Spinal Curvature and Lower Extremities Kinematics of Simulated Pregnancy during Stair Ascending and Descending

The purpose of this work is to investigate the effect of anteriorly-added mass to simulate pregnancy on lower extremities kinematic and lumbar and thoracic angles during stair ascending and descending. 18 healthy females ascended and descended, with and without a pseudo-pregnancy sac of 12 kg (experimental and control groups, respectively), a costume-made wooden staircase while instrumented with 20 reflective markers placed on the lower extremities and the spine. The movements were captured by 12 infrared cameras surrounding the staircase. Tracked position data were exported to MATLAB to calculate the required joints angles. SPSS was used to compare the ascent and descent phases of control group, and to find if there are any significant differences between control and experimental groups in the ascent phase as well as in the descent phase. When comparing the ascent and descent phases of control group, data revealed a higher hip flexion during ascending and greater ankle planter-flexion and dorsiflexion, lumbar, and thoracic angles during descending; however, no significant difference was shown in the knee flexion angle between ascending and descending. Non-pregnant data showed greater maximum hip flexion and ankle dorsiflexion during stair ascending compared to simulated-pregnant group; while ankle planter-flexion, knee flexion, and lumbar angle were greater for simulated-pregnant status. During stair descending, non-pregnant group had greater minimum hip flexion and ankle dorsiflexion compared to simulated pregnant group; while ankle planter-flexion, knee flexion, and maximum hip flexion were greater for simulated-pregnant group. However, the lumbar and thoracic angles were found to be similar for simulated-pregnant and non-pregnant groups during stair descending. In conclusion, the current study revealed important kinematic modifications pregnant women adopt while ascending and descending stairs at their final stage of pregnancy to increase their stability.

All mature forms of locomotion involve periods of unilateral stance. Unipedal hopping may provide useful information about the neuromuscular and biomechanical capabilities of a single lower extremity in adults. This study investigated whether hopping influenced vertical stiffness and lower extremity angular kinematics during human unipedal hopping. Vertical force and two-dimensional kinematics were measured in 10 healthy males hopping at three frequencies: preferred, +20%, and −20%. At +20%, compared to preferred, vertical stiffness increased 55% as hip flexion, knee flexion, and ankle dorsiflexion decreased, while at −20% vertical stiffness decreased 39.4% as hip flexion, knee flexion, and ankle dorsiflexion increased. As in bipedal hopping, the force-displacement relationship was more springlike at the preferred rate and +20% than at −20%. Given the prevalence of unilateral stance during walking, running, and skipping, findings related to unipedal hopping may be useful in the rehabilitation or conditioning of lower extremities.

Background and objective: Numerous tape applications have been used in patients with chronic ankle instability (CAI). However, the effect of prophylactic ankle taping on lower-extremity kinematics is still not well understood. This study aimed to investigate the effects of traditional taping, fibular repositioning taping, and kinesiology taping on the peak angles of the lower extremities in patients with CAI. Materials and Methods: A total of 14 men (age, 24.07 ± 4.46 years; height, 175.06 ± 5.10 cm; weight, 82.24 ± 10.38 kg (mean ± standard deviation)) with CAI identified using screening questionnaires (Cumberland Ankle Instability Tool, 17.64 ± 4.14; Foot and Ankle Ability Measure (FAAM) Activity of Daily Living, 86.69 ± 6.71; and FAAM Sports Subscale, 75.45 ± 6.70) participated. The peak angles of the hip, knee, and ankle joints during a stop-jump task, with and without tape application, were collected using a three-dimensional motion system. Results: The following peak angles were measured: hip flexion, hip adduction (ADD), hip internal rotation (IR), knee flexion, knee abduction (ABD), knee IR, ankle dorsiflexion, ankle inversion, and ankle ADD. No significant differences were observed in the peak angle of each joint across conditions (hip flexion, F(3,39) = 0.85, p = 0.47; hip ADD, F(1.729,22.478) = 1.90, p = 0.18; hip IR, F(1.632,21.220) = 0.67, p = 0.49; knee flexion, F(3,39) = 1.24, p = 0.15; knee ABD, F(1.691,21.982) = 1.24, p = 0.30; knee IR, F(1.830,23.794) = 0.44, p = 0.63; ankle dorsiflexion, F(3,39) = 0.66, p = 0.58; ankle inversion, F(1.385,18.007) = 0.85, p = 0.40; ankle ADD, F(1.865,24.249) = 2.23, p = 0.13). Conclusion: The application of different taping techniques did not significantly change the peak joint angles of the lower extremities during a stop-jump task. These results contradict those of previous studies, suggesting that ankle taping restricts joint range of motion.

Introduction: Ankle sprains are one of the most common sports injuries. This injury can affect the kinematics of the athlete's lower extremities. Therefore, the aim of this study was to compare the effect of 6 weeks of balancing and hopping strengthening training on the kinematics of the lower extremities of athletes with functional ankle instability while running. Methods: The present study  was a randomized clinical trial on 36 athletes with functional ankle instability who were randomly divided into three groups: hopping training, balance strengthening and control. The training lasted for six weeks and 3 sessions per week. Kinematic data were recorded by 3D OptiTrack camera while running at 10 km/h before and after training. SPSS software version 20 and ANCOVA test were used to compare the effect of exercises. Results: The results showed that there was a significant difference among the 3 groups in dorsiflexion (P = 0.009), inversion (P = 0.001) and knee flexion (P = 0.001). The results of Benferoni test also showed that hopping training in dorsiflexion, ankle inversion and knee flexion were significantly different from the control group (P = 0.009, P = 0.001, P = 0.001). The results also showed that balance strengthening training in the above movements were significantly different from the control group (P = 0.005, P = 0.002, P = 0.006) and in knee flexion, hoping training showed a significant difference compared to balance strengthening training (P =0.012). The results showed that both hopping and balance strengthening training improved the dorsiflexion, inversion and knee flexion angles in the initial contact in running. However, hopping exercises is better than balance strengthening in improving knee flexion angles. Conclusion: Therefore, according to the presented results, it can be concluded that the rehabilitation course of ankle sprains in athletes, coaches and sports rescue specialists should use a combination of balance strengthening training and hopping as a comprehensive program. In addition, hopping training can be used as a good alternative to balance training in places where there is not enough equipment.

Background: Pain control is considered as the key issue in modern midwifery. Along with medical painkillers, reflexology is viewed as a non-medical and noninvasive method. Hence, we aimed to investigate the effect of reflexology on the intensity of pain and length of labor. Materials and Methods: In this clinical trial, participants included 240 Iranian primiparous women with term and singleton pregnancy. Having a 3-4 cm cervical dilatation once they visited the hospital. Through a convenient sampling method, they were selected and then randomly divided into two groups. In the intervention group, reflexology was performed, and the intensity of pain during the active phase of labor along with the length of labor in the active phase was measured by visual analog scale (VAS) and compared with the control group. Data were analyzed using descriptive statistics, t-test, and Mann-Whitney U-test. Result: Data analysis showed a statistically significant difference between the intensity of pain in the 5-7 and 8-10 cm dilatation in the two groups (P=0.01). Moreover, the labor length in the active phase was found to be significantly shorter in the intervention group (P<0.001). Conclusion: It appears that reflexology can lead to a reduction in the pain and length of labor. Therefore, through instructing this technique, a goal of midwifery, which is reducing labor pain and its length can be achieved. Keywords: Reflexology, Labor Pain, Labor Length, Primiparous Women

Abstract Background Although almost a year has passed since the Coronavirus disease 2019 (COVID-19) outbreak and promising reports of vaccines have been presented, we still have a long way until these measures are available for all. Furthermore, the most appropriate corticosteroid and dose in the treatment of COVID-19 have remained uncertain. We conducted a study to assess the effectiveness of methylprednisolone treatment versus dexamethasone for hospitalized COVID-19 patients. Methods In this prospective triple-blinded randomized controlled trial, we enrolled 86 hospitalized COVID-19 patients from August to November 2020, in Shiraz, Iran. The patients were randomly allocated into two groups to receive either methylprednisolone (2 mg/kg/day; intervention group) or dexamethasone (6 mg/kg/day; control group). Data were assessed based on a 9-point WHO ordinal scale extending from uninfected (point 0) to death (point 8). Results There were no significant differences between the groups on admission. However, the intervention group demonstrated significantly better clinical status compared to the control group at day 5 (4.02 vs. 5.21, p = 0.002) and day 10 (2.90 vs. 4.71, p = 0.001) of admission. There was also a significant difference in the overall mean score between the intervention group and the control group, (3.909 vs. 4.873 respectively, p = 0.004). The mean length of hospital stay was 7.43 ± 3.64 and 10.52 ± 5.47 days in the intervention and control groups, respectively (p = 0.015). The need for a ventilator was significantly lower in the intervention group than in the control group (18.2% vs 38.1% p = 0.040). Conclusion In hospitalized hypoxic COVID-19 patients, methylprednisolone demonstrated better results compared to dexamethasone. Trial registration The trial was registered with IRCT.IR (08/04/2020-No. IRCT20200204046369N1).

Incomplete Kawasaki disease (iKD) is considered to be a less complete form of Kawasaki disease (cKD), and several differences in the laboratory presentations of iKD and cKD have been noted. We investigated serum procalcitonin levels in patients with iKD, cKD, and other febrile diseases (a control group). Seventy-seven patients with cKD, 24 with iKD, and 41 controls admitted to our hospital from November 2009 to November 2011 were enrolled in the present study. We obtained four measurements of serum procalcitonin levels and those of other inflammatory markers from each patient. Samples were taken for analysis on the day of diagnosis (thus before treatment commenced; D0) and 2 (D2), 14 (D14), and 56 days (D56) after intravenous immunoglobulin infusion. We obtained control group data at D0. The mean D0 serum procalcitonin levels of cKD patients (0.71±1.36 ng/mL) and controls (0.67±1.06 ng/mL) were significantly higher than those of iKD patients (0.26±0.26 ng/mL) (P=0.014andP=0.041, resp.). No significant difference in mean procalcitonin level was evident among groups at any subsequent time. In conclusion, the serum procalcitonin level of patients with acute-stage cKD was significantly higher than that of iKD patients.

Context:In jump-landing sports, the injury mechanism that most frequently results in an injury is the jump-landing movement. Influencing the movement patterns and biomechanical predisposing factors are supposed to decrease injury occurrence.Objectives:To evaluate the influence of a 3-mo coach-supervised jump-landing prevention program on jump-landing technique using the jump-landing scoring (JLS) system.Design:Randomized controlled trial.Setting:On-field.Participants:116 athletes age 15–41 y, with 63 athletes in the control group and 53 athletes in the intervention group. Intervention: The intervention program in this randomized control trial was administered at the start of the basketball season 2010–11. The jump-landing training program, supervised by the athletic trainers, was performed for a period of 3 mo.Main Outcome Measures:The jump-landing technique was determined by registering the jump-landing technique of all athletes with the JLS system, pre- and postintervention.Results:After the prevention program, the athletes of the male and female intervention groups landed with a significantly less erect position than those in the control groups (P < .05). This was presented by a significant improvement in maximal hip flexion, maximal knee flexion, hip active range of motion, and knee active range of motion. Another important finding was that postintervention, knee valgus during landing diminished significantly (P < .05) in the female intervention group compared with their control group. Furthermore, the male intervention group significantly improved (P < .05) the scores of the JLS system from pre- to postintervention.Conclusion:Malalignments such as valgus position and insufficient knee flexion and hip flexion, previously identified as possible risk factors for lower-extremity injuries, improved significantly after the completion of the prevention program. The JLS system can help in identifying these malalignments.Level of Evidence:Therapy, prevention, level 1b.

<p class="Abstract">Drawing – based modeling as learning approach, it allows students to create models based on drawing which help them to run a simulation. Students can use drawing–based modeling to learn a pair of interacting population known as predato–prey system while they usually conducted practicum using diagram of organisms. This study was to explore students’ argumentation and students’ understanding on the concept of the ecosystem. The study was conducted quasi – experimental using the matching – only post-test – only control group design. The participants consist of 60 grade 10 senior high school students in Subang, West Java. They are placed into two groups, 30 students in the experimental group and 30 students in the control group. Data was collected through argumentation test and selected response test for assessing students’ understanding. Students’ argumentation were analyzed using Toulmin’s argumentation pattern and statistical analysis. The result showed there is no significant difference between the experimental group and the control group for students’ argumentation. Most of the students are predominantly at level 2. But the coherency of arguments of the experimental group is more coherent than the control group. It means that the students in experimental group can make logical claim and supported by the correct and relevant grounds (data, warrant, and backing). The result of students’ understanding showed there is a significant mean score between the experimental group and the control group whether 72.33 for the experimental group and 62.13 for the control group</p>

Abstract: Central obesity is considered as a risk factor that strongly associated with several chronic diseases. Men with waist circumference ≥ 90 cm and women with waist circumference ≥80 cm are stated as central obesity. This study aimed to determine the effect of brisk walking on liver function in central obesity. This was an experimental field study with a non-randomized pre-post one group test and control group. This study used consecutive sampling method. There were 30 samples divided into two groups; 15 samples were given intervention and 15 samples as the control group. Data were analyzed with Wilcoxon signed ranks test.   The results showed that brisk walking for 1 month decreased the SGPT and SGOT levels, however, statistically there was no significant difference before and after intervention with a p value >0.05. Conclusion: There was no significant influence of brisk walking on liver function in central obesity. Sampel ditentukan dengan teknik pengambilan sampel konsekutif sampling. Terdapat 30 sampel yang dibagi menjadi 2 kelompok yakni 15 sampel yang diberi perlakuan dan 15 sampel sebagai kontrol. Data dianalisis dengan SPSS 20 dan uji Wilcoxon Signed Ranks Test. Hasil penelitian memperlihatkan terjadi penurunan kadar enzim hati SGPT dan SGOT pada sampel yang melakukan brisk walking selama 1 bulan tetapi secara statistik tidak ada perbedaan bermakna sebelum dan sesudah mendapatkan perlakuan dengan nilai p > 0,05.  Simpulan: Tidak terdapat pengaruh bermakna dari brisk walking (jalan cepat) terhadap fungsi hati pada obesitas sentral. Makanan adalah kebutuhan pokok untuk hidup manusia. Makanan dapat ditemui di restoran ataupun di pedagang kaki lima di tepi jalan yang mempunyai risiko penyakit yang ditransmisikan oleh bakteri patogen ketika tidak diolah dengan higienis. Alat makan merupakan salah satu risiko yang dapat menjadi sarana pertumbuhan dan perkembangbiakan mikroorganisme jika tidak dicuci dengan baik. Hal ini disebut dengan food-borne disease. Cara yang dapat digunakan untuk menekana terjadinya food-borne disease adalah dengan melakukan desinfeksi. Salah satu desinfektan yang dapat digunakan adalah daun jeruk purut yang dapat menurunkan angka kuman. Tujuan penelitian ini adalah menganalisis efektivitas daun jeruk purut terhadap angka kuman pada peralatan makan.Metode penelitian ini adalah eksperimental menggunakan rancangan non equivalent control group. Data dianalisis menggunakan uji annova dengan tes LSD (Least Significant Difference).Berdasarkan hasil penelitian didapatkan hasil yang signifikan bahwa daun jeruk purut dapat menurunkan angka kuman dan terjadi perbedaan yang signifikan pada kelompok kontrol.  Kelompok yang dapat menurunkan nilai angka kuman tertinggi adalah dengan konsentrasi 100 mg / 100 ml .Kesimpulan yang dapat ditarik dari penelitian ini adalah variasi konsentrasi daun jeruk purut dapat menentukan nilai penurunan angka kuman. Dalam penelitian ini, konsentrasi daun jeruk purut yang efektif untuk menirunkan angka kuman 100 mg / 100 ml. Keywords: daun jeruk purut, perelatan makan, angka kuman
