Background: Psychological factors can be associated with allergic disorders. However, no particular personality pattern associated with these disorders has yet been identified. Objectives: This study aimed at comparing the personality profiles of patients with perennial allergic rhinitis and normal controls. Materials & Methods: This cross-sectional study was done on 50 adult people with persistent allergic rhinitis for at least one year and 50 age- and sex-matched normal controls that completed the Minnesota Multiphasic Personality Inventory-2 (MMPI-2). A multivariate covariance analysis (MANCOVA) was carried out to discover possible personality differences between the two groups. The data were analyzed by SPSS v. 19 software. Results: The results of the MMPI showed no significant difference between the two groups in the validity scales (P=0.29). The most common modal validity presentation style in both groups was neutral self-presentation. The MANCOVA analysis showed significantly high scores on the hypochondriasis (F=18.13, P<0.005, partial η2 =0.16) and hysteria scales in allergic rhinitis subjects (F=8.68, P=0.04, partial η2 =0.08) compared with normal controls. The most common profiles with high scores in the allergic rhinitis subjects were schizophrenia (52%, P<0.01) and hypochondriasis (50%, P<0.01). These scales emphasized the feelings of isolation and discomfort in allergic people. The frequency of psychasthenia scale, which measures long lasting anxiety, was significantly different between the two groups (40% in allergic rhinitis subjects vs. 10% in normal controls, odds ratio=6 [95%CI 1.9-22.3], P<0.001). Urgent care (UC) is one of the fastest growing venues of health care delivery for nonemergent conditions. This study compared the patient experience during virtual and in-person urgent care visits. We conducted a cross-sectional study of patients with the same diagnosis during Virtual Urgent Care (VUC) and in-person UC visits with the same diagnosis with regard to time and cost over a period of one year. We recorded and analyzed 16 685 urgent care visits: In-person UC (n = 14 734), VUC (n = 1262). Significant differences were found in the average total time for a visit in an in-person UC (70.89 minutes), and VUC (9.38 minutes). The average total cost of VUC ($49) and in-person UC ($142.657) differed significantly. Significant difference was found between UC turnaround time and VC turnaround time (Dependent variable (DV): 53.77, P < .01). We found significant differences in cost and time between in favor of virtual visits. Our findings suggest additional policy reform to expand the use of virtual care among target populations to improve access, reduce costs, meet the needs of patients, and reduce emergency department visits.

Objective: To compare the efficacy of LigaSure Vessel Sealer in Near Total Thyroidectomy versus Conventional Clamp Knot Tie Technique in terms of bleeding, operative time and postoperative drainage. Methodology: This comparative cross sectional study was conducted at Department of Surgery, Liaquat University of Medical and Health Sciences, Jamshoro. Study duration was one year from November 2019 to October 2020. All patients of any age with benign multinodular goiter and either of gender were included. The study subjects were grouped into two categories by randomization (odd / even). The odd numbers were given to patients operated for ligasure and even numbers were given to patients operated with conventional clamp knot tie technique.  Outcomes were observed with respect to post-operative calcium level, intra-operative bleeding, operative time, post-operative pain & post-operative hospital stay. All the data was recorded via study proforma. Data was analyzed by using SPSS version 20. Results: Total 55 patients were observed. Mean age was 33.25±10.60 years in clamp knot tie procedure group and 35.16±07.96 years in ligasure technique group; without significant difference (p-0.448). Pre and post-operative calcium levels were statistically insignificant among both groups (p-0.358 and 0.163), while loss of blood, hospital stay, post-operative pain and operative duration were significantly greater in clamp knot tie technique group in comparison to ligasure technique group (p-<0.001). Conclusion: LigaSure Vessel Sealer is a feasible and reliable surgical technique and significantly more effective as compared to conventional clamp knot tie technique in terms of post-operative bleeding, operative time, post-operative pain and post-operative hospital stay. However, calcium level was statistically insignificant.

BACKGROUND: Bariatric surgery is considered the only effective method to treat refractory obesity, and especially for those in which clinical treatment was not successful. However, the appearance of food intolerances and clinical manifestations are quite common. AIM: To identify food intolerances and associated them to symptoms in patients undergoing Fobi-Capella technique without gastric ring. METHODS: This was a cross-sectional study of adult patients who had more than one year after surgery. Demographic, anthropometric, weight and preoperative height data were investigated. Nutritional status was classified according to the criteria established by the World Health Organization. It was considered food intolerance the presence of nausea, vomiting, diarrhea or bloating after eating a particular food. RESULTS: The sample consisted of 61 patients who attended the nutritional consultation of which 26 (42.6%) had food intolerance, mostly related to red meat (n=12; 34.3%) during the first six months of operation; there was a significant difference between the periods between 0 and 6 months, and 7 to 12 (p=0.02). Among the symptoms reported by patients, nausea was the most recurrent until the 6th month, but without significant differences between the two periods (p=0.06). CONCLUSION: The Fobi-Capella procedure without gastric ring promoted high frequency of intolerance to meat in general, especially for the red, chicken and fish, on this sequence; nausea was the most frequent symptom. These data suggest the need for adequate nutritional monitoring throughout the postoperative period.

Background and objectives: Although laparoscopic sleeve gastrectomy (LSG) is effective for obesity management, postoperative vitamin B12 (B12) deficiency is of major concern. In this cross-sectional study, we assessed the levels of B12 and its related functional biomarkers, namely, total homocysteine (tHcy), methylmalonic acid (MMA), folate, methylcitric acid (MCA), and hemoglobin (Hb), in one-year postoperative LSG patients and matched controls. Materials and Methods: Plasma B12, tHcy, MMA, folate, and MCA were measured in matched controls (n = 66) and patients (n = 71) using validated liquid chromatography-tandem mass spectrometry techniques and protocols in the United Arab Emirates (UAE). Results: The median B12 concentration in patients (177 pmol/L) was significantly lower (p < 0.001) than in the controls (334.7 pmol/L). The tHcy and MMA levels were significantly increased (p < 0.001 and p = 0.011, respectively) and folate levels were significantly decreased (p = 0.001) in the LSG patients compared to the controls. Interestingly, no significant difference in MCA levels were observed between the two groups. The levels of tHcy and MMA were concomitantly increased with the decreased folate levels in postoperative LSG patients when compared with the controls. The Hb levels were significantly lower in males and females in the patient group compared with those in the control group, respectively (p = 0.005 and p = 0.043). Conclusions: This is the first report of serum levels of B12 and its functional biomarkers in postoperative LSG patients among a local population from the UAE. Our findings revealed significant alterations of the B12 biomarkers, total B12, MMA, and tHcy in one-year postoperative LSG patients.

Abstract: Theoretically each profesion has a certain preferential personality. Albeit, it does not mean that to become a medical doctor (MD) needs a special certain personality.  However, it is suggested that some personalities be identified which are non supportive (high risk for dificulties) of the learning process to become a medical doctor. The aim of this study is to find out the personality profiles of students of the Medical Faculty of the  University of Sam Ratulangi (UNSRAT) that have not for some reason, finished the academic stages for more than six years by using Minnesota Multiphasic Personality Inventory-2 (MMPI-2). Methods: This was a cross-sectional study. Data were collected by using questionnaires and evaluation forms of the MMPI-2. We selected 34 students that had not passed the academic stages, but had been students of the Medical Faculty UNSRAT for more than six years. The result showed that based on sociodemographic, academic, and MMPI-2 profile characteristics, 47.06% of the students had specific non-supportive personalities. Meskipun demikian, perlu kiranya untuk dapat di identifikasi berbagai aspek kepribadian yang kurang mendukung (berisiko tinggi untuk terjadinya kesulitan) dalam menjalani pendidikan dokter. Tujuan. mengetahui profil kepribadian mahasiswa Fakultas Kedokteran Universitas Sam Ratulangi yang belum menye-lesaikan studi tahap akademik lebih dari enam tahun berdasarkan Minnesota Multiphasic Personality Inventory-2 (MMPI-2). Metode. Penelitian menggunakan rancangan cross-sectional. Pengumpulan data menggunakan kuesioner dan lembar penilaian MMPI-2 terhadap 34 orang mahasiswa yang belum menyelesaikan studi akademik selama lebih dari enam tahun. Hasil. Dari penelitian berdasarkan karakteristik sosiodemografi, karakteristik akademik dan karakteristik profil MMPI-2 didapatkan bahwa 47,06% mahasiswa memiliki beberapa ciri kepribadian spesifik yang kurang suportif. Background Dysmenorrhea is a common gynecological symptom reported in adolescent girls. Prevalence of the condition has been reported to be 45 - 75%. Absenteeism from work and school as a result of dysmenorrhea is common (13 - 51% of women have been absent at least once, and 5 - 14% are often absent due to the severity of symptoms).Objective To compare school performance in pubertal adolescent girls with and without dysmenorrhea.Methods This cross-sectional study was conducted in June 2010 in adolescent females aged 12 - 18 years from the Musthafawiyah School, Mandailing Natal district, North Sumatera. Adolescent females with and without dysmenorrhea were recruited for this study. All participants completed questionnaires including age of menarche, length of menstrual cycle, length of bleeding, number of sanitary napkins used daily and school absences. School reports from two consecutive semesters in one year were used to evaluate subjects’ academic performance. An academic score of higher than 7.5 was considered good performance while scores of less than 7.5 were considered poor. We used the chi-square test to analyze differences in school performance between girls with and without dysmenorrhea.Results One hundred and sixteen participants were divided into 2 groups, those with and without dysmenorrhea, of 58 subjects each. We found no significant difference in school performance between the two groups, P=0.176 (95% CI -0.009 to -0.048 and P=0.08 (95%CI -0.052 to 0.024).Conclusion There was no significant difference in school performance of girls with and without dysmenorrhea.

Background/Aim. Many studies investigated association between stress, anxiety or personality traits and sleep bruxism (SB), but results are still contradictory. We aimed to investigate whether there is a relation between clinically diagnosed sleep bruxism and salivary cortisol levels as one of the major stress biomarkers and to examine psychological factors and personality traits specific to sleep bruxism. Methods. A total of 23 sleep bruxism patients and 42 healthy non-sleep bruxism adults participated in this study. Diagnose of sleep-bruxism was assessed by selfreport and clinical examination and also confirmed by bedpartner. Morning saliva was collected from all participants for analyses of the cortisol level. Sleep bruxism patients underwent a psychodiagnostic personality interpretation using the Minnesota Multiphasic Personality Inventory ? MMPI-202 test. Results. Statistically significant difference between levels of morning salivary cortisol in the group of SB patients and the control group was recorded (t = 2.943, p < 0.01). Analysis of the personality profiles indicated that the sleep bruxism patients avoid contact with unpleasant feelings, especially depression, suppress the aggression and censor the expression of anger and rage. Conclusion. This study showed that patients with sleep bruxism have higher levels of salivary cortisol. Personality traits such as depression, hypomania and suppressed aggression were found to be common characteristics in patients with sleep bruxism. Present findings might support the hypothesis that sleep bruxism and psychological states such as stress may be related, but the cross-sectional nature of this study does not allow us to draw conclusions about the causal relationship between stress, personality traits and sleep bruxism.

Introduction: Prematurity may be related to the early onset of obesity and metabolic syndrome in adolescence. Breastfeeding and feeding are crucial factors in the genesis of cardio metabolic risk. Objective: To analyze the relationship between the type of breastfeeding and eating habits with the blood pressure, lipid, glycemic and anthropometric profile of adolescents born prematurely. Methods: Cross-sectional study with 50 adolescents born prematurely in western Paraná, Brazil, aged 10 to 19 years. Data on birth, breastfeeding and feeding were evaluated using the 24-hour Food Consumption Marker. Weight, height, abdominal circumference (AC), blood pressure (BP) were verified; concentrations of glucose, total cholesterol (TC) and triglycerides (TG) were measured by capillary puncture. Data analysis using descriptive statistics and analysis of variance. Results: Out of total, 78%   eat in front of screens and 52% do not take the main meals during the day. Regardless of the amount of meals a day, the lipid, glycemic and AC profiles did not show a statistically significant difference between the groups. There is a statistically significant association between BP and number of meals (p = 0.01), TC and breastfeeding (p = 0.03) and TG with consumption of sausages (p = 0.02) and products rich in carbohydrates (p = 0.01). Most of them (72%) consumed cow’s milk before completing one year and only 30% received exclusive breastfeeding until the age of six months. Related other values, 30% had high BP, 22% and 41% high TC and TG, respectively. Of the 30% overweight, 60% had high BP, 53% high TG, 33% high TC and 33% percentile AC ?90. Conclusion: Breastfeeding did not influence the metabolic profile, but it was evidenced as risk factors for adolescents to develop future cardiovascular problems due to prematurity, inadequate eating habits, overweight, abdominal circumference, and alterations in both blood pressure and lipid profile.

<b><i>Background:</i></b> Intranasal steroid (INS) is the most effective medication class for controlling allergic rhinitis (AR) symptoms; however, its effectiveness is limited by patient compliance. Previous studies have explored INS use, compliance, satisfaction, and experience. There is, however, no Asian study on these factors in entirety. <b><i>Objective:</i></b> We aimed to investigate the rate of compliance to usage of INS and explore the reasons for noncompliance in our local population. <b><i>Methods:</i></b> We conducted a prospective cross-sectional study on 65 AR patients in a tertiary hospital. Recruited patients were administered a questionnaire to collect data about symptoms, INS use, and concerns they may have. Statistical analysis was performed using SPSS. <b><i>Results:</i></b> The overall compliance rate to INS was 63.1%. Noncompliance was associated with increased frequency of dosing (<i>p</i> = 0.050), presence of sensory attributes (<i>p</i> = 0.041), and forgetfulness (<i>p</i> = 0.049). The top 3 most frequent sensory attributes experienced by patients include throat rundown (29.2%), aftertaste (21.5%), and immediate taste (20.0%). There was a significant difference between brands of INS with regard to sensory attributes experienced (<i>p</i> = 0.003) but no side effects (<i>p</i> = 0.070). <b><i>Conclusions:</i></b> Identifying risk factors for noncompliance to INS can help health-care providers address difficulties faced by patients and hence increase compliance, allowing better control of AR symptoms.

ABSTRACT Migraine is a chronic condition with complex pathophysiology. Although immunologic disturbances have been linked to migraine, only few attempts have been made to assess the possibility of allergic rhinitis as a worsening factor of migraine in adults. Objective This survey aimed to compare migraine disability between adult migraineurs with and without current allergic rhinitis. Methods This cross-sectional study comprised 118 adult migraineurs who were consecutively assisted at an outpatient clinic. After ordinary neurological evaluation, participants were evaluated for headache disability using the Migraine Disability Assessment (MIDAS). The presence of current allergic rhinitis, allergic rhinoconjunctivitis and seasonal allergic rhinitis was scored for each participant according to the International Study of Asthma and Allergies in questionnaire. Results There was no significant difference between the MIDAS scores of those with current allergic rhinitis, allergic rhinoconjunctivitis, or seasonal allergic rhinitis and nonatopic migraineurs. The disability caused by allergic symptoms also did not influence the MIDAS scores of patients with allergic rhinitis. The frequency of headache days during the last three months was higher in the subset of patients without allergic rhinitis (median and interquartile range 12 [8–19.2] vs. 8 [4–14]; p = 0.03). Conclusions Our results suggest that current allergic rhinitis, allergic rhinoconjunctivitis, and seasonal allergic rhinitis are not related to headache disability in adults with episodic migraine.
