Sharing the road with trucks is associated with increased risk of serious injury and death for passenger vehicle drivers. However, the onus for minimising risk lies not just with truck drivers; other drivers must understand the unique performance limitations of trucks associated with stopping distances, blind spots, and turning manoeuverability, so they can suitably act and react around trucks. Given the paucity of research aimed at understanding the specific crash risk vulnerability of young drivers around trucks, the authors employ a narrative review methodology that brings together evidence from both truck and young driver road safety research domains, as well as data regarding known crash risks for each driving cohort, to gain a comprehensive understanding of what young drivers are likely to know about heavy vehicle performance limitations, where there may be gaps in their understanding, and how this could potentially increase crash risk. We then review literature regarding the human factors affecting young drivers to understand how perceptual immaturity and engagement in risky driving behaviours are likely to compound risk regarding both the frequency and severity of collision between trucks and young drivers. Finally, we review current targeted educational initiatives and suggest that simply raising awareness of truck limitations is insufficient. We propose that further research is needed to ensure initiatives aimed at increasing young driver awareness of trucks and truck safety are evidence-based, undergo rigorous evaluation, and are delivered in a way that aims to (i) increase young driver risk perception skills, and (ii) reduce risky driving behaviour around trucks.

BackgroundYoung drivers of low socioeconomic status (SES) have a disproportionally high risk of crashing compared with their more affluent counterparts. Little is known if this risk persists into adulthood and if it differs between men and women.MethodsWe used data from a 2003/2004 Australian survey of young drivers (n=20 806), which included measures of drivers’ demographics and established crash risk factors. These data were linked to police-reported crash, hospital and death data up to 2016. We used negative binomial regression models to estimate the association between participants’ SES, with car crash.ResultsAfter adjusting for confounding, drivers of lowest SES had 1.30 (95% CI 1.20 to 1.42), 1.90 (95% CI 1.25 to 2.88), 3.09 (95% CI 2.41 to 3.95) and 2.28 (95% CI 1.85 to 2.82) times higher rate of crash, crash-related hospitalisation, crash in country areas and crash on streets with a speed limit of 80 km/hour or above compared with drivers of highest SES, respectively. For single-vehicle crashes, women in the lowest SES groups had 2.88 (95% CI 1.83 to 4.54) times higher rate of crash compared with those in the highest SES group, but no differences were observed for men from different SES groups.ConclusionYoung drivers who lived in areas of low SES at the time of the survey had a sustained increased risk of crash over the following 13 years compared with drivers from the most affluent areas. Our findings suggest that in addition to traditional measures, road transport injury prevention needs to consider the wider social determinants of health.

This study examines whether cell phone use stands apart from a general pattern of risky driving practices associated with crashes and impulsivity-related personality traits in young drivers. A retrospective online survey study recruited 384 young drivers from across the United States using Amazon’s Mechanical Turk to complete a survey measuring risky driving practices (including cell phone use), history of crashes, and impulsivity-related personality traits. Almost half (44.5%) of the drivers reported being involved in at least one crash, and the majority engaged in cell phone use while driving (up to 73%). Factor analysis and structural equation modeling found that cell phone use loaded highly on a latent factor with other risky driving practices that were associated with prior crashes (b = 0.15, [95% CI: 0.01, 0.29]). There was also an indirect relationship between one form of impulsivity and crashes through risky driving (b = 0.127, [95% CI: 0.01, 0.30]). Additional analyses did not find an independent contribution to crashes for frequent cell phone use. These results suggest a pattern of risky driving practices associated with impulsivity in young drivers, indicating the benefit of exploring a more comprehensive safe driving strategy that includes the avoidance of cell phone use as well as other risky practices, particularly for young drivers with greater impulsive tendencies.

Abstract Polycythemia vera (PV) is a myeloproliferative disorder associated with an increased risk of cerebrovascular diseases. In this narrative review, we aimed to analyze the relationships between acute ischemic stroke and PV. We conducted a PubMed/Medline and Web of Sciences Database search using MeSH major terms. We found 75 articles and finally considered 12 case reports and 11 cohort studies. The ischemic stroke resulted as the first manifestation of PV in up to 16.2% of cases; the cumulative rate of cerebrovascular events was up to 5.5 per 100 persons per year and stroke accounted for 8.8% of all PV-related deaths; age, mutations, and a previous history of thrombosis were the main risk factors. The best approach to reduce stroke recurrence risk is unclear, even if some evidence suggests a potential role of lowering hematocrit below 45%. Ischemic stroke represents one of the most common PV manifestations but, despite their relationship, patients with both diseases have a very heterogeneous clinical course and management. PV-related strokes often remain underdiagnosed, especially for the low prevalence of PV. An early diagnosis could lead to prompt treatment with phlebotomy, cytoreduction, and low-dose aspirin to decrease the risk of recurrences. Clinicians should be aware of PV as a risk factor for stroke when approaching the differential diagnosis of cryptogenic strokes. An early diagnosis could positively influence patients’ management and clinical outcomes. Further studies are required to evaluate the role of PV treatments in the prevention of cerebrovascular disease.

New technologies and traffic data sources provide great potential to extend advanced strategies in freeway safety research. The High Definition Monitoring System (HDMS) data contribute comprehensive and precise individual vehicle information. This paper proposes an innovative Variable Speed Limit (VSL) based approach to manage crash risks by intervening in traffic flow dynamics on freeways using HDMS data. We first conducted an empirical analysis on real-time crash risk estimation using a binary logistic regression model. Then, intensive microscopic simulations based on AIMSUN were carried out to explore the effects of various intervention strategies with respect to a 3-lane freeway stretch in China. Different speed limits with distinct compliance rates under specified traffic conditions have been simulated. By taking into account the trade-off between safety benefits and delay in travel time, the speed limit strategies were optimized under various traffic conditions and the model with gradient feedback produces more satisfactory performance in controlling real-time crash risks. Last, the results were integrated into lane management strategies. This research can provide new ideas and methods to reveal the freeway crash risk evolution and active traffic management.

As drivers age, their risk of being involved in a car collision decreases. The present study investigated if this trend is due, in part, to some risky drivers having a collision early in their driving lives and subsequently reducing their risky driving after that negative experience. Accelerometers and video cameras were installed in the vehicles of 16- to 17-year-old drivers ( N = 254), allowing coders to measure the number of g-force events (i.e., events in which a threshold acceleration level was exceeded) per 1,000 miles and the number of collisions. Among the 41 participants who experienced a severe collision, the rate of g-force events dropped significantly in the 1st month after the collision, remained unchanged for the 2nd month, and increased significantly in the 3rd month. There were no changes in the rate of g-force events at comparable time points for the drivers not involved in a collision. Being involved in a collision led to a decrease in risky driving, but this may have been a temporary effect.

Young drivers’ disproportionate involvement in road crashes due to cellphone distractions calls for remedial programs. Development of such programs requires an understanding of the reasons behind engagement in distractions and identifying factors to leverage, such as emotions. This investigation sought to determine how young drivers’ intention to engage in cellphone distractions is associated with negative emotions. An online survey was administered to 99 young drivers (18 to 25). Participants rated their intention to engage in cellphone distractions while driving and their anticipation of negative emotions while doing so. Our results showed an association between anticipating negative emotions and the intention to engage in cellphone distractions while driving. There was no difference between males and females in their reported intention. However, females reported greater anticipation of negative emotions compared to males. These findings have implications for emotion-based manipulations aimed at reducing distracted driving among young drivers.
