Atherosclerosis, myocardial infarction (MI) and heart failure (HF) are the main causes of mortality and morbidity around the globe. New therapies are needed to better manage ischemic heart disease and HF as existing strategies are not curative. Resveratrol is a stilbene polyphenolic compound with favorable biological effects that counter chronic diseases. Current evidence suggests that resveratrol is cardioprotective in animal models of atherosclerosis, ischemic heart disease, and HF. Though clinical studies for resveratrol have been promising, evidence remains inadequate to introduce it to the clinical setting. In this narrative review, we have comprehensively discussed the relevant compelling evidence regarding the efficacy of resveratrol as a new therapeutic agent for the management of atherosclerosis, MI and HF.

Ischemic Heart Disease is the number one killer in Malaysia and the total number of death has increased statistically. To make matters worse, the cost of medical related to this disease has increased approximately from 10 percent to 15 percent every year. Therefore, it is crucial to know the expected medical cost of heart disease and the risk exposure. This paper discussed on four different type of major illness relating to heart disease and the expected cost of procedure based on type of illness and age. The objectives are to analyse percentage of people getting heart disease based on age, gender and ethnic, calculation of the probability of four major categories of illness in heart disease and to estimate the total expected cost of heart disease. The result showed the expected cost of heart disease varies based on the cost of procedures and the severity of the heart disease of the patient; the higher the rate of severity, the higher the expected cost would be.

Objectives. To clarify the importance of clinical features and changes in the first electrocardiogram in 28-day and 1-year mortality in patients with diabetes. Material and methods. Men and women of Kaunas city aged 25–64 years with the first-ever myocardial infarction during 1983–1992 and with the first electrocardiogram were enrolled in the study. Electrocardiograms were coded using the WHO MONICA Project Protocol criteria and the Minnesota Code. The Kaunas Ischemic Heart Disease Register was the source of data; deaths from ischemic heart disease were identified via death register. Results. Diabetes was diagnosed in 124 patients: 65 (52.4%) men and 59 (47.6%) women. The 28-day (P=0.01) and 1-year mortality rates (P<0.001) were higher in diabetic than in nondiabetic patients with myocardial infarction. Among diabetic patients, who died during 28 days or one year, myocardial infarction was more often complicated by acute heart failure, and changes in ECG were more often detected than among those who were alive. Female gender (RR=30.2, P=0.02) was associated with an increased risk of death from a first-ever myocardial infarction during the first 28 days, while acute heart failure (RR=4.48, P=0.01) and anterior location of Q wave in the first ECG (RR=2.71, P=0.04) increased the risk of death from ischemic heart disease during one year after a first-ever myocardial infarction. Conclusions. Acute heart failure and Q-wave in derivations of the first electrocardiogram reflecting anterior site of myocardial infarction increased the risk of death from ischemic heart disease during the first year, and female gender – during the first 28 days in diabetic patients with myocardial infarction.

Abstract Introduction The interface between heart and bone emerges as a key trigger of post-infarction inflammation and progression of chronic heart failure (CHF). However, our knowledge on the underlying mechanisms of this interaction is incomplete. Bone vasculature, specifically so-called H-type (Endomucinhigh) endothelial cells (EC), plays a crucial role in maintenance of the bone integrity and regulation of hematopoietic stem cells (HSC). While previous studies in mice showed the reduction of H-type vessels by aging, the impact of ischemic heart disease is unclear. Therefore, we aimed to investigate the effects of myocardial infarction (MI) and chronic heart failure on the vascular bone cell composition in mice and humans. Methods and results Flow cytometric analysis of harvested bones at the different timepoints after MI induction in mice revealed a gradual loss of H-type endothelial cells in the time-course of developing chronic heart failure (P&lt;0.05 at day 7 and 14 vs. control; P&lt;0.0001 at day 28 vs. control). This results were confirmed in immunostainings of tibia sections showing a significant decrease of H-type vessel length after MI (p&lt;0.05 at day 14 and 28, accordingly). The loss of type-H endothelium was accompanied by a significant expansion of long-term HSC in the bone (P=0.0005 at day 28 post MI vs. control). Importantly, type H ECs were also significantly reduced in the bone of ischemic post-infarct HF patients (n=16) compared with control subjects (n=8; P=0.0003). To gain insights into the mechanisms underlying the changes in the vascular niche, we performed single cells RNA sequencing of human BM ECs. These studies confirmed the decrease in Emcnexpressing ECs in ischemic HF patients, which was accompanied by significantly increased expression of inflammatory genes, including IL1b (P&lt;0.0001). Inflammatory EC phenotypes and IL1b expression in HF could be further confirmed at protein level using cytospin immunostainings. Murine studies further revealed an early induction IL-1b specifically in H-type vessels already at day 1 after MI induction, which preceded the loss of H-type endothelium within the following 4 weeks. By inhibiting IL-1b production using a specific Nlrp3 inflammasome inhibitor (MCC950) we could observe a partial restoration of H-type EC frequencies (P=0.033) and a significant increase in H-type vessel length (p=0.035) at day 28 day after MI. Conclusions Our data show for the first time an impact of myocardial infarction and chronic heart failure on the bone marrow vasculature. These changes seem to be strongly associated with inflammatory response in H-type vessels, which precedes its loss after MI. Specifically, the induction of the inflammatory cytokine IL1b may contribute to the disturbed phenotype. This suggest that inhibition of IL1b (e.g. by canakinumab) be used as a novel strategy to prevent or reverse the deterioration of the vascular bone function in ischemic heart disease. : Subjects affected by ischemic heart disease with non-obstructive coronary arteries constitute a population that has received increasing attention over the past two decades. Since the first studies with coronary angiography, female patients have been reported to have non-obstructive coronary artery disease more frequently than their male counterparts, both in stable and acute clinical settings. Although traditionally considered a relatively infrequent and low-risk form of myocardial ischemia, its impact on clinical practice is undeniable, especially when it comes to infarction, where the prognosis is not as benign as previously assumed. Unfortunately, despite increasing awareness, there are still several questions left unanswered regarding diagnosis, risk stratification and treatment. The purpose of this review is to provide a state of the art and an update on current evidence available on gender differences in clinical characteristics, management and prognosis of ischemic heart disease with non-obstructive coronary arteries, both in the acute and stable clinical setting.

The aim of this work was to compare the prevalence of metabolic syndrome and smoking habits smokingduring a 10-year period and to evaluate the association between metabolic syndrome and smoking habits, and ischemic heart disease among Kaunas men aged 45–64 years. Material and methods. In this study, we have used data from two epidemiological studies, which had been carried out according to the MONICA study protocol (359 men aged 45–64 years were enrolled in 1992–1993 and 408 men aged 45–64 years – in 2001–2002). The association between metabolic syndrome and smoking habits, and ischemic heart disease was established according to the data of 2001–2002 years. Ischemic heart disease was diagnosed based on the following criteria: previous myocardial infarction, angina pectoris, or ischemic changes in electrocardiogram. Metabolic syndrome was defined by Adult Treatment Panel III (ATP III) criteria. Results. The prevalence of ischemic heart disease did not change among men aged 45–64 years during a 10-year period. During this period, the decreased prevalence of metabolic syndrome was observed; decreased rate of hyperglycemia, decreased high-density lipoprotein cholesterol level, increased rate of hypertriglyceridemia, and increased waist circumference were noted. During this period, the proportion of regular male smokers increased significantly. After the evaluation of association between and metabolic syndrome and smoking habits, and ischemic heart disease (according to the data of 2001–2002 years), it was determined that the highest rate of ischemic heart disease was among regular smokers with metabolic syndrome (32.3%), and the lowest rate of ischemic heart disease was noted among men who had never smoked and were without metabolic syndrome (11.6%) (OR=3.63; P=0.013). The highest rate of previous myocardial infarction and/or angina pectoris was determined among regular smokers with metabolic syndrome (19.4%), and the lowest rate of ischemic heart disease was determined among men who had never smoked and were without metabolic syndrome (3.6%) (OR=6.43; P=0.008). Conclusion. Combination of metabolic syndrome and smoking is significantly associated with ischemic heart disease among men aged 45–64 years.
