Music therapy with couples and families in the United Kingdom: a review

A review of UK sources relating to the use of music therapy in work with couples and families revealed reports on family work with pre-school children in the context of a child development centre, with vulnerable young children in a child and family psychiatric unit, and with adolescents and their carers in clinical settings. There are no reports of music therapy being used in helping couples struggling with relational difficulties, or with managing their special needs children. There is also no mention of therapeutic help being extended to strengthening co-parenting with behaviourally challenging children. Music therapists do not appear to regard couple work as being distinct from family work and this could explain the scarcity of literature relating specifically to couples. It may also be that by limiting the definition of music therapy to rehabilitative work following physical injury the British Association of Music Therapy (BAMT) has limited publications on couple and family work in the UK.

Music therapy is a small, but evolving profession. Numbers of music therapists are increasing, yet little is known regarding the workforce and its employment characteristics. To understand the current profile of the music therapy workforce in the United Kingdom (UK), the British Association for Music Therapy (BAMT) commissioned a national survey of its membership. This survey explores the profile of the UK music therapy workforce in terms of demographics, training and employment characteristics. An online survey was circulated to all BAMT members. Data were analysed using descriptive statistics and thematic analysis of open-ended responses. A total of 374 therapists responded (44% response rate). Following demographic information (including age, nationality and training background), we focus on employment characteristics such as income, types of work, settings, clients and age groups. Supervision and clinical fees are considered as well as commissioning and funding of self-employed and employed music therapy work. As an initial mapping of the current UK workforce, this study offers a pragmatic platform to consider development and strategic priorities and thus to re-vision the future of music therapy in the country. Potential implications for the international music therapy community are also discussed.

The concept of quality of life is a fundamental goal in the practice of music therapy, whether implicitly or explicitly defined in the philosophy of the practitioner. To improve the quality of life is often mentioned as one of the goals for music therapy treatment. However, the definition of quality of life has not always been clearly defined. The purpose of this article is to review the literature concerned with a definition for quality of life in other disciplines and to suggest a model for a more comprehensive definition of the quality of life for use in the practice of music therapy. Anecdotal examples are given to stir the issues toward a more comprehensive definition of the quality of life to be used in music therapy. As the title suggests, this article will not give a decisive answer for the definition of quality of life. However, this article will afford readers a chance to think and revisit their experiences as music therapists and define their own definition for quality of life.

Objective: To determine those patient groupings, based on volume and risk, whose optimal urethral reconstructive management might be provided by a reorganisation of UK reconstructive surgeons. Methods: Between 2010 and 2017, ~689 men/year were enrolled onto an online audit platform collecting data about urethral reconstruction in the UK; this accrual was compared against hospital episode statistics (HES). The available workforce, and where this was based, was collected. Individual and institutional incumbent patient volumes, pathology, surgical complexity and outcomes from treatment were collated to stratify volume/risk groups. Results: More than 90% of all HES-recorded data were accrued, being provided by 50 surgeons at 39 operative sites. Most reconstructive surgery was provided at 10 centres performing >20 procedures/year. More than 50% of all interventions were of a high-volume low-risk type. Of activity, 32.3% was intermediate volume or moderate risk, and 12.5% of men presented for lower-volume or higher-risk procedures. Conclusion: Correlation of detailed volume/outcome data allows the definition of patient populations presenting for urethral reconstruction. Stratification of each group’s management, to optimise the surgical outcome, may be applied to a hierarchical service delivery model based on the complexity of the patient’s presenting urethral pathology. Does current music therapy practice address the goals encapsulated in the UK Department of Health document, Living well with dementia: a national dementia strategy (the Dementia Strategy) published in 2009? A survey elicited the views of clients, family members, music therapists, care home staff and care home managers, about this question by focusing on the relationship between music therapy and the 17 objectives outlined in the Dementia Strategy. The results showed that the objectives that are related to direct activity of the music therapists (such as care and understanding of the condition) were seen as most fulfilled by music therapy, while those regarding practicalities (such as living within the community) were seen as least fulfilled. Although the responses from the four groups of participants were similar, differences for some questions suggest that people's direct experience of music therapy influences their views. This study suggests that many aspects of the Dementia Strategy are already seen as being achieved. The findings suggest that developments of both music therapy practices and government strategies on dementia care may benefit from being mutually informed.

Although there has been a fair amount of research and clinical practice related to neurological music therapy, speech production within a musical context and music psychotherapeutic voice work, we rarely find these practices intertwined. The following article provides history, rationale, definition of practice and theory that provides strong backing for the integration of the models and approaches currently available to music therapists.

This article takes the theoretical and philosophical lens of critical disability studies to critically reflect on the Health and Care Professions Council Standards of Proficiency for Arts Therapists. The discipline of critical disability studies, evolving from disability studies and the disability rights movement, is initially defined before multiple paradigms of disability are introduced as central tenets of these disciplines. The relationship between critical disability studies and music therapy is explored, with reference to seminal publications and the perceptions of music therapy within them. The Health and Care Professions Council Standards of Proficiency are then taken as a source of reflection to attempt to understand the perpetuation of medicalised perspectives in the profession and the potential friction between critical disability studies and music therapy. A selection of the Standards of Proficiency are analysed according to distinct paradigms of disability. Questions are posed to interrogate and contextualise the standards in relation to critical disability studies philosophy. From this critical reflection, a discussion emerges which reflects on the reach of these professional standards and how they might contribute to a continuing, outdated expert-model of music therapy in the United Kingdom. The article concludes by drawing these threads together in a series of recommendations to educators, practitioners and the wider profession.

Mela in the United Kingdom has become, in its short thirty year history, one of the most popular forms of festival entertainment. The word ‘mela’ itself, is based on the Sanskrit, meaning a community gathering or meeting, and in its many forms mela in the UK has remained true to this broad sense of people, families and communities congregating together in an atmosphere of festivity. At its roots, mela in the UK has evolved out of South Asian religious rites and rituals, and can also be seen to be built on South Asian folk and rural culture and traditions. However, at the core of the definition of mela is the notion of a gathering. This is most appropriate here in that it does not necessarily refer to any mono-cultural or religious focus, and is important when we observe how mela has ‘travelled’ and become ‘habituated’ in the UK. Carnegie and Smith (2006) identify Edinburgh Mela as having travelled but in this chapter, whilst recognising the travelled nature of mela that they refer to, we indicate that it is the habituated nature of mela that more clearly identifies its nature and existence in the UK. Therefore, this chapter will document that, after 25 to 30 years, mela in the UK can be seen to be adopting its own traditions and connotations. Moreover, by the very nature of the modern diverse British population, mela is now largely urbanised and many continue to reflect South Asian religious festivals, be they Boishakhi Melas (Brick Lane London), Holi Hai Melas (Oxford) or Eid Melas (Birmingham), but others have lost touch with these roots as the demands of festival and cultural event management and venue availability have led to other requirements taking priority. The focus of the research presented here is concerned with the manifestation of mela in the UK and, in particular, how it has adapted to the various town and city locations in which it is now a fundamental part of the cultural events calendar. The importance of mela in terms of economic impact and tourism may be one reason why mela is popular with local authorities.

Ansdell’s ‘Winds of Change’ paper articulated a distinction in UK music therapy between established ‘consensus’ practice drawing on psychotherapeutic principles and developing or previously hidden ‘community music therapy’ practices based on ecological or social-psychological principles. Writing that addresses different theoretical positions in music therapy (meta-theory) exists from European and American perspectives but far less from a UK perspective. This article considers the view that UK music therapy writing in general has continued along one or other of these paths and that there has been relatively little exchange between them; indeed, that UK music therapists tend to ‘talk past each other’. To explore the matter systematically, this article takes a critical discourse analysis approach to analysing three recent music therapy book reviews. Critical discourse analysis was chosen to identify underlying assumptions (‘ideologies’) that shape thinking and practice, as revealed by language use. Book reviews were identified as texts where reviewers typically engage with authors from different perspectives and in doing so offer potentially rich material for such analysis. The analysis identifies ways in which UK music therapy writing shows signs of stress across a divide between ecological and psychodynamic approaches, with reviewers going to some lengths to reconnect these different positions and so unify a discourse within which ‘fault-lines’ are present. It is suggested that, in the United Kingdom at least, ecological and psychodynamic music therapy writing are becoming more separated as discourses, with a lack of integrated meta-theoretical discussion or examples of shared practice. This inhibits coherent development of the discipline and the effective training of future practitioners. A case is made for greater integration in music therapy writing through both developments in meta-theory and by practitioners sharing examples of cross-theoretical practice.

AbstractThe definition of breeding objectives for farmed red deer in the United Kingdom (UK) was investigated. Options studied include the effects of discounting gene flow and the effect of altering variable costs/income (such as price of food or price received for sale animals). Three different herd structures were investigated. As the discount rate increased the importance of traits measured on the offspring also increased. Traits included in the economic breeding objective included number of calves weaned, carcass weights of stag calves and surplus hinds, hind calf live weight and food consumption for hinds and calves. The breeding objective derived does not include a measure of antler size, as antlers have little/no economic value in the UK market. Suggestions are made for further research.

At present there is no standardised procedure for music therapy assessment in the UK. Music therapists use a wide variety of methods to review their clients' progress. This paper begins by examining some of these different procedures and looking at their advantages and disadvantages. A particular system of analysis developed by the author is then explained in detail. Information about four different clients with severe learning difficulties from this analysis is compared to information gained simultaneously through video-analysis. The results seem to indicate that the author's assessment procedure is an efficient way of collecting information about music therapy sessions with these clients. The paper then looks at whether this same procedure can be used with other types of clients. Suggestions for further investigations are made.
