Pig production is one of the most important animal agricultural activities in South Africa, and plays a definite role in providing food security for certain population groups in the country. As with all animal production systems, it is subject to the risk of outbreak of transboundary diseases. In the present overview, evaluations of the perceived risk of selected transboundary animal diseases of pigs, as collated from the willing participants from the provincial veterinary services of South Africa, are presented. A scenario tree revealed that infected but undetected pigs were the greatest perceived threat. The provincial veterinary services, according to participants in the study, face certain difficulties, including the reporting of disease and the flow of disease information amongst farmers. Perceived strengths in surveillance and disease monitoring include the swiftness of sample despatch to the national testing laboratory, as well as the ease of flow of information between the provincial and national agricultural authorities. The four factors were identified that were perceived to most influence animal health-service delivery: transport, access, livestock policy and resources. African swine fever was perceived to be the most important pig disease in South Africa. Because the decentralisation of veterinary services in South Africa was identified as a potential weakness, it is recommended that national and provincial veterinary services need to work together and interdependently to achieve centrally controlled surveillance systems. Regionally-coordinated surveillance activities for certain transboundary diseases were identified as needing priority for the southern African region. It is proposed that an emergency preparedness document be made available and regularly revised according to the potential risks identified on a continuous basis for South Africa.

Infectious and zoonotic disease outbreaks have been linked to increasing volumes of legal and illegal trade. Spatio-temporal and trade network analyses have been used to evaluate the risks associated with these challenges elsewhere, but few details are available for the pig sector in South Africa. Regarding pig diseases, Limpopo province is important as the greater part of the province falls within the African swine fever control area. Emerging small-scale pig farmers in Limpopo perceived pig production as an important means of improving their livelihood and an alternative investment. They engage in trading and marketing their products with a potential risk to animal health, because the preferred markets often facilitate potential longdistance spread and disease dispersal over broad geographic areas. In this study, we explored the interconnectedness of smallholder pig farmers in Limpopo, determined the weaknesses and critical control points, and projected interventions that policy makers can implement to reduce the risks to pig health. The geo-coordinates of surveyed farms were used to draw maps, links and networks. Predictive risks to pigs were determined through the analyses of trade networks, and the relationship to previous outbreaks of African swine fever was postulated. Auction points were identified as high-risk areas for the spread of animal diseases. Veterinary authorities should prioritise focused surveillance and diagnostic efforts in Limpopo. Early disease detection and prompt eradication should be targeted and messages promoting enhanced biosecurity to smallholder farmers are advocated. The system may also benefit from the restructuring of marketing and auction networks. Since geographic factors and networks can rapidly facilitate pig disease dispersal over large areas, a multi-disciplinary approach to understanding the complexities that exist around the animal disease epidemiology becomes mandatory.

The article reviews the outbreaks and distribution of African swine fever (ASF) in South Africa since the first probable outbreak that occurred in the Koedoesrand Ward in 1926. Retrospective data on the ASF outbreaks in South Africa were obtained from the World Organisation for Animal Health (OIE) disease database and the South African veterinary services annual reports in addition to published articles and online sources. South Africa has experienced many outbreaks that can be divided into 2 time periods: the period before the development of the OIE diseases database (1993) and the period after. More than 141 outbreaks of ASF were reported during the first period. Since the development of OIE disease database, 72 outbreaks directly involving 2968 cases, 2187 dead and 2358 killed pigs mainly in smallholder pig farms were reported. The median number of cases for a given ASF outbreak is 17, but in 50% of outbreaks no pigs were killed for prevention. The most important ASF outbreak was reported in April 2014 in the Greater Zeerust district (North West province) involving 326 cases and 1462 killed pigs. However, the outbreak with highest mortality involving 250 pigs was reported in 2016 (Free State province). According to phylogenetic analysis, nine p72 genotypes (I, III, IV, VII, VIII, XIX, XX, XXI and XXII) have been identified in South Africa. Season-wise, more outbreaks were recorded during summer. It was also observed that the OIE disease database could contain errors that would have been introduced through compiled forms at country level. Spatiotemporal studies on ASF outbreaks in South Africa are therefore required in order to assess statistically and quantitatively the clustering of outbreaks over space and time.

The histories of the two swine fevers in southern Africa differ widely. Classical swine fever (hog cholera) has been known in the northern hemisphere since 1830 and it is probable that early cases of ‘swine fever’ in European settlers’ pigs in southern Africa were accepted to be that disease. It was only in 1921 that the first description of African swine fever as an entity different from classical swine fever was published after the disease had been studied in settlers’ pigs in Kenya. Shortly after that, reports of African swine fever in settlers’ pigs emerged from South Africa and Angola. In South Africa, the report related to pigs in the north-eastern part of the country. Previously (in 1905 or earlier) a disease assumed to be classical swine fever caused high mortality among pigs in the Western Cape and was only eradicated in 1918. African swine fever was found over the following years to be endemic in most southern African countries. Classical swine fever, however, apart from an introduction with subsequent endemic establishment in Madagascar and a number of introductions into Mauritius, the last one in 2000, had apparently remained absent from the region until it was diagnosed in the Western and subsequently the Eastern Cape of South Africa in 2005. It was eradicated by 2007. The history of these diseases in the southern African region demonstrates their importance and their potential for spread over long distances, emphasising the need for improved management of both diseases wherever they occur.

The historical evolution of veterinary services in South Africa is closely linked to the colonial history of the past and the eventual political formation of the Union of South Africa in 1910, as well as the establishment of a fully democratic South Africa in 1994. The majority of the early pioneering veterinarians had close links to military activities and were originally mostly of British origin. The appointment of the first colonial chief veterinary officers occurred in the late 1800s. These appointments were dictated by the need to combat devastating animal diseases, such as rinderpest and African horse sickness, mainly because they affected draught oxen (used for travel) and horses (used in combat). Veterinary field services was established in 1962 as a separate functional entity within government services when M.C. Lambrechts became Director of Veterinary Services of South Africa. In the context of this article, veterinary field services refers to that sphere of veterinary service delivery conducted by government-appointed or seconded veterinarians applying disease control and prevention, as required by animal health legislation. Paging through the history of veterinary field services in South Africa confirms that the problems faced by the veterinary services of today were just as real during the times of our pioneers. The pioneers of veterinary services transformed unknown animal diseases into textbook descriptions still used today and also demonstrated the important link to, and use of, the observations made by farmers, as well as the need for continued basic and applied research on animal diseases. This article provided a brief overview of the evolution of veterinary field services and the important role played by pioneers over the last two centuries to make South Africa relatively free and safe from the most important trade-sensitive and economically important animal diseases.

The purpose of this study was to set a benchmark for a monitoring and surveillance programme on the volumes of antimicrobials available and consumed by animals for the benefit of animal health in South Africa. This survey was collated from data available from 2002 to 2004. The authorised antimicrobials available in South Africa were firstly reviewed. The majority of available antimicrobials were registered under the Stock Remedies Act 36 1947. Secondly, volumes of antimicrobials consumed were then surveyed and it was found that the majority of consumed antimicrobials were from the macrolide and pleuromutilin classes, followed by the tetracycline class, the sulphonamide class and lastly the penicillin class.Results showed that 68.5% of the antimicrobials surveyed were administered as in-feed medications. 17.5% of the total volume of antimicrobials utilised were parenteral antimicrobials, whereas antimicrobials for water medication constituted 12% of the total and ’other‘ dosage forms, for example the topical and aural dosage forms, constituted 1.5% of the total. Intramammary antimicrobials represented 0.04% of the total. The surveillance systems for veterinary antimicrobials used by other countries were scrutinised and compared. It was concluded that a combination of the surveillance systems applied by Australia and the United Kingdom is the best model (with modifications) to apply to the animal health industry in South Africa. Such a surveillance system, of the volumes of veterinary antimicrobials consumed, should ideally be implemented in conjunction with a veterinary antimicrobial resistance surveillance and monitoring programme. This will generate meaningful data that will contribute to the rational administration of antimicrobials in order to preserve the efficacy of the existing antimicrobials in South Africa.

Abstract Background Effective animal health surveillance systems require reliable, high-quality, and timely data for decision making. In Tanzania, the animal health surveillance system has been relying on a few data sources, which suffer from delays in reporting, underreporting, and high cost of data collection and transmission. The integration of data from multiple sources can enhance early detection and response to animal diseases and facilitate the early control of outbreaks. This study aimed to identify and assess existing and potential data sources for the animal health surveillance system in Tanzania and how they can be better used for early warning surveillance. The study used a mixed-method design to identify and assess data sources. Data were collected through document reviews, internet search, cross-sectional survey, key informant interviews, site visits, and non-participant observation. The assessment was done using pre-defined criteria. Results A total of 13 data sources were identified and assessed. Most surveillance data came from livestock farmers, slaughter facilities, and livestock markets; while animal dip sites were the least used sources. Commercial farms and veterinary shops, electronic surveillance tools like AfyaData and Event Mobile Application (EMA-i) and information systems such as the Tanzania National Livestock Identification and Traceability System (TANLITS) and Agricultural Routine Data System (ARDS) show potential to generate relevant data for the national animal health surveillance system. The common variables found across most sources were: the name of the place (12/13), animal type/species (12/13), syndromes (10/13) and number of affected animals (8/13). The majority of the sources had good surveillance data contents and were accessible with medium to maximum spatial coverage. However, there was significant variation in terms of data frequency, accuracy and cost. There were limited integration and coordination of data flow from the identified sources with minimum to non-existing automated data entry and transmission. Conclusion The study demonstrated how the available data sources have great potential for early warning surveillance in Tanzania. Both existing and potential data sources had complementary strengths and weaknesses; a multi-source surveillance system would be best placed to harness these different strengths.

Information on the socioeconomic aspects and the health status of dogs in 2 resource-limited communities in the North West and Gauteng provinces of South Africa was gathered using semi-structured interviews and a standardised questionnaire. The dogs were examined clinically to determine their health status, and their body condition and age were scored. Most of the dogs (93 % in Jericho and 90 % in Zuurbekom) were infected with hookworm, which poses a threat to animal and human health in the 2 study areas. Many dogs were also being given a protein-deficient diet, which together with hookworm parasites would impact considerably on the dog's health. Dogs were mainly kept for security reasons. The need indicated to be most important by the residents of the 2 commnities was a lack of available and affordable veterinary services.

Background: Global decline in malaria episodes over the past decade gave rise to a debate to target malaria elimination in eligible countries. However, investigation regarding researchers’ perspectives on barriers and facilitating factors to effective implementation of a malaria elimination policy in South Africa (SA) is lacking.Aim: The aim of this study was to investigate the malaria researchers’ knowledge, understandings, perceived roles, and their perspectives on the factors influencing implementation of a malaria elimination policy in SA.Setting: Participants were drawn from the researchers who fulfilled the eligibility criteria as per the protocol, and the criteria were not setting-specific.Methods: The study was a descriptive cross-sectional survey conducted through an emailed self-administered semi-structured questionnaire amongst malaria researchers who met the set selection criteria and signed informed consent.Results: Most (92.3%) participants knew about SA’s malaria elimination policy, but only 45.8% had fully read it. The majority held a strong view that SA’s 2018 elimination target was not realistic, citing that the policy had neither been properly adapted to the country’s operational setting nor sufficiently disseminated to all relevant healthcare workers. Key concerns raised were lack of new tools, resources, and capacity to fight malaria; poor cross-border collaborations; overreliance on partners to implement; poor community involvement; and poor surveillance.Conclusion: Malaria elimination is a noble idea, with sharp divisions. However, there is a general agreement that elimination requires: (a) strong cross-border initiatives; (b) deployment of adequate resources; (c) sustainable multistakeholder support and collaboration; (d) good surveillance systems; and (e) availability and use of all effective intervention tools.

Background: South Africa faces a number of significant challenges apropos mental health service delivery, including a large treatment gap, a high rate of readmission, over-burdened specialist tertiary facilities, and slow integration of mental health into general health services. The South African National Mental Health Education Programme implemented between February 2019 and December 2019, aimed to upskill health workers to diagnose and manage mental disorders at primary and secondary levels of care.Aim: This study aimed to assess the evolution of training participants’ self-reported competency in mental health care and the number of referrals made to higher levels of care as well as to reflect on the possible broader effects of the training.Setting: The programme and study were conducted in South Africa with Medical Officers and Professional Nurses working at public sector primary and secondary level health care facilities.Methods: A descriptive observational study collected data from training participants through a pre- and post-course, and 3-month follow-up survey.Results: The average confidence ratings for performing mental health care activities and managing mental health conditions increased from pre- to post-course, and was either maintained or increased further at 3-month follow-up. A decrease in the self-reported percentage of patients being referred to a higher level of care was observed 3-months after the training.Conclusion: The evaluation suggests that a brief training intervention such this can go a long way in increasing the confidence of primary and secondary level health care workers in managing common mental health conditions and adhering to the provisions of legislation.
