The aim was to establish prevalence of bile acid malabsorption (BAM) and management in patients who underwent treatment for malignancy. Retrospective evaluation of data in patients seen within six months (August 2019–January 2020) was carried out. Demographic, nuclear medicine (Selenium Homocholic Acid Taurine (SeHCAT) scan result), clinical (previous malignancy, type of intervention (medication, diet), response to intervention) and laboratory (vitamin D, vitamin B12 serum levels) data were searched. In total, 265 consecutive patients were reviewed. Out of those, 87/265 (33%) patients (57 females, 66%) were diagnosed with BAM. Mean age was 59 +/− 12 years. The largest group were females with gynaecological cancer (35), followed by haematology group (15), colorectal/anal (13), prostate (9), upper gastrointestinal cancer (6), another previous malignancy (9). Severe BAM was most common in haematology (10/15; 67%) and gynaecological group (21/35; 60%). Medication and low-fat diet were commenced in 65/87 (75%), medication in 10/87 (11%), diet in 6/87 (7%). Colesevelam was used in 71/75 (95%). Symptoms improved in 74/87 (85%) patients. Vitamin D insufficiency/deficiency was diagnosed in 62/87 (71%), vitamin B12 deficiency in 39/87 (45%). BAM is a common condition in this cohort however treatments are highly effective.

Background: In this study, the aim was to evaluate the prevalence of vitamin D, vitamin B12, ferritin, and folate deficiencies in adolescence to clarify the need for early diagnosis and therapy. Methods: The medical records of adolescents between 10 and 18 years of age between 01 September 2018 and 28 February 2019 as healthy with non-specific complaints, or due to well-child care visits, were analyzed retrospectively. Results: A total of 1847/2507 (73.6%) adolescents were included in the study. The prevalence of vitamin D deficiency was 25.7% (n: 178/691). Vitamin B12 deficiency prevalence was 69.2% (n: 753/1088). The prevalence of anemia and ferritin deficiency was 4.8% and 13.26%. The prevalence of folate deficiency was 37.9% (n: 413/1088). VDD prevalence was statistically significantly higher in females than males (F/M:116/62). VB12D prevalence, the number and mean age of females with hemoglobin deficiency, and low ferritin levels was found to be statistically significantly higher in females than males. Conclusions: The prevalence of vitamin D, vitamin B12, folate deficiency and low ferritin levels was found to be high among adolescents. In particular, adolescents admitting with non-specific complaints and for control purposes in big cities must be considered to be at risk for the deficiency of these vitamins and low level of ferritin.

Abstract Low serum levels of vitamin D are associated with a higher frequency of at least eleven different malignancies including breast, colon, and prostate cancer (Holick, M.F. Vitamin D: Millenium Perspective. Journal of Cellular Biochemistry. 2003. 88:296–307). Low levels of vitamin B12 were found to contribute to a 2.5–4.0 times greater likelihood of breast cancer in postmenopausal women (Wu, K. et al. Cancer Epidemiol. 1999. 8:209–17) and hematological and mucosal tissue is more sensitive to chemotherapy in the presence of insufficient levels of B12. Only vitamin B12 that is complexed to transcobalamin as holotranscobalamin (HTCII) is metabolically active. It has been suggested that decreased HTCII serum levels are involved in the failure to methylate DNA resulting in the activation of oncogenes that would normally be dormant (Herbert, V. Methyl Metabolism: Epigenetics, Genomics, Proteomics. 2002 FASEB Summer Research Conference. Snowmass Village, Colarado.).Our study investigated vitamin D, total B12 and HTCII levels in 70 cancer patients. Vitamin D was measured as serum 25 OH-D3 (Nichols Advantage assay) and serum B12 was measured as both total B12 and as the metabolically active HTCII (Immulite B12 assay followed by glass adsorption: Vu, T. et al., Am J. Heme42: 202–211 1993). Vitamin D insufficiency has been defined based on differing physiologic sequelae of insufficiency and varies between values less than 50–75 nMol/L. When vitamin D insufficiency is defined as serum level <75nmol/L, 43 of 60 (72%) of cancer patients were found to be insufficient. At a lower definition of insufficiency, <50nmol/L, 24 of 60 patients (40%) were insufficient. Of 52 patients, only 3 (6%) were found to have insufficient serum levels of total B12 (normal >300pg/mL) while 17 of 52 (34%) were found to be HTCII insufficient (normal >69 pg/mL). Of these 17 patients, 14 (84.4%) had normal total B12 levels. Low levels of vitamin D strongly correlated with low serum HTCII. All 12 HTCII deficient patients were vitamin D insufficient at the <75nmol/L standard. Six of 12 HTCII deficient patients (50%) were vitamin D deficient at the <50nmol/L cut off. Chi-squared test for independence revealed a strong relationship between low levels of vitamin D and HTCII. Deficiency of vitamin D (70%) and holotranscobalamin (34%) is prevalent among newly diagnosed patients with cancer. The standard measurement of total serum B12 alone is inadequate for identifying patients with insufficient levels of metabolically active B12. Low vitamin D and holotranscobalamin levels may play a role in cancer development, progression and host response to tumor and therapy. Possible explanations for combined HTCII and D3 deficiency include age, the presence of atrophic gastritis in 30–50% of the elderly, and lack of sun exposure and deficient production of D3 in the elderly. Since both vitamins are conserved by cubulin/megalin mediated renal tubular reabsorption a defect of this mechanism could contribute to deficiency of both vitamins. Abstract Objectives There is no reliable and valid biomarker to identify Irritable bowel syndrome (IBS) and its subtypes. The aim of this study is to explore potential serum biomarkers that may be associated with IBS subtypes, particularly in the vitamin D pathway. Methods The study population comprised 75 IBS patients and 79 controls. Patients divided into IBS subtypes. Routine biochemical parameters, 25-OH-vitamin D, vitamin D binding protein (VDBP) and vitamin D receptor (VDR) serum levels were compared between IBS subtypes and controls. Factors related to IBS subtypes were examined by multivariate logistic regression analysis. Results Vitamin D levels were lower; VDBP and VDR were higher in all IBS patients than in controls (p<0.001; 0.047 and 0.029, respectively). According to logistic regression analysis, VDBP was a disease-related parameter as much as vitamin D in all IBS subtypes. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) were higher especially in diarrhea-dominant IBS (IBS-D) (p=0.041; 0.046) and vitamin B12 were significantly lower in constipation-dominant IBS (IBS-C) (p=0.001). Conclusions Increased VDBP levels were associated with all IBS subtypes. Patients, especially in IBS-D, had higher serum levels of VDBP, CRP and ESR. Vitamin B12 deficiency, which we consider as a result of the disease, was more common in IBS-C.

Abstract Objectives Advanced Glycation End Products, (AGEs) and their soluble receptor (sRAGE) have been implicated in the development of complications and mortality among individuals with type 2 diabetes (T2D). There is limited information on the relationship between AGEs and sRAGE and risk of cardiovascular diseases (CVD) in minority groups, who have a higher burden of T2D. The relationship between AGEs and sRAGE and CVD risks in adults with T2D and vitamin D insufficiency/deficiency was assessed in a minority population. Methods A cross sectional study of Hispanics and African Americans with T2D (n = 64, 41 women and 23 men, mean age = 54 ± 9) recruited from two clinics in Miami Dade. Systolic (SBP) and diastolic blood pressure (DBP), weight and height measurement and serum lipid profile were completed. ELISA kits were used to assess serum levels of AGEs (Biotang Inc/TSZ Elisa, Waltham, MA, USA) and sRAGE (Biotang Inc/TSZ Elisa, Waltham, MA, USA). Multiple linear regression was used to assess association between AGEs, sRAGE and CVD risks. Results A negative and significant association between AGEs and high-density lipoprotein cholesterol (HDL-C)(B = −0.551, P = 0.029) was found. The relationship between AGEs and HDL-C persisted after adjusting for covariates (P < 0.05). sRAGE was significantly associated with SBP (B = 0.015, P = 0.025) and diastolic blood pressure DBP (B = 0.0271, P = 0.037). Results loss significance when association between sRAGE and DBP and SBP were adjusted for covariates such as age, body mass index (BMI), smoking and alcohol intake. Conclusions Our results suggest that AGEs and sRAGE are related to markers of cardiovascular risk such as HDL-C, SBP and DBP in the study population of African Americans and Hispanics with T2D and vitamin D insufficiency/deficiency. Measures on reducing serum levels of AGEs and improving sRAGE and vitamin D are warranted in these populations for risk reduction of CVD. Purpose This paper aims to observe the possible relationship between skin color, sun exposure level, UV protection and food intake and serum levels of 25(OH) D in Japanese older adults. Design/methodology/approach Elderly (n = 131; 65-93 years old), followed by the Tokyo Metropolitan Institute of Gerontology in the Kusatsu (36°N) received a self-applicable questionnaire about the quantity and quality of the daily sun exposure and behavior to avoid the sun. A color analyzer measured each red (R), green (G) and blue (B) component of skin color, and dietary vitamin D was estimated by food frequency questionnaire. Serum 25(OH) D levels were collected and categorized as sufficiency (>30 ng/mL), insufficiency (between 20 and 30 ng/mL) and deficiency (<20 ng/mL). Findings High proportion of participants had insufficiency (53 per cent) and deficiency (25 per cent) levels of 25(OH) D. Insufficiency levels were more prevalent in women (57 per cent, p = 0.048) and in participants that use gloves (49 per cent, p = 0.054) and sunscreen on face (76 per cent, p = 0.003) as a sun protection way. Participants with sufficiency levels of 25(OH) D presented lower values of R (p = 0.067), G (p = 0. 007) and B (p = 0.001) of skin color (what is meaning darker skin) and a higher fish intake (12 times per week). Research limitations/implications The study is a cross-sectional design and brings a potential for measurement error in the recorded subjective variables. There is a memory bias in self-reported sun exposure and food consumption; however, in the multivariate analysis, it was demonstrated a significant association. Second, although the authors have sought to evaluate a number of variables that could affect the skin’s ability to synthesize vitamin D, there are many other factors that may affect this ability that could not be accounted for. Another limitation was the assessment of self-reported ultraviolet exposure data rather than direct measurement of exposure. Practical implications It was also concluded that darker skin color (a surrogate of longer-term sun exposure) participants had a lower prevalence of vitamin D insufficiency in this ethnic homogeneous population. When accessing patients’ skin color, the clinician must account for his or her ethnicity. Social implications Governments should regulate supplementation or food fortification with vitamin D, with special focus in countries with geographical location of insufficient solar radiation for skin synthesis of this vitamin. With this, it becomes a priority that a safe sun exposure ensures the sufficient serum levels of 25 (OH) D without the use of supplements. Originality/value This report was the first to analyze skin color components associated to vitamin D levels, finding that blue and green colors were significant. The clinical implication of this find is yet to understand. It was also concluded that darker skin color (a surrogate of longer-term sun exposure) participants had a lower prevalence of vitamin D insufficiency in this ethnic homogeneous population. When accessing patients’ skin color, the clinician must account for his or her ethnicity.

Vitamin D and vitamin B12 deficiencies are major public health problems; they may result from inappropriate low exposure to sunlight, autoimmune diseases or diminished intake. These two deficiencies have been extensively studied globally: causes, e"ects, treatment, as well as epidemiology. In Palestine the epidemiology of vitamin D and vitamin B12 deficiencies has not been addressed. This study was undertaken to determine the prevalence of vitamin D and vitamin B12 deficiencies in patients reporting to the West Bank (WB) governmental hospitals in the period between January 2015 and December 2018. It is a retrospective cross-sectional study for the data collected from medical records of patients tested for these deficiencies in 12 WB governmental hospitals for the three years period. Out of 30890 patients tested for vitamin D levels, 88% had insu!cient vitamin D levels (< 30 ng/ml), whereas out of 43532 patients tested for vitamin B12, 19% had insu!cient vitamin B12 levels (< 203 pg/ml). The percentage of patients with insu!cient vitamin D levels is alarming. The percentage of patients with insu!cient vitamin B12 levels falls within ranges reported by other studies in various countries. In conclusion, this study revealed an alarmingly high percentage (88%) of vitamin D deficiency below the reference su!ciency level among patients suspected to have such a deficiency. Around one fifth of the patients tested for vitamin B12 had insu!cient levels. Because testing for vitamin D is costly, we suggest, that medical suspicion of vitamin D deficiency would be adequate to initiate treatment to alleviate the expense, especially in high-risk groups such as elderly women. Future studies have to address major risk factors contributing to these deficiencies that are specific to our community.

[Purpose] The aim of this review was to discuss the effects of vitamin D on physical performance and musculoskeletal injuries in athletes and provide information on the field applications of vitamin D. [Methods] A systematic review was conducted to identify studies on vitamin D in athletes that assessed serum vitamin D levels, vitamin D and physical performance, vitamin D and musculoskeletal injuries, and practical guidelines for supplementation of vitamin D. [Results] Several studies reported that a high proportion of athletes had vitamin D insufficiency or deficiency. Low serum levels of vitamin D in athletes were more pronounced in winter than in other seasons, and indoor athletes had lower serum vitamin D levels than outdoor athletes. Low vitamin D levels have been demonstrated to have negative effects on muscle strength, power, and endurance; increase stress fractures and other musculoskeletal injuries; and affect acute muscle injuries and inflammation following high-intensity exercises. Therefore, periodic assessment and monitoring of vitamin D levels are necessary in athletes; the recommended serum level of 25(OH)D is > 32 ng/mL and the preferred level is > 40 ng/mL (-1). In those with low levels of vitamin D, exposure to sunlight and an improved diet or supplements may be helpful. Particularly, 2000–6000 IU of supplemental vitamin D3 can be consumed daily. [Conclusion] Vitamin D is a potential nutritional factor that can significantly affect physical performance and musculoskeletal injuries in athletes. The importance and role of vitamin D in athletes should be emphasized, and the current levels of vitamin D should be assessed. Therefore, it is essential to periodically evaluate and monitor serum vitamin D levels in athletes.

Qatar has a high burden of chronic diseases including obesity, cardiovascular disease and type 2 diabetes mellitus. Low serum vitamin D levels have been implicated in the development and progression of a range of these chronic conditions. The prevalence of vitamin D insufficiency or deficiency in the general population of Qatar has still not been investigated. The aim of this study was to carry out a systematic review of published studies documenting the prevalence of vitamin D insufficiency or deficiency in the Qatari population. A search strategy was developed for online databases (PubMed, Ovid MEDLINE, Embase and Embase Classic) between 1980 to the last week of August 2012, and bibliographies of the included studies were further searched for additional reports. Search terms used were QATAR and VITAMIN D. Studies reporting the serum levels of vitamin D in several Qatari sub-populations were identified. Weighted-average vitamin D serum levels and prevalence of low vitamin D status (&lt;75 nmol/L) were calculated. Subgroup analysis was carried out by age. The quality of each study was evaluated according to four criteria: national representativeness, representation of males and females, the sample size, and the sampling protocol. A total of 16 relevant publications were identified, and 8 of these (reporting from 7 unique studies) met our inclusion and exclusion criteria with a total number of 1,699 Qatari subjects. The pooled sample size weighted-average vitamin D concentration (±SD) was 45.3±14.3 nmol/L (95% CI: 44.6-46.0; range 29.2-66.9 nmol/L). The weighted-average prevalence of low vitamin D status was 90.4% (95% CI: 90.1-91.0; range 83%-91%). Age was inversely correlated with vitamin D levels and directly with its insufficiency/deficiency prevalence. There have only been a few studies on the prevalence of low vitamin D in Qatar a very high prevalence of vitamin D insufficiency/deficiency in Qatar that increases with age has been suggested. The present report underlines the need to develop a nationally representative study to further evaluate vitamin D status in Qatar. Given the growing evidence of the role of vitamin D in chronic disease, this study could help develop public health strategies for disease prevention in Qatar.
