Background: The etiology of benign prostatic hyperplasia and prostate cancer are unknown, with ageing being the greatness risk factor. Methods: This new perspective evaluates the available interdisciplinary evidence regarding prostate ageing in terms of the cell biology of regulation and homeostasis, which could explain the timeline of evolutionary cancer biology as degenerative, inflammatory and neoplasm progressions in these multifactorial and heterogeneous prostatic diseases. Results: This prostate ageing degeneration hypothesis encompasses the testosterone-vascular-inflamm-ageing triad, along with the cell biology regulation of amyloidosis and autophagy within an evolutionary tumorigenesis microenvironment. Conclusions: An understanding of these biological processes of prostate ageing can provide potential strategies for early prevention and could contribute to maintaining quality of life for the ageing individual along with substantial medical cost savings.

Multi-parameter magnetic resonance imaging has been widely used in the diagnosis and evaluation of prostate cancer, and has important guiding significance for clinical diagnosis of prostate cancer and their treatment. This article studies the value of transrectal multiparametric ultrasound (mpUSS) in the diagnosis of clinically meaningful prostate cancer. 102 patients with high risk factors for prostate cancer were examined by mpUSS and mpMRI. The transrectal biopsy (SB) results of the prostate system were regarded as the excellent standard, and the diagnostic value of mpUSS, mpMRl and mpUSS combined with mpMRl examination for clinically meaningful prostate cancer was analyzed. The results showed that 58 of the 102 patients with SB were diagnosed with prostate cancer. Among them, 43 cases were detected by mpUSS, 50 cases were detected by mpMRl, 42 cases were detected by mpUSS combined with mpMRI (series), and 56 cases were detected by mpUSS combined with mpMRl (parallel). Grouped by Gleason score, the detection rate of mpUSS for clinically significant prostate cancer was 83.74%, and the detection rate of mpMRl was 93.5%. The comparison between the two was not statistically significant (P > 0.05), but when the two inspection methods were combined. The detection rate was 97.8%, which was significantly higher than the two inspection methods alone. Therefore, we conclude that mpUSS can be used as an imaging test for the diagnosis of prostate cancer. In addition, mpUSS has a high application value in the diagnosis of prostate cancer. The detection rate of mpUSS combined with mpMRl examination for clinically meaningful prostate cancer is significantly higher than that of mpMRl examination alone, which can be used as a diagnostic technique for early diagnosis of meaningful prostate cancer and can be used as a guide clinicians’ early diagnosis and treatment of meaningful prostate cancer.

AbstractHomologous recombination deficiency (HRD) results in impaired double strand break repair and is a frequent driver of tumorigenesis. Here, we developed a genome-wide mutational scar-based pan-cancer Classifier of HOmologous Recombination Deficiency (CHORD) that can discriminate BRCA1- and BRCA2-subtypes. Analysis of a metastatic (n=3,504) and primary (n=1,854) pan-cancer cohort revealed HRD was most frequent in ovarian and breast cancer, followed by pancreatic and prostate cancer. Biallelic inactivation of BRCA1, BRCA2, RAD51C or PALB2 was the most common genetic cause of HRD, with RAD51C and PALB2 inactivation resulting in BRCA2-type HRD. While the specific genetic cause of HRD was cancer type specific, biallelic inactivation was predominantly associated with loss-of-heterozygosity (LOH), with increased contribution of deep deletions in prostate cancer. Our results demonstrate the value of pan-cancer genomics-based HRD testing and its potential diagnostic value for patient stratification towards treatment with e.g. poly ADP-ribose polymerase inhibitors (PARPi).

Angiotensin II (Ang-II) plays a key role as a vasoconstrictor in controlling blood pressure and electrolyte/fluid homeostasis. Recently it has also been shown that this peptide is a cytokine, acting as a growth factor in cardiovascular and stromal cells. In addition, the physiological function of Ang-II seems to be similar in prostate cancer and stromal cells. It is widely assumed that Ang-II facilitates the growth of both cells, and its receptor blockers (ARBs) have the potential to inhibit the growth of various cancer cells and tumors through the Ang-II receptor type 1 (AT1 receptor). The mechanism of cell growth inhibition by ARBs has been considered to be that of suppression of the signal transduction systems activated by growth factors or cytokines in prostate cancer cells, and suppression of angiogenesis. This review highlights the possible use of ARBs as novel agents for prostatic diseases including prostate cancer and benign hypertrophy, and covers related literature.
