The increasing number of problems that need to be addressed in the hospital sector calls for innovation in this field. It brings us the need to find cost-effective and memory-efficient solutions to handle the vast data and sector it into essential information to operate on the patient. There used to be many systems to manage clinical records which are fixed at a place. It is quite complicated to get the information and make this data available at a patient's bedside. This leads to a considerable amount of wasted time in moving to those storage PCs and also the cost afforded is comparatively high. A computer system that controls and accomplishes all the data in the hospital database to provide effective healthcare is called hospital information system (HIS). The introduction of HIS made billing and inventor easier for the staff. This paper discusses diverse methods that improve the cost, demands of HIS, and provide techniques to function efficiently using wireless networks. Also, the paper gives a comparative study on different aspects such as cost, quality of service, transportation, and security. A new system is proposed by combining the wireless healthcare system along prioritized alert notification.

Video streaming is becoming one of the most important services deployed over telecommunication networks such as the Internet and triple-play operators’ networks. This service differs from the rest in being loss sensitive and highly delay sensitive and requiring a considerable amount of bandwidth in order to offer a smooth transmission of packets through the network. While upgrading network elements with quality of service and multicast transmission capabilities becomes prohibitive for most network operators, peer-to-peer (P2P) architectures appear to be smart and efficient solutions to the previous issues. Many different P2P systems have been proposed and deployed to offer reliable video streaming services. These approaches address issues such as multicast transmission, quality of service enablement, mobility robustness, and video distribution according to network and user device capabilities. This chapter gives an overview of the different issues related to performance and reliability in multicast streaming over wireless networks and presents several alternatives facing them, including amendments to the already existing multicast mechanism of data distribution, video scalability and how peer-to-peer networking can provide a cost-effective solution to such problems.

The prevalence of wounds and the cost of treating them are increasing year on year. Improving the quality of wound care will improve patient outcomes and is a financial necessity. The Lens of Profound Knowledge is a tool that can be used to support quality improvement and identify where action is needed. It allows exploration of an organisation through four aspects—appreciate the system, understanding variation, psychology, and theory of knowledge—and working on all four aspects simultaneously is believed to increase the likelihood of achieving improvement. Improvements at and between all levels—microsystem (such as frontline services), mesosystem (health boards) and macrosystem (NHS Scotland)—would reduce variation in practice and prove to be both clinically and cost-effective. Given the rapidly growing population of people with unhealed wounds, wound care needs to be valued at all system levels and be adequately resourced.

Context: Stroke is one of the main causes of premature death and disability, imposing significant costs on the healthcare system, especially due to expensive hospital care. Home care service is one of the interventions used in the last two decades to reduce the cost of services provided for stroke patients in different countries. Objectives: The present study aimed to systematically review studies related to the economic evaluation of home care compared to hospital care for stroke patients. Data Sources: A search was conducted between January 1990 and January 2021. PubMed, Scopus, Web of Science, and Embase databases were searched systematically. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were used to select the studies. Data Extraction: To evaluate the quality of studies included in this systematic review, Drummond’s ten-item checklist was used. Results: Five economic evaluation studies were included in this review. The included studies reported different results regarding the effect of home care on improving different indicators and the cost-effectiveness ratio of home care to hospital care. Most previous studies reported that home care is a more cost-effective option for improving many indicators, such as physical function and quality-adjusted life years (QALY), and for reducing mortality and institutionalization, compared to hospital care. Conclusions: Home care is a more cost-effective option than hospital care for stroke patients with regard to some indicators, such as the Barthel index for Activities of Daily Living, Modified Rankin Scale (mRS), quality of life, mortality, and institutionalization. However, there are some exemptions to this conclusion. Due to limitations, such as heterogeneity of interventions in the existing studies, different levels of patients’ disabilities, different perspectives toward economic evaluation, and differences in the healthcare systems of countries, further research is needed according to the context of each country based on clinical trials.

Purpose The purpose of this paper is to apply security policies over the mobile ad hoc networks. A mobile ad hoc network refers to infrastructure-less, persistently self-designing systems; likewise, there is a noteworthy innovation that supplies virtual equipment and programming assets according to the requirement of mobile ad hoc network. Design/methodology/approach It faces different execution and effectiveness-based difficulties. The major challenge is the compromise of performance because of unavailable resources with respect to the MANET. In order to increase the MANET environment’s performance, various techniques are employed for routing and security purpose. An efficient security module requires a quality-of-service (QoS)-based security policy. It performs the task of routing and of the mobile nodes, and it also reduces the routing cost by finding the most trusted node. Findings The experimental results specify that QoS-based security policy effectively minimizes the cost, response time as well as the mobile makespan (routing cost and response time) of an application with respect to other existing approaches. Research limitations/implications In this paper, the authors proposed an enhancement of Cross Centric Intrusion Detection System named as PIHNSPRA Routing Algorithm (PIHNSPRA). Practical implications It maps the security with the secure IDS communication and distributes the packets among different destinations, based on priority. This calculation is proposed for the purpose of routing and security by considering greatest throughput with least routing cost and reaction time. Social implications When the concept is applied to practical applications. Quality of Service introduced in the proposed research reduces the cost of routing and improves the throughput. Originality/value The proposed calculation is tested by NS2 simulator and the outcomes showed that the execution of the calculation is superior to other conventional algorithms.

Providing security for the content that gets exchanged between physically and geographically different locations is challenging. The cost and resources to be used to meet this challenge has to be linked to the degree of security demanded by the content. In this chapter, the security associated with the transfer of the content is quantified and treated as a quality of service parameter. The user is free to select the parameter depending up on the content being transferred. As dictated by the demanding situations, a minimum agreed security would be assured for the data at the expense of the appropriate resources over the network.

In 2016, the U.S. Government health expenditures reached $3.35 trillion and the cost per person stood at $10,345. Health is seen as impacting both one's quality of life and finances. The Affordable Care Act (ACA) (2008 - 2016) brought the issue of cost to the forefront for all people especially those in the health disparate communities. Advances in health informatics coupled with new approaches to healthcare delivery may hold promise for this large industry in the USA that critically needs to be cost effective in order to sustain itself. This paper reports a study that investigated importance of health, mobile health (m-Health) and telemedicine awareness along with its adoption in a health disparate community that has one of the Historical Black Colleges & Universities (HBCUs) in the country. The findings were that, all participants owned a mobile (cell) phone with smart features. Although a large number them indicated that their health was very important to them, there was lack of awareness and adoption of m-Health and telemedicine.

This research aims to analyze factors affecting the satisfaction of tourists in the use of marine transportation from Bali toward Gili Trawangan. The benefits of this research useful to the company in improving the services for tourists who will use the services of a crosswalk leading to Gili Trawangan.The respondents in this research totalled 190 respondents, the selection of the sample using the method of purposive sampling, the data in this study were analyzed using the analysis factors that help by statistikal program SPSS version 17.0 for windows. The results of this study are: 1) the satisfaction of tourists in the use of sea transport in general are at a level of satisfaction. 2) there are three factors that affect the satisfaction of tourists formed i.e. factors quality of maids, the cost factor & ease factors and product quality. Quality of service factor is the most dominant factor influencing the satisfaction of travellers with variable reliability as the dominant variables, and the variable availability schedule is a variable that must be improved.

Production of high precision circular scales is a complicated process requiring expensive equipment and complex processes to achieve. Precision angle measurement equipment tends to be very expensive and therefore not accessible to all in need. Simplification of production of such devices can lead to reducing costs of angle measurement systems ensuring easier accessibility. A new method of producing precision circular scales using low cost mass production can reduce the costs of these devices drastically. Therefore, utilising a common CD technology as the basis for such scales is analysed. This paper deals with the analysis of the newest laser cutting method for plastic circular scales. Preliminary results of manufacturing such scales are presented in the paper as well as measurements of the grating of the scale were performed. The quality of different scales manufactured using different laser types is analysed in the study. The cost – effective alternative of manufacturing circular scales is discussed in the paper.
