Introduction: Indonesians have a low intake of dietary fibre, a key component for an increased incidence in constipation. Available data have documented the benefits of polydextrose (PDX) in healthy subjects. However, data on constipated subjects are lacking. This study aimed to investigate the effect of consuming a PDX (prebiotic) beverage on bowel habits and gastrointestinal symptoms of constipated subjects over seven days. Methods: This was a randomised, non-blinded, nonplacebo-controlled parallel design study involving 24 subjects, divided equally into two groups. Group A (active control group) consisted of 12 subjects, consuming one serving size of 6g PDX beverage. While Group B (intervention group) consisted of 12 subjects, consuming two servings of the same product, containing 12g PDX beverage. Changes in bowel habits (constipation score, stool frequency and stool consistency) and gastrointestinal symptoms (abdominal pain, bloating and flatulence) were monitored. Results: Within seven days, Group B showed 4.9% more reduction in overall constipation mean score than that of Group A. Positive improvement in gastrointestinal symptoms were reported: i.e. abdominal pain (∆M = -0.08±0.43), bloating (∆M = -0.29±0.37) and flatulence (∆M = -0.17±0.47). Majority of subjects had desirable stool frequency (87.5%, >3 defecations/week) and stool consistency (58.3%, type 4). These improvements were due to the fact that PDX provides physiological effects consistent with prebiotic fibre, which alters the gut microbiota composition during the fermentation cycle in the large intestine. Conclusion: Findings of this study suggested that daily PDX beverage consumption effectively improved bowel habits, with fewer constipated subjects reporting of gastrointestinal symptoms.

Objective: To assess the association between the Helicobacter Pylori (H.pylori) infection andrecurrent abdominal pain (RAP) by documenting the frequency of Helicobacter pylori infection in children aged 5 to 15years in our setup. Design: A comparative study. Place and Duration of Study: The study was conducted in pediatricunit Military Hospital Rawalpindi over one year period from 1st Apr 200 to 31st Mar 2002. Subject and Methods: Wedivided the patients into two groups; group A or the study group and the group B or the control group. In group A, atotal of 87 Children aged 5 to 15 years presenting with RAP in which no underlying cause was found on initialassessment underwent upper gastrointestinal tract (GIT) endoscopic examination. In group B (control) we included 68children in whom upper GIT endoscopy was done for any reason other than the RAP. In both the groups thegastroduodenal biopsy samples were sent for culture and histological examination for the diagnosis of H. pyloriinfection. The signs and symptoms of these patients were analyzed in detail. Results: In group A, out of 87 patientswho were endoscoped there were 44 female and 43 male. 54(62%) were found positive and 33(38%) were foundnegative for H.pylori on histopathology and culture examination of biopsy samples. In the control group (B) thefrequency of H.pylori infection was 35% (24 of 68) which was significantly lower than the study group (p-value .001).The frequency of H.pylori increased significantly with advancement in age (p-value .003) and lowering of thesocioeconomic status (p-value.002). The commonest endoscopic finding was antral gastritis (31 of 87; 35.6%). Antralnodularity was observed in 8(26%) out of 36 cases of antral gastritis and in 23(74%) cases there was antral erythemawithout nodularity. The remaining endoscopic findings were pan-gastritis (14 of 87;16%), duodenal ulcer (10 of87;11.5%), duodenitis (4 of 87;4.6%) and gastric ulcer (2 of 87;2.3%). The less frequent findings were gastric andduodenal erosions and esophagitis (about 1% each).Their was no abnormal finding in 22(25.3%) cases. Childrenpredominantly (52%) had the symptoms characteristics of ulcer-like dyspepsia. Recurrent epigastric pain wassignificantly more common in the H.pylori positive than the H. pylori negative children (44 of 87 vs 16 of 33 [p-value.001]). There was no difference for other symptom characteristics when comparing infected with non-infected children. Conclusion: The frequency of H.pylori infection is high in children with recurrent abdominal pain and there is a high degree of association between recurrent abdominal pain, endoscopic findings (chronic gastritis, duodenitis, gastric andduodenal ulcer) and the H. pylori infection.

Background and Aim. Distal cholangiocarcinoma associated with choledocholithiasis has not been reported, and the causal relationship remains to be established. We evaluated diagnosis of distal cholangiocarcinoma diagnosed after the removal of choledocholithiasis.Patients and Methods. We assigned 9 cases of cholangiocarcinoma with choledocholithiasis to Group A. As a control group, 37 patients with cholangiocarcinoma without choledocholithiasis were assigned to Group B.Results. Abdominal pain at admission is the only significant difference between Group A and Group B(P=0.001). All patients in Group A had gall bladder stones, compared with 7 patients (19%) in Group B(P<0.01). Of the 9 patients in Group A, endoscopic retrade cholangiopancreatography (ERCP) detected normality in 2 patients (22%) and abnormalities in 7 patients (78%). Of the 32 patients in Group B, ERCP detected normality in 4 patients (13%) and abnormalities in 28 patients (88%)(P=0.597). Intraductal ultrasonography (IDUS) detected a tumor in 8 patients in Group A, while in Group B, IDUS detected normality in 1 patient (3%) and tumors in 29 patients (97%)(P=1.000).Conclusions. IDUS after stone removal may potentially help in the detection of unexpected tumors. Therefore, we believe that IDUS after stone removal will lead to improve outcome and prognosis.

Abstract- This study was conducted to determine the effect of purgative Manna and clofibrate on unconjugated hyperbilirubinemia of term neonates. In this randomized clinical trial study, sixty neonates suffering from unconjugated hyperbilirubinemia were evaluated. The neonates were divided into three groups using balanced block randomization. Group A (control group-received only phototherapy), group B (intervention group-received purgative Manna and phototherapy) and group C (intervention group-received clofibrate and phototherapy). After the intervention, the amount of serum bilirubin reduction was compared between groups. There was no significant difference among group A, B, and C in terms of serum bilirubin reduction in 24, 48 and 72 hours after starting the intervention (P>0.05). The hospital stays in the control group was significantly longer than the intervention groups (P<0.05). No side effects were observed related to using purgative Mienna and clofibrate. The present study showed that prescribing of purgative Manna and clofibrate has no effect on reduction of serum bilirubin level in term neonates with unconjugated hyperbilirubinemia. Thus, it seems that the administration of these drugs is not necessary. Further studies in this regard are recommended.
