Introduction. Currently, medical organizations of Moscow Healthcare Department have a three-level system of internal control for assessing the quality and safety of medical activities. At the same time, insurance companies have adopted a logically consistent, alternating multi-level system of inspections (medical and economic control) and evaluations (medical and economic evaluation, quality of medical care evaluation). Methods, techniques, criteria and guidelines for external and internal control of the quality and safety of medical activities are different. In this regard, assessment of the effectiveness of internal quality control of medical care in medical organizations during control and supervision activities is an important task that requires an evidence-based decision. Objectives. To provide evidence for the effectiveness of assessment criteria when conducting internal quality control of medical care and to determine their impact on effective performance of medical organizations. Materials and methods. The authors studied the results of 10 inspections and 36 evaluations of regulatory and supervisory authorities (insurance companies, Federal Service for Surveillance in Healthcare (Roszdravnadzor), etc.) and systematized the reasons for applying deductions and penalties. We performed a comparative analysis with evaluation criteria used by the heads of structural divisions (departments, laboratories) of City Clinical Hospital named after brothers Bakhrushin of Moscow Healthcare Department, as well as other private medical organizations. Authors studied technical capabilities of various automated information systems (Megaclinic, Asklepius, Doctor’s Automated Workstation, EMIAS for polyclinics and hospitals, etc.). Results. Authors performed comparative analysis of criteria used in inspections and evaluations of external and internal control of the quality and safety of medical activities. Analysis showed that the currently used assessment criteria do not fully ensure fulfillment of requirements and guidelines, violation of which entails financial sanctions (deductions, fines). Conclusion. It is necessary to develop and implement methods for organizing and conducting internal control of quality and safety of medical activities to create «effective criteria» that will minimize financial sanctions (deductions, fines) applied during various inspections (evaluations) of control and supervisory authorities.

The main attention is focused on the issues of implementation of the Suggestions (recommended practice) of Roszdravnadzor on organization of the internal quality and safety control of medical activities, as well as particular points of the Order of June 7, 2019 No. 381n “On Approval of Requirements for organization and conduction internal control of the quality and safety of medical care in hospitals”.

The aim of the study, analyze and evaluate the quality control and safety of medical activities carried out in medical organizations. Results: the essence of the concept of quality control in the field of medical care was determined, criteria for assessing the quality of medical activities were identified, the main aspects of developing a program for internal quality control and safety of medical activities were characterized, key risks existing in medical activities were identified, directions for improving the quality and safety of professional activities of medical workers. Conclusion: modern approaches to the organization of quality control and safety of medical activities make it possible to ensure a high level of medical care provided to the population and improve the quality of life of the population as a whole.

The poor quality of expert reports prepared at the stage of preliminary investigation constitutes the main reason for summoning expert witnesses to testify before a court of law and posing numerous questions to them. The same applies to the examinations conducted on case materials, both in the course of criminal and civil proceedings. Тhe lack of internal quality control over forensic medical examinations in cases related to inadequate medical care may compromise the reliability and scientific validity of conclusions. Forensic medical examinations related to the cases of inadequate medical care constitute the primary source of evidence affecting the validity and legitimacy of decisions made by law enforcement agencies about the criminal liability of medical staff. In addition, scientifically unfounded expert conclusions may lead to investigative and judicial errors.Conclusion. Strict internal control over the forensic medical examinations is necessary to ensure that state forensic medical institutions produce expert reports fully compliant with the requirements of Law No. 73-FZ, which naturally should begin with a self-check.

Introduction. The article presents an example of implementation of the principles of total quality management (TMQ) in the system of internal quality and safety control of medical practice in a large medical organization. Results. The dispensary has established a unified quality service that unites all the structures of the clinic involved in the implementation of internal control and interaction with the subjects of external control, thus becoming a coordination center. Summary. The service functions include complex tasks, previously attributed to the functions of various divisions of the institution.
