BACKGROUND: The establishment of appropriate vertical dimension of occlusion (VDO) is paramount for successful and functioning complete dentures (CDs). AIM: The aim of this study was to assess the effect of alteration of VDO on the brain activity in CD wearers. MATERIALS AND METHODS: Ten completely edentulous patients participated in this study. Each patient received three sets of CDs: one control set and two duplicates’ CDs. The control CDs was designed with appropriate VDO (control - VDO), while the duplicate CDs were designated as followed: one with VDO lowered by –3 mm (low - VDO) the other one VDO was increased by +5 mm (high - VDO). The effect of alteration of VDO on brain activity was assessed by measuring alpha waves changes using electroencephalogram (EEG) before and after chewing gum. Statistical analysis was carried out using one-way ANOVA and dependent t-test. The significance level was set at p < 0.05. RESULTS: There was a significant increase in the alpha waves mean values after chewing (p < 0.05) gum compared to before except for low - VDO CDs (p > 0.05). The control-VDO revealed a statistically significant higher mean values compared to the duplicate dentures. CDs with high - VDO showed higher mean value than low - VDO dentures, however, it was statistically insignificant (p > 0.05). CONCLUSION: Appropriate determination of VDO is not only important for the success of CDs but also for the functional augmentation of brain activity.

The aim of this study is to demonstrate the use and the effectiveness of cephalometry and golden proportions analysis of the face in planning prosthetic treatments in totally edentulous patients. In order to apply this method, latero-lateral and posterior-anterior X-rays must be performed in addition to the common procedure. Two main concerns for totally edentulous patients are the establishment of the vertical dimension and the new position of the occlusal plane. The divine proportion analysis was carried out by the use of a golden divider. The prosthetic protocol was divided into three steps and a case was selected for better understanding. Referring to the golden relations, if the distance from the chin to the wing of the nose is 1.0, the distance from the nose to eye is 0.618. This proportion is useful and effective in determining the correct prosthetic vertical dimension. The incisal margin of the lower incisor must be positioned between Point A (A) and protuberance menti (Pm) according to the gold ratio 0.618 of the total height A-Pm. Posteriorly the occlusal plane must be placed 2 mm below the divine occlusal plane (traced from the incisal margin of lower incisors to Xi point). A prosthesis made in accordance with cephalometric parameters and divine proportions of the face helps to improve the patient’s aesthetics, function and social personality.

The purpose of this study was to compare the estimates of lean body mass (LBM) and percent body fat (%BF), as predicted by bioelectrical impedance (BIA) and sum of skinfolds (SF), with those derived by hydrostatic weighing (HW) obtained before and after a 10-wk diet and exercise regimen. The experimental (E) group consisted of 17 healthy male subjects; 20 healthy males served as the control (C) group. Post hoc Scheffe contrasts computed on E group data indicated that, for both LBM and %BF, the Lukaski and Segal BIA equations, as well as the Durnin SF equation, derived mean values that were not significantly different (0.05 significance level) from HW in both pre- and postregimen conditions. For LBM, the same equations derived the following significant (P less than 0.01) correlation coefficients for both pre- and postregimen data: Lukaski, 0.87 and 0.85; Segal, 0.89 and 0.87; and Durnin, 0.90 and 0.88. For %BF, the correlation coefficients were slightly lower but remained statistically significant (P less than 0.01). The findings of this study suggest that the BIA method, by use of either the Lukaski or Segal prediction equations, is a valid means of predicting changes in human body composition as measured by the Siri transformation of body density.

1. Five volunteers were studied before and after oral administration of NH4Cl (0.3 g/kg body wt.) given in order to create a moderate acidosis. 2. The quadriceps femoris muscles were stimulated electrically for 75 s and muscle biopsies for determination of pH and metabolite content were taken before, at the end of contraction and after 10 min in the recovery period. 3. Muscle pH at rest (mean 7.04) was not significantly decreased after acidification despite an extracellular pH decrease of 0.15 unit. 4. After contraction muscle pH was significantly lower after NH4Cl. Mean values before and after acidification were 6.70 and 6.54 respectively. 5. The buffer capacity calculated as the total capacity of the muscle to buffer H+ produced during the isometric contraction before and after NH4Cl ingestion was reduced from 68.6 sl to 54.5 sl. 6. The force produced by contracting muscle was significantly lower at the end of the contraction period after NH4Cl ingestion, 44.6% of initial compared with 55.4% without NH4Cl.

Introduction. Optimal reconstruction of vertical dimension of occlusion is crucial for functional and physiognomic rehabilitation of edentulous patients. This article is aimed at presenting attitudes and studies on application of cephalometric analysis in obtaining optimal vertical dimension of occlusion. The review of literature presents the studies which analyse the possibilities of cephalometric analysis aimed at improving the clinical methods for vertical dimension of occlusion determination in treatment of edentulous patients. The research carried out so far can roughly be divided into: cephalometric vertical dimension of occlusion evaluation in dentulous patients performed to determine precise indicators of vertical dimension of occlusion and to establish cephalometric standards for practical application in prosthodontics; the method of producing pre-extraction cephalometric registries involves the production of cephalometric radiographs for potential prosthodontic patients in dental pre-extraction period which are kept for reference to be used in later therapy; the cephalometric method of registering the position of physiologic rest position of the mandible involves measuring cephalometric parameters in cephalometric radiographs made when the mandible is in physiologic rest position; cephalometric evaluation of vertical dimension of occlusion in complete denture therapy after clinical determination of intemaxillary relationship is recommended for timely detection of possible mistakes, with a possibility of correction in the process of complete denture production; and cephalometric analysis in edentulous patients with old complete dentures for a planned vertical dimension of occlusion extension. Conclusion. Data from the literature give no proof of a scientific and universally accepted method for precise determination of vertical dimension of occlusion, which is a point many authors agree upon. Different methods proposed for vertical dimension of occlusion determination in everyday practice are usually recommended in combination with other methods. Determination of individual, morphological vertical dimension of occlusion indicators by cephalometric analysis is, in this sense, one of the directions for finding a better solution when planning an artificial occlusion complex.

There is clear clinical evidence that a drastic lowering of plasma LDL- Cholesterol (LDL) concentrations significantly reduces the rate of total and coronary mortality as well as the incidence of cardiovascular events in high risk hypercholesterolemic patients. We describe the case of a 51-year-old woman with coronary heart disease (CHD) who presented with increasing angina on exertion in 1995, at the age of 45. She suffered from a heterozygous familial hypercholesterolemia and in 1985 her total cholesterol (TCHO) was 328±62 mg/dl (mean value of ten analysis). After ten years of statins her mean values (20 analysis, 2 per year) were: TCHO 259±71, LDL 209±47, HDL 35±7 mg/dl. Coronary angiography (CA) performed in 1995 disclosed three vessel coronary heart disease with significant stenoses of the distal right coronary artery, multiple calcifications of the interventricularis artery and multiple plaques with significant stenoses in the ramus circumflexus. The woman underwent coronary by-pass surgery. Thereafter the patient was treated for six years with HELP in biweekly intervals, in combination with statins. TCHO, LDL, HDL and fibrinogen (fb) levels were measured before and after each treatment. Their mean values for an amount of 120 sessions were: TCHO pre 216±23, post 111±18 LDL pre 152±16 post 67±18, HDL pre 42±5 post 35±4 fb pre 306±48 post 125±31. In 2001 a new CA was performed. Calcifications disappeared and stenoses were identical to the previous CA or reduced. There were no further clinical manifestations of CHD. We trust that the clinical benefit of the HELP procedure will be substantial for those patients who have problems in clearing LDL from their plasma pool and who are at the same time sensitive to elevated LDL levels by the development of premature coronary sclerosis.

Objective Upper Crossed Syndrome (UCS) is a type of musculoskeletal system involvement that results in shortening of anterior muscles and weakening of posterior muscles. The aim of this study was to examine the effect of selected corrective exercises with physioball on the postural status of female computer users with UCS. Methods This quasi-experimental study was conducted on 24 female students with UCS who were selected using a purposive sampling method. The subjects were randomly divided into two groups of exercise (n=12) and control (n=12). The exercise group performed exercises for 6 weeks. Photogrammetry method was used to measure the angle of the forward head and round shoulder. A flexible ruler was used to measure the angle of kyphosis, and tape measure was used to measure the chest expansion. These measurements were performed before and after intervention. The collected data were analyzed using paired t-test and ANOVA at the significance level of P<0.05. Results There was a significant difference in mean values of kyphosis (P=0.001), round shoulder (P=0.001), forward head (P=0.002) and chest expansion (P=0.003) before and after exercise. Conclusion Improvement in forward head, round shoulder and thoracic kyphosis angles and chest expansion showed the effectiveness of applied exercise program. Therefore, it is recommended to use this exercise program in computer users with UCS.

Effects of home-based convergence insufficiency vision therapy on accommodation among school going children Wekesa Andrew1, Wanzala Maximilla1 1Masinde Muliro University of Science and Technology Kakamega, Kenya.   Abstract Background: Convergence and accommodation are always yoked together and any adjustment on the former affects the later. The most effective treatment for convergence insufficiency (CI) is vision therapy. Aim/Purpose: To determine the effects of home-based vision therapy on accommodation in school-going with CI attending the Masinde Muliro University Academic Vision Center in Kenya. Methods: A clinical experimental design involving 23 participants with the mean age of 14±2.4 years, were recruited into the study, however, only 18 were assessed after the therapy. The study took 9 weeks thus majority lost contact while others moved out of the town. Home-based vision therapy which is undertaken at home using a pencil or broke strings. During the therapy, accommodative values were monitored which were; negative relative accommodation, positive relative accommodation, dynamic accommodation, accommodative facility and near point of accommodation. Paired t-test used to compare mean values before and after the therapy. Results: The mean value of negative relative accommodation (NRA) before (mention what the mean values were here) and after (mention what the mean values were here) home-based therapy was statistically significant (p=0.01). However, the mean values before and after home-based therapy for the near point of accommodation (NPA), dynamic (Lag), relativity (PRA) and facility (MAF) showed no difference (p> 0.05). The mean value of the NRA before and after office-based vision therapy was statistically significant (p=0.01). Conclusion: The use of home-based vision therapy for the patients with CI had significant effects on accommodation changing from a lower limit to average after the therapy.   Keywords: Convergence insufficiency, Accommodation, Home-based vision therapy.

The treatment of skeletal disharmonies presents better results when performed during the growth period. The physical changes that occur in every individual express growth, which is ruled by genetic, general and environmental factors. In order to identify such disharmonies and hence concentrate the clinical actions during treatment and influence facial growth, cephalometrics appears as a useful a diagnostic tool for identifying facial growth patterns or growth direction. Jarabak's cephalometric analysis is used to assess facial growth pattern of subjects with normal occlusion or malocclusions. The purpose of this study was to obtain mean values for cephalometric measurements from Jarabak's cephalometric analyses of black Brazilian subjects resident in the city of Piracicaba, São Paulo state, Brazil and vicinities, who presented Class I molar relationship with normal overjet and overbite, mild or no tooth crowding or spacing, and no history of orthodontic treatment. A sample of 37 stone plaster casts and 37 lateral teleradiographs from both male and female individuals aged 10 to 14 years was evaluated. Data were analyzed statistically by Student's t-test at 5% significance level. There was no significant differences between genders. The cephalometric measurements obtained in this study were similar to the Jarabak's standards, except for S-N mean value in females (66.50 mm ± 3.16), which was significantly lower than the standard.
