Remote symptom tracking is critical for the prevention of Covid-19 spread. The qualified medical staff working in the call centers of primary health care units have to take critical decisions often based on vague information about the patient condition. The congestion and the medical protocols that are constantly changing often lead to incorrect decisions. The proposed platform allows the remote assessment of symptoms and can be useful for patients, health institutes and researchers. It consists of mobile desktop applications and medical sensors connected to cloud infrastructure. The unique features offered by the proposed solution are: (a) dynamic adaptation of Medical Protocols (MP) is supported (for the definition of alert rules, sensor sampling strategy and questionnaire structure) covering different medical cases (pre- or post-hospitalization, vulnerable population, etc.), (b) anonymous medical data can be statistically processed in the context of the research about an infection such as Covid-19, (c) reliable diagnosis is supported since several factors are taken into consideration, (d) the platform can be used to drastically reduce the congestion in various healthcare units. For the demonstration of (b), new classification methods based on similarity metrics have been tested for cough sound classification with an accuracy in the order of 90%.

The need to identify wood by its anatomical features requires a detailed analysis of all the elements that make it up. This is a significant problem of structural wood science, the most general and complete solution of which is yet to be sought. In recent years, increasing attention has been paid to the use of computer vision methods to automate processes such as the detection, identification, and classification of different tissues and different tree species. The more successful use of these methods in wood anatomy requires a more precise and comprehensive definition of the anatomical elements, according to their geometric and topological characteristics. In this article, we conduct a detailed analysis of the limits of variation of the location and grouping of vessels in the observed microscopic samples. The present development offers criteria and quantitative indicators for defining the terms shape, location, and group of wood tissues. It is proposed to differentiate the quantitative indicators of the vessels depending on their geometric and topological characteristics. Thus, with the help of computer vision technics, it will be possible to establish topological characteristics of wood vessels, the extraction of which would be used to develop an algorithm for the automatic classification of tree species.

A suitable nomenclature for atmospheric strata as well as a clear definition of the boundaries is proposed. The necessity of such a new classification is stressed. The atmosphere is divided into an inner and an outer atmosphere; from the latter particles may escape. The inner atmosphere is divided into three spheres—troposphere, stratosphere, and ionosphere—with each sphere in turn being subdivided into 3 or 4 layers. The new classification is based upon the thermal structure of the atmosphere.' Boundaries of each layer are fixed by a sudden change of lapse rate. The bottom layer, the ground layer, the advection layer, and the tropopause layer are subdivisions of the troposphere. The advantages gained by defining a separate tropopause layer as part of the troposphere are discussed in detail. Its upper boundary is assumed to be situated at 12 km over temperate latitudes. The stratosphere, consisting of an isothermal layer, a warm layer, and an upper mixing layer, extends from 12 to 80 km. The atmosphere between 80 and 800 km is occupied by the ionosphere, the subdivisions of which are the E-layer, the Flayer and the atomic layer. Above that height the exosphere exists.

Abstract. The definition of an atmospheric database is an important component of simulation studies in preparation of future earth observing remote sensing satellites. The Aeolus mission, formerly denoted Atmospheric Dynamics Mission (ADM) or ADM-Aeolus, is scheduled for launch end of 2013 and aims at measuring profiles of single horizontal line-of-sight (HLOS) wind components from the surface up to about 32 km with a global coverage. The vertical profile resolution is limited but may be changed during in-orbit operation. This provides the opportunity of a targeted sampling strategy, e.g., as a function of geographic region. Optimization of the vertical (and horizontal) sampling strategy requires a characterization of the atmosphere optical and dynamical properties, more in particular the distribution of atmospheric particles and their correlation with the atmospheric dynamics. The Aeolus atmospheric database combines meteorological data from the ECMWF model with atmosphere optical properties data from CALIPSO. An inverse algorithm to retrieve high-resolution particle backscatter from the CALIPSO level-1 attenuated backscatter product is presented. Global weather models tend to underestimate atmospheric wind variability. A procedure is described to ensure compatibility of the characteristics of the database winds with those from high-resolution radiosondes. The result is a high-resolution database of zonal, meridional and vertical wind, temperature, specific humidity and particle and molecular backscatter and extinction at 355 nm laser wavelength. This allows the simulation of small-scale atmospheric processes within the Aeolus observation sampling volume and their impact on the quality of the retrieved HLOS wind profiles. The database extends over four months covering all seasons. This allows a statistical evaluation of the mission components under investigation. The database is currently used for the development of the Aeolus wind processing, the definition of wind calibration strategies and the optimization of the Aeolus sampling strategy.

ABSTRACT Objective: To describe the modeling stages of food and nutrition surveillance in the Primary Health Care of the Unified Health Care System, considering its activities, objectives, and goals Methods: Document analysis and semi-structured interviews were used for identifying the components, describe the intervention, and identify potential assessment users. Results: The results include identification of the objectives and goals of the intervention, the required inputs, activities, and expected effects. The intervention was then modeled based on these data. The use of the theoretical logic model optimizes times, resources, definition of the indicators that require monitoring, and the aspects that require assessment, identifying more clearly the contribution of the intervention to the results Conclusion: Modeling enabled the description of food and nutrition surveillance based on its components and may guide the development of viable plans to monitor food and nutrition surveillance actions so that modeling can be established as a local intersectoral planning instrument.

Is systemic lupus erythematosus (SLE) is occurring more frequently now than in decades past? Despite improvements in the identification of patients with SLE, the development of new classification criteria, and the recognition of several biomarkers used alone or in combination, the diagnosis of SLE is still a challenge for clinicians, in particular early in the course of the disease, which makes the recognition of secular trends difficult to ascertain. Lacking a uniform definition of preclinical lupus or incomplete lupus, it is difficult to predict accurately which patients would go on to develop SLE. We will briefly review the classification criteria, early or preclinical SLE, the epidemiology of SLE, antinuclear antibodies-negative SLE, and biomarkers of the disease.

This article examines the meaning of appropriate technology in the World Health Organization's comprehensive definition of primary care. The author concludes that broad-ranging aspects of health maintenance, such as public health, personal lifestyles, and scientific research, as well as traditional diagnostic and therapeutic practices, need to be subjected to clear and careful scrutiny. It is vital that the results of these technology assessment studies be disseminated as widely as possible to both health care professionals and the public.

In 1978, at an international conference in Kazakhstan, the World Health Organization (WHO) and the United Nations Children’s Fund put forward a policy proposal entitled “Primary Health Care” (PHC). Adopted by all the World Health Organization member states, the proposal catalyzed ideas and experiences by which governments and people began to change their views about how good health was obtained and sustained. The Declaration of Alma-Ata (as it is known, after the city in which the conference was held) committed member states to take action to achieve the WHO definition of health as “state of complete physical, mental and social well-being and not merely the absence of disease or infirmity.” Arguing that good health was not merely the result of biomedical advances, health-services provision, and professional care, the declaration stated that health was a human right, that the inequality of health status among the world’s populations was unacceptable, and that people had a right and duty to become involved in the planning and implementation of their own healthcare. It proposed that this policy be supported through collaboration with other government sectors to ensure that health was recognized as a key to development planning. Under the banner call “Health for All by the Year 2000,” WHO and the United Nations Children’s Fund set out to turn their vision for improving health into practice. They confronted a number of critical challenges. These included defining PHC and translating PHC into practice, developing frameworks to translate equity into action, experiencing both the potential and the limitations of community participation in helping to achieve the WHO definition of health, and seeking the necessary financing to support the transformation of health systems. These challenges were taken up by global, national, and nongovernmental organization programs in efforts to balance the PHC vision with the realities of health-service delivery. The implementation of these programs had varying degrees of success and failure. In the future, PHC will need to address to critical concerns, the first of which is how to address the pressing health issues of the early 21st century, including climate change, control of noncommunicable diseases, global health emergencies, and the cost and effectiveness of humanitarian aid in the light of increasing violent disturbances and issues around global governance. The second is how PHC will influence policies emerging from the increasing understanding that health interventions should be implemented in the context of complexity rather than as linear, predictable solutions.

AbstractThis article is the first in a series of two articles about Occupational Therapy and Primary Care. This first article describes the health policy context in which primary health care should be strengthened. A definition of occupational therapy is given and the scope of the profession is explained. Based on a survey amongst the (experts of) member associations of COTEC, an overview is given of the main target groups and how occupational therapy is embedded and organized in different countries. In a position statement it is argued why occupational therapy can and should contribute to a comprehensive integrated primary care and challenges to strengthen the position of the profession are described.
