ABSTRACTObjective: to present the conceptual model of open systems and King's Theory of Goals Attainment, as well as its contribution for nursing practice, approaching the need of an health assistance with the objective of attend society needs and develop the critical conscience in order to provide positive changes in people's lifestyle and the care aiming the valorization of the other, in order to stimulate active participation of the clientele in the health-disease-care process. Method: this is a reflection on the theory as support to the nursing interactive assistance. Results: we believe that the presented theory can contribute to the nursing practice. Final considerations: based on the theoretical-conceptual reference we have an alternative of humanized nursing care ruled in the interaction between professional and customer in order to propitiate the improvement of health and well-being of the clientele. Método: reflejo sobre la teoría como soporte para la asistencia interativa de enfermería. Resultados: se cre que la teoria presentada podrá contribuir como posibilidad para la atención humanizada. Consideraciones finales: delante de marco teórico-conceptual, se mira una alternativa de atención de enfermería humanizado, basada en la interacción entre el profesional y cliente, para promover la mejoría y el bienestar de la clientela. ABSTRACT Background: Positive psychiatry offers an unique approach to promote brain health and well-being in aging populations. Minimal interventions through behavioral activation to promote wellness are increasingly available using self-guided apps, yet little is known about the effectiveness of app technology or the difference between clinician-supported behavioral activation versus self-guided app methodologies. Objectives: Investigate the difference in users and outcomes between two methods of the Fountain of Health (FoH) positive psychiatry intervention for behavioral activation to promote brain health and well-being: (1) clinician-assisted and (2) independent app use for behavioral self-management. Design and setting: As part of a larger knowledge translation intervention in positive psychiatry, two specific methods of a behavioral activation intervention were retrospectively compared. Participants: Two subsets of patients were compared; 254 clinician-assisted patients; 333 independent app users. Intervention: A minimal positive psychiatry intervention in frontline care using the FoH health and behavior change clinical tools Measurements: Main outcomes were changes in psychological (health and resilience, well-being scores) and behavioral indices (goal attainment, items of goal SMART-ness). User profiles (age, sex and completion rates) were also compared. Results: Clinician-assisted patients were more likely to be male, older, and have lower health and resilience scores at baseline. Clinician-assisted patients had notably higher completion rates (99.2% vs. 10.8%). Psychological outcomes (improved health and resilience, and well-being) were similar regardless of intervention method for those who completed the intervention. Behavioral outcomes revealed clinician-assisted patients set goals that better adhered to key goal-setting items. Conclusions: Clinician–patient relationships appear to be an important factor for intervention completion and behavioral outcomes, while further exploration of best practices for intervention completion using health apps in clinical practice is needed. A preliminary goal-setting methodology for effective behavioral activation, to promote brain health and wellness, is given.

Abstract: Meditation has been practiced throughout the centuries. This article explores meditation as a health patterning modality for nurses to employ for themselves and to facilitate clients' knowing participation in their change process. The theoretical framework for this interpretation is Rogers' Science of Unitary Human Beings. Meditation has the potential to promote awareness of the experience of flow in the human/environment patterning process. Out of this evolves an expanded awareness of creative potentials for change. Two clinical vignettes are offered to illustrate this process. Summary: Meditation is a health patterning modality that can facilitate knowing participation in change. It broadens awareness of potentials that can be actualized as nurses and clients seek to promote their own health and well-being. Meditation can assist both in experiencing the rhythm of their human/environment mutual process and open them to an expanded field image. According to Rogers, this experiencing is pandimensional, transcending traditionally perceived limitations of space and time. Meditation opens the door onto new and creative potentialities in the process of becoming.

ABSTRACT Objective: to apply nursing care systemization to a client submitted to angioplasty with placement of coronary stents in the light of King’s Theory of Goal Attainment. Method: convergent care research, carried out at the patient’s home. The following Nursing Diagnoses were elaborated and the interventions are proposed according to the connection between NANDA, NIC and NOC. The project was previously approved by the Research Ethics committee from University Federal do Ceará, under no protocol 61/08. Results: the nursing diagnoses: imbalanced nutrition: less than body requirements, impaired physical mobility, activity intolerance, chronic sadness, disturbed sleep patterns, self-care deficit control and ineffective family therapeutic regimen. the goals were: To obtain an adequate diet and fluid intake; Perform physical exercise safely and social interaction activities; Try and decrease dependence on the medication and adjust sleep times; Facilitate the accomplishment of self-care activities. The nursing plan attained a majority of the established goals, even if partially, which was expected in view of the proposed goals and implementation time. Conclusion: the use of Nursing Diagnoses is a technology needed for daily nursing care, as it permits comprehensive care and is relevant in home treatment, with an emphasis on health promotion.Descriptors: nursing process; nursing theory; coronary disease; care.RESUMO Objetivo: implementar a sistematização da assistência de enfermagem a um cliente submetido à angioplastia com colocação de stents coronarianos à luz da Teoria de Alcance de Metas de King. Método: pesquisa convergente-assistencial, realizada em um domicílio de Fortaleza, em 2010. Elaboraram-se os Diagnósticos de Enfermagem e propôs intervenções, segundo a ligação entre NANDA, NIC e NOC. O projeto foi previamente aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal do Ceará, sob nº de protocolo 61/08. Resultados: os diagnósticos de enfermagem: nutrição desequilibrada: menos do que as necessidades corporais, mobilidade física prejudicada, intolerância à atividade, tristeza crônica, padrão do sono perturbado, déficit no autocuidado e controle familiar ineficaz do regime terapêutico. As metas foram: Obter dieta e ingesta hídrica adequada; Realizar as atividades físicas e atividades de interação social; Tentar diminuir a dependência do medicamento e ajustar os horários de sono; Facilitar a realização das atividades de autocuidado. O plano de enfermagem alcançou a maioria das metas estabelecidas, mesmo que de forma parcial, fato esperado diante das metas propostas e o tempo de implementação. Conclusão: o uso dos Diagnósticos de Enfermagem é tecnologia necessária ao cotidiano de enfermagem, pois possibilita o cuidado integral e se mostra relevante no tratamento domiciliar, com ênfase na promoção da saúde. El plan de enfermería alcanzó la mayoría de las metas establecidas, aunque de forma parcial, hecho esperado delante de las metas propuestas y el tiempo de implementación. Conclusión: el uso del Diagnóstico de Enfermería es tecnología necesaria al cotidiano de enfermería, pues posibilita el cuidado integral y se muestra relevante en el tratamiento domiciliario, con énfasis en la promoción de la salud. O envelhecimento humano é considerado um fenômeno populacional reconhecidamente heterogêneo e multidimensional. Objetivou-se, nesse estudo, investigar a utilização da Avaliação Geriátrica Ampla (AGA) como subsídio para o processo de cuidar em enfermagem a pessoas idosas, em uma Caixa de Autogestão em Saúde, na cidade de Fortaleza-CE. Trata-se de um estudo documental, transversal, de abordagem quantitativa. A amostra foi composta de 70 prontuários de acompanhamento de idosos acima de 80 anos. A coleta dos dados ocorreu, por meio de um formulário semiestruturado, realizada no período de maio a junho de 2015. Constatou-se que 50% dos participantes apresentaram mais de oito anos de estudo. Verificou-se, ainda, que 57,1% são independentes para as atividades de vida diária (AVDs) e que 78,6% dos domicílios estão adequados para redução de risco de quedas. Dentre as comorbidades clínicas existentes, a hipertensão arterial está presente em 22%, seguida de Diabetes Mellitus, com 14,3%. Considera-se, nesse estudo, que a perda funcional ainda se constitui como uma condição de atenção ao cuidado de enfermagem para a população idosa, porquanto pode causar fraturas, síndrome da imobilidade, aumento do grau de dependência, depressão, isolamento social, dentre outras questões referentes ao processo de senilidade, implicando, desse modo, a necessidade de atuação de forma contínua e dinâmica em estratégias de educação em saúde a essa parcela crescente da população. Palavras-chave: Avaliação Geriátrica. Cuidados de Enfermagem.  AbstractHuman aging is considered a population phenomenon that is admittedly heterogeneous and multidimensional. The purpose of this study was to investigate the use of the Ample Geriatric Assessment (AGA) as a subsidy for the nursing care process for elderly people in a Health Self-Management Box in the city of Fortaleza-CE. This is a documentary study, cross-sectional with a quantitative approach. The sample consisted of 70 follow-up charts for the elderly over 80 years. The data were collected through a semi-structured form, carried out from May to June 2015. It was verified that 50% of the participants presented more than 8 years of study. It was also verified that 57.1% are independent for activities of daily living (ADLs) and that 78.6% of the households are adequate to reduce the risk of falls. Among the existing clinical comorbidities, arterial hypertension is present in 22%, followed by Diabetes Mellitus, with 14.3%. It is considered in this study that functional loss is still a condition of attention to nursing care for the elderly population, as it can cause fractures, immobility syndrome, increased dependency, depression, social isolation, among other issues relating to the senility process, implying, therefore, the need to act in a continuous and dynamic form in health education strategies to this growing part of the population. Keywords: Geriatric Assessment. As nurses, we seek to better understand how to gain nursing ‘wisdom’ and apply this wisdom in our daily practice. Yet the concept and experience of ‘wisdom in nursing practice’ has not been well defined. This article addresses wisdom-in-action for nursing practice. We briefly describe nursing theory, review the wisdom literature as presented in various disciplines, and identify characteristics of wisdom by analyzing four models of wisdom from other disciplines. We also present the ten antecedents of wisdom and the ten characteristics of wisdom identified in our analysis of the wisdom literature, discuss and summarize these antecedents, and conclude that understanding these ten antecedents and the ten characteristics of wisdom-in-action can both help nurses demonstrate wisdom as they provide nursing care and teach new nurses the process of becoming wise in nursing practice.

Background The Government’s aspiration to make Kenya a middle-income country and achieve the United Nations' sustainable development goal 3, ‘good health and well-being’, are threatened by poor quality of mental health services. Environment and lack of a conceptual model of nursing to guide care were some of the reasons that were attributed to poor quality of mental health services. The purpose of the study was to discover and describe an appropriate conceptual model of mental health nursing practice. This paper describes the environment which is one of the metaparadigms of a conceptual model grounded on data collected from Kenyan mental health nurses. Methods A grounded theory study was conducted with 33 mental health nurses selected by purposive, open and theoretical sampling procedures. The study sites were level 5 and 6 mental health facilities across the country. Data were collected over a period of 11 months through audio-recorded in-depth interviews and field notes. Analysis was performed using Straussian Grounded Theory steps of open, axial and selective coding processes aided by NVivo version 10. Dimensions and properties of environment metaparadigm grounded on nurses’ views were discovered. The study was conducted within the dictates of the institutional and national ethics and research review boards. Results Environment evolved as an intervening condition and a supra-system for mental health nursing. A homely environment emerged as a space with properties that nurture optimum mental health contrary to a hostile environment that precipitates mental disorders and prolongs recovery. Conclusion Grounded theory methodology was useful in discovering an environment metaparadigm as the context that influences mental health while nursing practice is the central phenomenon for optimum mental health. Nurses can ensure homely environments from diverse cultures and conduct comparative studies on the recovery of patients in the two environments.

Objetivo: A Sistematização da Assistência de Enfermagem é uma metodologia para organizar e realizar o cuidado de enfermagem orientado por princípios científicos. A Clínica Ampliada pressupõe uma abordagem singular do sujeito de forma integral, interdisciplinar e compartilhada. Neste artigo fazemos uma reflexão sobre os desafios que essas duas perspectivas trazem para inovar o ensino de saúde mental, procurando identificar interseções e contrapontos teóricos e formas de apropriação pelos enfermeiros de saúde mental. A articulação entre essas duas abordagens amplia a perspectiva de trabalho do enfermeiro em favor do cliente, valorizando suas potencialidades, transformando o processo de cuidar em algo mais dinâmico, participativo e solidário.
