BACKGROUND: Meningiomas are the most common primary tumors of the central nervous system worldwide. Routinely used immunohistochemical markers for diagnosis of confusing meningioma cases as epithelial membrane antigen lack specificity and sensitivity. MUC4 is glycosylated membrane-associated mucin expressed by normal epithelia and many cancers. However, it is recently noticed to be expressed in meningiomas. AIM: Intensity of MUC4 expression is needed to be verified whether it is the same among different subtypes or not. MATERIALS AND METHODS: Fifty cases of different intracranial meningioma subtypes and thirty cases of mesenchymal nonmeningothelial tumors were immunohistochemically stained with MUC4 antibody. The results of MUC4 expression intensity were associated with some clinical and pathological parameters. RESULTS: Most studied meningioma cases (84%) showed positive MUC4 expression. Meningothelial meningioma subtype showed characteristic pattern of diffuse and moderate to strong MUC4 staining. While fibroblastic meningioma showed mostly negative staining pattern and focal weak staining pattern if positive. A statistically significant relationship was detected between tumor subtype and intensity of MUC4 expression. On contrary, most included mesenchymal cases were MUC4 negative with statistical significance. Hence, the sensitivity of MUC4 as diagnostic marker for meningioma was 84%, while the specificity was 93.3%. Furthermore, meningioma histologic subtype showed significant relationship with age. CONCLUSION: The current study results suggest that MUC4 could be used as meningioma diagnostic marker with some limitations. Moreover, meningioma should be included in the differential diagnosis of MUC4 positive tumors.

BACKGROUND At present, there are many clinical markers and models to assess ovarian reserve, but none of them are ideal. The number of clinical samples is a key factor limiting the specificity and sensitivity of the markers and models, and traditional methods of subject recruitment are time and vigor. In addition, the model of ovarian reserve assessment for Chinese women need to be further explored. OBJECTIVE To explore the possibility of self-reporting for subjects through the WeChat mini program, and provide more data support for further optimization of the OvAge model, and to develop a predictive model of ovarian reserve that is specific to Chinese women. METHODS In this paper, with reference to the existing OvAge model, we developed an online OvAge calculator based on the WeChat mini program for data collection, and then applied the generalized linear model theory to obtain a predictive model of ovarian reserve which is in line with the characteristics of Chinese women. RESULTS Compared to traditional recruiting methods, the online OvAge calculator is able to collect a large number of samples in a short period of time, which is efficient and convenient. Optimized model of estimated OvAge =exp (3.5254-0.001*PRL-0.0231*AMH). This model showed a high statistical significance for each marker included in the equation. We applied the final equation on diminished ovarian reserve and polycystic ovary syndrome datasets and obtained a mean of predicted ovarian age significantly different from the mean of chronological age in both groups. CONCLUSIONS The OvAge calculator based on the WeChat mini program is a novel online subject self-reporting system that can collect many samples in a short period of time, continuously optimize the model and update the mini program version, which is economical, time-saving and efficient., and is worthy of promotion. In addition, the optimized OvAge model is suitable for Chinese women and provides a reference for clinical assessment of ovarian reserve. CLINICALTRIAL The study was approved by Chinese Clinical Trial Registry (registration No. ChiCTR2000037522) and Medical ethics committee of Jiangsu Province Hospital of Chinese Medicine (approved No. 2019NL-152-02).

Abstract Background Our goal was to find out the relation between mammographic densities and cancer of the breast according to the recent ACR classification. From the medical records of Kasereliny Hospital, 49,409 women were subjected to digital mammography for screening, of which 1500 breast cancer cases were collected. The mammographic categories of breast density were ACR-A, B, C, and D, which were detected by two senior radiologists. All radiological classifications were made using both standard mammographic views bilaterally. Two-sided tests of statistical significance were represented by all the P values. Results From 2014 to 2019, 49,409 women came for digital mammographic screening, their age ranges between 40 and 65, and all of them are included in the study. One thousand cases of breast cancer cases were radiologically and pathologically diagnosed. Different densities were arranged in descending pattern depending on the frequency of positive cases: D (13.7%), C (3.3%), B (2.7%), A (2.2%). There is positive significant risk ratio among every higher mammographic density in comparison to the lower density. Conclusion Our study results show that the risk of breast cancer is in close relation to the mammographic breast density.

Abstract The Silk Road pattern (SRP), which depicts the teleconnection pattern along the Asian jet, has been extensively investigated and commonly described as the leading mode of upper-tropospheric meridional wind anomalies in summer. In this study, the SRP is identified as having a significant relationship with the meridional displacement of the Asian jet (JMD), which manifests as the leading mode of upper-tropospheric zonal wind anomalies. This significant relationship is confirmed by the correlation coefficient between the indices for JMD and SRP, which is 0.39 and reaches statistical significance at the 0.01 level. When the Asian jet is in a northward (southward) displacement, the phase of SRP tends to be shown as anticyclonic (cyclonic) anomalies over western Asia and East Asia and cyclonic (anticyclonic) anomalies over Europe and central Asia. The authors propose an internal atmospheric mechanism for this relationship. In addition, it is found that the JMD is significantly affected by the tropical surface temperature anomalies. In particular, the negative (positive) SST anomalies in the tropical central and eastern Pacific of the preceding spring lead to significant cooler (warmer) tropical tropospheric temperatures in summer and may induce the northward (southward) displacement of the Asian jet through modifying the meridional gradient of tropospheric temperatures. The tropical tropospheric temperature anomalies may also affect the SRP through the JMD.

Alveolar echinococcosis (AE) is a rare anthropozoonotic parasitic disease, which can affects the liver, lungs and other organs. R0-radical liver resection is the sole curative therapy for the patients with AE. Size of the parasitic foci, distant dissemination, and involvement of main liver vessels – all this information allows the surgeon to make the right decision about practicability and volume of operation. Formerly, ultrasonography (US) was not method of choice for the qualitative pre-operative diagnostic of AE. Nevertheless nowadays development of new technologies allows US to be an equal to CT and MRI.The aim:to estimate the possibilities of US in the planning of surgery in patients with AE.Materials and methods.The data of 64 patients who were undergone complete liver resection or reduction surgery in A.V. Vishnevsky Surgery Institute in period from January 2008 to December 2016 we respectively analyzed. Specificity and sensitivity of US, CT and MRI were analyzed and ROC-curves were constructed. Statistical significance was calculated using Chi-square.Results.The efficiency of US was significantly comparable to CT and MRI when we analyzed the involved of porta hepatis, vena cava, hepatic veins. Assessment of involvement of liver arteries and vena porta was not statistical significant.Conclusion.Accumulated experience of A.V. Vishnevsky Surgery Institute shows the possibility of qualitative preoperationUS evaluation of AE-lesion, which has to include assessment of distant dissemination and involvement of the liver main vessels. In a big surgical hospitals, which has an experience of AE treatment, pre-operative US can become the method of choice in planning of surgical operation.

Background/Aim. Ovary is the organ of the female reproductive system most commonly affected by metastases. The aim of the study was to determine the frequency and features of metastatic ovarian tumors (MOT) depending on the site of the primary malignant tumor. Methods. The study group consisted of 488 patients with histopathologically confirmed ovarian cancers treated at the Clinic of Oncology, Clinical Center Nis, in the period from 1 January 1998 to 31 December 2005. MOT were found in 41 patients. Regarding the site of the primary malignant tumor, those with secondary ovarian tumor were divided into two groups: group A - primary malignant tumor involving the genital organs (n = 30) and group B - primary malignant tumor of extragenital origin (n = 11). Results. MOT were confirmed in 8.40% (41/488) of the patients. Secondary ovarian malignancies were the consequence of endometrial carcinoma spreading in 73.17%, breast carcinoma in 19.51%, stomach carcinoma in 4.88% and colon carcinoma in 2.44% of the cases. No significant differences were found between the group A and group B by the factors of age, body mass index, parity and menopausal status. Contrary to the group A, metastatic tumors in the group B patients were more commonly asymptomatic (p < 0.001), bilateral (p < 0.05), with larger ovarian diameter (p < 0.05), associated with ascites (p < 0.001) and abdominal metastases (p < 0.01), all of statistical significance. Conclusions. Metastatic tumors made up 8.40% of ovarian neoplasmas. With non-genital primary tumors, secondary ovarian deposits were frequently asymptomatic, bilateral, associated with larger ovarian diameter, ascites and abdominal metastatic deposits, compared to malignant tumors of genital origin.

Study Tool: The researcher built tow questionnaires, the first questionnaire to measure using of the transference leadership and it consist of two parts one is personal information anther is phrases for measuring the practice of heads educational management degrees' in Afif Educational management from the points of view of educational supervisors , also it contain forth transference leadership diminutions( typical effective, mental- excitation, personal considerations, accusatory motivation) in (39) sentences. The second questionnaire to measure the level of educational supervisors' job satisfaction in (28) sentences. Moreover, the researcher used the fifth Grading scale to measure the responses of study sample. Study Results: the results of the study can firmed the following: - The overall average of total to the degree of transference leadership applying for Heads Department (4,10) , so this is the mean between fourth category which indicated to high degree. - The general average of the responses of the study sample at the axis of educational supervisors job satisfaction for in Educational management in AFIF that is mean (3,85 over 5) indicated to (high) degree of study tool. The results proved that there is connective, direct, strong relation & statistical function at (0,01) between the practice of heads educational management degrees' in Afif Educational management for the style of transference leadership and job satisfaction of educational. Also, the result show that connective the relation between general degree of the measure of transference leadership and general degree of the measure of job satisfaction is (0,56) , so it is positive direct relation. Study Recommendations: - Encouraging and training the Heads of educational management to be attention in knowing the weakness points about their work which is hinder the transference leadership practice. - Caring of training and qualifying staff who have primitive readiness for having transference leadership properties and characters. - Necessity of hold periodical specialist courses to create leaders and trine them about modern administrative styles that make job satisfaction for personnel.

This study examined the Service Quality (SERVQUAL) model with the Internal Service Quality (ISQ) dimensions such as tangibles, reliability, responsiveness, assurance and empathy that impact on employee work engagement. This research work was conducted in Hotel Soaltee Crowne Plaza (SCP), one of the 5-star deluxe hotels of Nepal. The study has adopted an inferential research design to meet the study objectives regarding the impact of independent variables on work engagement of SCP Hotel. A structured questionnaire was distributed to 126 sample respondents out of a total of 503 employees of SCP hotel. The study reveals a significant relationship of work engagement with empathy and not with rest of the independent variables. Based on the findings it is recommended that employees should be taken good care of, and employers should be well aware of the employees’ emotional level since empathy has shown a highly significant relationship with work engagement. Study results have scope of future reference whereby implementing SERVQUAL dimensions for employee work engagement and reduction in employee turnover and improved.

Introduction/Objective. Presbycusis, elderly hearing loss, is a progressive, bilateral sensoryneural hearing loss characterized by reduced sensitivity of hearing and understanding speech in a noisy environment, thereby impairing communication and inducing anxiety. The objective was to examine the impact of hearing amplification on subjective hearing disability assessment and anxiety in people with presbycusis. Method. Sample consisted of 120 respondents aged 47?85 with presbycusis, 60 subjects with and 60 subjects with no auditory amplification. The standardized Hearing Handicap Inventory for the Elderly and the Spielberger State Trait Anxiety Inventory were used in the study. Results. In subjects with hearing amplification, test/retest has no statistical significance in the STAI and HHIE scales and subscales, except the HHIE-S (p = 0.004) with a lower score on the retest. Respondents in whom hearing amplification was performed during the year was statistically significant in HHIE (p = 0.016), HHIE-S (p = 0.004) and STAI-S (p = 0.029) which speaks of favorable effect of hearing amplification. In the group with no hearing amplification, statistical significance was observed in relation to the HHIE scores (p = 0.002), HHIE-E (p = 0.000), STAI (p = 0.000), STAI-S (p = 0.001) and STAI-T (p = 0.001) and it was noticed that anxiety, loss of emotional contacts, and more pronounced degree of hearing impairment were the result of unassisted hearing rehabilitation. Conclusion. Audiological practice should include tests for assessment of hearing disability and anxiety in order to preserve health in later life.

Background: Linear atelectasis is a focal area of subsegmental atelectasis with a linear shape. Linear atelectasis may occur as a consequence of subsegmental bronchial obstruction. Aims: We propose an early roentgen sign of obstructing lung tumors, namely perihilar linear atelectasis, and ascertain whether this phenomenon could be used as a sign to detect radiographically occult primary lung cancer. Materials and Methods: We performed a retrospective review of 45,000 posteroanterior chest radiographs to determine the frequency of appearance and characteristics of perihilar linear atelectasis. The perihilar region of chest radiographs was evaluated for the presence of linear atelectasis. When linear atelectasis was found, the total thickness was measured. Student's t-test was used to evaluate statistical significance, correlating the thickness of atelectasis and the presence of obstructing central primary lung cancer. Results: Perihilar linear atelectasis was demonstrated in 58 patients. Atelectasis was caused by an obstructing tumor in 21 (36%) cases and a variety of other conditions in 37 (64%) patients. A statistically significant relationship (P < 0.001) was observed between the dimension of perihilar linear atelectasis and primary lung cancer, with 16 of 19 patients with thick (>5.5 mm) perihilar linear atelectasis found to have primary lung cancer. Conclusion: Thick perihilar linear atelectasis is a new diagnostic roentgen sign that suggests subsegmental bronchial obstruction. In this patient subgroup, who are otherwise asymptomatic, a persistent linear atelectasis can be due to primary lung cancer.
