Patients with chronic wounds (leg ulcers, decubitus, and diabetic foot ulcers) suffer from marked restrictions in their quality of life and can often no longer adequately carry out their everyday tasks. The need for nursing and medical care increases when other illnesses and complaints are present at the same time. Qualified wound care and the treatment of comorbidities are therefore of particular importance. The treatment of this disease, which is increasing in number, requires a holistic, multimodal treatment approach which, in addition to professional wound care, also includes comorbidities in the treatment. This case study describes an old treatment method for refractory wounds, the so-called “maggot therapy”, and shows how this is integrated into a holistic, multimodal therapeutic approach.

Ulcers in lower extremity are generally caused by venous insufficiency, arterial insufficiency, and diabetic neuropathy. Inflammatory ulcers are also difficult to diagnose because of underlying pathophysiology rests on inflammation and immunologic phenomena. Definitive diagnosis and multidisciplinary treatment approach are needed to manage lower extremity ulcers. These ulcers may hamper the quality of life of patient. Here we reported a new case of post fracture of shaft of right tibia bone, presented with painful, hypertrophied, irregular shaped ulcer on anterior aspect of tibia gradually developing since 3 months. A male patient of 45 years old consulted to OPD with above complaint that was unable to bear weight on the effected leg (right limb). This case was managed with a local application of Thumari Gel and ulcers were almost healed within 6 weeks of wound care along with internal use of Ayurveda medicines. This case concluded that regular local application of Thumari Gel helpful in healing of chronic ulcer.

In the past decade, the frequency of chronic wounds in older population has increased, and their impact on quality of life is substantial. Chronic wounds are a public health problem associated with very high economic and psychosocial costs. These wounds result from various pathologies and comorbidities, such arterial and venous insufficiency, diabetes mellitus and continuous skin pressure. Recently, the role of infection and biofilms in the healing of chronic wounds has been the subject of considerable research. This paper presents an overview of various methods and products used to manage chronic wounds and discusses recent advances in wound care. To decide on the best treatment for any wound, it is crucial to holistically assess the patient and the wound. Additionally, multiple strategies could be used to prevent or treat chronic wounds.

ABSTRACTAdvanced wound care centres are now a well established response to the growing epidemic of chronic wounds in the adult population. Is the concept transferable to children? Whilst there is not the same prevalence of chronic wounds in children there are conditions affecting the integumentary system that do have a profound effect on the quality of life of both children and their families. We have identified conditions involving the skin, scars and wounds which contribute to a critical number of potential patients that can justify the setting up of an advanced skin, scar and wound care centre for children. The management of conditions such as giant naevi, extensive scarring and epidermolysis bullosa challenge medical professionals and lead to new and novel treatments to be developed. The variation between and within such conditions calls for a customizing of individual patient care that involves a close relationship between research scientists and clinicians. This is translational medicine of its best and we predict that this is the future of wound care particularly and specifically in children.

Background: The concept of wound bed preparation is a holistic approach to wound diagnosis and treatment of the cause, patient-centered concerns and optimizing the components of local wound care (débridement, infection and persistent infection, moisture balance) before edge effect for healable but stalled chronic wounds. This article has introduced the concepts of healable, nonhealable and maintenance wounds. Additionally, clinical criteria (mnemonic NERDS and STONEES) are provided on the use of topical agents for critical colonization or systemic antimicrobials for deep and surrounding infection. Objective: To present a holistic, evidence-informed approach to chronic wound care management. Methods: This article reviews the scientific evidence base, and forms an expert consensus of key opinion leaders to the Wound Bed Preparation model. Results and Conclusion: This article provides clinicians with evidence-informed wound caring approaches translating the evidence base into practice.

Europe's aging population presents an increasing number of chronic diseases, including vascular diseases that can lead to chronic wounds. These chronic wounds carry a significant health burden for individuals and economic burden for healthcare systems. Of chronic ulcers in the lower limbs, 80–85% are venous leg ulcers (VLUs). VLUs can be treated using a combination of compression therapy, modern wound dressings and wound-bed preparation based on tissue management, inflammation and infection control, moisture balance and epithelial advancement. This approach should improve the patient's quality of life in a way that is fast, cost-effective and minimises pain. Optimal treatment should involve holistic, comprehensive care for the individual patient, supported by multidisciplinary teamwork and patient education for self-care. An aging population also increases the need for community nursing care in patients' homes. Access to home wound care differs between European countries, particularly whether it is provided by registered nurses, as part of the healthcare system, or by homecare assistants, as part of the social care system. In countries like Slovenia, community nurses play an important role in the multidisciplinary treatment of VLUs, as they care for patients in the home environment and can best assess their condition. The GP first makes the initial wound assessment and diagnosis before authorising the community nurse to provide wound care in the patient's home. A case study is presented of a 91-year-old male patient with a VLU, alongside reduced mobility and multiple comorbidities. The treatment method provided optimal healing and is described and illustrated in detail.

Derfor er der brug for at se på nye forståelser af bevægelse i skolen, som kan udvikle bevægelsesorienteret praksis. I denne artikel med case i specialundervisningen på Skolehjemmet Marjatta belyser vi, hvordan varieret brug af bevægelse i undervisningen har betydning for væsentlige aspekter af funktionsnedsatte elevers udvikling og læring. Der præsenteres en holistisk forståelse af bevægelse i undervisningen og nye perspektiver på, hvordan bevægelse kan integreres meningsfuldt i undervisning i pædagogiske situationer.Abstract In recent years, an urgent political focus has been to include movement in teaching that aims to strengthen health, quality of life and learning amongst children in schools. Based on a case study on special education at Marjatta, a school of disabled children, we suggest that a holistic approach to movement in school teaching will improve central aspects of development and learning of disabled children. This article develops new perspectives on how movement can be meaningfully integrated in special education.

Conhecer a opinião de pessoas hospitalizadas acerca da terapia larval como opção terapêutica. Estudo do tipo transversal e descritivo, realizado no setor de clínica médica e cirúrgica de um hospital de ensino, por meio de entrevista. Participaram da pesquisa 105 pessoas, dentre essas, 6 souberam dizer o que é uma bioterapia, 95 afirmaram que utilizariam a terapia larval, 32 se sentiam motivadas em aderir o tratamento devido à possibilidade de diminuir seus níveis de dor e 73 referiram medo em perder a vida. A terapia larval foi bem aceita entre as pessoas hospitalizadas e que referiram medo de morrer. Ressalta-se a importância da divulgação e do acesso a essa terapia natural, para ampliar as opções de tratamento e a qualidade de vida das pessoas hospitalizadas.Descritores: Larvas, Feridas, Cuidado. Acceptability of maggot therapy in wound care Abstract: Know the opinion of people hospitalized about maggot therapy as a treatment option. Chronic wounds represent an economic burden to healthcare systems worldwide and a societal burden to patients, deeply impacting their quality of life. The incidence of recalcitrant wounds has been steadily increasing since the population more susceptible, the elderly and diabetic, are rapidly growing. Chronic wounds are characterised by a delayed wound healing process that takes longer to heal under standard of care than acute (i.e. healthy) wounds. Two of the most common problems associated with chronic wounds are inflammation and infection, with the latter usually exacerbating the former. With this in mind, researchers and wound care companies have developed and marketed a wide variety of wound dressings presenting different compositions but all aimed at promoting healing. This makes it harder for physicians to choose the correct therapy, especially given a lack of public quantitative data to support the manufacturers’ claims. This review aims at giving a brief introduction to the clinical need for chronic wound dressings, focusing on inflammation and evaluating how bio-derived and synthetic dressings may control excess inflammation and promote healing.

This paper describes an improved building treatment approach (IBTA) for use in urban inundation modeling. In this approach, the ground surface elevation was raised by the threshold (h) of the building entrance height to account for both the blockage and storage effect of areas with dense building coverage. A higher roughness coefficient was assigned to the areas where buildings were located to compensate for the resistance effects caused by the inner wall of the structure. The campus of Huazhong University of Science and Technology (HUST) in Wuhan City, China, was used as a case study. Comparison between IBTA and several traditional building treatment approaches suggested that the model results were sensitive to the building treatment method and the threshold used for terrain preprocessing in dense building regions. Furthermore, as the interaction between the surface water flow and dense buildings were adequately represented by using a new terrain preprocessing approach, the proposed IBTA provided better performance in terms of maximum inundation depth and the peak depth time than the traditional approaches in areas with dense building coverage, such as that of the campus.
