Effectiveness of microneedling in correction of involutive skin changes

In clinical practice, as a rule, patients with involutive changesas well havea photodamage, it should be considered when choosing a method of correction. Microneedling, including the use of PRP and hyaluronic acid, arevery promising methods.Material and methods. There were 50 patients under observation, who were divided into three groups: 1st group – 18 patients received microneedling and PRP, 2nd group – 17 people used microneedling of hyaluronic acid, 3rd group – 15 patients received microneedling. Four procedures were performed with an interval of 4 weeks.Results. According to dermatological status index, PRP microneedlingshowed the effectiveness, the index decreased by 86 % in the group. In group 2, the average index for the group decreased by 56 %, in group 3–41 %.Conclusions.Microneedling is an effective safe method for correcting involutive changes in the skin of the face, combined with signs of photoaging. To increase the effectiveness of microneedling, it is recommended to use autologous plasma with cells directly after the procedure applicationinto the form or the film.

The study of platelet-rich plasma efficacy in the treatment of patients with knee osteoarthritis (OA) was performed. 188 patients were included and divided into three groups. Group 1 - 82 patients with OA, where hyaluronic acid was used for treatment, Group 2 - 36 patients, and Platelet Rich Plasma (PRP) and hyaluronic acid were used, Group 3 - 70 patients, only PRP was used. It`s been found that the use of PRP for treatment is characterized by a pronounced clinical efficacy compared with use of hyaluronic acid, which manifests in reduced detection rate of edema, hyperthermia and hyperemia. The use of PRP leads to pain reduction in the knee joint and increase the functionality of patients, that confirmed by more pronounced declining of visual analog scale indicators, Leken index and the WOMAC index values increase. The presented results of study are consistent with authors who believe that the lack of side effects and complications when using PRP indicates the safety of its use in clinical practice.

Background.The appearance of the skin is of high social importance, largely determining the behavior and self-confidence of a person. The relevance of methods for the correction of involutive skin changes is growing every year. One modern treatment for photodamaged skin is photodynamic therapy. Objective:the study evaluated the effectiveness of photodynamic therapy using chlorin E6 as a photosensitizer for the correction of involutive changes in the skin of the face compared to using only light exposure. Methods.The study involved patients aged 4060 years with signs of involutive changes in the skin of the face. In group 1 (n=36), patients underwent therapy using only light coherent radiation. In group 2 (n=35), patients underwent PDT using light coherent radiation. Results.According to the results of elastometry, corneometry, assessment of transepidermal water loss, ultrasound examination of the skin in group2, more positive indicators were obtained than in group1. When assessing safety, the results in groups 1 and2 are comparable. Conclusions.The method of photodynamic therapy using chlorin E6 can be recommended for course use in wide clinical practice.

The osteochondral healing potential of hyaluronic acid (HA) plus diacerein was evaluated in subchondral-drilling- (SCD-) induced fibrocartilage generation in rabbits. A full-thickness chondral defect was created along the patellar groove of both knees and then SCD was subsequently performed only in the left knee. A week later, the rabbits were allocated into 3 groups to receive weekly intra-articular (IA) injection for 5 weeks with normal saline solution (NSS) (group 1) or with HA (group 2 and group 3). Starting at the first IA injection, rabbits were also gavaged daily for 9 weeks with NSS (group 1 and group 2) or with diacerein (group 3). The animals were then sacrificed for evaluation. The newly formed tissue in SCD lesions showed significantly better histological grading scale and had higher content of type II collagen in HA-treated group compared to NSS control. In addition, adding oral diacerein to HA injection enhanced healing potential of HA.

Purpose: We compared the accuracy of central corneal thickness (CCT) measurements according to CCT measured by noncontact Tono-pachymetry and ultrasound pachymetry (USP). Methods: CCT was measured in 90 eyes of 90 subjects by two optometrists. The CCT measurements were compared and the correlations between the measurements were analyzed. To evaluate whether the measurements varied depending on CCT, the subjects were classified into three groups according to CCT (group 1: thin thickness group; group 2: medium thickness group; and group 3: thick thickness group). The differences in CCT obtained by the two devices (△CCT = Tono-pachymetry-USP) were compared and analyzed among the three groups. Results: The average CCT measurements by Tono-pachymetry and USP were 523.26 ± 32.93 μm and 527.08 ± 37.33 μm, respectively. CCT by Tono-pachymetry was significantly thinner than by USP (△CCT= -3.82 ± 15.34, p = 0.020). The two measurements were strongly correlated (r = 0.912, p < 0.001). The △CCT values were 5.40 ± 12.13 μm in group 1, -6.37 ± 15.07 μm in group 2, and -10.50 ± 14.39 μm in group 3 (p < 0.001). Conclusions: CCT measured by tono-pachymetry was thinner on average compared to the value measured by USP and the differences in measurements between the two devices were different according to CCT. The thinner the CCT, the thicker the measurement, and the thicker the CCT, the thinner the measurement. Therefore, this trend should be considered when interpreting tono- pachymetry results in clinical practice.

According to recent studies, the prevalence of dry eye syndrome makes up 14% of cases in patients under the age of 50 years and 65-75% in patients over 50 years of age. Risk factors for the development of dry eye syndrome include aging, stress, side effect of frequent instillations of eye preparations, exposure to radiant energy, and diabetes mellitus. Therefore, the purpose of this study is to investigate the possibility of using “Delargin”, an antioxidant medicine, in the treatment of dry eye syndrome. 50 patients (100 eyes) aged 30-70 years were examined at the Department of Ophthalmology and then divided into three groups depending on the treatment used. Group 1 included 10 patients (20 eyes), who were prescribed to instil Oksial eye drops. Group 2 included 10 patients (20 eyes), who received basic therapy by instilling a solution of sodium salt of hyaluronic acid 0.0015 g / ml. Group 3 involved 20 patients (40 eyes), who instilled Delargin and hyaluronic acid. Monitoring the effectiveness of the treatment was carried out using the following methods: to assess basal tear production, a Schirmer 1 test was performed using the Ophthalic Test diagnostic bands (India); a Norn test was performed to control the stability of the tear membrane. Fluorescence test (staining of the cornea with 1% sodium fluorescein solution) was applied to detect corneal defects; Lipcof test was used to assess the degree of lid-parallel fold formation of the bulbar conjunctiva relative to the costal margin of the lower eyelid. As a result of the treatment, the patients of the Group 1 demonstrated that the Norn test scores increased to some extent from 6.8 ± 0.1 s up to 7.5 ± 0.1 s; in the patients of the Group 2 following the treatment, it was 8.5 ± 0.2 s and in the Group 3, the Norn test values were 11.2 ± 0.2 s (P<0.05). The frequency of blinking movements decreased significantly and approached the normal. It has been found out that the therapy of dry eye syndrome that includes the antioxidant Delargin, can significantly promotes normalizing of parameters typical for this process.

Objective: This study aimed at examining whether topical treatment of colon anastomosis with hyaluronic acid can influence the healing of colonic anastomosis in rats treated with systemic corticosteroid. Methods: Three groups of Wistar rats weighing 252 ± 7g were used, with six rats each, all submitted to anastomosis of the proximal colon: Group 1 - control rats, without treatment. Group 2 - rats treated with subcutaneous (s.c) corticosteroid, and topical application of 0.9% saline solution over the anastomosis. Group 3 - rats treated with corticosteroid s.c. and topical application of 0.4% hyaluronic acid (10 mg/ml) on colonic anastomosis. On the 7th postoperative day, under anesthesia and laparotomy, the anastomosis site was subjected to the determination of rupture pressure. Then, samples containing the anastomosis site were resected and fixed in 10% buffered formaldehyde and embedded in paraffin. Masson H-E and trichrome staining. Histometry evaluated the infiltration of inflammatory cells at the anastomosis site, using a numerical scale from 0 to 4. Continuous variables were assessed using the Tukey test. Differences considered significant with p<0.05. Results: All animals survived the experiments. There were no abscesses, fistulas and macroscopically detectable dehiscences in the anastomosis site. The weight of the animals on the 7th postoperative day showed a statistically significant difference (p<0.001) between the control (253.6±6.3g) and corticoid (221.6±15.4g) groups, as well as between the corticoid +. hyaluronic acid group (257.8±9.7g) and corticosteroids (221.6±15.4). There was no significant difference in the comparison between the control and corticoid + hyaluronic acid groups (p>0.05). In the analysis of intraluminal pressure of colonic anastomosis, there was a statistically significant difference when comparing groups 1 (286.8±9.9 mmHg) and 2 (155±6.0 mmHg), 1 (286.8±9,9 mmHg) and 3 (199.1±7.7) and 2 (155±6.0 mmHg) and 3 (199.1±7.7), with p<0.001 for all comparisons. Histopathological classification by the numerical scale: Group 1 - inflammatory response (H-E) grade 3 and collagen deposition by Masson Trichomic grade 1; Group 2 - inflammatory response (H-E) grade 4 and deposition of collagen and fibroblasts by Masson's Trichrome grade1\2; Group 3 - inflammatory response (H-E) grade 3 and deposition of collagen and fibroblasts by Masson's Trichrome grade 1\2. There was no significant difference between groups. Conclusion: The topical use of hyaluronic acid has a positive influence on the initial healing phase of colonic anastomosis in rats treated with systemic corticosteroid.

Abstract Background: Trisomy 21 is the most prevalent chromosomal anomaly among new-borns with mental disability. The phenotypes with trisomy 21 may involve short stature, mental retardation, shortness of the extremities, congenital heart disease and hydropsMaterial and methods: To evaluate the hyaluronic acid concentrations in amniotic fluid of trisomy 21 fetuses with and without hydrops.Materials and Methods: Totally 56 amniotic fluid supernatant samples were collected from pregnant women who underwent genetic amniocentesis for this study: group 1 were euploid fetuses without hydrops (n=30), group 2 were trisomy 21 fetuses without hydrops (n=16), group 3 were euploid fetuses with hydrops (n=6) and group 4 were trisomy 21 fetuses with hydrops (n=4). Hyaluronic acid concentrations in AF supernatants were evaluated by ELISA method.Results: There are no significant differences of hyaluronic acid concentrations between euploid and trisomy pregnancy in AF obtained from genetic amniocentesis. AF in the gestational age of amniocentesis among the four groups of fetuses; maternal age at amniotic fluid collected is oldest in group 2 fetuses. The amniotic fluid hyaluronic acid concentration is highest in group 4 fetuses, and is not significant difference among the other three groups. (P= 0.014, one way ANOVA test).Conclusion: Elevated hyaluronic acid concentration in amniotic fluid may associate with the pathophysiology of fetal hydrops in trisomy 21 fetuses.

BACKGROUND: Development and introduction of minimally invasive lifting techniques is considered a demanding and relevant objective of contemporary dermatocosmetology as these techniques result in nonsurgical lifting with minimal disruption of facial harmony due to the activation of a maximal number of key anatomic landmarks following the simultaneous insertion of lifting threads and hyaluronic acid fillers. AIMS: To study the clinical efficacy and safety of minimally invasive lifting technique and filling up of the tissue volume deficiency with the combination of thread lifting from the temporal approach and hyaluronic acid filler augmentation. MATERIALS AND METHODS: The study group included 193 female patients (mean age 41.3 8.5 years) with gravitational ptosis of the 1st, 2nd, and 3rd degrees. Group 1 (main) comprised 42 (21.8%) patients who underwent lifting by the method of implantation of 12 threads with shaped polydioxanone spikes DG-Lift and injection of 2 ml of hyaluronic acid biphasic filler. Group 2 (comparison) comprised 64 (33.2%) female patients who underwent lifting using only 1220 polydioxanone DG-Lift threads. Group 3 (comparison) comprised 87 (45%) female patients who received tissue volumization with 48 ml of biphasic hyaluronic acid filler in each case. RESULTS: A total of 82%, 59%, and 48% of patients in groups 1, 2, and 3, respectively, noted the optimal cosmetic result and full satisfaction with the result according to the Global Aesthetic Improvement Scale. The incidence of complications in group 1 (6.4%) was lower than that in group 2 (9.1%) and group 3 (17.7%). This is due to low invasive nature and lower tissue traumatization with the use of fewer fillers and threads to obtain optimal cosmetic effect. CONCLUSION: The proposed technique of thread lifting with biodegradable threads DG-Lift from the temporal approach combined with tissue volumization with hyaluronic acid fillers is safe and significantly effective for patients with gravitational ptosis of the 1st, 2nd, and 3rd degrees.

Objectives: The purpose of this study was to comparative analysis of effectiveness of isolated use of intradermal injections of modified hyaluronic acid (Hyalorepair 04) and its combination with platelet-rich autologous plasma in edentulous patients with perioral wrinkles of the skin. Materials and Methods: A total of 56 patient’s presence of perioral wrinkles of the skin participate in the study after dental implant prosthodontics rehabilitations. They were randomly divided into 2 groups in accordance with the applied therapy method (29 isolated implementation of modified hyaluronic acid bio-repairing and 27 bio-repairing combined with autologic plasmotherapy). Treatment included implant-prosthetic rehabilitation followed by hyaluronic acid injections in order to correct cheek-zygomatic sulcus, nasolabial folds and marionette wrinkles. Preparations in an amount from 1 to 4 ml were injected into the face area, depending on the zones to be corrected at the request of the patients. Results: The complex treatment restored the aesthetic profile of the face and oral cavity and increased the effectiveness of the chewing function. On M03 and M05, 2/3 of patients had significant improvement as assessed by physician and patient according to GAIS. Most of the patients also showed significant improvement at visit M12. Conclusion: A multidisciplinary approach to the treatment of edentulous patients with perioral wrinkles increases the functional and aesthetic effect of the treatment. Implementation of combined of bioreparation and autologous plasmotherapy is significantly more effective comparatively to the isolated implementation of modified hyaluronic acid. This study aims to evaluate the quality of eggs in laying hens reared at different densities in cage-free system, receiving diets with the addition of prebiotic additives. An observational study was carried out with 45 laying hens of the Novogen Brown scaling, which was in the phase of peak production (28 – 37 weeks), in prototypes of reduced scale without air conditioning and divided into group 1 (G1): density of two poultry /m² (11 poultry ) and diet with addition of a prebiotic pool; group 2 (G2): density of three poultry /m² (17 poultry ) and commercial diet without the addition of prebiotics and antibiotics; group 3 (G3): density of three poultry /m² (17 poultry ) and also diet with the addition of a prebiotic pool. In this study, the statistical evaluation was performed, the effects of densities 2 poultry /m² (G1) or 3 poultry /m² (G3) and the effects of diet with prebiotics (G3) and without prebiotics (G2) in laying poultry  were performed. The data were submitted to variance analysis by the GLM procedure of the SAS Program V9 (2002), using the Student's T Test at a level of 5% significance. The results referring to eggeso (g), albumen height (mm), yolk color,  haugh unit (UH), peel  resistance (Kgf) between G1 and G3 showed no statistically difference, since in relation to the thickness of the shell (mm) differed (P>0.05). In the analysis between G2 and G3, the statistical difference (P>0.05) obtained among all the panels analyzed. In the face of the above, it can be reported that the results of quality variables undergo the action of prebiotics in laying hens created in the Cage-free system.
