Gambling-Induced Analgesia: A Single Case Report

This paper describes a single case study of analgesia induced by gambling. The subject is a 48-year-old male diagnosed with pathological gambling problems, suffering chronic back pain resulting from a road trauma. The reported intensity of arousal associated with slot machines and roulette produced a state of dissociation or distraction that temporarily reduced levels of pain. Consistent with an operant conditioning model, this reduction in pain was a negative reinforcer that acted to elicit further gambling whenever the pain reached a certain level of discomfort. In the absence of any effective analgesic medication, he used gambling as his predominant strategy to manage pain. He began to enjoy gambling, and within a relatively short period, lost more than he intended and commenced chasing losses. Pain levels decreased following chiropractic interventions, but his gambling continued. The additional, positive reinforcing effects of the excitement generated by the slot machines and roulette gaming became sufficient to maintain persistence in gambling independent of pain experienced. This case highlights the possibility that psychological factors involved in establishing a gambling habit may differ from those involved in maintaining persistence.

This research aims to demonstrate how portfolio management influences the decision-making process in the projects of a financial organization. And to achieve this goal was used the single case study method. In order to reach this goal, the study began by means of bibliometric research on the subject of portfolio management and subsequent bibliographic research on the theme, decision making. Next, the relationships between portfolio management and decision making were studied. The results of the data collected confirmed the relationship between "the use of the project identification process in portfolio management to influence decision making" in order to add value to the business. It is also possible to demonstrate moderately that "the use of criteria for project selection influences the consequences of decision making", helping in the strategic management of the organization. One of the academic contributions was to note a migration of the portfolio management process, such as a tool that only controls the projects that will compose the portfolio of an organization, for a process that aims at a direct alignment with the strategic management of the organization. Regarding the practical implications, it was verified the importance of portfolio analysis for decision making, to the detriment of the evaluation of only one project. Taking into account the profitability and the return on investment of the projects, as the most important aspects for a decision making.

Purpose The purpose of this paper is to investigate the extent to which the organizational commitment and job satisfaction influence intent to turnover. Design/methodology/approach Following a quantitative approach regarding methodological aspects of this research, a case study was carried out in a company of information technology and communication located in Porto Digital, in the Northeast of Brazil. A data collection technique with 172 forms, a self-administered form with 18 closed questions with a Likert-type scale and an open questionnaire were used. In this analysis, Statistical Package for the Social Sciences (SPSS) software was used as the technique; besides the descriptive statistics, a correlation was made between dependent variables (intention of rotation) and independent variables (affective, normative, instrumental and work satisfaction). The level of organizational commitment in its three dimensions (affective, normative and instrumental), the job satisfaction in its five dimensions (satisfaction with nature of the task, with leadership, with colleagues, with salary and promotions) and the level of intent to turnover from the respondents were investigated. Findings The results suggest that affective and normative commitments and satisfaction with pay, satisfaction with promotions and satisfaction with the nature of the work are correlated with the intention of negative turnover significantly. Among the demographic factors, only the age showed a negative correlation with intent to turnover. By performing multiple regression analysis, we identified that the variables that most impacted the turnover intention are affective commitment, satisfaction with salary and normative commitment. The qualitative portion of this study was accomplished through a content analysis of the open question of the form. Finally, one of the main considerations is the findings that revealed variables other than those adopted in the study, which influence the permanence of the respondents. Research limitations/implications Among the limitations of the research a single case study is highlighted that replicates the experience in other ICT companies to verify if the results found are similar in other organizations and in other segments. The findings direct the construction and validation of new scales, the creation of qualitative protocols to identify the variables that influence the retention of a specific group of individuals to serve as a guide for the elaboration of a questionnaire, as well as creation of surveys of longitudinal nature to correlate the data of intention of rotation with the effective turnover. Practical implications Based on the results, organizations can reduce voluntary evasion by adjusting actions, policies and practices, directing those responsible for People Management to attract and retain good employees. It should be emphasized that voluntary turnover should be treated as one of the management indicators of greater relevance by organizations, representing a thermometer of all human capital management. Social implications The monitoring of employment and unemployment levels is part of the public policy agenda for generating employment and income in Brazil. For organizations, employee turnover can and should be managed and monitored so that appropriate levels are found and their consequences are minimized through effective solutions. The results are important both for technology-based organizations and for all stakeholders interested in the subject as the public power. Originality/value Although in the international literature turnover is a topic of study for many years, in the Brazilian context, there is a shortage of research on the subject, specifically in the sector of advanced technology where there is a great lack of skilled labor, a fierce competition and where to keep employees standard high becomes a survival factor. This study may contribute to the exploration in the local literature, as it will help locate the academy on the Brazilian reality and open the doors to new research works on turnover and its possible correlations with other variables besides organizational commitment and job satisfaction.

Atlanto-axial rotatory subluxation is a rare, controversial and frequently misdiagnosed condition occurring primarily in children. A single case study design was used to evaluate the effectiveness of cervical stabilisation training and correction of muscle imbalance, following reduction of this condition, in a nine-year-old boy. The study was conducted over a six-week period during which the subject maintained a daily diary to record his symptoms. The programme consisted of various muscle relaxation and lengthening techniques, endurance training of the deep cervical flexors and lower scapular stabilisers, as well as postural re-education. No treatment was directed at the articular component. The patients signs and symptoms were greatly reduced during the study period. This study supports the importance of correcting the muscle dysfunction component in cervicogenic pain sufferers.

The scope of this article is to explore the transaction profitability of frequent and sporadic buyers in the e-commerce arena. Evidence in relationship marketing literature stressing the impact of purchase frequency on customer transaction profitability as well as recent academic research challenging this approach and pointing out the importance of sporadic clients is analyzed and presented. A single case study research methodology was chosen for this article due to the exploratory facets associated with the subject and the industry under investigation. In order to gather relevant input to carry out this research, one of the largest retailing groups in Brazil was investigated. Conclusions are drawn showing that greater frequency of purchases does not necessarily translate into increased customer transaction profitability. Implications are presented, enabling practitioners and academics to grasp fully the real value of customers — both frequent and sporadic buyers — in order to develop coherent approaches for dealing with them adequately.


This chapter presents a Portuguese company of the information technologies industry that did an internationalization process and has implemented a management control system with a very interesting complexity level and that, in reality, crosses the theory present in the different bibliographical references on the subject, with the experience and business practice of its management. Such management practices were so helpful that, in a short period, the company could internationalize its activity and turnover went from 10 million euros to 60 million euros, becoming a reference of its industry in Portugal. It should be noted that the case study was developed through the triangulation of different information sources, such as interviews and meetings, direct observations of management control practices, as well as consultation of documents and internal reports. The authors characterize the company and its evolution, national and international, as well as the management control system implemented with reference to the instruments developed in the previous chapters.

The authors present a case of an incidental finding of a metallic foreign body located within the calcaneus of a 17-year-old girl. Although the presence of foreign material is a common occurrence in the foot, intraosseous metallic bodies are rare, especially so when they present as an incidental finding. A literature review of PubMed using keywords metallic, foreign body, and calcaneus revealed only a single case report, which did not present as an incidental finding. The presentation described here is rare and provides a platform for discussion about possible management options. Levels of Evidence: Therapeutic, Level IV: Case study

ABSTRACT Purpose: The purpose of this single case study was to examine the effectiveness of utilizing a wearable fitness tracker device in self-monitoring behavior change in complying with prescribed treatment. It was hypothesized that using a wearable self-monitoring device while involved in the treatment of multiple medical conditions will be beneficial to behavioral compliance and behavioral change in an overweight type 2 diabetic, geriatric subject being treated by a multidisciplinary health team utilizing an integrated treatment model. Methods: An exploratory single case study research design is employed to explore those situations in which the intervention, a wearable monitoring device, is employed. This observational case study model was applied to the utility of self-monitoring wearable smart technology marketed as FitBit for self-monitoring activity and exercise over a 36-week period. Results: After the 36-week intervention program, results revealed qualitative improvements in the (1) active minutes, (2) steps taken and (3) miles walked by the subject. In addition, the technology reported on calories burned, sleep hours and minutes logged and liquid consumed during each 24-hour period. The integrated allied health team was able to monitor changes made over time and noted improved time dedicated to exercise, walking and total miles walked. On miles walked per day, the results show that the subject increased miles walked from less than one mile per day to more than 4.6 miles per day which approached the recommended 5 miles per day goal. The calorie monitor aided the subject in changing calorie intake from that which exceeded 3500 per day to the recommended 2500 calories per day. Furthermore, the hours slept per night changed from less than 6.8 per night to 8.1 per day. Conclusions: It was concluded that a 36-week intervention program can be an effective intervention methodology for improved health and well-being as measured by standard healthcare and wellness markers utilizing a wearable fitness tracker device like the Fitbit. While further research is needed with a larger sample, it is therefore recommended that allied health professionals consider the utilization of wearable smart technology as an adjunct to self-monitoring and compliance with treatment planning and follow-up care with allied health care providers.

This paper is concerned with a generic feature geometry based model for  phonological disorder description of hypernasality routed in the voice profiles of an Iraqi child (aged 7 years) with a cleft palate. It primarily explores the applicability of this model to the resonance hypernasalization deficit elicited in this single-case study. The preliminary findings of the treatise reveal that the subject under study has a limited linguistic competence, due to the dysfunction of  the velopharyngeal valve, caused by the cleft palatology, which negatively militates his communicative skills. The most common resonance distortions resulted from this dysfunction are glottalization (25.95%), consonantal hypernasality(23.40%),vocalic hypernasality (21.019%), pharyngealisation (8.75%), labialization (7.48%), lateralization (7%), elision (4.93%), initial cluster simplification (1.11%), metathesis and vocalic shortening (0.15%), equally. Corpus analysis via manner association scheme (within the framework of Autosegmental Phonology) divulges that this model is significantly applicable to  hypernasalized vowels. However, this autosegmental model fails to capture the manner co-attachment association in certain cases of vocalic and consonantal hypernasality pointing in the direction of harmonic nasality.

Introduction: Herpes simplex is a viral infection caused by the herpes simplex virus. Oral herpes caus-es the group of blisters called cold sores or fever blisters over the face or mouth. These are having se-vere burning sensation which causes the dysphagia and difficulty in speaking. It is associated with fe-ver. There will be ulcer formation by the rupture of blisters. Oral herpes can be correlated to Pithaja mukhapaka. This is a case report of 41 years old male who complained of severe burning ulcers on hard palate since 1 month. Materials and Methods: The subject who approached Shalakya Tantra OPD of GAMC, with symptoms of burning ulcers over the palate was thoroughly examined and treat-ment was planned based on Chikitsa sutra of Pithaja mukhapaka. Result: The subject had shown good improvement both subjectively and objectively. Discussion: Pithaja mukhapaka is a Sarvasara Mukha-roga with Daha, Oshayuktha Vrana. There for the treatment is focused on pacifying Pitha.
