BACKGROUND Screening Wizard (SW) is a technology-based decision support tool aimed at guiding primary care providers (PCPs) to respond to depression and suicidality screens in adolescents. Separate screens assess adolescents’ and parents’ reports on mental health symptoms, treatment preferences, and potential treatment barriers. A detailed summary is provided to PCPs, also identifying adolescent-parent discrepancies. The goal of SW is to enhance decision making to increase utilization of evidence-based treatments. OBJECTIVE We describe a multi-stakeholder qualitative study with adolescents, parents, and providers to understand potential barriers to implementation of SW. METHODS We interviewed 11 parents and 11 adolescents, and conducted 2 focus groups with 17 healthcare providers (PCPs, nurses, therapists, staff) across 2 pediatric practices. Participants described previous experiences with screening for depression and were shown a mock-up of SW and asked for feedback. Interviews and focus groups were transcribed verbatim, and codebooks inductively developed based on content. Transcripts were double-coded, and disagreements adjudicated to full agreement. Completed coding was used to produce thematic analyses of interviews and focus groups. RESULTS We identified five main themes across the interviews and focus groups: (1) parents, adolescents, and pediatric PCPs agree that depression screening should occur in pediatric primary care; (2) there is concern that accurate self-disclosure does not always occur during depression screening; (3) Screening Wizard is viewed as a tool that could facilitate depression screening, and which might encourage more honesty in screening responses; (4) parents, adolescents and providers do not want Screening Wizard to replace mental health discussions with providers; and (5) providers want to maintain autonomy in treatment decisions. CONCLUSIONS We identified that providers, parents, and adolescents all have concerns with current screening practices, mainly regarding inaccurate self-disclosure. They recognized value in SW as a computerized tool that may elicit more honest responses and identify adolescent-parent discrepancies. Surprisingly, providers did not want the SW report to include treatment recommendations, and all groups did not want the SW report to replace conversations with the PCP about depression. While SW was originally developed as a treatment decision algorithm, this qualitative study has led us to remove this component, and instead focus on aspects identified as most useful by all groups. We hope that this initial qualitative work will improve future implementation of SW.

This study investigated the goals adolescents and parents have for the adolescent’s participation in an organized youth program. Open-ended interviews were conducted with 52 adolescents from 13 project-based youth programs and one of their caregivers. Adolescents (aged 13–18; 56% female) were ethnically diverse (46% Latinx, 29% European American, 19% African American, and 6% other). Caregivers ( M age 43.15 years) were predominantly female (80%) and biological or adoptive parents (94%); about half had been born in the United States. Qualitative and quantitative analyses were conducted to examine goal content and congruence between adolescents and parents. Interview data were inductively coded following a consensus team approach. Thirteen goal categories were identified: nine were described by both adolescents and parents (e.g., skill-building and socialization) and two were unique to adolescents and two to parents. More than half (56%) of adolescent-parent dyads reported at least one matching goal. In general, adolescents tended to emphasize short-term objectives and parents long-term outcomes. Collectively, findings indicate that adolescents and their parents view organized programs as salient contexts to pursue youth’s personal goals. Findings have implications for theory, research, and practice.

This study study aimed to examine (a) adolescents’ attributions and emotions for their subjectively perceived good and bad relationships with their parents, (b) the association of the intuitive and attributional appraisals of the adolescent-parent relationship with the subsequent emotions, and (c) the role of the perceived importance of the good adolescent-parent relationship in the generation of attributions and emotions, and in the impact of attributions on emotions. The sample comprised 670 adolescents, both genders, aged 14-17 years old, representing various parental socioeconomic levels. The results showed that: (a) It was extremely important for the adolescents to have good relationships with their parents, (b) the perceived good adolescent-parent relationships were attributed to internal, stable and personal controllable factors, along with parent- and self-parent interactive- related factors, while the estimated as bad relationships were attributed to external, stable, personal uncontrollable and external controllable factors (parents’ negative properties), (c) the adolescents experienced intense positive and negative emotions (mainly, general / outcome- dependent) for the perceived good and bad relationships with their parents, respectively, (d) both intuitive and attributional appraisals of the relationship were associated with the emotions, particularly in the perceived bad adolescent-parent relationship, and (e) the relative strength of the association of the attributional dimensions with the emotions varied between the perceived good and bad adolescent-parent relationship and across the various emotions. This study examined the ways in which Japanese adolescents handle disagreements with parents, and the extent to which the cultural values of emotional bonds within the family predict adolescents' reported handling of disagreements. Adolescents ( N = 1029), aged 12 to 25 years, reported their projected actions in response to six hypothetical adolescent—parent disagreements, and completed a scale of values of emotional bonds in the family. As hypothesized, self-assertion was the most common action, followed by negotiation and compliance. Although generally self-assertion and negotiation were higher and compliance lower with increasing age, adolescents' actions also varied depending on the situation. The values of emotional bonds predicted adolescents' reported handling of disagreements, as examined using structural equation modeling. Implications of these findings for the adolescent—parent relationship and adolescent development in Japanese cultural contexts are discussed.
