Carcinoma prostate is a common disease in urological aspect. Diagnosis of prostate cancer has evolved through the past century. In additions to estimations of serum PSA which has more predictive value than diagnostic value. In previous days diagnosis of prostatic cancer relied on three measures - DRE, needle biopsy and open biopsy. With development of medical science different biopsy techniques have evolved for prostatic biopsy. Each techniques has its own merits and demerits. This review article presented here discusses on ultrasonoguided transrectal prostatic biopsy following intraprostatic local anaesthesia. A thorough review literature search was done to resolve theses issues and finally a recommendation is drawn to develop a recommendated policy of biopsy for accurate diagnosis of prostatic cancer. Background: Cirrhosis is a common disease that destroys liver cells, and it has various etiologies. The early diagnosis of cirrhosis can be effective in improving prognosis.Considering the availability and affordability of ultrasound devices throughout the world, in this study we evaluated the diagnostic value of hepatic vein ultrasound examination in the early detection of liver cirrhosis.Materials and Methods: In this study, 45 patients referred to the radiology department of Ghaem Hospital for liver biopsy were evaluated for changes in the hepatic veins under ultrasound with a transducer of 5-7MHz. One piece of the hepatic vein was selected for ultrasound, and the wall of hepatic veins was examined for straightness and uniformity of echogenicity. Subsequently, patients underwent biopsy by ultrasound guide.  Of all the study subjects, eight patients presented mild chronic hepatitis without fibrosis, four patients were diagnosed with fatty liver, and 33 patients had severe cirrhosis or chronic hepatitis with fibrosis.Results: Our results showed that hepatitis patients with or without fibrosis had irregular and wavy hepatic vein wall (impairment in straightness) with non-uniform echo (impairment in uniformity of echogenicity). While patients without hepatitis and cirrhosis, but diagnosed with fatty liver, had a smooth and regular hepatic vein wall with a uniform echo.Conclusion: The present study showed that hepatic vein examination in terms of echogenicity and straightness could be used to diagnose hepatitis, its severity as well as its course towards fibrosis and cirrhosis. [GMJ.2018;7:e1140]

Tuberculosis is a common disease throughout the world, especially in developing countries. The most common form of the disease is pulmonary tuberculosis (PTB). Extra pulmonary tuberculosis (EPTB) is also a growing problem worldwide. The diagnosis of PTB and EPTB is even harder when smears and cultures are negative. Due to its nature, the diversity of clinical pictures, the diagnosis of EPTB is often difficult and late. Advances in rapid diagnostic techniques are urgently required both for the early management of the new cases of TB and for the individuals already infected with Mycobacterium tuberculosis and are at risk of developing disease. In addition to standard TB diagnostic techniques, use of new biochemical surrogate markers like LDH and gamma interferon are new suggested. This study was undertaken to evaluate the diagnostic value and accuracy of serum LDH and gamma interferon for the diagnosis of PTB and EPTB. This crosssectional study was carried out in the Department of Pathology and Microbiology, NIDCH, Dhaka and by convenient and purposive sampling, 131 patients (male 98, female 33) having age range of 20-90 years were enrolled for the study. Serum LDH and QuantiFERON-TB gold (QFT-G i.e. gamma interferon) were estimated and the result was compared with the culture and histopathology results. Performance tests were done. Prevalence was measured at 95% CI. Statistical significance was set at p<0.05. Of 131 patients 89 (67.9%) had pulmonary tuberculosis (PTB) and the rest 42 (32.1%) had extra-pulmonary tuberculosis (EPTB). Level of LDH and gamma interferon in serum of all patients was found significantly high. TB pleural effusion was detected in 64% cases. The sensitivity, specificity, positive predictive values, negative predictive values and accuracy of LDH and gamma interferon wre calculated both in PTB and EPTB. For LDH it was (SEN 89.22%, SPE 85.66%, PPV 88.82%, NPV 86.11%) and for gamma interferon (SEN 93.01%, SPE 91.64%, PPV 93.73%, NPV 91.18%) in PTB; and for LDH (SEN 89.27%, SPE 87.18%, PPV 84.35%, NPV 90.36%) and for gamma interferon (SEN 94.88%, SPE 93.65%, PPV 90.04%, NPV 95.82%) in EPTB respectively. When calculated in combination, the sensitivity and specificity was 100% and 50% respectively in both PTB and EPTB, and positive and negative predictive values of LDH and gamma interferon in combination were 94.00% and 91.58% in PTB and 90.43% and 96.71% in EPTB respectively. The study concludes that the diagnostic accuracy of LDH and gamma interferon is low when single test is done but it is significantly high when done in combination.Bangladesh J Med Biochem 2017; 10(2): 69-74

The centennial of the Azusa Street revivals of 1906 provides us with convenient poles for charting shifts in the landscape of Christian spiritual healing practices during the past century. Alongside unprecedented achievements in medical science, nearly 80 percent of Americans report believing that God supernaturally heals people in answer to prayer. Individuals who need healing, even after trying the best medical cures, readily transgress ecclesiastical, physical, and social boundaries in their quest for health and wholeness. The promise of a tangible experience of divine power, moreover, presents an attractive alternative to seekers disillusioned with what they perceive as the callous materialism of medical science and the religious legalism of traditional Christian churches. This essay calls for new narratives of sacred space that map the ways that pentecostal and charismatic healing practices have proliferated, diversified, and sacralized a growing number and variety of physical, social, and linguistic spaces in the past hundred years. At the turn of the twentieth century, modernist epistemological assumptions that privileged reason over experience encouraged fine intellectual distinctions between the sacred and the secular. In esteeming bodily experience as more trustworthy than disembodied doctrine and in resisting linguistic binaries as culturally constructed, postmodern epistemologies have multiplied the number and range of places available to be endowed with sacred meanings. I argue that boundaries between the sacred and the secular are dissolving at the same time that new boundaries are being established, privileging particular places and defining a new relationship among the United States, the Americas, and the world.

Background: Hepatocellular carcinoma (HCC) is one the most common primary malignancy of the liver and represents the third leading cause of cancer-related deaths worldwide. Incidence rates are highest in East Asia and Sub-Saharan Africa. A number of evidence suggests a possible role of interleukin-6 (IL-6), ?-Fetoprotein (AFP) and Des-?-carboxyprothrombin (DCP) in the pathogenesis of hepatocellular carcinoma (HCC). The high DCP may be related to increase tumour behaviour, such as the presence of vascular invasion and intrahepatic metastasis of HCC cells. Patients and methods: We studied IL-6, AFP and DCP in patients with HCC or in healthy controls. AFP was measured by chemiluminescent immunoassay; Serum IL-6 and DCP were measured by enzyme linked immunosorbent assay in 30 patients with primary hepatocellular carcinoma and 30 normal subjects.Results: IL-6, AFP and DCP were found high in the serum of patients initially diagnosed with HCC (18±9.8), (315.99±594.62) and (26.15±5.01) respectively compared with healthy subjects (4.29±2.10), (3.13±1.27) and (4.25±1.22). A significant positive correlation was found between mean levels of IL- 6 & AFP in HCC (P < 0.05), Combination of IL-6, AFP and DCP improved the sensitivity in diagnosing HCC or predicting future HCC development. Conclusions: IL-6, DCP along with AFP could be considered a promising tumor marker for HCC. DCP is a well recognized tumor marker for the screening and diagnosis of HCC. In particular, the diagnostic value of the test is significantly increased when combined with AFP.Bangladesh Journal of Medical Science Vol.15(2) 2016 p.230-233

Amlapiita is a very common disease of present era and a disease of Annavaha Strotas, and is commonly observed in these days of modernization and industrialization. 30% of the general population is suffering from gastro-oesophageal reflux and gastritis resulting in heartburn. It is very troublesome disease and can give rise to many serious problems if not treated in time. Signs and symptoms of Amlapiita are very similar to gastritis or hyperacidity. According to conventional medical science the most common causes of gastritis are H. pylori infections and prolonged use of Non-Steroidal Anti Inflammatory Drugs (NSAIDS). Gastritis is believed to affect about half of people worldwide. In 2015 there were approximately 90 million new cases of this condition. As people get older the disease becomes more common. It, along with a similar condition in the first part of the intestines known as duodenitis, resulted in 50,000 deaths in 2017. The five procedure of Panchakarma done through several procedures purify the body system by removing morbid Doshas from the body. These purification methods are essential components of the curative management of these diseases that are not controlled by palliative management.
