The aim of this paper is to assess the quality of services in the Ganeshman Singh Memorial Hospital and Research Center, Lalitpur, Nepal, considering that the quality is a key parameter in performance evaluation. Patients are the main actors in evaluating and judging the quality, therefore this study is based on a questionnaire completed by 53 patients and visitors between January and February, 2020. The results show that services by the paramedical staff (OPD nurses) have the highest mean value (mean = 4.17) which is closed to the excellent performance of hospital whereas the lowest mean was 3.57 rated for the cleanliness of the hospital, including the reception area, OPD and surrounding area. Though, in average (mean = 3.77), facilities of Hospital were rated as ‘Good’ in total. Similarly, the cost of medicine was slightly expensive in comparison of other cost like consultation fee, cost of diagnosis and lab test and registration. The patients had rated the cost of hospital in totality as ‘Good’ status, which indicates that the total cost of treatment was affordable for the patient. With the necessary inputs from the patients and the attendants by pointing various drawbacks or deficiencies should always be taken care of by the hospital administration that will turn into a good result of improvement in the hospital services to the satisfaction of the patients.

Perioperative bleeding occupies a leading place among all surgical complications and, despite the rapid development of surgery, remains relevant to this day. In addition to an increase in mortality, bleeding can cause the development of other postoperative complications, which lead to disability of patients and to a decrease in the quality of life in all age groups. Most perioperative bleeding are caused by technical errors. This article reviews the problem of perioperative bleeding from the point of view of impaired coagulation capabilities of the body. The main etiopathogenetic features of hemostasis during the development of this complication are considered. The analysis of postoperative complications, which were directly or indirectly caused by bleeding during or after surgery, is presented. The prevalence of these complications in various areas of surgery has been demonstrated. More detailed study of the hemostasis system and the identification of predictors of hemostasis difficulties before the surgery may cause an improvement in the results of surgical treatment and reduce the number of postoperative complications and the duration of hospital stay. Accordingly, this will lead to a decrease in the cost of treatment and an increase in patient satisfaction with the medical care. In connection with the above, there is a great interest among surgeons and anesthesiologists in preventing the development of perioperative bleeding.

The article presents the results of the analysis of judicial practice for five years (2013-2017) in civil cases related to defects of dental care. Studies have shown that the most common reasons for patients to go to court are claims to orthopedic dentists (in 42% of cases) and dental therapists (in 31% of cases). The specialty of the doctor is not of decisive importance for the occurrence of judicial risks, due to the fact that the quality of complex dental care is most often assessed (in 45% of cases). The largest amounts of payments, exceeding 92 times the cost of treatment, are determined by dentists-surgeons. The study substantiates the need to develop new assessments at the stages of treatment, criteria for assessing the achieved results of treatment and the development of the practice of insurance of professional responsibility of dentists.

The public urban transport market takes the form of a natural monopoly. In some cases, especially in the situation of supply of a relatively large volume of operation work, the implementation of competitive solutions may result in: improving the quality of services or reducing the cost of vehicle-kilometers. The purpose of the article is to present the determinants of functioning of regulated competition on the public urban transport market in Poland. The conducted analysis focuses on the issue of market openness and access to participation in competitive procedures of private operators. The research covered urban transport markets in cities over 200.000 inhabitants and their characteristics in the scope of: the model of public transport organization (with particular emphasis on the level of opening of markets), the level of competition on the operator market and the method of selecting operators. On the basis of the conducted research, the scope of applying pro-competitive solutions in the scope of contracting services by the largest organizers of public urban transport in Poland was characterized.

The high usage of wireless network will affect the level of data traffic. if there are multiple users to request a connection with the limited capacity of the connection between the user simultaneously then it will wait for their turn in the connect. The simple solution for institutions by adding capacity or bandwidth. But by adding bandwidth, the cost is also quite large. End-to-end monitoring method allows a provider to determine the quality of service QoS using actual data from the availability of services. This information in turn can be useful to know the characteristics of the use of the service, so that the results of monitoring in the form of hotspot service profile may be a reference to determine the user's perception. The final results of this research is profile of network quality service of UGM-Hotspot on PPTIK UGM. Three patients with cryoglobulinemia have been treated with automated plasma exchange procedures in which the replacement fluid was autologous plasma, obtained at a previous plasmapheresis and incubated in the cold to precipitate abnormal protein. All three responded with a reduction in cryoglobulin level and an improvement in clinical manifestations of cryoglobulinemia. The cost of treatment was less than that of conventional plasmapheresis, even though the quality of the replacement fluid was superior. The term “cryoglobulinpheresis” is suggested for this treatment process.

The organization of health care is one of the most complex present day challenges. Like other countries that run socialized health care systems, Icelanders face the question of the role of private enterprise in health care. The objective of this study was two-fold: to compare the cost of 17 private and state-run health care centers in the metropolitan area, and to compare consumer satisfaction related to these. At the beginning of Icelandic settlement, there were statutory laws decreeing that community services should be provided for those in need. By the Health Care Act in 1973, the Icelandic health care system fell under the Nordic welfare society with equal access and a tight safety net. The results show that the private health care centers had a low cost per work unit, but not the lowest. Four to seven state run health care centers had less expenditure per patient than the private centers. The cost of each doctor’s position was highest in one of the private clinics. Patient satisfaction surveys showed that there is no difference in the quality of services between these two different operating modes. A conclusion can be drawn from this study that it is not clear whether private health care improves the use of public funds or increases the quality of services.

The most common problem that the terminals face is inadequate infrastructure, long waiting times, delays, malfunctions, and unexpected problems due to which terminals are often congested for a long time, high costs, lack of expansion, etc. All this leads to reduced quality of services and the price increase for the user service. These problems do not occur only at the terminal of one type, but at all terminals. At present, the delays and uneconomical operations are unacceptable. Intermodal terminals are not immune to any of the above. In order to increase the service quality and to reduce the cost of transportation and transmission, all the bottlenecks in the process are detected and analyzed in the work. The identification and removal of the bottlenecks in the processes of transport and transfer from the initial to the final terminal increases the quality of services provided to customers. The quality of services includes shorter time of transport and transfer by using shorter routes as well as the acceptable price of the service itself.

Cystic fibrosis is a multi-organ genetic disease that reduces life expectancy and requires costly complex therapy. The economic burden of cystic fibrosis is determined by the cost of treatment, the conditions for optimizing medical technologies based on epidemiological knowledge of the disease course nature and the therapy effectiveness. In many countries cystic fibrosis patient registries have been created that allow to assess the quality, efficiency and optimality of medical care. Registries are formed at the regional, national, and international levels, which facilitates the collection of complete and reliable patient information for subsequent analysis, comparison and synthesis of clinical and epidemiological indicators. The review is devoted to the actual principles of organization of domestic and some foreign cystic fibrosis patient registries. The article selectively provides examples of information from registries. The application of positive experience of the work of foreign registries in Russia can improve the quality of medical care for patients with cystic fibrosis.

Mini C-arm image intensifiers are used commonly in surgery of the upper limb. With relatively low doses of emitted ionising radiation, portability and superior quality of image, they are a useful aid to the operating surgeon. However, these benefits are not so often used outside the theatre setting. This paper examines the use of a mini C-arm image intensifier in the out-patient clinic and presents an audit of 100 consecutive out-patients. We reviewed the potential benefits and effects on their care pathway. We also look at the specific radiation protection issues of the mini C-arm image intensifier in the out-patients clinic. We believe use of the mini C-arm image intensifier in the out-patient setting may speed treatment and reduce the cost of treatment.

Treatment of wastewater by the high rate land infiltration system is known as soil aquifer treatment (SAT). SAT is a proven technique for the improvement of wastewater quality. The quality of wastewater produced after SAT is suitable for unrestricted irrigation. The experimental study undertaken at Sulaibiyah proved that the method could be successfully used in Kuwait. The gatch layers within the subsurface region posed no major problems for the infiltration of the wastewater. The groundwater conditions at Sulaibiyah are suitable for the storage of large volumes of wastewater. The cost of treatment by SAT is considerably less than that of conventional methods.
