When breast milk is no longer enough to meet the nutritional needs of infants, complementary foods should be added to their diet. It is a very vulnerable period when malnutrition starts in many infants, contributing significantly to high prevalence of malnutrition in under-5 children world-wide. In Bangladesh, complementary feeding (CF) practices are not satisfactory. The objectives of the study were to look into the feeding patterns of under-2 children and to identify the causes which lead mothers/ caregivers to practice inappropriate CF. This cross-sectional study was done in the Pediatric department of Sir Salimullah Medical College Mitford Hospital, Dhaka and in a private chamber from a district town of Bangladesh from October, 2011 to December, 2011. Four hundred mother-child pairs were enrolled by non-random convenience sampling. Different aspects of feeding practices (age of initiation of CF, type of first complementary food, current main complementary food & its quantity, and frequency of CF) were analyzed. Bottle feeding, fast foods and lack of proper family support were most important barriers (p<0.05). High rate of early initiation of CF was mainly due to mothers’ perception that breast milk alone was not enough (81.8%) and main cause of late initiation was refusal of complementary foods by their babies (48.4%). Feeding practices were mainly influenced by relatives (25%), qualified doctors (15.3%), neighbors (14.5%) & mother-in-laws (13.5%). CF practices are still far from ideal. Strengthening of nutrition education to mothers/caregivers and family members/relatives along with awareness building in the community may change the wrong practices.J Bangladesh Coll Phys Surg 2015; 33(4): 195-201

Timely introduced complementary foods, which are appropriate for age, safe and nutrient dense are vital for child growth and development. The aim of the study was to identify associated factors that hampered timely introduction of complementary feeding practices among children 6-23 months. A community-based cross-sectional study was carried out. The data were collected from March 02-20, 2015 in seven-selected Kebeles of Wolaita Sodo town. Systematic sampling technique was employed to select 623 mothers/caregivers with children. The analysis was done using SPSS version 21. Principal component analysis was used to measure household economic status. Binary logistic regression was used to identify predictors of timely initiation of complementary feeding practice. The proportion of timely (at six months) introduction of complementary feeding was 71.2% [95% CI: 66.4%-75.3%], and 25.5% [95% CI: 21.7%- 30%] of children were experienced delayed introduction of complementary feeding. Variables like maternal education [AOR=0.33; 95% CI (0.14-0.78)], household economic status [AOR=0.40;95% CI(0.17-0.85)], history of child medical illness [AOR= 0.63; 95% CI (0.40-0.98)], early initiation of breastfeeding [AOR=2.59; 95% CI (1.02-7.35)] and having source of information about child feeding practices [AOR=0.41; 95% CI (0.24- 0.73)] were identified as independent predictors of introduction of complementary foods at the age of six months. The study came up with nearly three-fourth of children were initiated complementary food just at six months of age. It is not much higher than the target of the current national health sector development program. Maternal illiteracy, low socioeconomic status, presence of medical illness and lack of source of information about complementary feeding practices were associated with timely initiation of complementary feeding. Unreserved support and encouragement should be made to raise level of timely introduction of complementary foods and to diminish delayed introduction of complementary food.

Background: Adequate nutrition during infancy and early childhood is essential to ensure the growth, health, and development of children to their full potential. Complementary feeding is a process starting when breast milk alone is no longer sufficient to meet the nutritional requirements of infants, and therefore other foods and liquids are needed, along with breast milk. The objective was to study complementary feeding practices and the various factors influencing them in urban and rural areas.Methods: A community based cross sectional study was conducted in Urban and Rural area of Mangalore Taluk. Data was collected by interview method among 408 mothers using a predesigned pretested questionnaire, information regarding demographic profile, socio-economic status, complementary feeding practices, etc was collected.Results: As many as 186 (45.3%) i.e., 129/186 (69.3%) mothers in the rural area and 57/186 (30.6%) mothers in urban area started complementary feeds at the age of 6 months. The most common food preferred as complementary food was combination of rice and dal together. The number of meals per day given to the child varied from 2-4/day. The number of snacks given per day to the child varied from 1-4 /day; commonly preferred snacks were Biscuits both in urban and rural areas Bottle feeding was practiced by 181 (44.4%) of the mothers, that included 113/181 (62.4%) from rural area and 68/181 (37.5%) from urban area.Conclusions: Poor complementary feeding practices were observed both in rural and urban areas. Family member’s advice, poor knowledge and influence of baby food marketing strategies have resulted in inappropriate practices.

Background Appropriate complementary feeding has the potential to prevent 6 percent of all under five deaths particularly in developing countries. However, infant and young child feeding practices in Ethiopia are suboptimal. Data on complementary feeding practices in Horro district are also lacking. Therefore, this study aimed to determine magnitude and determinants of appropriate complementary feeding practice among mothers of children age 6–23 months in Horro district, Western Ethiopia. Methods Community based cross sectional study was conducted from February to March 2016 in six kebeles of Horro district, Western Ethiopia. A total of 325 mothers of children age 6–23 months were included in the study. Data were collected using pre-tested interviewer administered structured questionnaire; entered into EPI-INFO version 3.5.4 and analyzed using SPSS version 20. Odds ratio was calculated with 95% CI to identify determinants of appropriate complementary feeding practice. P-values less than 0.05 were considered as statistically significant. Results The magnitude of appropriate complementary feeding practice in the study area was 9.91%. Lower age of child (6-11months) [AOR: 0.195, 95% CI: (0.045–0.846)], having no formal education [AOR = 0.115:95% CI: (0.002, 0.290)] and giving birth for the first time [AOR = 0.271:95% CI: (0.011, 0.463)] were factors negatively associated with appropriate complementary feeding practice. Conclusion Only one tenth of mothers practiced appropriate complementary feeding. This strongly calls for sustained nutrition education targeting uneducated mothers, mothers who gave birth for the first time and those with very young children to improve the practice.

Background: Complementary feeding refers to food which supplements breast milk and ensures that the child continues to have enough energy protein and other nutrients to grow normally. Complementary feeding is started at 6 months of age, while continuing breast feeding. After 6 months of age breast milk alone is not enough to make an infant grow well.Methods: Cross sectional study conducted among 70 mothers coming to paediatric outpatient department. Data was collected regarding their knowledge and practices of complementary feeding. Mean, frequency table and chi-square test were used.Results: 77.1% women have sufficient knowledge about complementary feeding. 77.1% of mothers give complementary food after 6 months remaining i.e. 22.9% are unaware of age of starting it. All mothers are aware of the complementary food which is to be given to babies. 82.8% mothers properly know how to start with complementary feeds. 98.5% of mothers continue to breast feed their baby’s above 6 month.Conclusions: The study concluded that inadequate complementary feeding knowledge and practices were present among some study subjects.

A descriptive type of cross sectional study was carried out to assess the pattern of infant & young child feeding practices by the rural mothers of Bangladesh with a sample size of 450 during the months of January, 2015 to December, 2015. Out of 450 children, 51.1% are female & 48.9% are male. The proportions of infants with early initiation of breastfeeding (13.6%) and exclusive breastfeeding fewer than six months (57.3%) and infants who received complementary feeding at the age of 6-8 months (55.7%) were low. It showed that 90.4% of mothers have knowledge on exclusive breast feeding but only 57.3% of them have practiced it for 4-6 months. It further revealed that only 8.4% of mothers & 13.6 % of fathers are illiterate. The main problems revealed from the study were late initiation of breastfeeding, low rates of exclusive breastfeeding and inappropriate complementary feeding practices. It further revealed that 69.3% of the respondents had two or less children and only 30.7% had three or more children.Medicine Today 2016 Vol.28(1): 1-5

Early exposure to aflatoxins through complementary food is linked to impaired growth in childhood. The current study assessed the household’s practices on management of complementary foods in relation to the risk of aflatoxin exposure and poor nutritional status among infant and young children in Tanzania. A cross-sectional study of complementary feeding practices, aflatoxin exposure and nutritional status was conducted to 101 infants and young children aged between 6-23 months in Dodoma region of Tanzania. The intake of complementary food was estimated by using repeated 24 h dietary recall. Flour used as complementary food was sampled from each of the 101 families and aflatoxins were analysed using high-performance liquid chromatography. A deterministic approach was used to estimate dietary exposure of aflatoxins in the complementary foods. Anthropometric measurements were taken and rates of stunting, underweight and wasting estimated according to the WHO standard procedures. Multivariate logistic regression analysis was used to assess the association between feeding practices and aflatoxin exposure or the growth performance among subjects. The average consumption of complementary flour was 118 g per child per day and 52% of the flours contained groundnuts. AFB1was detected in 42.5% of the flour and levels ranged from 0.3 to 2,128.0 μg/kg (mean 228.11±49.84 μg/kg). Dietary exposures of aflatoxin B1ranged from 0.1 to 23,172.81 ng/kg body weight per day (mean 1,337±392.5 ng/kg). Of the subjects, 40.4% (95% CI; 29.8; 50.9) were stunted and significant association was found between stunted growth and dietary exposure of AFB1(adjusted odd ratio (AOR)=5.9; 95% CI: 0.019-0.028). Early introduction of cereal-and groundnut-based complementary foods in Tanzania is associated with high risk of aflatoxin exposure and impaired growth in children. There is need to integrate aflatoxin management measures in the guidelines for Infant and Young Children Feeding of Tanzania.

Background In developing countries, 5-10% of infants suffer from failure to thrive. Adequate feeding is the most crucial factor for optimal growth in early life.Objective To assess the differences in growth velocity at 4 to 6 months of age, based on the infant feeding practices.Methods This cross-sectional study involving 4 to 6 month-old babies from 6 public health centres in Yogyakarta was performed from August to November 2016. Data on body weight, and growth velocity as they related to weight at birth were collected. Subjects were divided into groups according to their feeding practices.Results Of 173 subjects, 130 (75%) infants were exclusively breastfed, 19 infants (11%) were given breast milk and formula, 14 (8%) infants were given breast milk and complementary food (8%), and 10 (6%) infants were given formula and complementary food. The mean growth velocity z-scores by group were as follows: exclusively breastfed 0.04 (SD 1.15) (95%CI -0.16 to 0.24), breast milk and formula -0.61 (SD 0.84) (95%CI -1.01 to -0.21), breast milk and complementary food -0.69 (SD 1.14) (95%CI -1.35 to -0.04), formula and complementary food 0.23 (SD 1.50) (95%CI: -0.84 to 1.31). The mean difference in growth velocity between the exclusively breastfed vs. breast milk and formula groups was 0.65 (SD 0.28) (95%CI: 0.10 to 1.20; P=0.02); vs. breast milk and complementary food was 0.73 (SD 0.32) (95%CI: 0.10 to 1.37; P=0.02); and vs. formula and complementary food was -0.19 (SD 0.37) (95%CI: -0.93 to 0.55; P=0.61).Conclusion Exclusively breastfed have the most optimal growth velocity compared to infants who experience other feeding practices.

Introduction: Lack of knowledge and proper feeding practices contribute to higher childhood morbidity and mortality. The aim of the study wasto evaluate the mother’s knowledge and practices in terms of quantity, quality and timing of complementary feeding in infant and young child.Methods: A hospital based cross-sectional study was conducted between June, 2010 to October, 2011, at Kanti Children's Hospital, Nepal, involving 1100 mothers of children from 6 to 24 months of age who attended outpatient department of this hospital, applying systematic sampling technique using semi-structured questionnaire.Results: Eighty-seven percent of mothers had knowledge about the duration of exclusive breast feeding but only 33.0% practiced it and 21.0% breast fed their children up to 3 months. Eighty-seven percent of mothers knew when to start complementary feeding and 53.27% of mothers used the marketed weaning food. lito alone was offered by 28.27% as complementary food. Though 36.6% had proper knowledge of frequency of complementary feeding, only 33.27% were actually practicing it and 9.9% were offering more frequent thanrecommended. About half of the mothers fed their child with the food of appropriate consistency and 66.0% fed with the appropriate amount. But only 15.82% mothers fed their children with ideal frequency, sufficient amount and ideal quality.Conclusions: There was a knowledge and practice gap of duration of exclusive breast feeding and initiation and continuation of ideal complementary feeding. After approximately 6 months of age, term breastfed infants are increasingly depend on other sources of iron to avoid iron deficiency anemia, due to the depletion of the low concentration of iron in human milk. The appropriate complementary feeding must include a balance composition of foods containing an adequate amount of macro- and micronutrients to avoid iron deficiency anemia. This study aimed to compare the risk of iron deficiency and growth in breastfeed infants receiving commercial fortified complementary foods or home-made. A cross-sectional study was held on April-June 2016 to evaluate infants aged 6-24 months with breast feeding intake for 6 month of life. Complementary feeding practices were determined by questionnaire; an unquantified food frequency and feeding practices questionnaire was used to determine usual food intake. Biochemical assessment of haemoglobin (Hb), serum ferritin (SF) and serum iron (SI) level were measured. Anthropometric were assessed using WHO Child Growth Standard 2005. Statistical analysis used were Chi-square Test. Thirty eight infants were enrolled, mean age of 16.2 (SD 10.5) months. 17 infants consumed commercial complementary foods and 21 infants use home-made. Infants with home-made had lower Hb level, SF and SI  than those receiving commercial complementary food, and had higher risk of underweight, stunted and wasted. Infants with home-made complementary food had lower haemoglobin, serum feritin and serum iron levels than those in fortified complementary food CF; and a higher risk of stunted and wasted than children with commercial fortified CF.
