BACKGROUND: Dengue is a mosquito-borne disease caused by any one of four closely related Dengue virus (DENV 1-4). The clinical sign Dengue virus infection can vary from mild (mild febrile illness), Dengue Fever (DF), Dengue Hemorrhagic Fever (DHF) to Dengue Hemorrhagic Fever with shock (Dengue Shock Syndrome, DSS). AIM: This study was designed to determine the relationship of obesity with the severity of Dengue Hemorrhagic Fever in children. METHODS: It is a case-control study. The data of patients were retrospectively collected from the Department of Child Health at the Wangaya General Hospital between March 2019 to May 2019. It uses consecutive sampling. The total sample of 22 children with DHF with shock and 22 children with DHF without shock were investigated. Statistical analysis has been performed by SPSS Statistics 20.0 for Mac (IBM Corp., Armonk, New York, USA). DHF positive results were compared by the Chi-square test and binary logistic regression. RESULTS: Prevalence of DHF with shock is fifty per cent's and DHF without shock is 50%. Prevalence of obesity is 40.9%. The result of binary logistic regression analysis of obesity in children and the severity of DHF was significantly correlated with P-value 0.004 and OR = 7.734. CONCLUSION: Obesity is associated with the severity of Dengue Hemorrhagic fever in children.

Numerous studies have been carried out on patients afflicted with dengue hemorrhagic fever (DHF), but various issues related to the disease, including the characteristics of the dengue virus (DENV), remain unclear. To address this deficiency, the current research was conducted to determine the correlation of clinical and subclinical presentations with dengue serotypes and plasma viral load. This prospective cohort study, which was performed at Tien Giang General Hospital from 2009 to 2014, involved 481 children who were under 15 years of age and had DHF for less than 72 hours. Results showed that among the patients, the highest proportion were composed of those suffering from DENV-1 infection (44.7%). The progression of the disease to dengue shock syndrome (DSS) owing to infection with DENV-2 and DENV-1 was significantly higher than that caused by infection with DENV-3 and DENV-4. No statistically significant differences in DENV viremia were found between the non-shock DHF and DSS groups. Finally, no correlation was found between dengue plasma viral load and clinical and subclinical presentations. The findings led to the conclusion that dengue serotypes can be used as a basis in ascertaining the prognosis of DSS and DHF.

Abstract Background Intestinal obstruction is defined as a blockage or partial blockage of the passage of the intestinal contents. It is a potentially risky surgical emergency associated with high morbidity and mortality. Its pattern differs from country to country and even from place to place within a country. Therefore, this study aimed to find out the magnitude, pattern and management outcome of intestinal obstruction in Arba Minch General Hospital. Methods A retrospective cross-sectional study was conducted in Arba Minch General hospital from January 09, 2015, to November 09, 2018. The data collection period was from December 15, 2018, to February 09, 2019. A simple random technique was applied to select 801 study participants. Then, the required data entered into Epi Info version 7.2.1.0 and exported to the statistical package for the social sciences software package version 20 for analysis. The binary logistic regression analysis has been done to determine crude statistical associations between independent variables and dependent variables. Linearity, Multivariate normality and multicollinearity were checked between independent and dependent variables by using scatter plot and Q–Q plot respectively. Variables with a p-value of less than 0.25 in the binary logistic regression analysis were entered into multivariable logistic regression. Statistical significance factors were identified based on a p-value of < 0.05 and with a 95% confidence interval. Result This study revealed that the overall magnitude of intestinal obstruction was 40.60% with 95% CI (34.95–45.95). The magnitude of unfavorable management outcomes and deaths during the study period were 22.3% with 95% CI (18.00–27.00) and 7.1% with 95% CI (4.00–10.00) respectively. Persistent tachycardia 10.3 (3.28–32.42), Dehydration 13.7 (3.34–56.56), elevated serum creatinine 10.2 (1.89–54.94), gangrenous small bowel volvulus 2.7 (1.27–5.84), ischemic bowel 3.4 (1.17–9.81) and perforated bowl 7.68 (2.96–19.93) were significantly associated with the management outcome of intestinal obstruction. Conclusion and recommendation Intestinal obstruction was the most common among all acute abdomen cases and its management outcome highly associated with dehydration. Adequate early preoperative resuscitation and proper post-operative care with appropriate surgical techniques and wound care with sterile techniques would help to reduce further mortality. This could be achieved by increasing public awareness of health-seeking behavior. Moreover, health facilities capable of handling patients with small bowel obstruction should be available within the reach of the community.

Dengue hemorrhagic fever is a severe and fatal infection that occurs in tropical regions such as Indonesia. In 2014, recorded that dengue morbidity rate was 5.17 per 100,000 inhabitants (approximately 13031 cases) with mortality rate of 0.84% (110 deaths). Demographic and societal changes such as population growth, urbanization, and modern transportation appear to play an important role in the increased incidence and geographical spread of dengue virus. Aedes aegypti, the urban yellow fever mosquito, is also the principal dengue-carrying vector. The Aedes aegypti mosquitoes as a vector of dengue virus normally live and breed in clean water reservoirs that are not directly related to the land such as: bath, bird drinks, water pot, water jars / barrel, cans, old tires, etc. In Indonesia, dengue outbreaks often occur when the seasons change from dry to rain hor vice versa. This study aimed to figure out the influence of natural environment and the existence of the vector’s larva. A logistic regression was performed to ascertain the effects of temperature, humidity, water replacement and the existence of water reservoir on the likelihood of the existence of the vector’s larva . The logistic regression model was statistically significant, p &lt; .005 and the model explained 69% (Nagelkerke R2). This finding shows that the existence of the mosquito’s larva, influenced by temperature, water replacement activity and the existence of water reservoir. Health education about vector control and environmental engineering are necessary to break the chain of mosquito breeding.

Pare Community Health Center or Puskesmas is one of Puskesmas in Kecamatan Pare which has the highest number of Dengue Hemorrhagic Fever incidence that is 64 incidence in 2016. The purpose of this study is to analyze the risk factors that affect the incidence of Dengue Hemorrhagic Fever in the work area of Puskesmas Pare. It was an observational study with cross sectional approach. Interviews and environmental observations were conducted on 100 respondents, randomly, in the working area of Puskesmas Pare, Desa Tulungrejo. The independent variables of the research were larvae existence , maya index status and 4M Plus behavior, with the dependent variable of the research was Dengue Hemorrhagic Fever incidence in 2016.This data was analized by binary logistic regression test.The results of this study indicate that risk factors that affect the incidence of Dengue Hemorrhagic Fever is the action of 4M Plus (p = 0,017).Therefore, the 4M Plus actions should be further improved independently and promoted evenly to the community in order to suppress the incidence of Dengue Hemorrhagic Fever.

Abstract Background Antibiotics are synthetic or natural substances used to treat diseases of infectious origin. When used incorrectly, they can be a factor in the development of antimicrobial resistance, increased treatment time, costs, and mortality. The present study aimed to assess the pattern inappropriate use of antibiotics and their predictors in pediatric patients admitted to the Hospital Central de Nampula, Mozambique Methods A cross-sectional, retrospective study with a quantitative approach was conducted between January and July 2019. The population consisted of children aged 0 to 10 years hospitalized in the pediatric ward I. Binary logistic regression was used to determine risk factors for inappropriate use of antibiotics with 95% CI. Results The prevalence of antibiotic use among pediatric patients was 97.5%. Of the 464 antibiotics prescribed, 39.9% were for patients suffering from gastroenteritis, 21.8% and 9.1% Affected with pneumonia and malaria. Most antibiotics were for systemic use (95.9%, 445/464). Most (36.5%) of prescriptions had errors. Duration of treatment (74.6%) and dosage (25.4%) were the most frequent errors. Binary logistic regression analysis revealed that patients were prescribed with ≥ 3 antibiotics (OR = 2.920, 95% CI = 1.283–6.646, p–value = 0.011) and with a short hospitalization time (OR = 1.726, 95% CI = 1.040–2.864, p–value = 0.035) were more likely to experience some inappropriate use of antibiotics. Conclusion The study showed a higher prevalence of antibiotic use, however, a greater number of prescriptions contained errors. Errors related to dose and duration of treatment were the most common among pediatric patients. These results reveal a concern, since inappropriate and excessive use of antibiotics is a major factor for the development of antibiotic resistant microorganisms. Therefore, strategies concerning the use of antibiotics in order to reduce their inappropriate and excessive use are necessary.

Dengue is a mosquito-borne disease (female mosquitoes of the Aedes genus, principally Aedes aegypti) caused by any one of four closely related dengue viruses. It is endemic in tropical and subtropical continent. World health organization (WHO) currently estimates there may be 50 -100 million dengue infections worldwide every year with over 2.5 billion people at risk of dengue. Symptomatic dengue virus infection may manifests as undifferentiated fever, classical dengue fever (with or without unusual hemorrhages), and dengue hemorrhagic fever (with or without shock). Isolated organopathy or expanded dengue syndrome (EDS) was coined by WHO in the year 2012 to describe cases, which do not fall into either dengue shock syndrome or dengue hemorrhagic fever. The atypical manifestations noted in expanded dengue are multisystemic and multifaceted with organ involvement, such as liver, brain, heart, kidney, central/peripheral nervous system, gastrointestinal tract, lympho reticular system. Dengue virus has long been considered as a non-neurotropic virus, as animal studies have shown that virus does not cross blood brain barrier. Hyponatremia may be found in association with dengue fever and is thought to be caused by peripheral fluid extravasation and resulting intravascular hypovolaemia. But hyponatremia due to syndrome of inappropriate secretion of anti-diuretic hormone (SIADH) in Dengue fever is rare. We report a 40 years old male who was diagnosed as Dengue fever (Dengue Ns1Ag positive) with thrombocytopenia and hyponatremia. He was admitted and further investigations revealed SIADH. He responded well to cautious sodium replacement and addition of tolvaptan. He recovered completely and was discharged after one week. Thus, all clinicians should keep in mind the possibility of SIADH as a part of expanded dengue syndrome.

ABSTRACT Background: In response to the global (COVID-19) epidemic, the United Arab Emirates (UAE) government is taking precautionary action to mitigate the spread of the virus and protect the safety and well-being of citizens, residents, and visitors. The knowledge and practices of individuals will probably have an important bearing on the course of the coronavirus disease 2019 pandemic. The aim of this study was to evaluate the knowledge and practices toward COVID-19 among the general public in the UAE during the current outbreak COVID-19. Methods: A cross-sectional online survey of 1356 of respondents in the UAE we conducted during the epidemic outbreak between 9th to 24th June-2020. The questionnaire consisted of three sections: Socio-demographic, participants knowledge, and participants practices. Independent- samples t-test, one-way analysis of variance (ANOVA), chi-square, and binary logistic regression have used. A p-value of (p < 0.05) was considered statistically significant. Results: Of the total sample, 72% were females, 47% % were aged between 30-49 years, 57.2% were from Sharjah, 65.6% had a college degree, and 40.6% were unemployed. The total correct score of knowledge and practices questions was high 85% and 90%, respectively. Males gender, other marital status, and illiterate/primary educational levels had a lower level of knowledge and practices than others. participants aged 18-29 had little higher knowledge than other ages but had a lower level in practices, people who live in Abu Dhabi had better knowledge and practice than other emirates, employed people had a lower level of knowledge but higher in practice. Binary logistic regression analysis presented that females, 18-29 years, and married participants significantly associated with a higher score of knowledge, while female gender, over 30 years old, the martial status of singles, college-level and higher, unemployed, were significantly associated with high mean practice score. Conclusions: To our knowledge, the current study is one of the first studies to evaluate the knowledge and practices of UAE population toward COVID-19. Most of the respondents demonstrate an excellent level of knowledge and awareness as well as proper conscious practices. Continuing to implement the health education programs pursued by the UAE is highly important to maintain the appropriate level of awareness among the public.
