Effectiveness of the curative complex consisting of photodynamic therapy, intravenous laser irradiation and local laser irradiation aimed at correcting hemostatic parameters at the initial stages of squamous cell skin cancer

Purpose. To assess the effectiveness of curative complex consisting of photodynamic therapy (PDT) + intravenous laser blood irradiation (ILBI) + local tumor irradiation (TR) at hemostatic parameters at the initial stages of squamous cell skin cancer (SCSC).Material and methods. The present work analyzes SCSC course in 185 patients who had outpatient and inpatient treatment in medical institutions of cities Engels and Saratov in 2015–2019. 129 (69.7%) males and 56 females (31.2%). Patients were divided into three groups. Group I – 74 (40%) patients who had combined treatment: surgery with wide tumor excision under intravenous anesthesia + PDT + ILBI + TR; Group II – 111 (60%) patients who had only surgical treatment. Group III was created for controlling and comparing the obtained laboratory parameters; it included 17 patients without established pathology.Results. It was found out that hemostatic disorders in SCSC are characterized by increased activity of AT III and vWF and by simultaneous decrease of tPA-PAI-1 complex as well as by increased activity of VIIIa factor and resistance of Va factor – indices which indicate dysfunction of endothelial vascular wall. Combination of PDT + ILBI + TR prior to surgical treatment in patients with squamous cell skin cancer at the initial stages of the disease allow to normalize homeostatic indices and reduce the number of complications.Conclusion. 1. Squamous cell skin cancer at the initial stages changes the hemostatic system. 2. Effects of SCSC mechanism may be characterized as follows: damage of the endothelium vascular wall which is confirmed by endothelin increase; it results in vasoconstriction and increased blood procoagulant activity and, ultimately, in changes of hemostatic parameters. 3. Combination of PDT + ILBI + TR applied before surgery in patients with squamous cell skin cancer at the initial stages of the disease normalizes homeostatic indices and reduces the number of complications.

The work is based on an analysis of the course of the disease in 185 patients with squamous cell skin cancer. The study program included studies in three major groups. The first included 74 (40%) patients who underwent combined treatment: an operation involving widespread excision of a tumor under intravenous anesthesia + photodynamic therapy (group 1). The second group was 111 (60%) patients with squamous cell carcinoma of the skin who received only operative treatment. To control and compare the obtained laboratory indicators, a second group was created, which included patients without established pathology. This group included 17 people who voluntarily agreed to conduct the study. The study traced the dynamics of changes in hemostasis scores and metalloproteinases in the two groups being compared. In the group of patients with squamous cell skin cancer, there was a statistically significant decrease in the metalloproteinase-1 inhibitor index, which led to an increase in all metalloproteinase-2, -7 and -9 indicators. Performing surgical treatment did not lead to correction of these indicators, both in the near and distant periods after treatment. The mechanism of influence of photodynamic therapy in squamous cell skin cancer can be characterized as follows: there is a decrease in the inhibitor of metalloproteinase-1, which leads to an increase in matrix metalloproteinases-2, -7 and -9 in the blood, as a result of damage to the endothelium of the vascular wall, which is confirmed by an increase in endothelin, this, in turn, led to the development of vasoconstriction and increased procoagulant activity The use of photodynamic therapy restores the index of metalloproteinase-1 inhibitor, this leads to the restoration of matrix metalloproteinases-2, -7 and -9 in the blood and the prevention of damage to the vascular wall. Comprehensive treatment of patients with squamous cell skin cancer in the initial stage of the disease should include photodynamic therapy with simultaneous intravenous blood irradiation, laser tumor training, and then surgical treatment, consisting in excision of the tumor with observance of oncological principles.

Abstract Background The primary aim of this study was to observe the effect of 5-ALA-mediated photodynamic therapy on oral squamous cell carcinoma in vitro. Methods SCC25 cells were divided into the observation group and the blank control group. Different concentrations of 5-ALA and SCC25 cells were co-incubated for different times, and the concentration of protoporphyrin IX was detected by flow cytometry. SCC25 cells were divided into the 5-ALA group (100 mg/L), the laser irradiation group alone, the 5-ALA plus laser irradiation group, and the blank control group (0 mg/L 5-ALA), and the methyl thiazolyl tetrazolium (MTT) solution method was used (each group was incubated for 4, 8 and 12 h in turn). The cell survival rate was calculated. Using annexin V-fluorescein isothiocyanate/propidium iodide method, the apoptosis of SCC25 cells was detected by flow cytometry. Results The level of protoporphyrin IX in SCC25 cells increased with increased concentrations of 5-ALA and length of incubation. However, after 12 h, protoporphyrin IX level in SCC25 cells was gradually stabilized, and similar effect was obtained with 100 mg/L or more 5-ALA, indicating that the level of protoporphyrin IX in SCC25 cells was determined by 5-ALA concentration and incubation time. 5-ALA plus laser irradiation exerted an inhibitory effect on the growth of SCC25 cells, which was highly associated with drug dose and incubation time. Compared with the control group, laser irradiation alone or 5-ALA alone had no effect on the apoptosis of SCC25 cells. Different concentrations of 5-ALA combined with laser irradiation showed a remarkable effect of apoptosis, and a higher apoptosis rate was seen with higher drug concentrations. Conclusion 5-ALA-mediated photodynamic therapy affects the growth of SCC25 cells in vitro, which may provide a new idea for the clinical treatment of oral squamous cell carcinoma.

Aim:  Our aim in conducting this study was to ascertain the clinical, pathological, and radiological characteristics of patients diagnosed with idiopathic granulomatous mastitis in our clinic and to contribute to related literature by creating a therapy algorithm for granulomatous mastitis. Method: A total of 62 patients diagnosed with granulomatous mastitis in our clinic were classified into three groups as per the mode of treatment they received. Among these, 20 patients who had only been administered corticosteroids were allocated to Group I, while 18 patients who had received surgical treatment in addition to corticosteroid treatment were allocated to Group II, and 24 patients who had only received surgery were allocated to Group III. Results: All the cases were female patients. The mean age of the patients was 33.67 ± 6.35 (21-48). No statistically significant difference was found among the groups as per the mean age figures, pregnancy, breastfeeding, oral contraceptive administration, smoking and alcohol use, and family history (p>0.05). 49 (77.4%) patients were diagnosed with granulomatous mastitis by thick-needle biopsy, 7 (11.2%) by incisional biopsy, 4 (6.4%) by excisional biopsy, and 3 (4.8%) by fine-needle aspiration biopsy (FNAB) before the onset of treatment. The median value for the follow-up of the patients was 40 (6-81) months and there was no statistically significant difference among follow-up periods (p: 0.51). 15 (24.1%) patients among all the cases had recurrence. There was a statistically significant difference among the groups with regards to recurrence (p: 0.015). Conclusion:  Granulomatous mastitis may simulate breast cancer both clinically and radiologically. Recurrence is observed in patients who only received corticosteroid or surgical treatment. The combination of corticosteroid and surgical treatment significantly reduces the rate of recurrence. Therefore, this combination can be recommended as selective treatment.

The authors consider the possibilities of pain management during photodynamic therapy (PDT) of visible tumors based on the observation of 102 patients. Of the total number of patients, 62 had verified basal cell skin cancer, 10 people - squamous cell skin cancer, another 10 - oral and oropharynx mucosa cancer, 8 - oral leukoplakia and dysplasia, in 6 - lower lip cancer, in 4 - breast cancer, in 2 - other localizations of neoplasms. In 15 patients, nonsteroidal anti-inflammatory drugs (NSAID) were used as pain management, in 69 - a combination of NSAID with tramadol, in 14 - nerve block anesthesia, in 4 - PDT was performed under general anesthesia. The intensity of pain syndrome during laser irradiation of the tumor was assessed on the verbal rating scale (VRS). The absence of pain was recorded in 9% of cases. Mild pain was noted by 58% of patients, moderate pain - 20%, severe pain - 10%, very severe pain was noted by 3% of patients.The degree of expression of pain syndrome during PDT depends on the incidence of a lesion, histological form of tumor, and method of anesthesia. NSAID alone, or in combination with an opioid analgesic, allows effective control of pain syndrome in PDT of basal cell skin cancer in 89%, in PDT of squamous cell skin cancer in 66% of observations. Nerve block anesthesia allows stoping pain syndrome during PDT of oropharyngeal tumors.

The objective: to optimize treatment of endometriosis using a combined hormonal Luvyna medicine. Materials and methods. The study included 50 patients 22-36 years old with a diagnosis of endometriosis. In line with the patient’s therapy are divided into groups: I group (n = 25) received the Luvyna on a continuous 63/7 scheme, then 3 months in scheme 21/7; group II (n = 25) after surgical treatment of endometriosis (gistero-, laparoscopy) also received the medicine in a similar way. The mode of menstrual function disorders, pain on a visual rating scale of pain, psycho-emotional status, concomitant dysfunction of adjacent organs were evaluated. Results. According to the results of therapy established algodismenorei reduction in symptoms I group – 80%, II – 83%, abnormal uterine bleeding I – 96%, II – 100%; the disappearance of heavy uterine bleeding, improving the function of adjacent organs, psycho-emotional status in both groups. Reduced pain on a pain scale from 8±2.0 to 2±0.5 in the group I and from 7±2.5 to 1±1.0 in the group II. Conclusion. Luvina is a highly effective medicine for the treatment of endometriosis, comparable to the combined treatment - surgery, followed by the appointment of combined oral contraceptives. Recommended mode 63/7, followed by a reception 21/7, including after surgical treatment. Key words: endometriosis, Luvyna, conservative therapy, combined therapy.
