BACKGROUND: Neck pain (NP) is common among high performance aircrew, yet evidence remains insufficient to guide examination, treatment, and prevention. The purpose of this randomized pilot study was to collect baseline data for neck function for F-15E aircrew and determine efficacy and feasibility of two separate exercise protocols in measuring short-term outcomes of subjective and objective neck function in order to inform future study design. METHODS: Randomized to either progressive (PRO) or general (GEN) exercise groups were 41 F-15E aircrew. Data collection occurred at baseline, 3 wk, and 3 mo. RESULTS: At baseline, 39% of the subjects reported current NP, 79.5% reported a history of NP attributed to flying, 12.8% reported being removed from flying duties due to NP, and 10% reported receiving medical care for NP. PRO and GEN group randomization showed similar baseline assessment data. Blinding was successful and exercise logs showed 31.6% compliance with prescribed exercise regimens. There were small but statistically significant increases in neck range of motion in both groups over the course of the study. Aircrew with current NP had significantly higher F-15E flight hours. DISCUSSION: This study supports the high prevalence of NP in aircrew, yet low frequency of seeking care for NP. Future studies to assess NP prevention and treatment in aircrew require an integrated approach that includes operational exercise policy and long-term data collection in flying units with dedicated resources for assessment and analysis. Objective. Most research on health problems of children in foster care has been cross-sectional, resulting in overselection of children who have been in care long-term and underrepresentation of children who are in care for a short time. Methodology. This paper reports on the health of a large cohort of children who had complete health examinations at the time of entry into foster care in a middle-size city during a 2-year period. Results. Results indicate that >90% of the children had an abnormality in atleast one body system, 25% failed the vision screen, and 15% failed the hearing screen. The children were also lighter and shorter than the norm. Mental health screening revealed that 75% had a family history of mental illness or drug or alcohol abuse. Of children older than 3 years of age, 15% admitted to or were suspect for suicidal ideation and 7% for homicidal ideation. Of the children younger than 5 years of age, 23% had abnormal or suspect results on developmental screening examinations. At the time of entry into foster care, 12% of the children required an antibiotic. More than half needed urgent or nonurgent referrals for medical services and, for children >3 years of age, more than half needed urgent or nonurgent referrals for dental and mental health services. Just 12% of the children required only routine follow-up care. Conclusions. The high prevalence and broad range of health needs of children at the time they enter foster care necessitate the design and implementation of better models of health care delivery for children in foster care.

Lichen sclerosus affecting the male genitalia is a poorly understood but potentially devastating condition. The natural history of the condition is beginning to be understood better with longer follow-up of patients. Recent long-term data suggest that circumcision for lichen sclerosus limited to the prepuce may not be curative as was once thought. In addition, surgical treatments should be followed up for longer periods as recurrences may occur after urethroplasty and perineal urethrostomy.

Reducing plastic pollution in rivers, lakes, and oceans is beneficial to aquatic animals and human livelihood. To achieve this, reliable observations of the abundance, spatiotemporal variation, and composition of plastics in aquatic ecosystems are crucial. Current efforts mainly focus on collecting data on the open ocean, on beaches and coastlines, and in river systems. Urban areas are the main source of plastic leakage into the natural environment, yet data on plastic pollution in urban water systems are scarce. In this paper, we present a simple method for plastic hotspot mapping in urban water systems. Through visual observations, macroplastic abundance and polymer categories are determined. Due to its simplicity, this method is suitable for citizen science data collection. A first application in the Dutch cities of Leiden and Wageningen showed similar mean plastic densities (111–133 items/km canal) and composition (75–80% soft plastics), but different spatial distributions. These observations emphasize the importance of long-term data collection to further understand and quantify spatiotemporal variations of plastics in urban water systems. In turn, this will support improved estimates of the contribution of urban areas to the plastic pollution of rivers and oceans.

Estimates of long-term temporal trends and variability in growth are often not available for many commercially exploited fish stocks. An integrated estimation framework that combines growth information from tagging studies, direct age estimates from hard parts, and modal progression estimates from length–frequency data is applied to data on southern bluefin tuna (Thunnus maccoyii, SBT) collected over four decades, from 1960 to 2000. By using an integrated approach, a comprehensive set of growth estimates can be obtained for each of these four decades even though substantive deficiencies exist in the coverage of the historical data from any single source. The results confirm previous findings that cohorts from the 1980s grew substantially faster at young ages than cohorts from the 1960s. The results also suggest that the 1970s was a period of transition and that growth of fish up to about age 4 was faster in the 1990s than in the 1980s. The changes in SBT growth over these four decades are consistent with density-dependent responses given the history of exploitation and stock assessment estimates of changes in population size.

Background: The quality of the HbA1c assay is inversely proportional to the variation of the assay. Most published measures of HbA1c variation are limited by the data collection period, the statistical treatment of outliers, and even the noncommutability of the products used to generate the variation measurements. We have used an alternate approach to derive HbA1c variation, using serial patient data. Methods: HbA1c measurements of outpatient blood sample pairs drawn within 30 days of each other were made on three different immunoassay systems: the Roche INTEGRA® 700, the Roche INTEGRA® 400, and the Dade Dimension® RxL; and two high-performance liquid chromatography assays: the Tosoh G7 and the Tosoh 2.2+. The standard deviation of duplicates was calculated for the following time intervals: 1 to 3 days, 4 to 6 days, 7 to 9 days, …, 28 to 30 days. These intra-individual variations were then plotted; extrapolation to time zero yields the long term total random error which consists of both analytic and pre-analytic error. Data collection periods were usually 2 years. Results: At the mean HbA1cs of 7.08%, 7.14%, 7.20%, 6.96%, and 7.51% for populations tested on the Roche INTEGRA 700, Roche INTEGRA 400, Dade Dimension RxL, Tosoh 2.2+, and Tosoh G7, respectively, the total analytic imprecisions (coefficient of variation) were 2.56%, 2.29%, 2.25%, 1.66%, and 1.14%, respectively. Conclusion: Assessment of the HbA1c long term total imprecisions shows that while the three immunoassay systems are acceptable, the Tosoh HbA1c analyzers demonstrate superior analytic performance.

The impact of COVID-19 on the organs of the genitourinary system is of particular interest to the urologist. There is insufficient information about this influence up to date. The studies are actively developing and require long-term data analysis to determine possible long-term complications, persistent changes in physiological parameters and anatomical and histological structures, as well as to establish the possibility of regression of these changes and complications. The results obtained will undoubtedly improve not only the diagnosis, treatment and prevention of coronavirus infection and its complications, but also make it possible to predict certain disease's outcomes and changes in the function of organs and systems. In turn, this will give an understanding of the measures that need to be taken to completely avoid or minimize these complications and changes.This review focuses on the impact of COVID-19 on genitourinary organs, particularly its place in the development of the lower urinary tract and reproductive organs lesions, as well as the role of androgens in the course of SARS-CoV-2.

Abstract. This paper describes the GRUAN-wide approach to manage the transition from the Vaisala RS92 to the Vaisala RS41 as the operational radiosonde. The goal of the GCOS Reference Upper-Air Network (GRUAN) is to provide long-term high-quality reference observations of upper air Essential Climate Variables (ECVs) such as temperature and water vapor. With GRUAN data being used for climate monitoring, it is vital that the change of measurement system does not introduce inhomogeneities in to the data record. The majority of the 27 GRUAN sites were launching the RS92 as their operational radiosonde, and following the end of production of the RS92 in the last quarter of 2017, most of these sites have now switched to the RS41. Such a large-scale change in instrumentation is unprecedented in the history of GRUAN and poses a challenge for the network. Several measurement programmes have been initiated to characterize differences in biases, uncertainties and noise between the two radiosonde types. These include laboratory characterization of measurement errors, extensive twin sounding studies with RS92 and RS41 on the same balloon, and comparison with ancillary data. This integrated approach is commensurate with the GRUAN principles of traceability and deliberate redundancy. A two-year period of regular twin soundings is recommended, and for sites that are not able to implement this burden sharing is employed, such that measurements at a certain site are considered representative of other sites with similar climatological characteristics. All data relevant to the RS92-RS41 transition are archived in a database that will be accessible to the scientific community for external scrutiny. Furthermore, the knowledge and experience gained about GRUAN's RS92-RS41 transition will be extensively documented to ensure traceability of the process. This documentation will benefit other networks in managing changes in their operational radiosonde systems. The mass die-off of Caribbean corals has transformed many of this region’s reefs to macroalgal-dominated habitats since systematic monitoring began in the 1970s. Although attributed to a combination of local and global human stressors, the lack of long-term data on Caribbean reef coral communities has prevented a clear understanding of the causes and consequences of coral declines. We integrated paleoecological, historical, and modern survey data to track the prevalence of major coral species and life history groups throughout the Caribbean from the pre-human period to present. The regional loss of Acropora corals beginning by the 1960s from local human disturbances resulted in increases in the prevalence of formerly subdominant stress-tolerant and weedy scleractinian corals and the competitive hydrozoan Millepora beginning in the 1970s and 1980s. These transformations have resulted in the homogenization of coral communities within individual countries. However, increases in stress-tolerant and weedy corals have slowed or reversed since the 1980s and 1990s in tandem with intensified coral bleaching. These patterns reveal the long history of increasingly stressful environmental conditions on Caribbean reefs that began with widespread local human disturbances and have recently culminated in the combined effects of local and global change.
