This study was carried out to evaluate the efficacy, safety and adverse effects of topical isotretinoin 0.05% gel in the treatment of acne vulgaris. This prospective study was undertaken in Skin & VD outpatient department of Rajshahi Medical College Hospital, Rajshahi. One hundred patients with mild tomoderate acne vulgaris were enrolled and were instructed to apply isotretinoin 0.05% gel once daily at night for 12 weeks. Patients were followed up at 2,4,8 and 12 weeks for efficacy and tolerability. Efficacy was measured by counting facial inflammatory and noninflammatory lesions and by grading acne severity. Cutaneous tolerance was assessed by determining erythema, scaling and burning with pruritus.Response was excellent in 80%of cases.None of the cutaneous reactions was severe, all were mild and well tolerated.Nobody had to discontinue the therapy for side effects. This study confirms that isotretinoin 0.05% gel is safe and effective topical therapy for mild to moderate acnevulgaris.TAJ 2016; 29(2): 1-5

This study was conducted to evaluate the efficacy, safety and adverse effects of topical clindamycin phosphate 1% lotion in treating acne vulgaris in a group of Bangladeshi people. This prospective study was undertaken in skin & VD out patient department of Rajshahi Medical college Hospital, Rajshahi. A total of 30 patients, 15 (50%) males and 15 (50%) females between ages 15 and 35 years (mean 18 years) with acne of grades I, II and III of duration <3 years to 10 years (mean 3.33 years) were studied. Clindamycin phosphate 1% lotion was administered twice daily for 12 weeks to each patient. Response was excellent in 73.33%, good in 20%, fair in 3.33% and poor in 3.33% of cases. Clinical adverse effects were noted in 20% of cases where oiliness was in 6.67%, irritation and burning in 3.33%, puritans in 3.33%, erythema i3.33% and peeling in 3.33% of cases. None of the reactions was severe, all were mild and well tolerated and most occurred within the first month of initiation of treatment and resolved with continued use of drug and completely cured after the treatment completed. No body had to discontinue the therapy for side effects. 80% of the total patients had no side effects. Objective: To compare the untoward effects of isoniazid (inh) and placebo as an anti-tuberculosis therapy in tuberculosispatients. Study Design: Prospective study. Setting: Free T.B Clinic, Muhammad Medical College Hospital Mirpurkhas Sindh. Period: June2007 to Dec 2007. Patients, Method and Results: Patients were selected with filling of consent forms from free T.B Clinic. Prospective study ofadverse effects of INH Isoniazid in tuberculosis treatment. Conclusions: It is concluded like other anti-biotics, Isoniazid must be carefully,mentioned in combination therapy of Anti-Tuberculosis Therapy. The major side effects are those giving rise to serious health hazards, requirediscontinuation of the drug and referral to chest physician.

This study was done in the outpatient department of Rajshahi Medical College Hospital from January 2004 to December 2005. In this study the efficacy of topical tretinoin, benzoyl peroxide and oral tetracycline was studied in 50 patients. Forty five patients completed the course of treatment and 5 patients were defaulter. Complete remission was seen in 32 (71.11%), marked improvement in 08 (17.78%) and good response in 05 (11.11%) patients. Application of topical tretinoin cream (0.05%) in the evening, benzoyl peroxide cream (5%) in the morning and oral tetracycline (500 mg bd) before meal was found very effective, safe and cheap in the treatment of comedonal, pustular and pustulo-nodular acne vulgaris.   doi: 10.3329/taj.v19i1.3161 TAJ 2006; 19(1): 10-12

Background: Oral cancer burden in developing countries is huge. In Bangladesh and India incidence were 10.6% and 10.4% respectively on 2018. At the same time worldwide incident was only 2%. Majority patients present at advanced stage and their survival rate is poor. Diagnostic delay from patient side and health delivery system side is significantly longer and it is more in developing countries. Like other malignancy early diagnosis can save a lot of these patient. Methods: This prospective study was carried out in Khulna Medical College Hospital from July 2017 to July 2019. Patients who will meet the criteria for this prospective study and capable and willing to give informed consent were enrolled. 20 second rinse/gargle with 10ml of 1% Tolonium chloride solution was done. Biopsy were taken from stained lesion, biopsy report were compared with TC stained lesions. Results: Out of 1650 patients in our outdoor department for various problem and only 30 cases were selected for this study according to selection criteria. Of 30 patients, 18 (60%) were male and 12 (40%) were female. The male-to-female ratio was 3:2. It was found that the sensitivity of 1% Tolonium Chloride rinse for oral precancer and cancer detection was 83.33%, whereas the specificity was 84.21%. Conclusion: Tolonium chloride rinse is a good screening test for oral cancer diagnosis with sensitivity 83.33% and specificity 84.21%. Bangladesh J Otorhinolaryngol; April 2020; 26(1): 11-17

Cataract surgery is no more a blind rehabilitation surgery, it absolutely gives normal vision. In the era of modern cataract surgery patients expectations are also high about visual outcome. This prospective study was carried out to investigate the magnitude and pattern of pre-existing corneal astigmatism in age related cataract patient at Faridpur Medical College Hospital, Faridpur and Agha Yusuf Adhunik Hospital, Kustia, from July 2009 to June 2012. We examined 850 eyes of 730 patients who underwent cataract surgery. The mean age at the time of surgery was 61.9±8.1 (40 to 70) years. Corneal astigmatism was measured by Auto Refracto Keratometer at least two times for each patient. Astigmatism was calculated from diopteric difference of vertical reading from horizontal reading. With the rule (WTR) astigmatism was considered when steep meridian at 90°± 20°. Against the rule (ATR) astigmatism was considered when steep meridian at 180°±20°. Astigmatism is in other direction is defined as oblique. On keratometry, when vertical reading (k1) was found greater than horizontal (k2) was considered WTR astigmatism and the reverse reading for ATR astigmatism. The percentage of corneal astigmatism was 1D or less was 69.6%, more than 1D and less than 1.5D, 27.6% and more than 1.5D and less then 2D 2.8%. Prevalence of ATR astigmatism was more than WTR astigmatism and prevalence of ATR astigmatism increases significantly with age. Background: Cough variant asthma (CVA) is a subset of asthma where the only symptom is chronic persistent cough. Many cases go unrecognized due to lack of proper evaluation. Response to asthma medication with features supportive of airway hypersensitivity helps in management of this disease. Objective: To find out the proportion of cough variant asthma among the patients attending medicine outpatient department of Enam Medical College, Savar, Dhaka. Materials and Methods: This cross sectional study was conducted in Enam Medical College Hospital, Savar, Dhaka over a period of two years from July 2009 to July 2011. Cough variant asthma was diagnosed mainly on clinical ground as chronic cough without wheezing, fever, weight loss, shortness of breath or sputum or any other apparent cause that persisted for more than eight weeks with absolutely normal physical examination of chest, normal chest radiography and blood count except raised eosinophil count and IgE level. Patients who met these criteria were given 2 weeks course of inhaler beclomethasone propionate and were assessed for improvement. Those who improved after steroid inhalation were categorised as having cough variant asthma. Results: Out of purposively selected 148 patients complaining only of chronic dry cough for more than eight weeks, 92 patients met the primary selection criteria for cough variant asthma. These 92 patients were given 2 weeks trial of 250 ìgm beclomethasone inhalation twice daily. Seventy nine patients reported almost complete recovery from chronic cough after 2 weeks and were categorized as having CVA. Thirteen patients did not improve and were not categorized as CVA. Conclusion: These findings suggest that cough variant asthma is the most common among the patients with chronic cough not due to any apparent cause. The efficacy of inhaled corticosteroid suggests that early intervention is effective in the treatment of this disease. Clubfoot is the commonest congenital deformity in babies. Around 80% of the cases occur in developing nations. There are many options for treating club foot, Ponseti technique is one of them. This observational study was carried out at Chittagong Medical College Hospital, From July 2014 to December 2015 to evaluate the results of idiopathic club foot / feet within 1 year of age by Ponseti technique. Patients with idiopathic congenital club foot (CTEV) is attended at the outpatient department of CMCH. Among the 35 patients, 1 patients 2.85% required 1 plaster, 2 patients (5.74%) required 2 plaster, 3 patients (8.57%) required 4 plaster, 5 patients (4.28%) required 5 plaster, 16 patients (45.71%) required 6 plaster, 5 patients (14.28%) required 7 plaster cast and 3 patients (8.57%) required 8 plaster cast with or without percutaneous tonotomy. The final score after completion of the management (with or without tenotomy) were 0 of 39 feet (75%) which were normal and 0.5 of 10 feet (19.24%) and score 1 of only 3 feet (5.76%) which were better than moderately abnormal.Medicine Today 2017 Vol.29(2): 42-45

Background: Ever since the early reports of human immuno deficiency infection, it presented as wide range of infectious and non-infectious dermatoses which correlate with the degree of immunodeficiency. Skin assessment remains a vital tool in the diagnosis and management of HIV infection due to relative easiness of examination. Most of skin diseases are amenable to diagnosis by inspection and biopsy. In this descriptive study, we have enumerated in detail the dermatological manifestations of Link ART centre clients.Methods: To analyze the dermatological manifestations in people living with HIV, we undertook a prospective observational study of all PLHIV on ART visiting IRT Perundurai Medical College Hospital link anti- retroviral therapy centre (LAC) during July 2015 to August 2016 with focus to skin manifestations.Results: Among the 140 clients, 22 in 2010, 12 in 2011, 6 in 2012, 51 in 2013, 36 in 2014, and 9 in 2015, 3 up to June 2016 enrolled for follow up. 31 were transferred to nearby ART, Link ART centres during this period as per their request, 4 lost for follow up and 6 deceased after enrolment, finally ninety-nine (39 male; 60 female) were utilizing our centre during the study period. Majority of participants were on ZLN (zidovudine, lamivudine, and nevirapine) regimen, hailing from rural zones, belonged to low or low middle income group, were undertaking farming, cattle rearing. CD4 counts of the asymptomatic clients in the ZLN group noted increase and decrease in the ZLE (zidovudine, lamivudine, and efavirenz), TLN (tenofovir, lamivudine, and nevirapine) group.  In the symptomatic clients, decline in the individual and the mean counts except in female ZLN subset. 70% were clinically asymptomatic and 30 % were having some dermatological manifestations. Multiple manifestations were seen in 5 clients during the study period. Dermatological manifestations observed commonly were fungal and viral infections, xerosis/ichthyoses, adverse drug reactions like lipodystrophy, and discoloration of nails.Conclusions: Skin manifestations observed in this study were due to aging and long term cosmetic side effects of highly active antiretroviral therapy  (HAART). Lipodystrophy posed significant aesthetic distress in HAART clients. Hence, early management would decrease the most offending cosmetolgical side effects of the disease and drugs. Therapy yields the declining trend in the inflammatory, infectious dermatoses.
