Annotation. The lack of unified approach or algorithm for expert assessment of the endodontic treatment quality is the reason for registration of various prevalence indicators considering unsuccessful outcomes of pulpitis and periodontitis treatment, misinterpretation of the results and occurrence of problematic situations during the process of forensic dental assessment of iatrogenic dental status changes. Objective – to systematize and analyze the existing problematic aspects of expert assessment of the endodontic treatment quality and identify the most reasoned approaches for objectification of the endodontic interventions consequences. During provided research the calculation methodologies of indexes designed for assessing the consequences and results of endodontic treatment were studied. Additionally, a retrospective analysis of publications with described cases of complex and commission forensic medical and forensic dental examinations of dental care defects was provided, including endodontic treatment cases, cases with errors during endodontic interventions, and also relevant expert conclusions. Revealed that following index indicators could be used as the most expedient approaches for the objectification of endodontic interventions consequences: Peri-Apical Index (PAI), CBCT-PAI, Complex Periapical Index (COPI), Endodontically Treated Tooth Index (ETTI) and PESS (Periapical and Endodontic Status Scale); however, such indexes do not fully assess the quality of iatrogenic manipulations in the structure of the endodont. Thus, for a comprehensive assessment of the derived parameters associated with the dental treatment quality, and quality of endodontic interventions in particular, it is advisable to include in the forensic dental evaluation protocol the main domains, such as technical characteristics, level of patient satisfaction, level of service provided, additional components of the treatment process, and also additional ones, such as verification of experience, quantification of quality of life associated with changes in dental status, and stratification of the effectiveness of treatment.

Objective: The aim of this study was to examine the quality of root canal fillings and their relationship with apical periodontitis (AP) in a selected Turkish population. Material and Methods: The cases in this study was formed of patients attending routine dental treatment at the Faculty of Dentistry, University of Birun. 1024 patients were examined with panoramic radiographs and 1780 endodontically treated teeth with present periapical radiographs. Endodontic treatment quality was evaluated by obturation length, density, taper quality and iatrogenic errors. Presence of AP was analysed according to Ørstavik’s periapical index (PAI). The chi-square test was used for statistical analyses. Results: Statistically significant relationship was found between both the tooth type and location with obturation quality (p: 0.000; p < 0.05). Obturation quality of maxillary and mandibular molars are significantly less than the other tooth types in the same location. Apical periodontitis were noticed in 740 teeth (41.6%). Inadequate obturation quality was found to be significantly effective on the periapical status of tooth types both in maxilla and mandible (p < 0.05). Conclusion: The frequency of inadequate root canal obturations of endodontically treated teeth of the selected Turkish population was high, especially in molar teeth and women. Even though radiographic obturation quality is not the only success criteria in endodontic treatment, it is still one of the most important criteria regarding the positive correlation of “inadequate” obturations with AP.KEYWORDSPeriapical periodontitis; Radiography; Root canal obturation; Root canal theraphy.

People with masticatory discomfort are unable to consume a balanced diet, which impacts their general health. We studied the relationship between quality of life and dental care associated with masticatory discomfort. Data from Korea’s representative 6th Korea National Health and Nutrition Examination Survey (KNHANES) were used. Complex sampling analysis with the stratification variable, clustering variable, and weight was applied. Demographic and dental treatment characteristics and activity limitations were compared through chi-square tests. The comparison of quality of life according to masticatory discomfort was performed using linear regression. The risk of masticatory discomfort was high in people who did not undergo regular oral examinations and preventive and definitive caries treatment and in those who received periodontal, surgical, endodontic, or prosthetic treatments. Generally, people with masticatory discomfort engaged in less activity owing to other disorders like arthritis, rheumatism, and back, neck, and oral disease. People with masticatory discomfort scored low on quality of life. People who received regular oral examinations and preventive care had a low level of masticatory discomfort, and the treated persons had high masticatory discomfort. Therefore, in order to reduce masticatory discomfort, more diverse and active care should be provided for prevention, specifically regular oral examinations.

Subject. Pulp tissue diseases are characterized by a wide prevalence, which makes it possible to consider the quality of endodontic dental treatment one of the most urgent problems of modern dentistry. High-quality treatment of complicated forms of caries is the prevention of odontogenic inflammatory processes, which include periodontitis, periostitis, osteomyelitis, abscess and phlegmon. Therefore, new tools and methods of treatment of complicated forms of caries are proposed, aimed at long-term preservation of teeth as a functional unit of the dental system. Objective: to analyze modern laser technologies in the treatment of complicated forms of caries. Materials and methods. The study was based on the search and study of original articles on the endodontic treatment of complicated forms of caries using laser radiation in the databases: Russian state library, eLibrary, PubMed, the Cochrane Library, Google Scholar. The main selection of materials was carried out by keywords. Results. The literature review describes the positive properties of laser radiation, its mechanism of action, the basics of its use in endodontic treatment, and its application in various branches of dentistry. Conclusions. Analyzing the literature data, we can conclude that the use of laser technologies in the treatment of complicated forms of caries is promising. It is possible to achieve high-quality cleaning of the root canal system and the best long-term results. However, the literature does not sufficiently cover the use of Er, Cr:YSGG laser with a wavelength of 2780 nm in the treatment of purulent pulpitis and pulp necrosis. In addition, taking into account the possibility of using lasers at a therapeutic appointment, it is possible to significantly improve the professional level of a dentist, improving the quality of dental care, reducing the number of complications and preventing various dental diseases.

Evaluation (including within the framework of medico-legal examination) of the quality and efectiveness of endodontic dental treatment is a matter of current interest in the clinical dentistry.Obiectives. Te aim of the work was to develop an algorithm of medico-legal examination in case of endodontic treatment, to test this algorithm in the work of the commissions of medical care quality control in the departments of therapeutic dentistry of outpatient dental clinics.Material and methods.Te material of the study was the reports of daily activities of dental clinics of various forms of ownership (municipal, departmental, private) in 3 regions of Russia (Moscow, St. Petersburg, Rostov-on-Don), including periodic reports and reference reports, ofcial statistical reports of the activities of the examined clinics, including registration books of the work of the commissions of the quality of medical care. Tese materials were used as a source of primary statistical information. Afer grouping of the information contained in them, the main data characterizing organization, condition and quality of endodontic care were recovered taking into account nosological entities: pulpitis and periodontitis. Taking into account the medical, statistical and analytical nature of the study, the main conclusions and recommendations were formulated on the basis of a retrospective study of these materials.Results. Based on the studies carried out and recommendations of the European Endodontic Association on provision of the standard of endodontic treatment, an algorithm for medico-legal examination of endodontic treatment was proposed. Tis algorithm allows to assess the quality and efectiveness of endodontic treatment of pulpitis and periodontitis in diferent terms (immediately afer treatment, remote period)Conclusion. Te use of the developed algorithm in clinical practice in internal quality control of endodontic care has shown its efectiveness. A large number of clinical cases of insufcient quality of endodontic treatment and its low efectiveness in the remote period indicate persistence of the potential for claims from patients.

Compliance with the algorithms of endodontic treatment in most cases ensures the quality of treatment. However, real, long-term and sometimes immediate results that occur in clinical practice are not always successful. In general, the main reason for the unfavorable outcome of dental treatment is an incomplete sanitation of the root canal system. Modern publications by J. F. Siqueira, I. N. Rocas, and D. Ricucci (2019) note that many issues of endodontic treatment are unresolved. It follows that improving the quality of endodontic treatment is still relevant.Aim. To compare two systems of instruments for root canal preparation with different types of rotation.Materials and methods. To identify a tool that will increase the efficiency of root canal preparation, taking into account their anatomy using CBCT, 24 teeth were removed, taken according to orthodontic or pristidintic indications, without previous endodontic treatment. Among them there were 2 incisors, 1 canine, 6 premolars, 15 molars. In total, 60 canals were examined.Results. The shape changes throughout the canal, turning into a round, oval, slit-like, complementing the configuration by the isthmuses between the canals in 13.3% of cases. WaveOne Gold showed a change in cross-sectional shape after preparation, while ProTaper Universal retained a rounded section of 6.67% after preparation of the slit-like channels.Conclusion. Identified priority technique of root canal preparation, allowing to reduce the number of instruments and, as a consequence, the possibility of technological failures in the root canal to reduce machining of the mouth of the channel, retaining dentine playing a major role in the further prosthesis and positive prediction of remote results.

High health indicators, including dental health, are a condition of professional activity of cosmonauts. The assessment of indicators of dental status in members of the squad of astronauts on the basis of the survey on the Map-assessment of dental status of the who in the age groups under 35 years 35 - 44 years old over 45 years old. The values of the intensity of caries and periodontal disease, the quality of light-curing seals and endodontic treatment, the presence of orthopedic structures, including implants are presented. The lower intensity of caries, as well as the prevalence and intensity of periodontal diseases among the members of the cosmonaut team in comparison with the Russian indicators; insufficient quality of previous dental treatment regarding fillings and root canal obturation; low need for treatment of caries and dental prosthetics in connection with timely and quality treatment and replacement of removed teeth in specialized clinics of Moscow after enrollment in the cosmonaut corps, as well as in connection with the real dispensary dental care of cosmonauts.

Abstract Background Shelter-in-place orders and social distancing guidelines, in response to the COVID-19 pandemic, have limited traditional face-to-face interactions and led to many clinical providers transitioning to the use of videoconferencing platforms. The present study aims to assess how the COVID-19 pandemic has impacted adolescents’/young adults’ (AYA) eating disorder (ED)-related care, and how access to, changes in, perceived disruptions to, and quality of care are associated with ED thoughts and behaviors. Methods AYA enrolled in the RECOVERY study, a pre-existing web-based longitudinal study, and completed a COVID-19-specific survey (n = 89). We examined bivariate associations of four markers of care: i) access to care, ii) changes in care, iii) perceived disruption to care, and iv) quality of care. Using multiple logistic regression, we examined the associations of pandemic-related markers of care with changes in ED thoughts and behaviors. We excluded those not engaged in treatment pre-pandemic (n = 16). Results In the remaining 73 participants, reported access to care was high, with 92% of respondents continuing care with at least one ED provider during the pandemic; however, 47% stopped some treatment during the pandemic. Nearly one-third (32%) perceived a disruption in treatment. Quality of care remained high with 67% reporting care to be better than or as good as pre-pandemic. Respondents acknowledged heightened symptomatology: 81% reported increased ED thoughts and 81% reported increased ED behaviors due to COVID-19-related factors. However, none of the markers of care described were significantly associated with ED thoughts or behaviors in regression analyses adjusting for demographic variables and baseline characteristics, except our quality of care measure which was approaching significance (p = 0.07). Conclusions Our findings show the majority of AYA who had care prior to the pandemic continued receiving some element of their multi-disciplinary ED treatment and perceived their care as high quality. None of the markers of care described were statistically associated with increased ED thoughts and behaviors.

Objective: To identify the number of companies providing Do-It-Yourself (DIY) orthodontics and explore information available on websites for DIY brace providers operating in the UK. Design: Web search and review of websites providing DIY braces. Setting: Leeds, UK. Methods: A Web search was completed in November 2020 and April 2021 of all companies providing DIY braces for UK consumers. Each website was evaluated, and the following data collected: name; year started operating; costs; process; involvement of a dental professional; average ‘treatment’ length; retention; consent process; information on risks and benefits; aligner material; social media presence; age suitability; and consumer ratings on Trustpilot. Quality of website information was assessed via the DISCERN tool. Results: Seven DIY orthodontic companies were operating in the UK. Websites reviewed revealed the following: product costs were in the range of £799–£1599, ‘treatment’ length quotes were in the range of 4–12 months; Trustpilot reviews were in the range of 1.6–4.8 stars; and websites claimed their aligners were suitable for individuals with an age range of 12–18 years. Quality of content regarding risks described on websites varied, and there was limited information regarding involvement of a dental professional. Quality of websites information scored ‘poor’ or ‘very poor’ on the DISCERN scoring. Conclusions: There has been an increase in the number of DIY orthodontic companies operating in the UK over the last three years. There is a need to determine whether these products constitute dental treatment in their own right. If so, it is crucial to ensure these are regulated appropriately with adequate information available to satisfy informed consent and have greater transparency over dental professional involvement to safeguard the public.

The research revealed in the paper considers the improvement of secondary treatment of wastewater in the aerobic reactor to provide removal of organics and nutrients. There were five types of polymer biofilm carriers taken into account initially; however, two of them were decided not to apply due to technological reasons. The main part of the research was divided into three substages to investigate each type of biofilm carrier. According to the literature review, the optimal efficiency may be reached if the carrier filling ratio is 10 to 30% of reactor volume. On this basis, there were three benches launched at each sub-stage with a corresponding filling ratio of 10, 20, and 30%. The fourth reactor at each sub-stage had no floating carrier to control the experiment. The research of all three types of carriers showed the effect of BOD removal in the range of 95–96% for benches equipped with a floating carrier, which can be considered similar to the control bench with the efficiency of 92%. In the case of ammonia nitrogen, the removal control bench showed only 55% of efficiency, while floating carriers helped to increase the efficiency up to 70–86%. Despite obtaining relatively positive results, the research has to be continued to achieve regulation requirements in treatment quality.
