Beyond the acute character, two important aspects confer specificity to the dento-periodontal traumas: the age of the patients and the multidisciplinary attend of the therapeutic approach. The traumas, together with the dento-periodontal infections, are real pedodontic emergencies. Dento-periodontal traumas are lesions requiring careful examination to establish a correct diagnosis and a rapid establishment of appropriate treatment. They require a rapid diagnosis, an optimal therapeutic attitude, to resolve dento-periodontal structures and further harmonious development of the jaws. The coronary traumas that amputate the dental crown result from trauma that develop high intensity forces, with a point of application to the tooth cervical area, accompanied by other types of injuries. Radical status evaluation is performed paraclinically by imaging exploration methods. Information on root length, implantation, absence or presence of periapical diseases, endodontic treatment quality are fully provided by these methods.Clinically, the diameter of the roots of the teeth is evaluated. The analysis and synthesis of these data will allow the determination of the method of substitution and the planning of working methods. Material and Method: The study group comprises 39 children aged 6-16, most of them present for emergency treatment. Results and Discussion: The traumatic agent�s force exceeds the resistance and elasticity of tissues, causing discontinuities in the tissue structure, whether soft or rough. Conclusions: The provisional or final treatment of dento-periodontal traumas aims at restoring the morphological, functional and psychological integrity of the traumatized individual.

Intracranial germinomas (GE) are malignant neoplasms most commonly found in the suprasellar region, which may cause anterior and particularly posterior pituitary hormone deficits with central diabetes insipidus (DI). Differential diagnosis of pituitary stalk thickening includes granulomatous, inflammatory, infectious, and neoplastic lesions. Although careful analysis of clinical, laboratory, and imaging findings may facilitate the diagnosis, transsphenoidal biopsy is indicated to confirm the disease, as the correct diagnosis directs the appropriate treatment.

The purpose of the presented research is to analyze, systematize and formulate an enterprise strategy based on the analysis and synthesis of opportunities and threats of the external environment in combination with its strengths and weaknesses. Unconventional structuring of SWOT-factors, as well as their local synthesis for determination of the driving and inhibiting forces of the enterprise, has been offered. Ways to identify trends and patterns of external and internal environment based on a generalized model of SWOT-factors have been identified. Decisive in formulating a strategy (self-organized in nature) is a harmonization model of the goals system. General theoretical methods (analogy, abstraction, analysis and synthesis, idealization, imaginary modeling) and practical methods of research (observation, comparison, matching, analysis of statistical and other indicators) were used to solve the problems of the research. Methods such as empirical, statistical, graphical, etc. were used to process and summarize information.Thus, the approach study to the enterprise strategy formation based on the analysis and synthesis of the environment opportunities and threats in combination with its strengths and weaknesses allows to state that the development of a common structured list of major areas of enterprise activity is carried out taking into account the synthesis of all SWOT-analysis of the brand product, competitors and market segments, which allows the company to achieve a certain specific purpose of its activities.

Testicular dislocation after blunt perineal trauma is a rare event and a diagnosis that can be easily overlooked. Careful examination can help facilitate early and appropriate treatment. Timely diagnosis and surgical management are of paramount importance to preserve normal spermatogenic function in the dislocated testicle. We describe a case of testicular dislocation and discuss some of the issues surrounding diagnosis and treatment.

Leiomyomatosis peritonealis disseminata (LPD), a rare and unusual condition affecting mainly women of reproductive age, causes peritoneal and subperitoneal nodules formed by smooth muscle. Very few cases have been diagnosed since the disease was first described. We present a 42year old female who was managed for infertility and uterine myomata at a Municipal hospital in Ghana. Following a pelvic ultrasound diagnosis of multiple uterine myomata the patient was booked for myomectomy. At surgery to remove her myomata, the patient was found to have several peritoneal nodules some of which were attached to peritoneum, omentum and the surface of bowel loops in addition to a uterine myoma. The disease has since recurred twice after two laparotomies. The diagnosis was made by histopathology of ultrasound-guided biopsy of the nodules, and she has since been on GnRH analogue treatment. LPD simulates peritoneal carcinomatosis; thus, a good history, clinical evaluation, radiological imaging, and histopathologic analysis must be accurately diagnosed. Surgeons’ and Radiologists’ knowledge of the condition is fundamental to ensuring correct diagnosis and appropriate treatment and to minimising the probability of malignant transformation.

Background: Diagnosis of abdominal tuberculosis as well as histopathological confirmation is difficult because of suboptimal access to the intraperitoneal pathology. Laparoscopy provides minimally invasive access to the peritoneal cavity and materials can be collected for confirmation of diagnosis. Objectives: To study the importance of laparoscopy as a tool for the diagnosis of abdominal tuberculosis and initiation of appropriate treatment without delay. Materials & Methods: In this study 25 patients with suspected abdominal tuberculosis were selected within the period of May, 2014 to October, 2014. Diagnostic laparoscopy performed on all patients with biopsy of tissue from accessible sites. Results: Diagnostic laparoscopy with biopsy confirmed the diagnosis in 24 (96%) patients, 23 of these patients (96%) had nodules at different site of abdominal cavity and 19 of these patients (76%) had ascites. In two cases there were nodules over liver surface; biopsy was taken also from both liver nodules. One nodule revealed fibrosis and another nodule revealed tuberculosis. Conclusion: Imaging and culture of ascitic fluid may fail to confirm or exclude abdominal tuberculosis in clinically suspected cases. Laparoscopy with peritoneal tissue biopsy provided rapid and correct diagnosis of abdominal tuberculosis and should be performed early in suspected cases. KYAMC Journal.2021;12(01): 14-17

AbstractAbnormal hemoglobins can have major consequences for tissue delivery of oxygen. Correct diagnosis of hemoglobinopathies with altered oxygen affinity requires a determination of hemoglobin oxygen dissociation curve (ODC), which relates the hemoglobin oxygen saturation to the partial pressure of oxygen in the blood. Determination of the ODC of human hemoglobin is typically carried out under conditions in which hemoglobin is in equilibrium with O2 at each partial pressure. However, in the human body due to the fast transit of RBCs through tissues hemoglobin oxygen exchanges occur under non-equilibrium conditions. We describe the determination of non-equilibrium ODC, and show that under these conditions Hb cooperativity has two apparent components in the Adair, Perutz, and MWC models of Hb. The first component, which we call sequential cooperativity, accounts for ∼70% of Hb cooperativity, and emerges from the constraint of sequential binding that is shared by the three models. The second component, which we call conformational cooperativity, accounts for ∼30% of Hb cooperativity, and is due either to a conformational equilibrium between low affinity and high affinity tetramers (as in the MWC model), or to a conformational change from low to high affinity once two of the tetramer sites are occupied (Perutz model).

Abstract Deafness has a very variable disease. It may occur as a result of external auditory canal involvement or a deficiency in the sound conduction mechanism (transmission deafness) or impairment of the cochlear, cochlear nerve or central auditory perception. Genetics is the most common cause, as approximately 70% of hearing disorders are of hereditary origin. 1/3 of hereditary deafness is syndromic (associated with other symptoms) and 2/3 are non-syndromic (isolated deafness). At this date, 173 loci of deafness gene have been reported in the literature (69 DFNA, 94 DFNB, 6 X-linked DFN, 2 DFNM, 1 DFNY and 1 AUNA1). For syndromic deafness, approximately 400 syndromes associated with hearing disorders are already described. Thus, the determination of causal mutations is a valuable aid for accurate and early diagnosis. This makes it possible to better guide the management since forms of deafness respond better to the cochlear implant than others. The correct diagnosis also gives an idea of ​​the evolutionary profile of deafness and whether it is a syndromic deafness requiring special surveillance. In this study, we have examined the genetic causes of sensorineural hearing loss in Moroccan patients through whole exome sequencing (WES) to identify candidate genes for six severely deaf Moroccan families. The results revealed four genetic variants in the genes GJB2, COL4A3, ATP6V1B1 and EDNRB, which are therefore common causes of syndromic and non-syndromic deafness.

The concept of polyhedra with articulated faces is investigated in this paper. Polyhedra with articulated faces can be described as polyhedral frameworks, whose faces are constrained to remain planar. A mechanical arrangement based on a single type of component is proposed for the construction of the polyhedra. Then, the determination of their infinitesimal and full-cycle mobility is addressed. In some cases, they are rigid structures while in others they are articulated mechanisms. Finally, examples are given, using simulation and physical models, and several new families of articulated polyhedra are synthesized.

A young Indian man presented with nine-month history of chronic diarrhea, occasionally mixed with blood and intermittent colicky abdominal pain. He also complained of generalized body swelling for the last three months. On examination, he had diffuse hyperpigmentation of the skin and dystrophic nail changes. Upper and lower gastrointestinal endoscopy revealed multiple sessile polyps in the stomach, small bowel, and colon and rectum. Biopsy of polyps showed adenomatous changes with stromal edema and dilated glands. Cronkhite-Canada syndrome (CCS) was diagnosed and treated with glucocorticoids and enteral nutritional supplementation. There was an associated small intestinal bacterial overgrowth (SIBO) and stool was positive for clostridium difficile toxin. After 12 weeks of treatment, the patient achieved remission. Close correlation with clinical findings, including pertinent ectodermal abnormalities, endoscopic studies, and careful examination of biopsies will ensure a timely and correct diagnosis of CCS.

Self-system therapy (SST) is an appropriate treatment for clients with a primary diagnosis of major depressive disorder (MDD) or persistent depressive disorder, with symptom severity ranging from mild to severe using standardized measures. SST was developed for individuals with depression characterized by problematic self-regulation, but it can also be used for treating comorbidities such as anxiety. Clients should receive a thorough diagnostic evaluation and medical examination to rule out nonpsychological causes of mood disturbance. Diagnostic assessment also includes a determination of whether the client meets the criteria for MDD, identification of contraindicated comorbidities, and a comprehensive evaluation of the client’s current problems and strengths. Chapter 3 discusses the recommendations for determining whether a client can benefit from SST and includes guidelines for diagnostic assessment.
