Atopic dermatitis is a disease characterized with increasing prevalence around the world. It has chronic relapsing course and has a negative impact on the quality of life. Topical steroids (TS) due to their mechanisms of action remain the mainstay of topical treatment for AD patients. At the same time, well known side effects associated with corticosteroid therapy, although seen more frequently with incorrect TS use, and rare with the use of modern TS, in some instances limit their use. That is a reason for proper selection of topical treatment algorythms of AD including emollients and other medications in addition to the TS.

Clobetasol-17 propionate is a dihalogenated corticosteroid which, on the basis of the vasoconstrictor assay, is classified as the most potent of all topical steroids. Since 1973 it has been marketed specifically for the short term topical treatment of moderate and severe cortico-responsive dermatoses for its antinflammatory, immunosuppressive and antimitotic effects. Psoriasis, atopic dermatitis, chronic eczema, boullous disorders and itching dermatosis of the genital area are usually well controlled by clobetasol with a great improvement of quality of life in patients. In this manuscript, we reviewed data concerning pharmacology, efficacy and safety profile of clobetasol-17 proprionate, updated studies on its use, efficacy of different formulations and impact on quality of life in corticosteroid-responsive dermatoses.

Atopic dermatitis (AD) is an inherited inflammatory disease of skin, multifactor or monogenic nature, characterized by itching, chronic recurrent current and age-specific features of localisation and morphology of lesion centers. The proportion of the disease in the structure of skin diseases is currently between 10 and 30%. When the process acquires a persistent course with an unparalleled effect from the performed therapy and exacerbations of the process up to 3-4 per year with increase of their duration, it becomes sluggish (torpedo) [1, 3]. The torpedo course of AD leads the patient to a decrease in the quality of life due to physical discomfort and emotional disadaptation. Approaches to treatment in theory depend on the mechanism of disease development.

Background: The World Health Organization declared COVID-19 a global pandemic on 11 March 2020, due to the number of newly reported confirmed cases and the rapid increase in deaths. Therefore, countries around the world limited their population to policies such as “social distancing” or “staying at home” to prevent the spread of the virus. The purpose of this study was to evaluate differences in lifestyle pre and post the outbreak COVID-19 among older adults in South Korea and to identify the impact of lifestyle differences on depression and quality of life. Methods: An online single questionnaire covering sociodemographic data, lifestyle details, depression status, and quality of life level was distributed using mailing lists and social media. To assess lifestyles differences in older people pre and post the outbreak of COVID-19 pandemic, the online single questionnaire was used post COVID-19 pandemic. Based on the participants’ memories, they responded lifestyles at two time points (pre and post COVID-19 pandemic). Results: The results showed that there was a significant decrease in physical activity and activity participation during the pandemic. In terms of nutrition, there was no statistically significant change pre and post the outbreak COVID-19, except for the intake of protein, fat, and vitamins. Additionally, the results showed that the resulting lifestyle differences seem to have had a negative impact on depression and quality of life among older adults in South Korea. Conclusion: There was a significant difference the lifestyle patterns among the participants in South Korea between the current period and pre COVID-19 pandemic. Additionally, it is observed that these differenced lifestyles were associated with depression and quality of life among the participants. Our findings may help to develop public health programs that support healthy lifestyles in pandemic conditions.

In the article the principles of improving the quality of life of the population of Ukraine were analyzed, a method of calculating the quality of life of the population of the world was developed. The solution of the tasks was carried out with the use of general scientific research methods: abstraction, analysis and synthesis, generalization, formalization, as well as special methods of analysis: structural, statistical, logical. The authors have analyzed the state of factors influencing the quality of life of the population of Ukraine: purchasing power index, safety index, health care index, cost of living index, property price to income ratio, traffic commute time index, pollution index, climate index, GDP by PPP per capita in international dollars, rating of political and civil liberties, life expectancy at birth, unemployment rate, gender equality index. General recommendations for improving the condition of these factors and priority areas have been identified. The general approaches to development of methods of calculation of quality of life of the population of the world countries are defined. A method for calculating the quality of life of the population has been developed: 5 indicators that have a positive impact on the quality of life and 5 indicators that have a negative impact on the quality of life of the population have been identified; mentioned weights of each of these indicators. The Index of Quality of Life of the Population of Ukraine, neighboring countries and developed world countries was calculated according to the developed calculation method, the obtained results have been compared with the Numbeo Quality of Life Index.

Objectives: To investigate the quality of life (QOL) of and to characterize patients with atopic dermatitis (AD) in Portugal. Methods: This was a cross-sectional study of patients with AD and other eczemas. Skindex-29, Skindex-teen, and the Childhood Atopic Dermatitis Impact Scale (CADIS) were the instruments used to assess QOL in adults, teenagers, and children, respectively. The SF-12 was also used, and disease severity was evaluated using the Patient-Oriented SCORAD (PO-SCORAD) instrument. Associations with QOL were assessed based on the odds ratio (OR). P values <.05 and 95%CIs were considered statistically significant. Results: The study population comprised 162 participants aged 0.5-74 years. We found that 37.3% of AD patients consider their disease disabling and that more than half of the patients feel stigmatized by society. The mean Skindex score for AD was 39.68, and the impact on QOL was severe in 44%. “Symptoms” was the most affected category in adults. AD was moderate to severe in 87% of the sample. One of the factors that most influenced poorer QOL in AD was age: with increasing age, the Skindex is likely to increase (OR, 1.03; 95%CI, 1.00-1.06). “Considering the disease a disability” was also associated (OR, 6.72; 95%CI, 2.56-17.63). QOL worsens with increasingly affected body area (OR, 1.07; 95%CI, 1.03-1.11) and the presence of edema (OR, 2.0; 95%CI, 1.23-3.40). Conclusions: This is the first study to provide data on QOL in patients with AD in Portugal. Our data show an expected negative impact. More awareness-raising activities are needed to increase knowledge, decrease stigmatization, and, consequently, address the factors involved in the QOL of patients with AD.

Hepatic encephalopathy is a complication of decompensated liver disease that consumes considerable health care resources and has a negative impact on quality of life. For several decades, maintenance therapy with lactulose has been considered the standard of care, but emerging data from clinical trials demonstrate that other therapeutic options may be available and could have a better tolerability profile than lactulose. This brief review will discuss the pathophysiology of hepatic encephalopathy and proposed mechanisms of action of zinc and probiotics in this context, discussing the findings of key trials.
