South Africa’s interventions to address complex social challenges rely on coordination across several sectors and between different levels of government and society. Improved alignment, planning and coordination are needed when addressing the causal factors of these social challenges. These causal factors include the environments in which people live and their behaviours. Furthermore, emphasis is placed on the recurring engagement of civil society, especially of marginalized stakeholders, as participants in the efforts to address the challenges. The study draws from the promise shown by stakeholder networks, termed Innovation Platforms, in other Sub-Saharan Africa countries to address such complex social challenges. The study aimed to improve the understanding of how a stakeholder network’s engagement practices impact the effectiveness of the network. To this end, a conceptual framework and management tool for stakeholder engagement in IPs is proposed. The study followed the conceptual framework analysis procedure to develop, evaluate and refine the conceptual framework. The article describes the core research outcomes of the framework development approach, starting with a systematized literature review to identify core concepts, followed by interviews with experts and a case study to refine the framework content. The case study applied the framework to develop recommendations for improved engagement in a stakeholder network which has been established around the challenge of vagrancy in Stellenbosch, South Africa. The result of the approach is a multidimensional framework for conceptualizing stakeholder engagement practices in a variety of contexts. The focus of the framework content remains on the practices of engagement which enable effective and fruitful stakeholder interactions within and around a network. The study delivered valuable insights into the nature of some development initiatives in South Africa and the impact of stakeholder engagement on them.

Purpose – The purpose of this paper is to explore the disconnect between policy intent and policy implementation in relation to regional/local (sub-national) TV deliverables in South Africa between 1990 and 2011, and evaluate the impact of this disconnect in pursuit of public interest objectives. Design/methodology/approach – The article is based on a research case study in which data extracted from policy documents and interviews were qualitatively analysed via the Kingdon “policy streams” framework and the Feintuck and Varney public interest media regulation framework. Findings – It was found that ruptures in deliberative policymaking, and policy implementation missteps, undermined sub-national TV delivery and, in turn, undermined pursuit of the public interest. Originality/value – By combining a political science conceptual framework with a media policy conceptual framework, the article provides unique insights into South African TV policymaking in the early democratic era.

This article reports on a study that investigated the impact of organisational culture on internal knowledge production and assessed the challenges of producing knowledge at the Africa Institute of South Africa (AISA), which is seen as a model knowledge producing think tank in sub-Saharan Africa. The broad objectives of the study were: identifying AISA’s achievements in knowledge production; finding out the challenges AISA confronts in producing knowledge; examining how AISA’s organisational culture impacts on internal knowledge production; and suggesting ways in which knowledge production at AISA and other think tanks may be improved. A case study was conducted and self-administered questionnaires, face-to-face interviews, document analysis, and observation were used to collect data. The findings showed that AISA’s knowledge production efforts are confronted by several challenges, including: organisational culture and employees’ negative attitudes towards sharing knowledge freely, and employees encountering difficulties in finding the information and knowledge they need. If these challenges could be identified and clearly confined, it is argued that AISA would be in a better position to effectively produce and utilise knowledge, enabling it to achieve its objectives more efficiently. It is recommended that AISA acquire knowledge from external sources; produce knowledge internally which it uses and is used by its clientele; and establish itself as a knowledge-based organisation by creating a knowledge friendly culture as a framework for addressing the issue of organisational culture. The study results will hopefully lay a foundation for understanding ways of improving knowledge production at AISA and thus influence positive public policy in sub-Saharan Africa.

Objective: The number of people living with HIV (PLWH) over 50 years old in sub-Saharan Africa is predicted to triple in the coming decades, to 6-10 million. Yet, there is a paucity of data on the determinants of health and quality of life for older PLWH in the region. Methods: A review was undertaken to describe the impact of HIV infection on aging for PLWH in sub-Saharan Africa. Results: We (a) summarize the pathophysiology and epidemiology of aging with HIV in resource-rich settings, and (b) describe how these relationships might differ in sub-Saharan Africa, (c) propose a conceptual framework to describe determinants of quality of life for older PLWH, and (d) suggest priority research areas needed to ensure long-term gains in quality of life for PLWH in the region. Conclusions: Differences in traditional, lifestyle, and envirnomental risk factors, as well as unique features of HIV epidemiology and care delivery appear to substantially alter the contribution of HIV to aging in sub-Saharan Africa. Meanwhile, unique preferences and conceptualizations of quality of life will require novel measurement and intervention tools. An expanded research and public health infrastructure is needed to ensure that gains made in HIV prevention and treamtent are translated into long-term benefits in this region.

A Conceptual Framework for Evaluating Joint Venture Opportunities Between Hospitals and Physicians This paper reviews the changes in the competitive and regulatory environment and examines the impact of those changes on the relationships between hospitals and physicians. Transaction cost economics (TCE) provides a conceptual framework for examining the emergence of closer linkages between hospitals and physicians than the traditional independent hospital and medical staff organisations. TCE predicts that as investments in support of transactions become more specialised, closer linkages are more efficient. To illustrate, two case studies of successful hospital-physician joint ventures are presented. The first case study describes a joint venture between hospitals and physicians to purchase durable medical equipment. The second case describes the breakdown of an informal arrangement and the subsequent formation of a joint venture to organise a clinical programme. The discussion reports the rationale for choosing these structural arrangements and their key features, pointing out how TCE would account for the decision to establish a joint venture. The conclusion discusses the implications of this argument for the strategic decisions of health care managers.

Extended shifts are common in medical practice. This is when doctors are required to work continuously for more than 16 h, with little or no rest, often without a maximum limit. These shifts have been a part of medical practice for more than a century. Research on the impact of fatigue presents compelling evidence that extended shifts increase the risk of harm to patients and practitioners. However, where the number of doctors is limited and their workloads are not easily reduced, there are numerous barriers to reform. Some of these include a perceived lack of safer alternatives, concerns about continuity of care, trainee education, and doctors’ preferences. As such, working hour reorganisation has been contentious globally. South Africa, a middle-income country where extended shifts are unregulated for most doctors, offers a useful case study of reform efforts. The South African Safe Working Hours campaign has promoted working hour reorganization through multi-level advocacy efforts, although extended shifts remain common. We propose that extended shifts should be regarded as an occupational hazard under health and safety legislation. We suggest options for managing the risks of extended shifts by adapting the hierarchy of controls for occupational hazards. Despite the challenges reform pose, the practice of unregulated extended shifts should not continue.

Abstract We investigate factors which may drive the number of agents who compete for a specific piece of farmland in the French region of Brittany by fitting count models to data originating from a local committee, the CDOA, which is responsible for guidance in delivering the necessary “authorisations to farm”. Results are analysed in the light of a conceptual framework which provides an explanation why the subset of farmers who actually apply for an authorisation to farm may differ from the whole set of potential applicants. Several results reflect the impact of an informal mediation by local farmer unions, which aims to lower potential conflicts and hence competition.

Over the past twenty years, the focus of development policy has shifted from the state to the private sector. Privatisation is now central to utility reform in much of SSA. This paper sets out developments in water privatisation and reviews the evidence regarding its impact. Water privatisation has been carried out to some degree in at least fourteen countries in the region, and many other governments are at various stages in the privatisation process. However, in some cases privatisation has been difficult to achieve, and a few countries have successfully provided water under public ownership. Evidence on the impact of privatisation indicates that the performance of privatised utilities has not changed dramatically, but that enterprises have continued to perform well, or not so well, depending both on their state when they were privatised and on the wider economic context. The evidence points to internal improvements in terms of financial management. However, governments face considerable difficulties in attracting investors and regulating private utilities. Furthermore, privatisation fails to address some of the fundamental constraints affecting water utilities in SSA, such as finance, the politicised nature of service delivery, and lack of access for the poor. A preoccupation with ownership may obscure the wider goals of reform.

Having viewed the successful social franchising partnerships pilot programme that serviced sanitation facilities at 400 schools in the Butterworth District of the Eastern Cape of South Africa, the Amathole District Municipality (ADM) expressed interest in exploring how well the partnership model could empty household pit latrines in its jurisdiction. The impact and effectiveness of the model was demonstrated by the emptying, by five franchisees over a period of only six weeks, of the contents of 400 household ventilated improved pit latrines in Govan Mbeki Village, and the safe disposal of their content. The paper describes the methods and results in removal and disposal of faecal sludge. Problems were encountered, and the solutions (technical, institutional and social) are described. Not unexpectedly, the amount of effort involved in this work – including time, training required, equipment required and ingenuity – varied enormously. The main variables included the type of top structure, the nature of the pit contents, whether or not there was broad consistency of type and contents in an area, distances (between pits, from home base to work site, from pits to disposal site, from location of specialized equipment to work site), logistical delays (e.g. non-arrival of equipment) and bureaucratic hold-ups (especially payment delays).
