pathways. An observational and retrospective study at Manerbio Hospital Internal Medicine Unit (IMU) was carried out to define: i) characteristics of IMU patients; ii) role of IMU physician in management of poly-pathological patients; iii) alternative organizational models. After a Literature review, clinical severity was defined by modifying early warning score, complexity and co-morbidities by cumulative illness rating scale (CIRS) and by intensity of care through care intensity index (IIA). All medical records of patients admitted in the first quarter of 2016 were analyzed. A total of 393 medical records were examined: 199 M/194 F, median age 81 years. Critical patients (requiring continuous monitoring using advanced equipment): 27% of the sample (10% with intensive care transfer criteria). Co-morbidity: between 5 and 6 active diseases for most of the sample; 53% with CIRS between 7 and 12. Elevated care intensity (IIA) was found in 46% of the sample, remaining constant throughout hospital stay. Medium stay: 9.35 days. 27% of IMU patients needs subintensive care. About a quarter of patients has unresolved social problems contributing to acute presentations in the emergency room. Agreement on appropriate links between hospital and community care structures is advisable to reduce hospital stay, adequately responding to patients’ needs.

ABSTRACTObjective: to present the conceptual model of open systems and King's Theory of Goals Attainment, as well as its contribution for nursing practice, approaching the need of an health assistance with the objective of attend society needs and develop the critical conscience in order to provide positive changes in people's lifestyle and the care aiming the valorization of the other, in order to stimulate active participation of the clientele in the health-disease-care process. Method: this is a reflection on the theory as support to the nursing interactive assistance. Results: we believe that the presented theory can contribute to the nursing practice. Final considerations: based on the theoretical-conceptual reference we have an alternative of humanized nursing care ruled in the interaction between professional and customer in order to propitiate the improvement of health and well-being of the clientele. Purpose This paper aims to explore the concept and practice, of dignity as understood and experienced by older adults and district nursing staff. The paper adds a new, nuanced, understanding of safeguarding possibilities in the context of district nursing care delivered in the home. Design/methodology/approach The research used an ethnographic methodology involving observations of care between community district nursing clinicians and patients (n = 62) and semi-structured interviews with nursing staff (n = 11) and older adult recipients of district nursing care (n = 11) in England. Findings Abuse is less likely to occur when clinicians are maintaining the dignity of their patients. The themes of time and space are used to demonstrate some fundamental ways in which dignity manifests. The absence of dignity offers opportunities for abuse and neglect to thrive; therefore, both time and space are essential safeguarding considerations. Dignity is influenced by time and how it is experienced temporally, but nurses are not allocated time to “do dignity”, an arguably essential component of the caregiving role, yet one that can become marginalised. The home-clinic exists as a clinical space requiring careful management to ensure it is also an environment of dignity that can safeguard older adults. Practical implications District nurses have both a proactive and reactive role in ensuring their patients remain safeguarded. By ensuring care is delivered with dignity and taking appropriate action if they suspect abuse or neglect, district nurses can safeguard their patients. Originality/value This paper begins to address an omission in existing empirical research regarding the role of district nursing teams in delivering dignified care and how this can safeguard older adults.

Los cuidados de Enfermería constituyen un elemento fundamental y articulador en la atención integral que se brinda en el medio hospitalario. Esto se evidencia en los documentos que registran la atención al paciente; en este sentido, las notas de Enfermería son una importante fuente de información que permiten dar continuidad al cuidado. En este trabajo, se realiza una revisión bibliográfica de aspectos que deben tenerse en cuenta para la realización de las notas de Enfermería. Más en concreto, se trata de conceptos deontológicos de la profesión y de aspectos legales que deben tenerse en cuenta para la construcción de la historia clínica. El insumo final para el análisis fue integrado por 50 artículos científicos de revisión, reflexión e investigación incluidos en las bases de datos SCIELO, OVID, DIALNET y PUBMED, así como documentos emanados de la legislación colombiana. En la estrategia de búsqueda, se utilizaron las palabras clave: enfermería, registros de enfermería, atención de enfermería y servicios de enfermería. En conclusión, quedó claro que las notas de Enfermería forman parte esencial de los expedientes de cualquier institución hospitalaria; en ellas se registran las intervenciones que realiza Enfermería en su labor asistencial. Por tanto, es necesario contar con documentos legales que avalen el rol autónomo de la profesión para así facilitar la investigación clínica, docente y cumplir con los requerimientos fundamentales para defender eficazmente litigios y retos legales.ABSTRACTA fundamental key element and articulator in the comprehensive care that is provided by the hospital environment is the nursing care. The documents that record the patient care are evident, in this sense, this is evident in the documents that record the patient care; in this sense, the nursing notes are an important source of information that allows you to give continuity to the care. In this work paper, a bibliographical review about aspects that must be taken into account for doing nursing notes is done. It’s about concepts of ethic of the profession and legal aspects which must be taken into account in order to report the clinical history, more specifically, final input for the analysis of 50 scientific articles in order to review was composed, reflection and research included in the SCIELO, OVID, DIALNET and PUBMED databases, and documents issued by Colombian legislation. The search strategy the key words: nurse, nursing, nursing care and nursing services records were used. In conclusion, it is clear that nurses notes are an essential part of any hospital; there nursing care interventions made by nursing care work are recorded. So, to have legal documents demonstrating the role of the autonomous profession to facilitate clinical research, is necessary, teaching and comply with the fundamental requirements for effectively defending litigation and legal challenges

ABSTRACTIntroduction: the theoretical and methodological components are the proper expertise in nursing, and it refers to models, theories, care process, taxonomy of nursing diagnoses, system of nursing intervention classification, and system of outcomes classification, which base nursing care into professional practice. Methodology: research was performed on Google Scholar, reviewing the databases of Scielo, Ciberindex, Index Enfermería, Dialnet, Redalyc, Medline, identifying 70 published articles between 2005-2015, and selecting 52 of them. The keywords used were: nurse care, nursing diagnostic, classification, nursing theory, in spanish and portuguese. Results: training students, receive knowledge in the nursing process, NANDA International, classification of the interventions, nurse results and theoretical components. The Dorothea Orem, Callista Roy, Nola Pender, Virginia Henderson, Florence Nightingale, and Betty Neuman theories are applied. The application of the nursing process is limited and low familiarity with the international taxonomy by nurse professionals in the assistance area is noticed. La aplicación del proceso de enfermería es limitada y hay escasa familiaridad con la taxonomía internacional por parte de los profesionales de enfermería en el área asistencial. Nursing is a professional health discipline and nursing models based on nursing theories form its scientific foundations. Neuman’s Systems Model is one of these models. The model focuses on the individual's well-being and is based on holistic nursing care. In the model, the individual is an open system interpreting the stressors he perceives. By defining these stressors, the nurse develops the defense mechanisms of the individual on the primary, secondary and tertiary level. The nurse is responsible for the planning, implementation and evaluation. The cervical disc hernia that affects life activities is helped cope with the stressors perceived by the individual receiving the diagnosis. Thus, the quality of life of the individual can be increased, the treatment process can be shortened and the cost can be reduced. In this study, the nursing care process of a patient who was diagnosed with cervical disc herniation was examined according to Neuman’s Systems Model. Person-specific, interpersonal and extrinsic stressors were identified in the direction of the obtained data, and nursing diagnoses suitable for the patient were established in the direction of stressors. As a result, it was found that the use of the model in this case was appropriate and it was suggested to be applied in other patient groups to evaluate the effectiveness of the model. Extended English summary is in the end of Full Text PDF (TURKISH) file.ÖzetHemşirelik profesyonel bir sağlık disiplinidir ve bilimsel bilgi kaynağını kavram,[Mh1]  kuram ve modellerden almaktadır. Neuman Sistemler Modeli bu modellerden birisidir. Model bireyin iyilik haline odaklanmakta ve bütüncül hemşirelik bakımını esas almaktadır. Modelde birey, algıladığı stresörleri yorumlayan açık sistemdir.  Hemşire bu stresörleri tanımlayarak bireyin savunma mekanizmalarını birincil, ikincil ve üçüncül düzeyde geliştirecek bakımın planlanması, uygulanması ve değerlendirilmesinden sorumludur. Günlük yaşam aktivitelerini olumsuz etkileyen servikal disk herni tanısı alan bireyin algıladığı stresörlerle başa çıkmasına yardım edilerek bireyin yaşam kalitesi artırılabilir, tedavi süreci kısaltılabilir ve maliyet azaltılabilir. Bu çalışmada servikal disk hernisi tanısı alan bir hastanın hemşirelik bakım süreci Neuman Sistemler modeline göre incelenmiştir. Elde edilen veriler doğrultusunda kişiye özgü, kişiler arası ve kişi dışı stresörler belirlenmiş, stresörler doğrultusunda hastaya uygun hemşirelik tanıları konulmuştur. Sonuç olarak, modelin bu olguda kullanımının uygun olduğu görülmüş, modelin etkinliğini değerlendirmek amacıyla başka hasta gruplarında da uygulanması önerilmiştir.
