This book identifies a range of issues and emerging trends in relation to medical tourism in the 21st Century. One basic issue is the relationship between medical tourism and the wider concept of health and wellness as a framework for living in the present, and another is determination of the extent to which concepts of health and wellness act as a background for medical tourism and the way in which it is constructed and/or practiced. There is no doubt that wellness tourism is a fundamental part of the wider range of treatments now afforded the medical tourist in addition to the medical procedures themselves, and health is of course the condition that all tourists and the members of a host community aspire to. This chapter examines health and wellness tourism as a necessary part of medical treatment for many patients, as one of the reasons why medical tourism has become popular, and its importance as a background to medical tourism.

Bali sebagai ikon pariwisata Indonesia menjadi salah satu penyumbang devisa negara terbesar melalui sektor pariwisata. Salah satu daya tarik Bali yang telah populer di kalangan wisatawan adalah health and wellness tourism, oleh karena itu tujuan wisatawan berkunjung ke Bali saat ini tidak hanya untuk berlibur, melainkan juga untuk berobat yang dikenal sebagai perjalanan medis (medical tourism). Salah satu pusat pelayanan kesehatan di Bali yang terinspirasi dari konsep perjalanan medis ini adalah Rumah Sakit Bali Royal. Upaya yang dilakukan rumah sakit dalam meningkatkan kualitas adalah mengembangkan fasilitas dan melakukan manajemen sumber daya manusia agar tercipta sumber daya manusia yang berkualitas. Oleh karena itu rumah sakit perlu memerhatikan aspek penting dalam diri karyawan yang berguna bagi kesuksesan organisasi. Salah satu aspek tersebut adalah komitmen karyawan terhadap organisasi. Komitmen organisasi dalam diri karyawan akan muncul apabila organisasi dapat memberikan dukungan berupa kesempatan atas pengembangan karir. Melalui pemberian kesempatan pengembangan karir, organisasi dapat menunjukkan rasa kepedulian dalam membantu karyawan mencapai tujuan karir. Penelitian ini bertujuan untuk mengetahui peran pengembangan karir terhadap komitmen organisasi karyawan Rumah Sakit Bali Royal. Hasil analisis menunjukan nilai koefisien regresi sebesar 0,882, nilai koefisien determinasi sebesar 0,778 dan nilai signifikansi sebesar 0,000 (p<0,05). Hasil tersebut menunjukan bahwa pengembangan karir berperan sebesar 77,8% terhadap komitmen organisasi karyawan Rumah Sakit Bali Royal. Kata kunci: Karyawan, komitmen organisasi, pengembangan karir, rumah sakit.

What can clinical psychology contribute to medical tourism? Well, of course, clinicians are a very important part of any form of medical treatment, but psychologists are important because of their training in the mental condition of the patient, an aspect of medicine that is often not valued as highly as that of physical intervention in disease control or accident recovery. And yet, the whole concept of health and wellness (of which medical tourism is increasingly a part) relies heavily on the addition of psychological support in the healing and rehabilitation process. This chapter provides some insights into the processes and value of psychological intervention in the physical and mental wellbeing of patients in the medical system, provides information on the Japanese variants of this, and seeks to extend understanding of the value of this intervention for medical tourists.

Wellness tourism is currently one of the fastest growing tourism niche markets having experienced exponential growth over the past two decades (Global Wellness Institute, 2018). The attributed reasons for the exponential growth is wellness being an essential factor in shaping people’s lives, as well as being increasingly influential in patterns of consumption and production. The wellness industry plays a crucial role as an important driver for future business growth and major innovations (Voigt and Pforr, 2013; Pyke et al., 2016). This chapter defines the relative terms of health, wellness, spa and medical tourism, identifies the current trends in the health and wellness sector, details the various wellness providers and considers the future direction of health and wellness in connection with tourism and destination development. The chapter concludes with a case study discussing the success factors of wellness spa tourism in Thailand.

The author has proved the prospects of medical tourism development in Ukraine. The analysis of the concepts of “tourism” and “health tourism” has been carried out; their features have been singled out. The expediency of distinguishing the concept of “health tourism” on “medical” and “health” has been proved. It has been offered to amend the Art. 1 of the Law of Ukraine “On Tourism” by defining the terms of “medical tourism” and “health tourism”. The content of the concept of “medical tourism” has been studied; its constituent elements have been singled out; the author’s definition of the concepts of “health tourism”, “medical tourism” has been formulated; the sphere of their law-enforcement has been defined. The author has developed propositions to improve the regulatory acts regulating the circulation of medical tourist services (health and medical) in Ukraine, namely: the Tax Code of Ukraine, the Law of Ukraine “On Public and Private Partnership”, the Law of Ukraine “On Licensing of Economic Activities”. The author has also offered to develop and approve the National Program for the Development of Medical Tourism in Ukraine.

Health and wellness tourism is a new form of tourism in which people seek to experience a healthy lifestyle through tourism. From the perspective of demand side, this paper discusses the tourism motivation of health and wellness tourists, and based on a new tourism motivation model, studies the influence mechanism of pull motivation on revisit intention. The results showed that:(1) Pull motivation had a significant positive effect on revisit intention; (2) pushing motivation negatively moderates the relationship between pull motivation and revisit intention.

SummaryWhen the International Normalised Ratio (INR) is used for control of oral anticoagulant therapy the same result should be obtained irrespective of the laboratory reagent used. However, in the UK National External Quality Assessment Scheme (NEQAS) for Blood Coagulation INRs determined using different reagents have been significantly different.For 18 NEQAS samples Manchester Reagent (MR) was associated with significantly lower INRs than those obtained using Diagen Activated (DA, p = 0.0004) or Instrumentation Laboratory PT-Fib HS (IL, p = 0.0001). Mean INRs for this group were 3.15, 3.61, and 3.65 for MR, DA, and IL respectively. For 61 fresh samples from warfarin-ised patients with INRs of greater than 3.0 the relationship between thromboplastins in respect of INR was similar to that observed for NEQAS data. Thus INRs obtained with MR were significantly lower than with DA or IL (p <0.0001). Mean INRs for this group were 4.01, 4.40, and 4.59 for MR, DA, and IL respectively.We conclude that the differences between INRs measured with the thromboplastins studied here are sufficiently great to influence patient management through warfarin dosage schedules, particularly in the upper therapeutic range of INR. There is clearly a need to address the issues responsible for the observed discrepancies.

Objective: Tourism has become the main engine of economic, social and environmental development in several countries, so promoting tourism awareness among tourists and the local population should be a priority. The present study aims to suggest a status of the research carried out on the topic of tourism awareness. Design / methodology / approach: The type of analysis is through a retrospective and exploratory bibliometric study. The analysis materials were scientific articles and a training manual published between 2000 and 2020, registered by Scopus, Emerald insight and Dialnet, using “tourism awareness” as the keyword. Results: When considering the three senses in which tourism awareness ought to operate, it is concluded that studies are more focused on the relationship and contact of the host community with the tourist. It is observed that four out of six articles in this sense consider that education, training, and government policies around tourism awareness should be developed in a better way in the destinations, in order to be an element that contributes to the development of communities and reduces poverty in developing countries. Study limitations / implications: It is considered a limitation not to include thesis dissertations. Findings / conclusions: It is necessary to make visible the importance of tourism awareness as a local development strategy for communities, in addition to including tourism awareness on the part of tourists.

The objective: to еxplore the relationship between the activity of endometrial proliferation and the state of the local immune response in the uterus in the conditions berprestasi process. Patients and methods. Examined 228 women of reproductive and perimenopausal age with endometrial pathology using ultrasound and then performing hysteroresectoscopy. Determination of the concentrations of the cytokines IL-1, IL-2, IL-6 and TNF was performed by solid phase ELISA. Results. Found a trend that confirms the loss of sensitivity to hormones at the stage of malignancy of the endometrium and can be used as diagnostic determinants in determining the nature of intrauterine pathology and criterion of the effectiveness of conservative therapy. Conclusion. Improving etiopatogenetice approach to the therapy of hyperplastic proce.sses of endometrium with determination of receptor phenotype of the endometrium is a research direction in modern gynecology, which will help to improve the results of treatment and prevention of intrauterine pathology. Key words: endometrial hyperplasia,the receptors for progesterone and estrogen, immunohistochemical method.

Vertical axis wind turbine technology has been applied last years, very long after horizontal axis wind turbine technology. Aerodynamic problems of vertical axis wind machines are discussible. An important problem is the determination of the incidence law in the interaction between wind and rotor blades. The focus of the work is to establish equations of the incidence depending on the blade azimuth, and to solve them. From these results, aerodynamic torques and power can be calculated. The incidence angle is a parameter of velocity triangle, and both the factors depend not only on the blade azimuth but also on the ratio of rotational speed and horizontal speed. The built computational program allows theoretically selecting the relationship of geometric parameters of wind turbine in accordance with requirements on power, wind speed and installation conditions.
