Abstract BackgroundCompared to body mass index (BMI), waist circumference (WC), and adiposity measurements, adipose tissue morpho-functionality evaluations are more consistent predictors of cardiometabolic abnormalities. However, these evaluations require determination of adipokines and other non-routine biochemical parameters, which is not feasible in clinical practice. The present study establishes dysfunctional adiposity index (DAI) as a simple, accessible, and reliable marker of early adipocytes morpho-functional abnormalities and cardiometabolic diseases.MethodsTo establish the DAI constant parameters, 340 subjects (134 males and 206 females) without cardiovascular risk factors were selected from a cross-sectional study. Then, DAI was calculated in 36 healthy subjects who underwent subcutaneous adipose tissue biopsy, for whom adipocytes number and size, body composition, circulating adipokines, glucose, insulin, and lipids were also determined. The correlation of DAI with adipocyte morphology (size/number of adipocytes) and functionality (adiponectin/leptin ratio) was analyzed. The receiver operating characteristic curve was used to define the optimal DAI cut-off point to identify metabolic abnormalities. Finally, the independent association of DAI with cardiometabolic abnormalities was determined in 1418 subjects from the cross-sectional study through multivariate analyses.ResultsThe constant parameters to calculate the DAI were [WC/[22.79+[2.68*BMI]]]*[triglycerides (TG, mmol/L)/1.37]*[1.19/high density lipoprotein-cholesterol (HDL-C, mmol/L)] for males, and [WC/[24.02+[2.37*BMI]]]*[TG(mmol/L)/1.32]*[1.43/HDL-C(mmol/L)] for females. In subjects underwent biopsy, DAI correlated with adipocytes mean area (r=0.358; p=0.032), adipocyte number (r=-0.381; p=0.024), adiponectin/leptin ratio (r=-0.483; p=0.003), and systemic inflammation markers. Compared to BMI, WC, and visceral fat, DAI was the only determination associated with insulin resistance (area under the curve: 0.743; p = 0.017). In the cross-sectional study, DAI ≥1.065 was independently associated with diabetes (OR: 1.96; 95%CI: 1.36-2.84), non-alcoholic fatty liver disease (OR: 2.57; 95%CI: 1.98-3.33), subclinical atherosclerosis (OR: 1.74; 95%CI: 1.02-2.94), and hypertension (OR: 1.44; 95%CI: 1.10-1.88).ConclusionsThe present study establishes the constant parameters to calculate the DAI and highlights that a DAI ≥ 1.065 is associated with early cardiometabolic abnormalities independently of adiposity and other risk factors. Since DAI is calculated using accessible parameters routinely used in the clinic, this indicator can be easily incorporated in clinical practice for the early identification of adipose tissue abnormalities in apparently healthy subjects.

ABSTRACT Background: Actually the lifestyle exposes the population to several risk factors related to alimentary habits and less physical activity that contributes to chronic diseases appearance worldwide. Aim: To analyze the association between salivary cortisol and the components of metabolic syndrome. Methods: This is a cross-sectional study. As part of it, 28 individuals aged 30-59 years presenting three or more of the following findings: CA: ≥88 cm for women and ≥102 cm for men; SBP>130 mmHg and DBP>85 mmHg; GL>100 mg/dl; TG>150 mg/dl; HDL<40 mg/dl for men and <50 mg/dl for women. Was performed analysis of salivary cortisol (by radioimmunoassay) from 25 salivary samples collected throughout the day, for evaluating changes in the circadian rhythm of this hormone (8AM, noon and 8PM). Results: 28 evaluated individuals had a mean age of 51.9±7.5 years, mostly women (64.3%) and a mean of BMI 33.6±3.2 kg/m². The cortisol level from the 8AM averaged 18.7±4.8 ng/dlL. Individuals with FPG>110mg/dl, have significantly lower average levels of cortisol than ones with FPG <110 (12.8±5,2 vs. 17.3±4.2). Significant correlations were HOMA vs. WC (r=0,465; p˂0,005) and TG (r=0,473; p˂0,005), WC vs. FG (r=0,446; p˂0,005) and BMI (r=0,730; p˂0.0001); TG vs. HDL (r=0,441 p˂0,005) and FG (r=0,440; p˂0,005). Conclusion: Morning salivary cortisol in subjects with chronically elevated blood glucose can represent a downregulation of the hypothalamic-pituitary adrenal axis. This is an important finding not yet well investigated.

Objective: To explore the prevalence and socio-demographic factors associated with stunting and thinness in adolescent females. Method: The survey-based cross-sectional study was conducted in randomly selected schools of the Kotri Taulka of Jamshoro District in Sindh, Pakistan, from October to December 2019, and comprised healthy female adolescents aged 1019 years. Data about demographic and anthropometric factors was collected using a pre-tested questionnaire. Data was analysed using SPSS 23. Results: There were 393 subjects with mean age 14.93±2.18 years, mean weight 45.9±8.85kg, and mean height 151.6±6.25cm. The prevalence of stunting and thinness was 127(32.31%) and 42(10.68%) respectively. Stunting was more frequent in participants with father’s income ?15000 Pak rupees (p<0.05). Conclusion: The increased prevalence of stunting and thinness in adolescent females needs to be addressed by the policy-makers. Key Words: Stunting, Thinness, Adolescent, Females, Sindh, Pakistan. Introduction: Hypertension is a chronic life threatening non- communicable disease. It increases the risk for cardiovascular diseases even leading to premature death. Almost half of the adults with hypertension had elevated blood pressure during childhood. With globalisation and lifestyle changes, adolescents are exposed to various risk factors. However, diagnosis in this population is difficult due to absence of symptoms. Thus, regular blood pressure screening is essential in these groups. This study aimed to find out the prevalence of hypertension and its determinants among the school going adolescents of Kathmandu, Nepal. Methods: This was a descriptive cross sectional study which included five private secondary schools of Kathmandu, Nepal. The adolescent students studying in classes VIII, IX and X were the study subjects. Two stage cluster random sampling technique was used to select 356 participants. Data collection was done by doing anthropometric measurements, blood pressure measurement and through self administered questionnaire. Results: The study depicted that the prevalence of elevated blood pressure was 12.4%, stage 1 hypertension 32.3% and stage 2 hypertension 9.8%. Similarly, 13.8% were overweight and 1.4% were obese. Bivariate analysis depicted significant association of prevalence of hypertension with gender, religion and obesity (p < 0.05). The multivariate analysis shows that the significant predictors of elevated blood pressure and/or hypertension were gender and obesity. Males were six times more likely to have elevated blood pressure (AOR = 6.058, CI = 2.571 - 14.274) and 2.8 times more likely to be hypertensive (AOR = 2.838, CI = 1.688 - 4.773) compared to females. Similarly, compared to obese/ overweight students, thin and normally built ones have less likelihood of having elevated blood pressure and hypertension. Conclusions: Hypertension has been prevalent among adolescents, due to various behavioural risk factors. This is really challenging and of public health significance. Regular screening of adolescents is essential for early detection and management of hypertension.
