Background: The popliteus muscle fulfills a function of rotational stabilizer of the knee. Its particular shape and innervation pattern are the interest of anatomists and surgeons. The aim of this work was characterizing the anatomy and innervation of the popliteus muscle of knees from a sample of mestizo-raced population, predominant in Latin America.   Methods: This cross-sectional study was made in 23 knee segments 14 right knees and 9 left knees coming from 14 men and 9 women aged 67 ± 22 years.   Results: The popliteus muscle presented a scalene-like triangle shape and its medial base was covered by the superficial expansion of the semimembranosus tendon. The length of its base was 79,2±12,8 mm with a superior and inferior side that measured 58.0±7.4 mm and 101.7±11.7 mm respectively. The popliteus muscle innervation was supplied by the tibial nerve (TN) by the arising of two branches in 22 cases (95.7%). The first branch arose proximal to the inter articular line and presented a length of 91,1 mm. The second branch arose distally to the inter articular line with a length of 48.4±1.3mm. The third was observed in just one case (4.3%) Its length was 108±1.1mm and it arose distally to the inter articular line.   Conclusions: The morphological characteristics of the popliteus muscle and its innervation patterns found in our study differ from what was reported in previous studies. The presence of these findings can be considered during the planning of surgical procedures in the posterior aspect of the knee.

AbstractPurposeHigher incidence of mental illnesses and less access to care is previously reported concerning migrants but few studies focus on the needs and care of migrant groups in psychiatry. The aim of this study was to compare differences in needs and care between migrant and nonmigrant groups of severely and persistently mentally ill (SMI) after the 1995 Swedish mental health care reform.MethodsIn a Swedish area, inventories were made in 2001 and 2006 of persons considered as SMI. These persons were interviewed and their needs were assessed. In a cross-sectional study in 2006, needs and care were compared between migrants and nonmigrants. In a longitudinal study, migrants and nonmigrants interviewed in both 2001 and 2006 were compared concerning the development of needs and care.ResultsThe needs of the migrant group were less taken care of. In 2006, there were more unmet needs in this group concerning accommodation, physical health, psychological distress, basic education and economy.ConclusionThe improvement of groups considered as SMI concerning functional disability and efforts of care found in the actual area did not seem to include the migrant group, at least not to the same degree.

There is little information on a country-wide basis in sub-Saharan Africa about how the diagnosis of extra-pulmonary tuberculosis (EPTB) is made. A country-wide cross-sectional study was carried out in 40 non-private hospitals in Malawi which register and treat (TB) patients in order to assess diagnostic practices in adults registered with EPTB. All patients aged 15 years and above in hospital on treatment for EPTB were reviewed using TB registers, case note files and clinical assessment. There were 244 patients, 132 men and 112 women whose mean age was 36 years. In 138 (57%) patients, all appropriate procedures and investigations, commensurate with hospital resources, had been carried out. Of 171 EPTB patients with cough for 3 weeks or longer, 138 (81%) submitted sputum specimens for smear microscopy of acid-fast bacilli (AFB). A confirmed diagnosis of TB was made in 15 (6%) patients based on finding AFB or caseating granulomas in specimens. In 157 (64%) patients, the diagnosis of EPTB was considered to be correct. In 46 (19%) patients the diagnosis was considered to be TB, although different from the type of EPTB with which the patient was registered. In 39 (16%) patients an alternative non-TB diagnosis was made and in two (1%) patients it was not possible to make a decision. Diagnostic practices need to be improved, and ways of doing this are discussed.

ResumenObjetivo: Diseñar y validar encuesta, mediante el modelo de Rasch, para identificar motivaciones internas, externas o desmotivación en docentes de educación básica y media convocados a capacitación en educación para la sexualidad. Materiales y métodos: Estudio de corte transversal cuantitativo con enfoque empírico analítico no experimental para evaluar la encuesta y establecer calidad de los ítems, confiabilidad y validez; la encuesta fue elaborada a partir de la teoría de Deci y Ryan. Participaron 155 docentes de 12 colegios públicos de los municipios de Piedecuesta y El Socorro del departamento de Santander, Colombia. Resultados: Se aplicaron 51 encuestas iniciales y 104 finales. En la prueba inicial solo se logró analizar el componente de motivación externa y se observó un buen ajuste, unidimensionalidad, buena confiabilidad y separación para ítems de 0.97 y 5.76 respectivamente; En la segunda prueba los tres componentes (motivación interna, externa y desmotivación) mostraron ajuste a Rasch con unidimensionalidad, confiabilidad de ítems de 0.80, 0.97 y 0.89 y separación de 2.01, 5.78 y 2.83 respectivamente; no hubo ítems con funcionamiento diferencial por sexo. Conclusiones: Se dispone de encuesta validada y confiable para evaluar motivación docente frente a procesos de capacitación en educación para la sexualidad. AbstractObjective: To design and validate a survey using the Rasch model with the aim of identifying internal and external motivation or discouragement in teachers of basic education and media invited to training in sexuality education. Materials and methods: A quantitative cross-sectional study with  non-experimental analytical empirical approach was made in order to evaluate the survey and establish quality items, reliability and validity. The survey was  developed from the theory of Deci and Ryan. 155 teachers of 12 public schools from the municipalities of Piedecuesta and El Socorro from the department of Santander, Colombia participated. Results: 51 initial surveys and 104 final ones were applied. In the initial test, it was only possible to analyze the component of external motivation and a good fit, unidimensionality, good reliability and separation for items of 0.97 and 5.76 respectively were observed. In the second test the three components (internal, external and  discouragement) showed Rasch fit in terms of unidimensionality, reliability of items of 0.80, 0.97 and 0.89 and separation of 2.01, 5.78 and 2.83 respectively. There were no items with differential performance by sex. Conclusions: There is a validated and reliable survey to evaluate teachers’ motivation in front of training processes for sexuality education.

Background: BCR-ABL translocation is the most common genetic abnormality associated with adult Acute Lymphoblastic Leukaemia (ALL) with poor outcome. Objective: The aim of the study was to determine the association of myeloid aberrant antigens and the presence of BCR-ABL gene rearrangements in acute lymphoblastic leukaemia (ALL) patients in our context. Method: A total of 38 ALL patients were included in this cross-sectional study from August 2018 to July 2019 according to selection criteria. BCR-ABL was detected by Real Time-Polymerase Chain Reaction (RT-PCR). Results: The median age at diagnosis was 27.5 years with male (76.3%) predominance. Aberrant myeloid markers, e.g.CD13 was present in 9(64.3%) patients who were BCR-ABL positive which was statistically significant (p:<0.05). Conclusion: Early suspicion about BCR-ABL positivity can be made in ALL patients who show aberrant myeloid expression.

ABSTRACT Context: In cases of chronic facial palsy, where direct neurotisation is possible, ipsilateral masseteric nerve is a very suitable motor donor. We have tried to specifically locate the masseteric nerve for this purpose. Aims: Describing an approach of localisation and exposure of both the zygomatic branch of Facial nerve and the nerve to masseter, with respect to a soft tissue reference point over face. Settings and Design: Observational cross sectional study, conducted on 12 fresh cadavers. Subjects and Methods: A curved incision was given, passing about 0.5cms in front of the tragal cartilage. A reference point “R” was pointed out. The zygomatic branch of facial nerve and masseteric nerve were dissected out and their specific locations were recorded from fixed reference points with help of copper wire and slide callipers. Statistical Analysis Used: Central Tendency measurements and Unpaired “t” test. Results: Zygomatic branch of the Facial nerve was located within a small circular area of radius 1 cm, the centre of which lies at a distance of 1.1 cms (±0.4cm) in males and 0.2cm (±0.1cm) in females from the point, ‘R’, in a vertical (coronal) plane. The nerve to masseter was noted to lie within a circular area of 1 cm radius, the centre of which was at a distance of 2.5cms (±0.4cm) and 1.7cms (±0.2cm) from R, in male and female cadavers, respectively. Finally, Masseteric nerve's depth, from the masseteric surface was found to be 1cm (±0.1cm; male) and 0.8cm (±0.1cm; female). Conclusions: This novel approach can reduce the post operative cosmetic morbidity and per-operative complications of facial reanimation surgery.

The purpose of this study was to examine the plain AP radiograph of foot and lateral radiograph of knee in order to determine the prevalence and size of sesamoid bone in different part of foot and on the posterior aspect of the knee joint. A cross-sectional study was performed in Department of Radiology and Imaging, Tribhuvan University Teaching Hospital from July to September 2014. A total of 206 radiographs of foot (55) and knee (155) were collected. 121 (58.7%) were female and 85 (41.3%) were male. The most common site of sesamoid bone in foot was in the first metatarsal joint (96.4%) whereas the sesamoid bone in knee joint, fabella was found in 12.3% of total cases. The prevalence is more common in female than in male. The length and breadth of the sesamoid bones were measured and then area of bones was calculated. The size of medial and lateral sesamoid bone in the first MTP Joint was measured 0.6225 cm2 and 0.8261 cm2 respectively. The mean size of sesamoid bone of knee, fabella was 0.2818 cm2.

IntroductionThe digital boom and the media coverage of the sexuality urge the youngest to expose itself more and more via the social networks and other digital applications.Aims/objectiveTo explore the virtual sexuality of the Tunisian teenagers.MethodsIt's a cross-sectional study made in August 2015, including 104 Tunisian teenagers from different social backgrounds and living in Tunis, chosen aimlessly. An auto-railing drafted in Arabic, anonymous and carrying on various items was distributed.ResultsWe included 52 boys and 52 girls. Seven of them (6.1%) was not schooled and 68.3% (n = 71) were at high school. Ninety-one of the teenagers (87.5%) reported not have sexual education at school and 74% (n = 77) have already turned to web sites to inquire about sexuality. A regular practice of cybersex at the rate of at least once a week, was founded at 36.5% (n = 36) and 41 teenagers (39.4%) have reported to have already undressed in front of their webcam. The candidates have already consulted a pornographic site at least once in their life in 76.9% (n = 80) of the cases. The allocated time for these virtual activities is made to the detriment of the sleep in 75% of the cases (n = 27). The connection was accompanied with a masturbatory activity in 38.9% (n = 14).ConclusionContrary to preconceived ideas, the computer screen does not protect from sexual risk behaviors. Therefore, a sexual education for the youngest is more than ever necessary to avoid such drift.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Objective: This study aimed to gather evidence of progress made in the provision of emergency obstetrics and newborn care (EmONC) services in Zanzibar. Methods: This cross-sectional study was conducted in 83 health facilities in ten districts of Unguja and Pemba islands in Zanzibar in 2018 using the validated Averting Maternal Death and Disability (AMDD) program module. Results: Proportion of non-EmONC facilities is lower compared to 2012 (35% vs. 55.7%). Moreover, partially BEmONC facilities have increased compared to 2012 survey (58.7% vs. 28%). Only 2.5% and 3.8% provided CEmONC and BEmONC respectively. Concerning population size, EmONC availability is met at only 33.3% while a similar number lacks Comprehensive EmONC.
