The aim of the study, analyze and evaluate the quality control and safety of medical activities carried out in medical organizations. Results: the essence of the concept of quality control in the field of medical care was determined, criteria for assessing the quality of medical activities were identified, the main aspects of developing a program for internal quality control and safety of medical activities were characterized, key risks existing in medical activities were identified, directions for improving the quality and safety of professional activities of medical workers. Conclusion: modern approaches to the organization of quality control and safety of medical activities make it possible to ensure a high level of medical care provided to the population and improve the quality of life of the population as a whole.

Introduction. Currently, medical organizations of Moscow Healthcare Department have a three-level system of internal control for assessing the quality and safety of medical activities. At the same time, insurance companies have adopted a logically consistent, alternating multi-level system of inspections (medical and economic control) and evaluations (medical and economic evaluation, quality of medical care evaluation). Methods, techniques, criteria and guidelines for external and internal control of the quality and safety of medical activities are different. In this regard, assessment of the effectiveness of internal quality control of medical care in medical organizations during control and supervision activities is an important task that requires an evidence-based decision. Objectives. To provide evidence for the effectiveness of assessment criteria when conducting internal quality control of medical care and to determine their impact on effective performance of medical organizations. Materials and methods. The authors studied the results of 10 inspections and 36 evaluations of regulatory and supervisory authorities (insurance companies, Federal Service for Surveillance in Healthcare (Roszdravnadzor), etc.) and systematized the reasons for applying deductions and penalties. We performed a comparative analysis with evaluation criteria used by the heads of structural divisions (departments, laboratories) of City Clinical Hospital named after brothers Bakhrushin of Moscow Healthcare Department, as well as other private medical organizations. Authors studied technical capabilities of various automated information systems (Megaclinic, Asklepius, Doctor’s Automated Workstation, EMIAS for polyclinics and hospitals, etc.). Results. Authors performed comparative analysis of criteria used in inspections and evaluations of external and internal control of the quality and safety of medical activities. Analysis showed that the currently used assessment criteria do not fully ensure fulfillment of requirements and guidelines, violation of which entails financial sanctions (deductions, fines). Conclusion. It is necessary to develop and implement methods for organizing and conducting internal control of quality and safety of medical activities to create «effective criteria» that will minimize financial sanctions (deductions, fines) applied during various inspections (evaluations) of control and supervisory authorities.

According to the requirement of 20CrMnTiH steel, the effects of casting peed, EMS current intensity and secondary cooling system on internal quality of billets are studied. The results show that, it is beneficial to internal quality control of 20CrMnTiH billets when the casting speed is in the range of 1.2~1.4m/min, and when the current intensity of EMS is 380A, the level of center porosity can be controlled in 1.0, the index of carbon segregation can be controlled in 1.08 and equiax crystal ratio reaches 34.9%. Furthermore, it is beneficial to internal quality improvement when weak secondary cooling system is adopted. With the application of the studies, the quality of billet macrostructure and hardenability can meet the requirement of 20CrMnTiH steel.

Evaluation (including within the framework of medico-legal examination) of the quality and efectiveness of endodontic dental treatment is a matter of current interest in the clinical dentistry.Obiectives. Te aim of the work was to develop an algorithm of medico-legal examination in case of endodontic treatment, to test this algorithm in the work of the commissions of medical care quality control in the departments of therapeutic dentistry of outpatient dental clinics.Material and methods.Te material of the study was the reports of daily activities of dental clinics of various forms of ownership (municipal, departmental, private) in 3 regions of Russia (Moscow, St. Petersburg, Rostov-on-Don), including periodic reports and reference reports, ofcial statistical reports of the activities of the examined clinics, including registration books of the work of the commissions of the quality of medical care. Tese materials were used as a source of primary statistical information. Afer grouping of the information contained in them, the main data characterizing organization, condition and quality of endodontic care were recovered taking into account nosological entities: pulpitis and periodontitis. Taking into account the medical, statistical and analytical nature of the study, the main conclusions and recommendations were formulated on the basis of a retrospective study of these materials.Results. Based on the studies carried out and recommendations of the European Endodontic Association on provision of the standard of endodontic treatment, an algorithm for medico-legal examination of endodontic treatment was proposed. Tis algorithm allows to assess the quality and efectiveness of endodontic treatment of pulpitis and periodontitis in diferent terms (immediately afer treatment, remote period)Conclusion. Te use of the developed algorithm in clinical practice in internal quality control of endodontic care has shown its efectiveness. A large number of clinical cases of insufcient quality of endodontic treatment and its low efectiveness in the remote period indicate persistence of the potential for claims from patients.

Abstract Introduction/Objective The management of internal quality control (IQC) in Sidra Medicine Clinical Chemistry Division has been evaluated in order to promote a more consolidated and efficient process of IQC management. The statistical data produced from Cerner QC Module are transferred to IQC review templates consisting of formulas to auto- calculate parameters such as multiple of expected QC failure frequency and desirable comparison limit between analyzers. The IQC review and documentation process using the in-house excel template requires several hours to complete, hence a faster and more efficient IQC management module is required. The main objective of this study is to improve the initial IQC management set up, work flow and review procedures and to implement Biorad Unity Real Time (URT) program to develop a more efficient IQC management system. Methods The URT software has been recently configured and implemented to consolidate and streamline IQC management. URT is built through Sidra Medicine IT Enterprise level which allows multiple users to login. IQC data are downloaded using scripts from Cerner which are filtered through Biorad Unity Connect (UC) software. Additional quality tools are also explored such as various user defined statistical reports, IQC analysis using peer reviewed total allowable error (TeA) and assignment of the most appropriate Westgard rules. Determination of sigma metrics and uncertainty of measurement is also performed using the URT application. Results The generation of any IQC report is less cumbersome and time consuming as compared with the previous process. However, some user defined formulas in the IQC templates are not found on the URT reports. The URT Levey Jennings chart are also more user friendly and directly compares the daily IQC data with Unity inter-laboratory peers enabling the production of instant and monthly reports through QCNet site when assay investigation is required and for IQC report documentation. Conclusion The combination of Cerner IQC, Unity Real-time, QCNet Inter-laboratory reports and in house IQC templates produce a high level and very detailed IQC review which effectively evaluate assay performance to assist on IQC troubleshooting and root cause analysis to be able to apply the most appropriate corrective actions.

Lead is widely knows as one of the harmful pollutants for humans. Contaminated fishery built by lead may endanger the consumption body. Therefore, contamination monitoring for the fishery products is necessary to be conducted. To ensure the validity of data, Oyster Tissue 1566b certified reference material was used as Internal Quality Control. Marlin fish powder and certified reference materials were analyzed with the same method, personnel, duration or time, equipment and environmental conditions. As the results, it was found that the lead contained in CRM was 0.306 mg.kg-1 with 11.9 % rsd, which is within range of lead content in  the certificate. The method was applied for Marlin Fish. Lead content obtained in this study (1.821 mg.kg-1 with 11.06 % rsd) can be reported with high level of confidence.Keywords: Lead, Certified Reference Materials, Marlin fish

Can we match formal procedures for quality assurance with the creativity and individuality that defines honours? In a session at the Utrecht Honours Conference 2016: Honours Futures we discussed experiences in quality assurance of honours programmes in the United States of America, the Netherlands and Belgium. To illustrate the possibilities, a case study of using an external audit for honours education and two case studies of the use of internal quality assurance tools in an honours programme were presented. It is concluded that a form of quality assurance is needed to continuously improve. So honours educators must at least care for internal quality control. Student involvement is stimulating for the engagement of all participants in the process of quality control. Internal quality control may result in an annual report. Annual reports can be used both internally and in external quality control.

Therefore, the system for ensuring public health should not receive funding on a residual basis, and the mechanism for providing medical care and preventing morbidity should be based on market principles.

The poor quality of expert reports prepared at the stage of preliminary investigation constitutes the main reason for summoning expert witnesses to testify before a court of law and posing numerous questions to them. The same applies to the examinations conducted on case materials, both in the course of criminal and civil proceedings. Тhe lack of internal quality control over forensic medical examinations in cases related to inadequate medical care may compromise the reliability and scientific validity of conclusions. Forensic medical examinations related to the cases of inadequate medical care constitute the primary source of evidence affecting the validity and legitimacy of decisions made by law enforcement agencies about the criminal liability of medical staff. In addition, scientifically unfounded expert conclusions may lead to investigative and judicial errors.Conclusion. Strict internal control over the forensic medical examinations is necessary to ensure that state forensic medical institutions produce expert reports fully compliant with the requirements of Law No. 73-FZ, which naturally should begin with a self-check.
