Road is a vital public facility that connects one region to another region. Hence, the existence of public street lighting is a key to supports people’s activities at night. But not all roads are equipped with public street lighting. For example, the village roads in Sukarame Village, Sajira sub-district, Lebak regency, Banten. This village is fully electrified but the supply for public street lighting is still limited. Therefore, it is neccesary to install public street lighting in the form of solar lamps as an effective alternative for street lighting and public facilities. Solar lamp is chosen because it doesn’t need power supply and the village’s geographical is very potential for utilizing solar lamps. Solar lamps installed in 3 strategic points, which are village road in front of local kindergarten, village road in front of public field, and mosque’s terrace. Besides solar lamp installation, this research also focused in economic analysis of this program. The analysis was done by comparing total cost needed for conventional public street lighting using bulbs that connect to grid with total cost needed for public street lighting using solar lamps. Despite solar lamp’s price is more expensive, for the long-term usage, public street lighting using solar lamps is more economically profitable compared to conventional public street lighting with the cost difference of Rp 636,582.80 per lamppost for 3 years usage period.

The existence of street lighting and or public facilities is one of the things that is very important for the smooth running of residents' activities, especially at night. However, not all of them had lighting installed as in the Al-Falah Serua Ciputat Mosque in South Tangerang. This village is a village that is electrified but electricity supply for public facilities and roads is still minimal, due also to the large cost of using electricity. Therefore it is necessary to install public and street lighting facilities in this case in the form of solar lights as an effective alternative to solutions in lighting and to reduce the cost of using electricity from PLN. The solar lights are very suitable to be installed in this location, considering that the lights do not require electricity from residents' houses and PLN, and geographically the location is very potential for utilizing solar power because it is still very green. Solar lights are installed at strategic points on the road to Al Falah Mosque and the front porch of Al Falah Mosque itself. The lamp used is a HILIOS SL-40W lamp with 40Watt power. In addition to street lighting and public facilities, an economic analysis is also carried out on this activity by comparing the total costs required between incandescent lighting with solar power. Where the nominal difference obtained in rupiah is Rp 311,055.2 lighter than PJU incandescent lamps, and solar lights are not affected by TDL (basic electricity tariff).

Purpose Positive behavioural support has been considered as a valuable alternative to residential care for children and adolescents with learning disabilities and behaviour that challenges. While recent evidence suggests it has a positive impact on behaviour and carer ability to cope, there is little evidence of its economic costs or benefits. The paper aims to discuss this issue. Design/methodology/approach An exploratory cross-sectional study was conducted to evaluate the cost of providing positive behavioural support to ten children and adolescents with learning disabilities and behaviour that challenges living in the community in Ealing, West London. Comparison was also made with the cost estimate of possible alternative support packages for children and adolescents with learning disabilities and behaviour that challenges in the UK, as obtained through a Delphi exercise. Findings Total cost of services per child was £1,454 per week for young people supported short-term, and £1,402 supported long-term. Children and adolescents were making use of a range of social care, education and health services. Over the full sample, half of the total cost was accounted for by education services. The Delphi exercise estimated the weekly cost of residential-based care as more expensive than the cost of community-based care for children and adolescents with learning disabilities and behaviour that challenges. At the end of the ITSBS, all ten children and adolescents initially at risk of imminent residential placement were living in the community with less service-intensive and less expensive support. This suggests that avoiding residential-based care could reduce costs in the long term. Originality/value Positive behavioural support has potential to support people with learning disabilities and behaviour that challenges in the community, leading to potential cost advantages. However, this is a small study and more robust research is needed.

Objectives: The logistics hub construction has always been the short board of logistics network planning in China. In order to improve the decision-making efficiency of logistics enterprise’s hub selection and reduce its comprehensive operation cost, this paper establishes a cost difference model for hub-and-spoke(H-S) and point-to-point(P-P) networks considering the fixed cost of hubs, transportation and route costs based on the 0-1 integer nonlinear programming. The model aims at minimizing the cost difference between the two networks, and divides the fixed cost of the hubs into three situations: full lease, lease and self-built, and fully self-built. Finally, this paper takes tobacco transportation logistics as an example, and use particle swarm algorithm to solve the model by using tobacco transportation logistics data of a logistics enterprise in Jiangsu Province. The results show that: (i) in the case of complete leasing, the total cost of the H-S network decreases with the increase of the number of hubs, and the cost change has a point of intersection with the total cost of the P-P network;(ii) when the lease and self-build are mixed, the increase is first reduced and then increased, it is U-shaped and has a minimum value, and there are two intersections with the total cost of the P-P network;(iii) the situation of completely self-built and fully leased is just the opposite. This paper takes tobacco transportation logistics as a representative, and provides a reference for logistics companies to choose the appropriate regional logistics network structure and different pivot points.

e15601 Background: Cytokines are a first-line treatment option for a subset of advanced RCC patients in the US. After progression on cytokines, NCCN guidelines recommend targeted agents, such as axitinib and sorafenib. Subgroup analysis of post-cytokine patients in the phase III AXIS trial found that axitinib increased median progression free survival (PFS) compared with sorafenib (12.0 vs. 6.6 months, p<0.0001), while overall survival (OS) showed no difference (29.4 vs. 27.8 months, p=0.144). An economic analysis for this subgroup was conducted from a US healthcare payer perspective. Methods: A cohort partition model with monthly cycles was constructed to estimate direct medical costs and health outcomes, discounted at 3.0% per annum, over cohort lifetime. Patients were apportioned into 3 health states (progression-free, progressed and dead) based on OS and PFS Kaplan-Meier curves for the post-cytokine subgroup in the AXIS trial. Active treatment was applied until progression, followed by best supportive care (BSC) alone thereafter. The wholesale acquisition costs were based from RedBook. Adverse event (AE) management costs were obtained from published studies. AE rates and utility values were informed by the AXIS trial. Administrative claims data from MarketScan Database were analyzed to estimate costs for BSC and routine care of second-line advanced RCC patients. Results: The total per-patient lifetime costs were estimated to be $242,750 for axitinib and $168,880 for sorafenib and most of the cost difference (84%) was due to the higher total medication cost of axitinib. The cost difference was sensitive to dose intensity and length of treatment. The difference in quality-adjusted life-years (QALY) for axitinib versus sorafenib was minor (1.3 versus 1.2) and the incremental cost-effectiveness ratio (ICER) for axitinib compared with sorafenib was $683,209/QALY. Conclusions: For cytokine-refractory advanced RCC patients, axitinib resulted in an ICER > $650,000/QALY versus sorafenib due to high drug costs and lack of OS benefit, indicating that axitinib may not present good value for money as 2nd line treatment when compared to sorafenib in the US.

The aim – to conduct clinical effectiveness, meta-analysis of 30 and 120-days mortality data, pharmacoeconomic evaluation of levosimendan treatment compared with dobutamine in patients with severe acute decompensated chronic heart failure (ADCHF) who require inotropic support. Materials and methods. The PubMed and Cochrane databases were searched for direct randomized clinical trials of levosimendan treatment compared with dobutamine in patients with ADCHF. The clinical efficacy of levosimendan and dobutamine was analyzed. Pharmacoeconomic analysis was carried out using the cost-effectiveness method with an assessment of the incremental cost-effectiveness ratio. A decision tree model of levosimendan or dobutamine treatments was constructed. The efficacy endpoints and impact on the budget were analyzed in terms of long-term effectiveness of levosimendan and dobutamine use. Discounted was conducted with rate of 3 %. Sensitivity analysis was carried out in terms of price changing of drugs, the cost of drugs in mg, the likelihood of re-hospitalization of the patient in a 3-year horizon and survival in the long term.Results and discussion. Analysis of clinical data and meta-analysis of randomized clinical trials found that mortality rates with levosimendan and dobutamine in the 30-day period were 9.6 % and 13.8 %, RR 0.71 (95 % CI 0.53–0.95) and in the 120-day period – 13.5 % and 25.2 %, RR 0.54 (95 % CI 0.32–0.92), respectively. The total cost of the course of treatment, taking into account the price of the drug, medical devices, staff services, diagnostic procedures and treatment of adverse reactions when using levosimendan, was 34 003.02 UAH per patient and 18 787.28 UAH when treated with dobutamine. The weighted average hospital stay was 6.4 days in case of levosimendan treatment and 7.5 days of dobutamine treatment. Extrapolation of the data from clinical trials to the 3-year survival rate of patients allowed us to determine an additional indicator of efficacy – the number of life years saved with levosimendan – 2.64 and 2.37 with dobutamine treatment. A cost-effectiveness analysis found that levosimendan is more efficient but more expensive technology compare to dobutamine. The incremental cost-effectiveness ratio for the additional life year saved of a patient with severe CHF is 43,473.55 UAH, which is 6 times less than the likely threshold of willingness to pay in Ukraine.Conclusions. The multivariate sensitivity analysis detected the model sustainability to the most crucial parameters of the model – drug price; the cost of drugs associated with their actual use in mg, the possibility of re-hospitalization of the patient in a 3-year horizon, and long-term survival, which is associated with the time horizon of the model. The total cost of a cohort of patients with ADCHF in Ukraine when using scenario 1 (100 % distribution of costs for dobutamine treatment) over 5 years is 268 188 351.94 UAH, when using scenario 2 (100 % distribution for treatment with levosimendan) total budget costs will be in amount of 485 393 073.09 UAH, if scenario 3 is applied (gradual 5 % transition in the treatment of patients with ADCHF with dobutamine for treatment with levosimendan within 5 years), the total budget costs will amount to 289 916 431.92 UAH

What if electronic voting performs in a rural area that has a limited condition of infrastructure and people who are peculiar with technology such as a computer? This article answers this question. Elections in Indonesia at national, province and district level are still used ballot paper. However, there is a village in Indonesia that use remote electronic voting system as a tool to vote head of the village. This research used the interview as a primary data collection and this research interviewed 17 key informants that directly involved in the village electronic voting. The research found the village voters more convenient in an electronic voting than the ballot paper system and the electronic voting can perform in a place that the people not familiar with modern technology. The unique finding of this research is the cost for electronic village election came from the village head candidates that shared the total cost of electronic voting. The other findings are electronic voting can be performed in an area with limited infrastructures and community that peculiar with technology.hence, it is possible for Indonesia to implement electronic voting in a level that higher than village voting, such as regent or mayor election.

AbstractAs an extension of previous work on the risk of prematurity in singletons and on the social cost of twin births, an analysis has been carried out into the cost effectiveness of preventing premature delivery in twin pregnancies. The cost of prevention is assessed in terms of early diagnosis through ultrasound screening and of an extra 11 weeks of work leave to expectant mothers. When this cost is compared to the social cost involved in the transfer of newborns to neonatal intensity care units and in supporting handicapped children, it is concluded that the total cost of prevention corresponds to one-third of the long-term costs associated to lack of prevention.

This research work is mainly focused on comparative economic analysis of Zero Budget Natural Farming (ZBNF) for Kharif Groundnut under Central Dry Zone of Karnataka. The purpose of conducting the study is to examine the cost of cultivation for ZBNF with other treatments. This is mainly due to farmers are facing high cost of cultivation by practicing conventional farming as it is highly depends on external inputs. ZBNF is low cost technology where, farming is practiced by using jeevamrutha, beejamrutha, mulching and whapsa etc. The Groundnut experiment was conducted at ZAHRS, Babbur Farm, Hiriyur during Kharif 2019-20 in order to estimate economics for different treatments. Each treatment was having different input usage. For calculating economics of each treatment, conventional cost of cultivation method was followed (i.e. calculation of Variable cost, Fixed cost, Gross returns, Net returns and B:C).The results of economic analysis revealed that the per hectare yield (17.46 q/ha) total cost ( 53,019), gross returns ( 88,871), net returns ( 35,852) and B:C (1.68) were found to be highest in RPP among all treatments i.e. ZBNF, OPS and Control. The cost of cultivation of ZBNF 22.55 per cent lesser than RPP. The B:C ratio was found to be more in ZBNF (1.66) than OPS (1.58).This is due to highest cost for purchase of excess quantity of FYM in case of OPS. It is concluded from the study that RPP was best treatment among all treatments. This is due to highest yield, net returns and B:C was found in RPP treatment  as compared to all treatments. In the economic point of view, there was reduction in total cost in case of ZBNF with fewer yields as compared to RPP.

Aims: The programme was undertaken to study the economic analysis of Kamrupa and local chicken in Dhemaji district of Assam under backyard system of rearing. Study Design: The data on various expenses and returns thus collected were tabulated and subjected to statistical analysis as per the methods described by [1].    Place and Duration of Study: The study was conducted in Sissiborgaon, Dhemaji and Jonai development blocks of Dhemaji district during the period January, 2018 to July, 2019 by Krishi Vigyan Kendra, Dhemaji. Methodology: For the purpose thirty numbers of farmwomen from three different development blocks, thus a total of ninety numbers of farmwomen, of Dhemaji district were selected on the basis of their early experience in keeping local poultry along with Kamrupa chicken at backyard system. Items of cost included fixed cost e.g. land and building, equipments and variable costs e.g. cost of day-old chick, cost of feed, vaccine, medicine, labour, depreciation in poultry shed and miscellaneous cost. Return items included eggs, cocks and spent hens. Results: The cost of labour accounted for 72.44 percent of the total cost of production of Kamrupa chicken followed by feed cost (9.79%), chick cost (7.21%) and depreciation of poultry house (6.44%) up to 18 months of age. The total cost of production up to 72 weeks of age was found to be higher in Kamrupa (Rs. 3,882.48) than its local counterpart (Rs. 3,512.48). The maximum amount of income was contributed by selling of eggs (46.60%) followed by sale of cocks (26.76%) and sale of spent hens (26.64) in case of local chicken. The benefit-cost (B:C) ratio in Kamrupa and local chicken were recorded as 2.64 and 2.14, respectively, in the present study. Conclusion: From the study, it can be concluded that the small scale Kamrupa rearing is a profitable venture for farmwomen in the state of Assam.

Based on the improved methodology, an economic assessment of the mowing technology for hay harvesting is given. The total cost (44.8–47.5 thousand rubles/ha) for the creation of early and medium-mature long-term herbage with the dominance of rhizomatous grasses paid off in 1.6–1.8 years. In the next 25 years, annual production costs amounted to 20.1–21.6 thousand rubles/ha with the cost of 1 feed unit of 5.1–5.6 rubles and a conditional net income of 15.1–20.1 thousand rubles/ha.
