Despite significant progress in trisomy 21 (T21) diagnostic tools, amniocentesis is still used for the confirmation of an abnormal fetal karyotype. Invasive tests carry the potential risk of miscarriage; thus, screening biomarkers are commonly used before undergoing invasive procedures. In our study, we investigated the possible application of oxidative stress markers in the prenatal screening of trisomy 21. The DNA/RNA oxidative stress damage products (OSDPs), advanced glycation end (AGE) products, ischemia-modified albumin (IMA), alfa-1-antitrypsin (A1AT), asprosin, and vitamin D concentrations were measured in both maternal plasma and amniotic fluid in trisomy 21 (T21) and euploid pregnancies. The obtained results indicated increased levels of DNA/RNA OSDPs and asprosin with simultaneous decreased levels of vitamin D and A1AT in the study group. The diagnostic utility of the plasma measurement based on the area under the received operative characteristic (ROC) curve (AUC) calculation of asprosin (AUC = 0.965), IMA (AUC = 0.880), AGE (AUC = 0.846) and DNA/RNA OSDPs (AUC = 0.506) in T21 screening was demonstrated. The obtained results indicate a potential role for the application of oxidative stress markers in the prenatal screening of T21 with the highest screening utility of plasma asprosin.

Background: According to a recent World Health Organization (WHO) report, India, with 32 million diabetic individuals, currently has the highest incidence of diabetes worldwide; these numbers are predicted to increase to 80 million by the year 2030. Deficiency of vitamin D has been associated with increased risk of developing Type 2 diabetes mellitus (DM) and cardiovascular diseases. Vitamin D deficiency is highly prevalent in our country. About 70% of adults in both rural and urban areas were found showing manifestations of vitamin D deficiency. Therefore, we designed this study to assess the vitamin D status of the study population by measuring serum 25(OH) D levels, and its association with oxidative stress markers in type 2 diabetes mellitus.Methods: This is a cross sectional study with Group 1 (n=147): Newly diagnosed type 2 diabetics and Group 2 (n=147): Apparently healthy individuals. Blood was collected by venipuncture. 5ml of blood was collected and allowed to clot. Serum was separated and stored in refrigerator to estimate the oxidative stress markers and vitamin D levels. Estimation of vitamin D levels and oxidative stress markers were carried out by commercially available kits.Results: Vitamin D levels are significantly low in newly diagnosed type 2 diabetics when compared to controls (p < 0.000), whereas the FBG levels are significantly high in newly diagnosed type 2 diabetics when compared to controls (p < 0.000). The TAOS levels are significantly low in newly diagnosed type 2 diabetics when compared to controls (p <0.000), whereas the MDA levels are significantly high in newly diagnosed type 2 diabetics when compared to controls (p < 0.000). The oxidative stress marker TAOS (r = 0.71; p < 0.000), was positively correlated and MDA (r = - 0.85; p < 0.000), was negatively with Vitamin D in newly diagnosed type 2 diabetics.Conclusions: From this study, it is concluded that, lower levels of vitamin D is associated with increased oxidative stress. Therapeutic interventions to increase the vitamin D levels and reduce the oxidative stress should be included as a part of treatment inn newly diagnosed type diabetics.

Introduction. Not only is diabetic nephropathy (DN) the most common cause of end-stage renal disease worldwide, but it also increases the risk of mortality up to fourteen times compared to normoalbuminuric diabetic patients. Aim. The aim of the current study was the evaluation of the renoprotective effects of vitamin D in DN and the possible interplay between autophagy and mTOR pathways. Materials and Methods. Fifty male Wistar albino rats were divided (10/group) into control, DN group, insulin-treated DN group, vitamin D-treated DN group, and combined insulin and vitamin D-treated DN group. Assessments of systolic blood pressure, albuminuria, creatinine clearance, serum glucose, insulin, urea, creatinine, inflammatory cytokines, oxidative stress markers, and rat kidney gene expression of mTOR were performed. Histopathological and immunohistochemical assessments of autophagy marker LC3 in rat kidneys were also performed. Results. DN was associated with significant increases in SBP, urinary albumin, serum glucose, urea, creatinine, inflammatory cytokines, MDA, and mTOR gene expression (P<0.05). However, there was significant decrease in creatinine clearance, serum insulin, GSH, and H score value of LC3 when compared with control group (P<0.05). The combination of insulin and vitamin D treatment significantly restored DN changes when compared with the other treated groups, except in oxidative stress markers where there was an insignificant difference between the combination-treated and insulin-treated groups (P>0.05). Conclusion. It has been concluded that vitamin D is a potent adjuvant therapy in treatment of DN via downregulation of mTOR gene expression, stimulation of autophagy, and antioxidant, anti-inflammatory, and hypotensive effects.
