Introduction. The activities of health workers are associated with the impact of many harmful factors that lead to loss of health. Compared with other professional groups, health care workers are ill longer and harder, which may be due to polymorbidity pathology.The aim of the study based on the survey data to study the representation of health workers about working conditions and to identify their impact on the formation of occupational diseases.Materials and methods. In order to get a real idea of the attitude of medical workers to their working conditions in 2018, an anonymous survey was conducted of 1129 doctors and 776 employees of secondary and junior medical personnel working in health care institutions in St. Petersburg, Moscow, Krasnoyarsk, Vologda and Orel.Results. Research of working conditions and health of physicians allowed to establish that work in medical institutions imposes considerable requirements to an organism of working, its physical condition and endurance, volume of operational and long-term memory, ability to resist to mental, moral and ethical overloads.Conclusions: The most important method of combating the development of occupational diseases is their prevention. Organizational and preventive measures should be aimed primarily at monitoring the working conditions and health of medical staff .

Development of health care of Siberia in the 18<sup>th</sup>&nbsp;— first half of the 19<sup>th</sup> century was closely connected with economic and political development of the Russian state. Before the medical personnel emergence in Siberia its population received medication only in the form of folk remedies. Development of its territory, insanitary conditions of life of Siberians, constant contingent of exiled to the indigenous people, and severe climate contributed to the spread of such diseases as smallpox, syphilis, and others.<br> The first medical institutions of Siberia were military hospital and infirmaries. In the days of reign of the Empress Anna Ioannovna, the city medical care started developing, and thanks to an initiative of the industrial enterprises owners&nbsp;— medical care for miners. After the establishment of the Public Charity Orders in Siberia, hospitals under their jurisdiction began to function in Tobolsk, Irkutsk, and Tomsk. A worthy contribution to the development of local medical institutions was made by donators Tolstopyatov, Bednyagin, Chupalov, and others. However, deficiency of financing, hospitals, and the medical personnel couldn’t adequately satisfy the needs of Siberian inhabitants in medical care.<br> The purpose of the article is to analyse the condition of public health, causes of morbidity and mortality of the population, as well as the process of medical care development in Siberia the turn of the 18<sup>th</sup> century by means of a historical and genetic method.

Aim. To assess the importance, role, methods and conditions for creating and sustaining positive motivation of medical personnel in deploying a modern management system based on lean production to optimise the production flow in medical institutions, improve quality, availability and efficiency of public medical care.Materials and methods. The authors employed historical, bibliometric and statistical methods to analyse domestic and foreign scientific archives on lean production, motivation and motivational risks as key factors in implementing lean technologies in medical institutions.Results. Implementation of lean production (LP) as a manufacturing management system is among key conditions for creating a new model of the medical institution. Principles of LP are applied at three interfaces: with patients, personnel and resources. Positive motivation of employees is prerequisite for accomplishing a project, which requires supervisors to know and leverage the basics of motivational management. Creation of a motivating environment for various categories of employees in developing a LP system requires taking into account the type and management culture level of a medical institution.Conclusion. In order to create and sustain motivation for realising creative potential of the team, it is necessary to account for the motivation risks, develop and implement a motivation system differentially by employee categories and project stages. Effective motivational measures are necessarily based on a system of material and non-material incentives with clear and achievable criteria, amounts and forms valued by employees and rooted in medical and economic feedback of implementing the LP principles.

The article is devoted to the impact of healthcare reform on the situation in the industry and the requirements for managers of medical organizations. On the data from the expert survey, participants of which were physicians with practical experience in the field of health management; economists studying the work of the public sector; lawyers involved in the protection of patients’ rights, etc., it provides characteristics of today’s financial, logistical support and staffing of medical institutions, their relationships with founders, insurance companies and patients; there are given experts’ opinions on who should be the leaders of medical organizations — managers or clinicians, how to create their personnel reserve, what should be changed in the system of professional development and retraining of medical personnel. It is shown that the working conditions of medical organizations have radically changed, and the amount of attracted extra-budgetary funds has become an important indicator of their effectiveness. At the same time, the institutional context, in which they function, is highly controversial, devoid of a number of fundamentally important elements, and is not balanced in terms of the rights and obligations of various actors involved in the provision of medical care. This significantly complicates the activity of the management of medical institutions, generates a «conflict of interests» among them, which ultimately negatively affects patients. According to experts, today an experienced manager should be at the head of the medical institution, but — with the obligatory good knowledge and understanding of the specifics of the medical industry. Since medical institutions are now managed by medics, their training in economics and management within the system of continuing professional education becomes very important. The study determines the impact of regional measures of socio-economic stimulation of medical personnel on the process of employment in medical institutions in rural areas. The analysis of Federal and regional legislation regulating the process of encouraging doctors to find employment in the rural health system is carried out. It was found that measures of socio-economic support for the provision of ready-made housing in the framework of regional events, contributed to a more active involvement and long-term consolidation of medical personnel in rural areas.

Purpose: The article is devoted to the analysis of organizational measures to overcome the medical consequences of two large-scale events: the Chernobyl accident on 26.04.1986 and the COVID-19 pandemic. Results: A comparison of the causes, key factors, numbers affected and involved in these events persons, and the availability of the source of knowledge about the clinical picture, diagnosis, prevention and treatment of diseases, developing as a result of radiation exposure and the virus SARS-Cov-2. The article considers the availability of special medical institutions, the infectability of medical workers, the presence of long-term consequences for the health of victims and for the economy. Conclusions: In the development of such catastrophic events, an important role should be played by 1) Timely and adequate information to the population. 2) It is necessary to have a sufficient number of beds that can be repurposed in accordance with the needs and provided with appropriate equipment to support the vital functions of the body. 3) Planned training of qualified medical personnel should be carried out. 4) It is necessary to have stocks of emergency drugs close to the institutions where it is possible to receive victims.

The experience of organizing the treatment of wounded and sick military men of the Red army in Saratov at the final stage of the Great Patriotic War is studied. Priority tasks that were solved in conditions of the location of medical institutions in the deep rear are given. The structure of injuries and injuries with which soldiers and officers of the Red Army were sent from the front to the evacuation hospitals of the city and the region was investigated. Attention is drawn to the change the severity and location of wounds, depending on the progress of our units towards Germany. The factors that contributed to the successful treatment and return to the system of heavy contingents of the wounded and sick are indicated. The importance of the initiative work, which was carried out by medical personnel of evacuation hospitals on the development and introduction of new methods to improve the effectiveness of medical measures in everyday clinical practice, was stressed. Published data on the number of beds that were transferred from the Saratov hospital base in the army. In addition, evacuation hospitals and the Saratov Medical Institute were those institutions of which, even at the final stage of the war, medical personnel were sent for service to the front. At the same time, many honorably and honorably fulfilled their civic duty to the Motherland. Examples of patronage over hospitals from industrial enterprises and public organizations of Saratov and the region are given. A brief overview of the treatment and prophylactic tasks, which were solved in the interests of the civilian population of the city and the region, is given.

Relevance. In the context of the pandemic, the current psychological state and the likely delayed deterioration of the mental health of medical personnel, regardless of their professional duties, are of concern. Taking into account the stressful circumstances in which the pandemic has placed all medical workers, assessing the level of perceived stress, as well as identifying risk factors and factors that mitigate stress, is an urgent task, the solution of which will contribute to the effective organization of psychological support for medical personnel.Methods. The study used a shortened version of the Perceived Stress Scale (PSS10) as the main methodology – a tool designed to study a person's attitude to stressful situations and determine the degree to which life is assessed as stressful and uncontrolled (Cohen et al., 1988). The methodology is widely used by the scientific community, including during the pandemic, and the original English version of the PSS has been translated into many languages and adapted in different countries. The study was conducted through an online survey.Respondents. The study, conducted in the period from May 9 to June 26, 2020, when there was a steady increase in SARS-CoV 2 virus infections in Russia, involved 1,287 employees of medical institutions in various subjects of the federation (1,079 women and 208 men) aged 19 to 80 years. The sample of the study is represented by doctors, middle and junior medical staff, heads of medical departments and institutions, as well as volunteers.Results. A high average overall index of perceived stress among employees of medical institutions was revealed. The highest level of stress is observed in the youngest age group (from 19 to 30 years), and with age, the level of stress decreases. There were no differences in the level of stress between male and female health workers in any of the considered stress indicators. Protective factors against increased stress are the presence of children and living with family members or relatives, while the presence of a spouse / partner does not affect the overall indicator of perceived stress. From different categories of medical workers, the index of perceived stress is higher in managers and doctors compared to middle and junior medical personnel. There were no significant differences in the level of stress between the staff working and not working in the "red zone". Differences in the level of stress of medical workers depending on the current situation (the number of infected and dead) in the region of residence were revealed: the level of stress is higher for those specialists who are forced to work in stressful conditions due to the complex epidemiological situation. The relationship between different beliefs about COVID19 and the level of stress is shown: the level of stress is higher in those who take its danger more seriously. In terms of the sources of stress, the most likely predictors are anxiety about exposure to COVID19 at work and the likelihood of infecting loved ones, fear of catching it yourself and getting sick, inability to meet the usual personal needs and economic instability.Conclusions. Practical interventions, the provision of psychological support and the development of actions to reduce stress for staff during a pandemic, should take into account the role of factors in increasing stress among staff working in specific conditions and focus on the prevailing beliefs and stressors in specific categories of medical factors.

The study aimed to evaluate the influence of coronavirus on the functioning of the system of healthcare and the associated pharmaceutical industry in Russia. The study of the influence of coronavirus infection on the system of healthcare and pharmaceutical market in Russia was made using a statistical method of comparison of the parameters. The degree of influence on the system of healthcare was estimated by the monthly dynamic of the morbidity rate, which reflected the load on medical institutions of the budgetary system of healthcare of the RF. The changes on the pharmaceutical market were evaluated through quantitative and price factors: quantitative factor was defined by the dynamics of general volume of sales and price – by the changes of the average-weighted prices on the market. As a result, it was established that the dynamics of the morbidity rate with coronavirus in Russia varied wavelike, significantly increasing in the periods of pandemic peaks in May and December 2020. This indicates that the system of healthcare lacks mechanisms of fighting the infection spread in complicated epidemiologic conditions, the bed capacity is low, and there is a deficit in medical personnel. During pandemics, a significant increase in the drug cost was observed on the pharmaceutical market. Especially, it was evident for drugs used to limit the spread of coronavirus, which was confirmed by the shift of drugs sales to the group of antimicrobials. Coronavirus pandemic significantly affected the structure and tendencies in the development of healthcare and Russian pharmaceutical market.

The article presents results of medical sociological survey of attitude of various categories of medical personnel of municipal medical institutions of the Irkutskaia oblast to HIV-infected patients. The study used qualitative and quantitative sociological techniques that permitted to obtain representative data about characteristics of attitude of medical workers to patients and to establish risks of professional contamination and factors effecting development of models of behavior of medical workers in the system of relationships "physician-patient". The sociological survey fixed ambiguous and contradictory character of attitude of medical workers to HIV-infected patients related to fear of professional risk of contamination. The correlation and typological analysis permitted to establish stigmatization and discrimination of HIV-infected individuals manifesting at emotional psychological and behavioral levels and fixing in striving of particular groups of respondents to isolate HIV-infected patients. The results of survey testify necessity of development in all categories of medical personnel rational pragmatic attitudes of perception of risk of contamination permitting to manage this perception and to develop program of psychological and behavioral trainings for various categories of medical workers.
