scholarly journals Investigation of Nutritional Status of Hemodialysis Patients in Istanbul/Turkey

Background: This study aims to determine protein, sodium, potassium, phosphorus consumptions and energy intake in endstage renal disease (ESRD) patients undergoing hemodialysis 3 times a week and compare these quantities with recommended requirements. Methods: This study was conducted with 198 end-stage renal disease patients between the ages of 20 and 65 who underwent hemodialysis 3 times a week. Data were obtained via a survey form and Food Frequency Questionnaire. Results: No significant relationship was identified between gender and protein consumption (p>0.05). However, a significant relationship was found between the body mass indices (BMI) and protein consumption of the patients (p<0.05); when groups were compared, protein consumption reduced while the BMI increased (p<0.01). When age and protein consumption were examined, protein consumption was found to be higher in patients aged 20-44 as compared to patients aged 56-65 (p<0.05). Conclusion: The protein consumption and energy intake of hemodialysis patients appears to be below the recommended level. This suggests that hemodialysis patients are receiving insufficient nutrition in Istanbul/Turkey. Hemodialysis patients should be given better nutritional education and their nutritional status should be optimized.

Abstract. Malnutrition is not uncommon in patients with end-stage renal disease treated with maintenance dialysis. The presence of several abnormal parameters of nutritional status are reported to be predictive of poorer outcomes in these patients, compared to dialysis patients without evidence of malnutrition. This article describes methods that may be used to recognize the presence of malnutrition in end-stage renal disease patients and the management of protein and energy intake. Whether the correction of malnutrition will improve outcomes, such as morbidity and mortality, is unknown.

In end-stage renal disease patients, the leading causes of mortality are of cardiovascular (CV) origin. The underlying mechanisms are complex, given that sudden heart failure is more common than acute myocardial infarction. A contributing role of oxidative stress is postulated, which is increased even at early stages of chronic kidney disease, is gradually augmented in parallel to progression to endstage renal disease and is further accelerated by renal replacement therapy. Oxidative stress ensues when there is an imbalance between reactive pro-oxidants and physiologically occurring electron donating antioxidant defence systems. During the last decade, a close association of oxidative stress with accelerated atherosclerosis and increased risk for CV and all-cause mortality has been established. Lipid peroxidation has been identified as a trigger for endothelial dysfunction, the first step towards atherogenesis. In order to counteract the deleterious effects of free radicals and thereby ameliorate, or delay, CV disease, exogenous administration of antioxidants has been proposed. Here, we attempt to summarize existing data from studies that test antioxidants for CV protection, such as vitamins E and C, statins, omega-3 fatty acids and N-acetylcysteine.

Background: The mean arterial pressure serves as an expression of blood pressure in patients on chronic hemodialysis. Serum calcium phosphorus product is considered as a risk factor of vascular calcification that is associated with hypertension in the patients of end stage renal disease. The literature regarding this relationship is inconsistent therefore this study is designed to determine the correlation between calcium phosphorus product and mean arterial pressure among hemodialysis patients with end stage renal disease. Methods: A total of 110 patients of end stage renal disease on hemodialysis for at least one year, 20 to 60 years of age were included. Patients with primary or tertiary hyperparathyroidism, peripheral vascular disease, malignancy, hypertension secondary to any cause other than kidney disease were excluded. Mean arterial pressure was calculated according to the standard protocol in lying position. Blood samples for estimation of serum calcium and phosphorous were taken and was sent immediately to the laboratory for serum analysis. Results: Mean age was 44.17 ± 10.94 years. Mean calcium phosphorous product was 46.71 ± 7.36 mg/dl and mean arterial pressure was 103.61 ± 12.77 mmHg. The values of Pearson correlation co-efficient (r) were 0.863 for age group 20 to 40 years and 0.589 for age group 41 to 60 years. This strong positive correlation means that high calcium phosphorous product goes with high mean arterial pressure (and vice versa) for both the age groups. Conclusion: A strong positive relationship exists between the mean arterial pressure and calcium phosphorous product and is independent of patients’ age.

The incidence of renal disease is increasing at an alarming rate in Bangladesh. Malnutrition is directly correlated with the prevalence with these diseases. A total of 120 dialysis patients from both sexes having mean age 45±13. 79 years undergoing hemodialysis were enrolled for the study in Khulna city, Bangladesh. Information regarding disease history and associated co morbidities were collected from patient’s files. The personal information, prevalent vices, exercise behavior, dietary modifications and clinical and health status of the patients were recorded by using pre-tested questionnaire. Etiology of the chronic renal disease, complications and biochemical parameters were documented from the case files. Renal failure was more prevalent among male than in female. Diabetes mellitus (31.67%) were the most common etiology for chronic renal failure. Pallor of eyes was the most prevailing sign and the most common symptoms are breathlessness (98%), oliguria (88%), anorexia (67%) and nausea (60%). Nutritional status was assessed by anthropometric measurements using body mass index (BMI), mid arm circumference (MAC) and waist hip ratio (WHR). According to BMI, 43% male are grouped in normal category whereas majority of females (52%) are underweight with no overweight female respondents. 80% male had low adequacy of energy whereas all the female patients had adequacy of energy, carbohydrate and protein on low level. Dietary data was obtained by using 24-hour dietary recall and food frequency questionnaire. Actual intake of calories, protein, fats and minerals was lower than the recommended intake for hemodialysis patients. More than 50 percent of patient on hemodialysis were at risk of malnutrition. Around 32% respondents included fruits as per doctor’s advice whereas water and salted foods were restricted to three fourth of patients to protect kidney. Proper assessment of nutritional status, nutritional management and dietary modification can play a central role in preventing malnutrition of hemodialysis patients. Asian J. Med. Biol. Res. Abstract: Humans need a certain amount of energy in order to support the growth and activity. Energy can arise due to combustion derived from carbohydrates, fats and proteins in foods consumed by the body, therefore to have enough energy one should consume enough and balanced food.Nutritional status is a state of the body that is the final result of a balance between the nutrients into the body and its utilization. Adolescence (10-19 years) is a period that is often prone to nutritional problems, because in this period there is less and over nutrient intake. This study aims to determine the relationship between energy intake and Student’s Nutrition Status of  Faculty of Medical Education, University of Sam Ratulangi Manado Year 2013. This study was an observational analytic using cross - sectional approach. Results of statistical analysis using the Spearman rank test shows that, the value of the correlation coefficient (r) of - 0.234 on IMT and 0.077 on WHR and p value of < α = 0.05 on IMT and 0.514 > α = 0.05 on WHR. From the results it is concluded that there is a significant relationship between energy intake with BMI, while the relationship between energy intake with WHR there is no significant relationship. Energi dapat timbul karena adanya pembakaran yang diperoleh dari karbohidrat, lemak dan protein dalam makanan yang di konsumsi oleh tubuh, karena itu agar energi tercukupi perlu  mengkonsumsi makanan yang cukup dan seimbang. Status gizi adalah keadaan tubuh yang merupakan hasil akhir dari keseimbangan antara zat gizi yang masuk ke dalam tubuh dan utilisasinya.Masa remaja (10-19 tahun) merupakan masa yang sering rentan terhadap masalah gizi, dikarenakan pada masa ini terjadi asupan gizi kurang dan asupan gizi lebih. Penelitian ini bertujuan untuk mengetahui hubungan asupan energi dengan status gizi pada mahasiswa Program Studi Pendidikan Dokter Angkatan 2013 Fakultas Kedokteran Universitas Sam Ratulangi Manado. Penelitian ini bersifat observasional analitik dengan menggunakan pendekatan cross-sectional (potong lintang). Kesimpulan: Hasil analisis statistik dengan menggunakan uji spearman rank menunjukkan bahwa, nilai koefisien korelasi (r) sebesar -0,234 pada IMT dan 0,077 pada WHR serta nilai p sebesar < α = 0,05 pada IMT dan 0,514 > α = 0,05 pada WHR. Dari hasil tersbut disimpulkan bahwa ada hubungan bermakna antara asupan energi dengan IMT sedangkan hubungan antara asupan energi dengan WHR tidak terdapat hubungan yang bermakna.
