Wilson disease is a rare, inherited autosomal recessive disease of copper metabolism. Loss of ATP7B func-tion leads various grades of reduced biliary excretion of copper accumulation and toxicity of copper in the liver, brain and other tissues results in liver toxicity and other myriad manifestations of the disease. In clas-sics there is no exact description of the disease entity which exactly matches the feature of Wilson disease, but it can be correlated with Sahaja Vyadhi or Janamjata Vyadhi. A 16-year-old female child diagnosed with Wilson disease complaints of improper gait, altered speech, chronic constipation and altered mental level etc. Investigation shows S. Ceruloplasmin- 0.4mg/dl, S. copper- within normal limit, slit lamp study for KF ring was positive. This patient was treated with Ayurvedic procedures i.e. Abhyanga, Swedana, Bas-ti and Nasya along with oral medication. After two treatment protocol patient got symptomatic relief in clin-ical features. So, it can be concluded that quality of life (QOL) of patients with chronic disease can be im-proved with the help of internal Ayurvedic medication along with Panchkarma procedure.

Depression is common disorder and contributes largely to nonfatal health loss characterized by feeling of sadness or loss of interest in activities which can be correlated to kaphaja Unmada. In Ayurveda, choice of treatment for Kaphaja Unmada is Vamana with Satwavajaya Chikitsa, Shamana Aushadhi, and Medhya Rasayana. A treatment protocol was designed based on signs and symptoms of the patient and was administered Sarvanga Udvartana with Bashpa Sweda, Sirotalam, Vamana, Patent medicine (Composed of Jyotishmati Taila, Shuddha Gandhaka, Shankhapushpi, Vacha, Jatamamsi and Bhringraja) and Saraswatarishta. Interstitial Lung Diseases(ILD) are a group of diffuse parenchymal lung disorders, most of which cause progressive scarring of lung tissue associated with substantial morbidity and mortality as the scarring in ILD eventually affects the ability to breathe and get enough oxygen into the blood- stream. Prolonged ILD may result in pulmonary fibrosis. As there is no definite treatment for the disease, it has remained a great problem for the patients. Several conventional treatment modalities are in existence but not much comprehensive. Though the permanent cure for the disease is not available but Ayurveda can provide cost-effective and conveniently the better treatment which relieves the symptoms to a good extent and improves the quality of life of the diseased. In this article, a case study presented on a case of ILD. A 63 years old female, diagnosed case of ILD, attended Kayachikita OPD , with chief complaint of breathlessness since 2018 but severe from 1 month. Patient got admitted in IPD and treated on the Ayurvedic principles. At the time of discharge, symptomatic relief was found in breathlessness, effectiveness in FEV1( Forced Expiratory Volume in 1 sec)and also maitained saturation , but no significant changes in X-Ray Chest was observed after treatment.

Background: Wilson disease is an autosomal recessive disorder of copper metabolism, where excessive copper accumulation occurs in various tissues. Although hepatic and neurological symptoms predominates, it may present with some other unusual features which sometimes confuses clinicians and makes a diagnostic dilemma. We present here an 11 years old boy presented with gradual darkening of whole body over last 2 months and jaundice for 5 month. His clinical features and laboratory parameters were suggestive of Wilson disease. Being one of the common causes of chronic liver disease (CLD) in childhood, Wilson disease may present with some atypical features like darkening of complexion.Bangladesh J Child Health 2018; VOL 42 (1) :43-45

Primary tinnitus has a severe negative influence on the quality of life of a substantial portion of the general population. When acoustic coordinated reset (CR) neuromodulation stimuli are delivered for several hours per day over several weeks a clinically significant symptom reduction in patients with primary tonal tinnitus has been reported by several clinical sites. Here, we reported the first case where CR neuromodulation was delivered through a hearing aid. A 52-year-old man with chronic primary tonal tinnitus was previously considered untreatable with sound therapy. He initially received the classic CR treatment protocol with signals delivered with the separate proprietary device with his hearing aids removed during treatment. He was subsequently treated with the therapy being deployed through a set of contemporary hearing aids. After 5 months of classic CR treatment with the separate custom device, the THI and VASL/Ascores worsened by 57% and 13%/14%, respectively. Using the hearing aid without CR treatment for 5 months no change in tinnitus symptoms was observed. However, after three months of CR treatment delivered through the hearing aids, the THI and VASL/Ascores were reduced by 70% and 32%/32%, respectively.

Wilson Disease (WD) is a genetic metabolic disease of copper metabolism. The implicated gene is ATP7B, encodes a P-type ATPase which transports copper. The resultant defective metabolism of copper results in copper accumulation in multiple tissues especially liver, eye and central nervous system. WD occurs worldwide, usually between 5 and 35 years; a wider age range is also reported. Clinical presentations are diverse and include combinations of hepatic, neurological, ophthalmic and psychiatric manifestations. Other organs or tissues may also be affected. Biochemical abnormalities such as serum ceruloplasmin and 24-h urinary copper excretion are important for the diagnosis but are not always abnormal in WD. The liver histopathology has several different patterns from mild nonspecific changes to acute fulminant hepatitis and cirrhosis. Copper histochemistry is helpful in diagnosis. Genetic testing is another diagnostic tool. It is important to diagnose WD because it is fatal when overlooked, curable when diagnosed. The diagnosis should be keep in mind at all ages in patients with hepatic disease, neurological disease, or psychiatric symptoms.

Purpose Misophonia is a sound tolerance disorder condition in certain sounds that trigger intense emotional or physiological responses. While some persons may experience misophonia, a few patients suffer from misophonia. However, there is a dearth of literature on audiological assessment and management of persons with misophonia. The purpose of this report is to discuss the assessment of misophonia and highlight the management option that helped a patient with misophonia. Method A case study of a 26-year-old woman with the complaint of decreased tolerance to specific sounds affecting quality of life is reported. Audiological assessment differentiated misophonia from hyperacusis. Management included retraining counseling as well as desensitization and habituation therapy based on the principles described by P. J. Jastreboff and Jastreboff (2014). A misophonia questionnaire was administered at regular intervals to monitor the effectiveness of therapy. Results A detailed case history and audiological evaluations including pure-tone audiogram and Johnson Hyperacusis Index revealed the presence of misophonia. The patient benefitted from intervention, and the scores of the misophonia questionnaire indicated a decrease in the severity of the problem. Conclusions It is important to differentially diagnose misophonia and hyperacusis in persons with sound tolerance disorders. Retraining counseling as well as desensitization and habituation therapy can help patients who suffer from misophonia.
