Heat-strengthened rails of the R65 type are the main type of rails used on the railway network of Russian Railways. Reducing the number of cropped and acutely defective rails is possible due to the rational use of the design features of intermediate rail fasteners and their current content. The article presents study results of the stiffness influence of intermediate fasteners on the operational durability of rails. The general statistics of the use of various types of intermediate rail fastenings on the network of Russian railways is considered. The main results of previously published studies on the effect of the stiffness of intermediate rail fasteners on the accumulation of contact fatigue damage in rails are briefly presented.Calculations of the accumulation of contact-fatigue damages in rails, carried out by the authors, are based on the data on the vertical and horizontal transverse stiffness of intermediate rail fastenings obtained from the results of bench tests. Calculations of the accumulation of contact-fatigue damage were carried out using the “Universal Mechanism” software package. In the process of modeling, four types of intermediate rail fasteners were considered: ARS-4, ZhBR-65Sh, ZhBR-65PShM and W-30. Calculation results were obtained for a curve with a radius of 650 m on a continuous welded track section.

Abstract Background This study aimed to identify the current challenges of health services provided by senior centers in South Korea and to determine recommendations for improving service to community-dwelling older adults, focusing on the nurse’s role. Methods Quantitative data were obtained from a survey of 30 nurses at senior centers in Seoul, South Korea. In addition, focus group interviews were conducted with 6 senior center nurses, 5 health experts, and 4 attendees of senior centers (n = 15). Content analysis was performed to analyze the qualitative data. Results The study results revealed several challenges, including insufficient health services; a lack of human resources; non-systematic and overlapping health services; and a lack of legal clarification of nurses’ roles. Our recommendations for improvement are that senior centers should: focus on disease prevention and chronic disease management; be hubs to connect health and welfare services; empower nurse’s role and capacity; and establish legal regulation and adequate staffing for nurses. Conclusions These findings are important for improving the senior centers in playing a key role in contributing to health promotion, disease prevention, and chronic disease management among older individuals in South Korea.

Neonatal Intensive Care Units (NICU) plays a major role in providing care for premature neonates, sick neonates or neonates require surgery very soon following delivery. The experience of being a parent of a baby in NICU is more stressful especially if there is a little support or no support rendered from the nursing staff. Parents need support from the nursing staff to gain knowledge and understanding about the neonate’s illness, treatment measures and hospital rules. However, no studies about this issue have been conducted in Namibia. The aim of this study was to investigate the perception of parents related to the nursing support rendered by nurses to parents whose neonates were admitted to NICU at a state hospital in Windhoek, Namibia. A quantitative, cross sectional survey was used to identify and describe parents’ perceptions related to the nursing support rendered by nurses to parents whose neonates were admitted to NICU. The study results indicated that the majority of the participants perceived that they got the good support from the nurses. However, not all parents received needed support from nurses which could increases their stress level. The findings of this study could be used to improve the parents support to reduce stress among parents and neonates and to promote positive coping with regard to the stressful situation of newborns hospitalization.

Nowadays, a change of material properties is evaluated on the basis of the results of "traditional" methods (for example, tensile testing and impact bending test). Unfortunately, "traditional" mechanical testing requires quite large samples, that leads to a damage of studied objects integrity. That is why testing with the application of compact samples, which allows converting obtained results into regular parameters, such as tensile properties and fracture resistance, was developed. Due to the novelty of this method, influence of different factors on testing results is understudied. Here arises the aim to study the influence of the level of accumulated damage on testing results. Regular changes of small punch testing tensile diagrams, depending on the number of worked out cycles, are under study in the present paper. Based on the study results, algorithm for determination of mechanical properties of steel 20 with different levels of cumulative fatigue damage with the application of compact samples was developed.

In recent years, the production decline of some oilfields is obvious, and a large number of oil wells begin to be in inefficient or ineffective production. Only by controlling these wells can the oilfields maintain effective production. By means of dynamic and static multidisciplinary integrated research, through elaborate dissection of typical potential blocks, the optimal treatment measures are selected from four aspects of “ reservoir heterogeneity and production status, type and distribution law of remaining oil, causes of low efficiency wells, classification and comprehensive treatment countermeasures and methods”, so as to provide technical support for potential tapping and comprehensive treatment of old oilfields.

Many scholars claimed that due to maintenance, high reconstruction and construction costs in Malaysia, LCC shows an important role in estimating the cost of rigid pavement. Because of the government's expenditure and high budget allocation on pavement rehabilitation and maintenance work in Malaysia, choosing the most cost-effective pavement maintenance, maximizing the budget allocation and rehabilitation alternatives is essential for transportation agencies. A useful technique LCC of economic analysis that measures the different solutions of cost-effectiveness to rehabilitation and maintenance within the same category of roads. To generate a broad and accurate LCC output, however, it is necessary to ensure that the value information inputs are ready and of sufficient quality to use in the estimation of the LCC. This article shows the findings of Malaysia's systematic literature review inputs for LCC cost information study of dynamic pavement rehabilitation and maintenance. The literature study results showed that the greater part of key players in the construction industry are aware of the need for LCC analysis in the rigid rehabilitation and maintenance of the pavement. In Malaysia the LCC current state system of rigid pavement rehabilitation and maintenance, however, is still in its not only infancy stage but also is not ready for the difficulties to be implemented in achieving quality cost data to generate a reliable and comprehensive analysis of life cycle cost. The present findings of the paper which provide information on cost data current state inputs in the construction industry from the LCC analysis of rigid pavement rehabilitation and maintenance.

Abstract Meniscal tears and osteoarthritis (osteoarthrosis, degenerative arthritis, or degenerative joint disease) are two of the most common conditions involving the knee. This article includes definitions of apportionment and causes; presents a case report of initial and recurrent tears of the medial meniscus plus osteoarthritis (OA) in the medial compartment of the knee; and addresses questions regarding apportionment. The authors, experienced impairment raters who are knowledgeable regarding the AMA Guides to the Evaluation of Permanent Impairment (AMA Guides), show that, when instructions on impairment rating are incomplete, unclear, or inconsistent, interrater reliability diminishes (different physicians may derive different impairment estimates). Accurate apportionment of impairment is a demanding task that requires detailed knowledge of causation for the conditions in question; the mechanisms of injury or extent of exposures; prior and current symptoms, functional status, physical findings, and clinical study results; and use of the appropriate edition of the AMA Guides. Sometimes the available data are incomplete, requiring the rating physician to make assumptions. However, if those assumptions are reasonable and consistent with the medical literature and facts of the case, if the causation analysis is plausible, and if the examiner follows impairment rating instructions in the AMA Guides (or at least uses a rational and hence defensible method when instructions are suboptimal), the resulting apportionment should be credible.
